Oligodeoxyribonucleotides (ONs) containing two incorporations of 2 ′-N-(pyren-1-yl)acetyl-2 ′-amino-α-L-LNA monomer Y are sensitive probes for detection of single nucleotide polymorphisms (SNP) in DNA. ObjectiveThis study was conducted to assess the incidence of sudden cardiac death (SCD) in post myocardial infarction patients and to determine the predictive value of various risk markers in identifying cardiac mortality and SCD.MethodsLeft ventricular function, arrhythmias on Holter and microvolt T wave alternans (MTWA) were assessed in patients with prior myocardial infarction and ejection fraction ≤ 40%. The primary outcome was a composite of cardiac death and resuscitated cardiac arrest during follow up. Secondary outcomes included total mortality and SCD.ResultsFifty-eight patients were included in the study. Eight patients (15.5%) died during a mean follow-up of 22.3 ± 6.6 months. Seven of them (12.1%) had SCD. Among the various risk markers studied, left ventricular ejection fraction (LVEF) ≤ 30% (Hazard ratio 5.6, 95% CI 1.39 to 23) and non-sustained ventricular tachycardia (NSVT) in holter (5.7, 95% CI 1.14 to 29) were significantly associated with the primary outcome in multivariate analysis. Other measures, including QRS width, heart rate variability, heart rate turbulence and MTWA showed no association.ConclusionsAmong patients with prior myocardial infarction and reduced left ventricular function, the rate of cardiac death was substantial, with most of these being sudden cardiac death. Both LVEF ≤30% and NSVT were associated with cardiac death whereas only LVEF predicted SCD. Other parameters did not appear useful for prediction of events in these patients. These findings have implications for decision making for the use of implantable cardioverter defibrillators for primary prevention in these patients.  相似文献

Ulcerative colitis and Crohn's disease are the two major forms of inflammatory bowel disease (IBD). A series of reports have hypothesized interplay of genetic and environmental factors in the pathogenesis of IBD. Polymorphism in the mannan-binding lectin-2 (MBL-2) gene is known to affect the structural assembly and function thereby predisposing subjects to various diseases. The present study was designed to evaluate effect of MBL-2 gene polymorphism on MBL levels and function in IBD patients. Genomic DNA was isolated from blood samples collected from 157 ulcerative colitis, 42 Crohn's disease and 204 control subjects. Genotyping for different polymorphic sites at exon1 of MBL-2 gene was performed by refractory mutation system-PCR and amplification followed by restriction digestion (PCR-RFLP). Serum MBL concentration and C4 deposition levels were estimated using ELISA. Mannan-binding lectin-2 genotypic variants were calculated in IBD and healthy controls. DevS and DosT from Mycobacterium tuberculosis (MTB) are paralogous heme-based sensor kinases that respond to hypoxia and to low concentrations of nitric oxide (NO). Both proteins work with the response regulator DevR as a two-component regulatory system to induce the dormancy regulon in MTB. While DevS and DosT are inactive when dioxygen is bound to the heme Fe(II) at their sensor domain, autokinase activity is observed in their heme Fe(II)-NO counterparts. To date, the conversion between active and inactive states and the reactivity of the heme-oxy complex toward NO have not been investigated. Here, we use stopped-flow UV-vis spectroscopy and rapid freeze quench resonance Raman spectroscopy to probe these reactions in DevS. Our data reveal that the heme-O(2) complex of DevS reacts efficiently with NO to produce nitrate and the oxidized Fe(III) heme through an NO dioxygenation reaction that parallels the catalytic reactions of bacterial flavohemoglobin and truncated hemoglobins. Autophosphorylation activity assays show that the Fe(III) heme state of DevS remains inactive but exhibits a high affinity for NO and forms an Fe(III)-NO complex that is readily reduced by ascorbate, a mild reducing agent. On the basis of these results, we conclude that upon exposure to low NO concentrations, the inactive oxy-heme complex of DevS is rapidly converted to the Fe(II)-NO complex in the reducing environment of living cells and triggers the initiation of dormancy.  相似文献

It is of interest to evaluate the prevalence of systemic disorders in patients undergoing minor oral surgeries at a dental hospital. This will help to take necessary precautions prior to oral surgeries. We used the digital case records of 1288 patients who underwent minor oral surgeries in a hospital. Demographic details and systemic diseases of the patients were recorded from digital case records. Data shows that 103 patients (7.9%) of the total number of patients undergoing minor oral surgeries had systemic diseases with 3.8% of patients diagnosed with diabetes. Statistically significant associations were found between type of minor oral surgery and the type of systemic disease (p<0.001); age of patients and type of minor oral surgery (p<0.001); age and type of systemic diseases (p<0.001) and gender of patient and type of minor oral surgery (p = 0.005). Thus, data shows the prevalence of systemic diseases in patients undergoing minor oral surgeries was 7.9%.  相似文献

We report the synthesis of a series of novel structurally related protic chiral ionic liquids (PCILs) derived from ephedrines. Enantiopure norephedrine, ephedrine, and methylephedrine were neutralized by use of fluorinated acids, bis(trifluoromethanesulfonyl)imide, and bis(pentafluoroethanesulfonyl)imide to afford six PCILs with protonated primary, secondary, and tertiary amines. The goal of this study is to investigate the influence of structure on both chiral recognition abilities and physicochemical properties of these closely related PCILs. The newly synthesized PCILs were characterized by use of nuclear magnetic resonance (NMR), thermal gravimetric analysis, differential scanning calorimetry, circular dichroism (CD), mass spectrometry, and elemental analysis. The PCILs were thermally stable up to 220°C and had glass transition temperatures between -60 and -30°C. Both enantiomers of the PCILs retained chirality throughout the synthesis as demonstrated by use of CD measurements. More interestingly, these ephedrinium PCILs displayed strong chiral recognition capabilities as evidenced by peak splitting of the chemical shift of the trifluoro group of potassium Mosher's salt by use of (19)F-NMR. In addition, these PCILs demonstrated enantiomeric recognition capabilities toward a range of structurally diverse analytes using steady-state fluorescence spectroscopy.  相似文献

A series of novel aryl and thiophenyl tethered dihydro-6H-quinolin-5-ones have been synthesized in very good yields through CeCl3·7H2O-NaI catalyzed one-pot condensation of β-enaminones derived from the respective methyl ketones; 1,3-cyclohexanedione & 5,5-dimethyl-1,3-cyclohexanedione and ammonium acetate refluxing in 2-propanol. Dihydro-6H-quinolin-5-ones 3a-f was further derivatized to the respective hydroxymethyl analogs using proline as an organocatalyst in aqueous media. Among the all 18 compounds screened for in vitro antimycobacterial activity against Mycobacterium tuberculosis H37Rv (MTB), dihydro-6H-quinolin-5-ones 4e and 4f were found to be most active with MIC 3.13 μg/mL.  相似文献

Electrospinning was used to prepare the nanofibrous membrane (NFM) of the composite comprising poly(vinylidene fluoride) and poly(aminophenylboronic acid) (PVdF/PAPBA-NFM). The PVdF/PAPBA-NFM displayed an excellent linear response to the detection of glucose for the concentration range of 1 to 15mM with a response time of less than 6s. Further experiments on amperometric sensing of glucose were performed in the presence of interferents such as uric acid, ascorbic acid, acetaminophen, fructose, mannose, etc. using PVdF/PAPBA-NFM. The interferents did not give significant overlapping current signal during the determination of glucose. Also, PVdF/PAPBA-NFM possesses better reproducibility toward glucose detection and storage stability.  相似文献
