Background

Deciduous Molar Hypomineralisation (DMH) and Molar Incisor Hypomineralisation (MIH) are common developmental disturbances in pediatric dentistry. Their occurrence is related. The same determinants as suggested for MIH are expected for DMH, though somewhat earlier in life. Perinatal medical problems may influence the prevalence of DMH but this has not been studied sufficiently.

Objective

This study aimed to identify possible determinants of DMH in a prospective cohort study among 6-year-old children.

Study Design

This study was embedded in the Generation R Study, a population-based prospective cohort study from fetal life until young adulthood. The the data were used to identify the determinants of DMH. Clinical photographs of clean, moist teeth were taken with an intra-oral camera in 6690 children (mean age 6.2 years; 49.9% girls). Data on possible determinants that had occurred during pregnancy and/or the child''s first year of life were on the basis of manual standardized measurements (like length and weight) and questionnaires. Multivariate analyse with backward and forward selection was performed.

Results

A number of factors in the pre-, peri- and postnatal phase were found to be associated with DMH. After multivariate logistic regression analyses, Dutch ethnic background, low birth weight, maternal alcohol consumption during pregnancy, and fever episodes in the first year of the child''s life were found to play a role in the development of DMH in 6-year-old children.

Conclusion

This study shows that Dutch ethnicity, low birth weight, alcohol consumption by the mother during pregnancy and any fever in the first year of the child''s life are associated with DMH. Not only childhood factors but also prenatal lifestyle factors need to be taken into account when studying determinants for DMH.  相似文献

PTP-BL is a large phosphatase that is implicated in cellular processes as diverse as cytokinesis, actin-cytoskeletal rearrangement, and apoptosis. Five PDZ domains mediate its cellular role by binding to the C termini of target proteins, forming multiprotein complexes. The second PDZ domain (PDZ2) binds to the C termini of the tumor suppressor protein APC and the LIM domain-containing protein RIL; however, in one splice variant, PDZ2as, a 5 residue insertion abrogates this binding. The insert causes distinct structural and dynamical changes in the alternatively spliced PDZ2: enlarging the L1 loop between beta2 and beta3, both lengthening and changing the orientation of the alpha2 helix, giving the base of the binding pocket less flexibility to accommodate ligands, and destabilizing the entire domain. These changes render the binding pocket incapable of binding C termini, possibly having implications in the functional role of PTP-BL.  相似文献

Inner hair cells (IHCs), the primary sensory receptors of the mammalian cochlea, fire spontaneous Ca2+ action potentials before the onset of hearing. Although this firing activity is mainly sustained by a depolarizing L-type (CaV1.3) Ca2+ current (I Ca), IHCs also transiently express a large Na+ current (I Na). We aimed to investigate the specific contribution of I Na to the action potentials, the nature of the channels carrying the current and whether the biophysical properties of I Na differ between low- and high-frequency IHCs. We show that I Na is highly temperature-dependent and activates at around −60 mV, close to the action potential threshold. Its size was larger in apical than in basal IHCs and between 5% and 20% should be available at around the resting membrane potential (−55 mV/−60 mV). However, in vivo the availability of I Na could potentially increase to >60% during inhibitory postsynaptic potential activity, which transiently hyperpolarize IHCs down to as far as −70 mV. When IHCs were held at −60 mV and I Na elicited using a simulated action potential as a voltage command, we found that I Na contributed to the subthreshold depolarization and upstroke of an action potential. We also found that I Na is likely to be carried by the TTX-sensitive channel subunits NaV1.1 and NaV1.6 in both apical and basal IHCs. The results provide insight into how the biophysical properties of I Na in mammalian cochlear IHCs could contribute to the spontaneous physiological activity during cochlear maturation in vivo.  相似文献

Context

Publication bias jeopardizes evidence-based medicine, mainly through biased literature syntheses. Publication bias may also affect laboratory animal research, but evidence is scarce.

Objectives

To assess the opinion of laboratory animal researchers on the magnitude, drivers, consequences and potential solutions for publication bias. And to explore the impact of size of the animals used, seniority of the respondent, working in a for-profit organization and type of research (fundamental, pre-clinical, or both) on those opinions.

Design

Internet-based survey.

Setting

All animal laboratories in The Netherlands.

Participants

Laboratory animal researchers.

