Background

Exclusive breastfeeding (EBF) during the early months of life reduce infant morbidity and mortality. Current recommendation in Sri Lanka is to continue exclusive breastfeeding up to six months of age. Exclusive breastfeeding rates are generally assessed by the 24 recall method which overestimates the actual rates. The objective of this study was to determine actual exclusive breast feeding rates in a cohort of Sri Lankan children and to determine the reasons that lead to cessation of breastfeeding before six months of age.

Methods

From a cohort of 2215 babies born in Gampaha district, 500 were randomly selected and invited for the study. They were followed up at two (n?=?404), four (n?=?395) and six (n?=?286) months. An interviewer administered questionnaire asked about feeding history and socio-demographic characteristics. Child health development record was used to assess the growth.

Results

Exclusive breastfeeding rates at two, four and six months were 98.0%, 75.4% and 71.3% respectively. The main reasons to stop exclusive breastfeeding between two to four months was concerns regarding weight gain and between four to six months were mothers starting to work. Majority of the babies that were not exclusively breastfed still continued to have breast milk. Mothers above 30 years had lower exclusive breastfeeding rates compared to younger mothers. Second born babies had higher rates than first borns. There was no significant association between maternal education and exclusive breastfeeding rates.

Conclusions

Exclusive breastfeeding rates were high among this cohort of children. A decrease in EBF was noted between two and four months. EBF up to six months does not cause growth failure. Mothers starting to work and concerns regarding adequacy of breast milk were the major reasons to cease EBF. The actual exclusive breastfeeding rates up to six months was 65.9%.

This study was undertaken to assess the effects of exogenous alpha-agonists on the effector response to transmural nerve stimulation in canine saphenous vein rings. The response to a fixed train (5 s duration) of transmural nerve stimulation (8 Hz, 0.3 ms, 9 V) applied every 5 min was determined in the control state and in the presence of subthreshold (for contraction) concentrations of noradrenaline, adrenaline, clonidine, and methoxamine. The maximum potentiations achieved by the three drugs were 246.2 +/- 36.9, 220.5 +/- 38.8, 384.3 +/- 78.7, and 353.3 +/- 68.0%, respectively. The potentiation observed was significantly inhibited by indomethacin (10(-6) mol/L) and propranolol (5 X 10(-6) mol/L). Both indomethacin and propranolol potentiated the response to transmural nerve stimulation. The potentiation of the responses to transmural nerve stimulation by alpha-agonists suggests that, presynaptic alpha 2-inhibition by circulating catecholamines is likely to be of limited biological significance in modulating the effector responses in the canine saphenous vein.  相似文献

According to Ayurvedic literature of Sri Lanka, roasted seeds of Artocarpus heterophyllus Lam. (Family: Moraceae) has aphrodisiac activity. However, some reproductively active young men in rural areas of Sri Lanka claim that consumption of these seeds few hours prior to coitus disrupts sexual function. Because of these two conflicting claims, it was thought useful to scientifically investigate the effects of A. heterophyllus seeds on male sexual function and fertility. This was done using a seed suspension in 1% methylcellulose (SS) in rats. In a sexual behaviour study using receptive female rats, an oral administration of 500 mg/kg dose of SS markedly inhibited libido, sexual arousal, sexual vigour and sexual performance within 2 hr. Further, the treatment induced a mild erectile dysfunction. These antimasculine effects on sexual function was not evident 6 hr post treatment indicating rapid onset and offset of action. Further, these actions on the sexual behaviour was not due to general toxicity, liver toxicity, stress or reduction in blood testosterone level but due to marked sedative activity. In a mating study, SS failed to alter ejaculating competence and fertility. These results suggest that A. heterophyllous seeds do not have aphrodisiac action, at least, in rats.  相似文献

This study investigated the impacts of climate variability on thermotolerant coliform bacteria (TCB) transport in the Upper Pearl River watershed (UPRW) in Mississippi. The Soil and Water Assessment Tool (SWAT) was applied using daily observed stream flows and TCB concentration data. The SWAT model was successfully calibrated and validated using both manual and automatic methods from February 2011 to June 2012 (NSE and R2 up to 0.79). The Long Ashton Research Station Weather Generator (LARS-WG), a stochastic weather generator, with the global climate model, CCSM3, which was developed by the U.S. National Center for Atmospheric Research (NCAR) was used for future climate variability simulations. The Special Report on Emissions Scenarios (SRES) A1B of the Intergovernmental Panel on Climate Change (IPCC) was simulated for the mid (2046–2065) and late (2080–2099) 21st century. The SWAT model simulated TCB concentrations in surface water and demonstrated reasonable performances (R2 up to 0.59 and NSE up to 0.58). During mid-century climate, average monthly TCB levels increase to 175%, while late-century average monthly TCB levels increase to 297% from the watershed. Although late-century climate variability impacts were determined more critical than mid-century climate impacts, appropriate watershed management practices are required to adapt to maintain and improve water quality.  相似文献

Hot-compressed water treatment of lignocellulose liberates numerous inhibitors that prevent ethanol fermentation of yeast Saccharomyces cerevisiae. Glycolaldehyde is one of the strongest fermentation inhibitors and we developed a tolerant strain by overexpressing ADH1 encoding an NADH-dependent reductase; however, its recovery was partial. In this study, to overcome this technical barrier, redox cofactor preference of glycolaldehyde detoxification was investigated. Glycolaldehyde-reducing activity of the ADH1-overexpressing strain was NADH-dependent but not NADPH-dependent. Moreover, genes encoding components of the pentose phosphate pathway, which generates intracellular NADPH, was upregulated in response to high concentrations of glycolaldehyde. Mutants defective in pentose phosphate pathways were sensitive to glycolaldehyde. Genome-wide survey identified GRE2 encoding a NADPH-dependent reductase as the gene that confers tolerance to glycolaldehyde. Overexpression of GRE2 in addition to ADH1 further improved the tolerance to glycolaldehyde. NADPH-dependent glycolaldehyde conversion to ethylene glycol and NADP+ content of the strain overexpressing both ADH1 and GRE2 were increased at 5 mM glycolaldehyde. Expression of GRE2 was increased in response to glycolaldehyde. Carbon metabolism of the strain was rerouted from glycerol to ethanol. Thus, it was concluded that the overexpression of GRE2 together with ADH1 restores glycolaldehyde tolerance by augmenting the NADPH-dependent reduction pathway in addition to NADH-dependent reduction pathway. The redox cofactor control for detoxification of glycolaldehyde proposed in this study could influence strategies for improving the tolerance of other fermentation inhibitors.  相似文献

The synthesis and (or) release of endothelium-dependent relaxant factor released by acetylcholine is impaired in New Zealand white rabbits fed an atherogenic diet. Experiments were designed to investigate whether the synthesis and (or) release of the endothelium-dependent relaxant factor from rabbit aortas are restored after reversal from an atherogenic diet to a non-atherogenic diet. Atherosclerosis was induced by feeding a diet containing lipids and 2% cholesterol for 6 weeks. Rabbits were sacrificed after 6 weeks on the atherogenic diet and 36 weeks after return to a standard laboratory diet. Synthesis and (or) release of the factor from the thoracic aorta was assayed using a bioassay system. The relaxant responses produced in the assay tissue were impaired both in the acute stage and after 36 weeks on non-atherogenic food. This impaired relaxation is probably due to a persistent functional abnormality in the aortic endothelium resulting in the failure to synthesize and (or) release endothelium-dependent relaxation factor 36 weeks after induction of atherosclerosis.  相似文献
