Two hundred and sixteen consecutive patients seen for the first time with symptomatic haemorrhoids entered trials of conservative and minor surgical treatment. They were divided into two groups according to their pretreatment maximal anal pressure. Patients with pressures of 100 cm H2O (73.7 mm Hg) or more (108 patients) were treated by anal dilatation (37), sphincterotomy (34), or high-fibre diet (37). Four and 12 months later anal dilatation had produced significantly better results than sphincterotomy or diet. Furthermore, anal dilatation was the only treatment associated with a significant reduction in anal pressure at four and 12 months. Patients with pressures under 100 cm H2O (108 patients) were treated by rubber-band ligation (35), cryosurgery (36), or diet (37). Four and 12 months later significantly more patients were improved by rubber-band ligation than by cryosurgery or diet. These results suggest that haemorrhoids in patients with excessive activity of the internal anal sphincter are best treated by anal dilatation and that in all other patients rubber-band ligation is the treatment of choice. 相似文献

Eight chalcones and benzalacetones were tested for their virulence induction on Agrobacterium tumefaciens. With one exception, they had a strong action, and in particular 4-(3,5-dimethoxy-4-hydroxy-phenyl)-but-3-en-2-one (10a) was very effective with a virulence induction about 1.5–2 times that of acetosyringone (AS). When applied to Agrobacterium-mediated gene transfer of onion, both 10a and AS at 250 μM, led to increased gene transfer of between 25 and 35% when compared with the control. Many derivatives of AS were tested and the indispensable chemical functions required to induce vir genes were determined; for example, the presence of at least one methoxy group and a carbonyl group as in acetyl, aldehyde and acid functions. The most effective vir-inducing compound used was the original AS with two methoxy groups and an acetyl function. By testing the corresponding β-glucosides and glucosyl esters (from acids), we established that a phenolic function was also essential for virulence induction. These glucosides led to a decreased toxicity to the bacteria in relation to the original product. However, the presence of a β-glucosylated phenol function led to the total loss of vir induction while the corresponding esters, particularly the glucosyl sysingate ester, conserved a good vir induction. 相似文献

Currently, azoospermia is one of the most common diseases of male infertility. Stem cell research is the new hope for novel therapy with a higher degree of safety and lower cost. This study aimed to investigate the effect of umbilical cord blood-derived stem cells (” and mesenchymal “UCB-MSCs”) and mono-cell layer implanted into the induced azoospermic mice testis. Stem cells were isolated from umbilical cord blood and CD34+ve cells were separated from negative one by Mini MACs column. At 5th week after single injection of busulfan, stained mesenchymal (CD34-ve), hematopoietic stem cells (CD34+ve) and their conjugate (mono-cell layer) were injected locally into testis. At the end of the study, MSCs group showed that mRNA levels of genes related to meiosis (Vasa, SCP3, and PgK2) were increased with significant decrease of FSH and LH levels, compared to control group. Histologically, most of the tubules restored normal architecture. In contrast, HSCs and mono-cell layer groups showed statically insignificant change of FSH, LH, and gene expression, compared to control group. Histologically, distorted seminiferous tubules, with reduction in sperm content, and interstitial mononuclear cellular infiltration were seen. There was significant increase in the optical density of PCNA immune reaction in MSCs group than azoospermia, HSCs, and mono-cell layer, while there was non-significant difference between MSCs and control group. The present study suggested that injection of MSCs into chemotherapeutic-induced azoospermia in mice improved testicular failure; histologically and functionally, by restoration of spermatogenic gene expression while HSC and mono-cell layer showed no effect on spermatogenesis added to that mono-cell layer may induce testicular tissue damage. 相似文献