Main Outcome Measure(s)

Median (interquartile ranges) strengths of beliefs on 5 and 10-point scales (1: totally unimportant to 5 or 10: extremely important).

Results

Overall, 454 researchers participated. They considered publication bias a problem in animal research (7 (5 to 8)) and thought that about 50% (32–70) of animal experiments are published. Employees (n = 21) of for-profit organizations estimated that 10% (5 to 50) are published. Lack of statistical significance (4 (4 to 5)), technical problems (4 (3 to 4)), supervisors (4 (3 to 5)) and peer reviewers (4 (3 to 5)) were considered important reasons for non-publication (all on 5-point scales). Respondents thought that mandatory publication of study protocols and results, or the reasons why no results were obtained, may increase scientific progress but expected increased bureaucracy. These opinions did not depend on size of the animal used, seniority of the respondent or type of research.

Conclusions

Non-publication of “negative” results appears to be prevalent in laboratory animal research. If statistical significance is indeed a main driver of publication, the collective literature on animal experimentation will be biased. This will impede the performance of valid literature syntheses. Effective, yet efficient systems should be explored to counteract selective reporting of laboratory animal research.  相似文献

Low-temperature environments interfere with plant reproduction by reducing the frequency of pollinators, and this may favour reproductive strategies such as self-pollination and apomixis. Tibouchina pulchra is a common tree species that occurs at high and low sites of the Brazilian Atlantic rainforest. This study focussed on the pollination biology and breeding system of this species, describing the pollinators and the reproductive success at the two sites of an elevational gradient. Observations were made to determine extent of flowering and fruiting, to identify the richness and abundance of pollinators, and to record data on the floral and reproductive biology at these two sites. Despite more dense flowering at the high site, five visits of bees (two species) were recorded during the observation time (60?h), whereas at the low site there were 948 visits (seven species) during the same period. In contrast with the low site, the flowers of the high site released and received few pollen grains on the stigma. At the high site less fruit was set with fewer seeds as a result of open pollination than at the low site; at that site, however, more seeds were obtained from cross-pollination than at the low site. Tibouchina pulchra is self-compatible; however it is not apomictic and needs pollinators for seed set at both sites. Life-history traits other than the breeding system, for example more dense flowering, advantage of greater fertility in cross-pollination, and multiple reproductive events during the lifetime of the tree may reduce inbreeding depression, increase the hybrid vigour, and balance the lack of pollinators at the high site.  相似文献

Background

Governments and donors all over Africa are searching for sustainable, affordable and cost-effective ways to improve the quality of malaria case management. Widespread deficiencies have been reported in the prescribing and counselling practices of health care providers treating febrile patients in both public and private health facilities. Cameroon is no exception with low levels of adherence to national guidelines, the frequent selection of non-recommended antimalarials and the use of incorrect dosages. This study evaluates the effectiveness and cost-effectiveness of introducing two different provider training packages, alongside rapid diagnostic tests (RDTs), designed to equip providers with the knowledge and practical skills needed to effectively diagnose and treat febrile patients. The overall aim is to target antimalarial treatment better and to facilitate optimal use of malaria treatment guidelines.

Methods/Design

A 3-arm stratified, cluster randomized trial will be conducted to assess whether introducing RDTs with provider training (basic or enhanced) is more cost-effective than current practice without RDTs, and whether there is a difference in the cost effectiveness of the provider training interventions. The primary outcome is the proportion of patients attending facilities that report a fever or suspected malaria and receive treatment according to malaria guidelines. This will be measured by surveying patients (or caregivers) as they exit public and mission health facilities. Cost-effectiveness will be presented in terms of the primary outcome and a range of secondary outcomes, including changes in provider knowledge. Costs will be estimated from a societal and provider perspective using standard economic evaluation methodologies.

Background, aim and scope  Different ways of performing a life cycle assessment (LCA) are used to assess the environmental burden of milk production. A strong connection exists between the choice between attributional LCA (ALCA) and consequential LCA (CLCA) and the choice of how to handle co-products. Insight is needed in the effect of choice on results of environmental analyses of agricultural products, such as milk. The main goal of this study was to demonstrate and compare ALCA and CLCA of an average conventional milk production system in The Netherlands. Materials and methods  ALCA describes the pollution and resource flows within a chosen system attributed to the delivery of a specified amount of the functional unit. CLCA estimates how pollution and resource flows within a system change in response to a change in output of the functional unit. For an average Dutch conventional milk production system, an ALCA (mass and economic allocation) and a CLCA (system expansion) were performed. Impact categories included in the analyses were: land use, energy use, climate change, acidification and eutrophication. The comparison was based on four criteria: hotspot identification, comprehensibility, quality and availability of data. Results  Total environmental burdens were lower when using CLCA compared with ALCA. Major hotspots for the different impact categories when using CLCA and ALCA were similar, but other hotspots differed in contributions, order and type. As experienced by the authors, ALCA and use of co-product allocation are difficult to comprehend for a consequential practitioner, while CLCA and system expansion are difficult to comprehend for an attributional practitioner. Literature shows concentrates used within ALCA will be more understandable for a feeding expert than the feed used within CLCA. Outcomes of CLCA are more sensitive to uncertainties compared with ALCA, due to the inclusion of market prospects. The amount of data required within CLCA is similar compared with ALCA. Discussion  The main cause of these differences between ALCA and CLCA is the fact that different systems are modelled. The goal of the study or the research question to be answered defines the system under study. In general, the goal of CLCA is to assess environmental consequences of a change in demand, whereas the goal of ALCA is to assess the environmental burden of a product, assuming a status-quo situation. Nowadays, however, most LCA practitioners chose one methodology independent of their research question. Conclusions  This study showed it is possible to perform both ALCA (mass and economic allocation) and CLCA (system expansion) of milk. Choices of methodology, however, resulted in differences in: total quantitative outcomes, hotspots, degree of understanding and quality. Recommendations and perspectives  We recommend LCA practitioners to better distinguish between ALCA and CLCA in applied studies to reach a higher degree of transparency. Furthermore, we recommend LCA practitioners of different research areas to perform similar case studies to address differences between ALCA and CLCA of the specific products as the outcomes might differ from our study.  相似文献

Background

This practice based explorative study aims to provide insight into the ways in which case managers shape and fill up the evaluation phase of their support of the informal care network of persons with dementia.

Method

A combination of quantitative and qualitative research methods were used. A group of 57 case managers of persons with dementia in three different organisational networks took part in this study.

Results

Results from the quantitative and qualitative data are organized into four themes: (1) attitude towards evaluation, (2) forms of evaluation, (3) implementation of evaluation and (4) content of evaluation. There are different ways in shaping evaluation and the content of it. The importance of interim and final evaluation is recognized, but is difficult to realize in a methodical way. Barriers experienced by the case managers include various factors associated both with clients as professionals.

Conclusion

Case managers evaluate continuously and in an informal way to assess whether the extent of their assistance is meeting the needs of the client and informal network. Case managers do not use systematic evaluation to measure the quality of care they offer to persons with dementia and their caregivers. The findings demand a discussion on the level of clients, as well as on the professional and societal level about the way case managers should evaluate their support.

In this work, we explore the ability of several characterization approaches for phenotyping to extract information about plant cell wall properties in diverse maize genotypes with the goal of identifying approaches that could be used to predict the plant’s response to deconstruction in a biomass-to-biofuel process. Specifically, a maize diversity panel was subjected to two high-throughput biomass characterization approaches, pyrolysis molecular beam mass spectrometry (py-MBMS) and near-infrared (NIR) spectroscopy, and chemometric models to predict a number of plant cell wall properties as well as enzymatic hydrolysis yields of glucose following either no pretreatment or with mild alkaline pretreatment. These were compared to multiple linear regression (MLR) models developed from quantified properties. We were able to demonstrate that direct correlations to specific mass spectrometry ions from pyrolysis as well as characteristic regions of the second derivative of the NIR spectrum regions were comparable in their predictive capability to partial least squares (PLS) models for p-coumarate content, while the direct correlation to the spectral data was superior to the PLS for Klason lignin content and guaiacyl monomer release by thioacidolysis as assessed by cross-validation. The PLS models for prediction of hydrolysis yields using either py-MBMS or NIR spectra were superior to MLR models based on quantified properties for unpretreated biomass. However, the PLS models using the two high-throughput characterization approaches could not predict hydrolysis following alkaline pretreatment while MLR models based on quantified properties could. This is likely a consequence of quantified properties including some assessments of pretreated biomass, while the py-MBMS and NIR only utilized untreated biomass.  相似文献
