In a review of the operative treatment of 1,271 patients with pulmonary tuberculosis in an 11-year period, it was noted that, beginning with 1947, there was a great increase, relatively, in the number of cases in which pulmonary resection was carried out. In 1943, the first year of the period of study, there were six minor operative procedures to every four major operations; in 1953 the ratio was one minor to nine major. This reversal reflects the discoveries of antibiotics for conservative therapy on the one hand and the advances in surgical techniques for major operative treatment on the other. Now that it is safer, resection will probably be used more and more-including bilateral resection in "salvage" cases. On the other hand, with specific antibiotics available, there is a tendency at present to treat conservatively for longer periods in cases in which, formerly, minor operative procedures would have been carried out early. 相似文献

Isolates of Fusarium species obtained from freshly harvested bean grains for human consumption collected from different Argentinean
regions, were investigated for their ability to biosynthesise trichothecenes and zearalenone either on rice grains or beans.
Low incidence of toxigenic fungi was observed. These mycotoxigenic species produced several toxins when grown on rice but
none or little amount when cultured on beans. The results of this report suggest that contamination of Argentinean beans with

The aim of this research was to determine whether all memory T cells have the same propensity to migrate to the joint in patients
with juvenile idiopathic arthritis. Paired synovial fluid and peripheral blood mononuclear cell proliferative responses to
a panel of antigens were measured and the results correlated with a detailed set of laboratory and clinical data from 39 patients
with juvenile idiopathic arthritis. Two distinct patterns of proliferative response were found in the majority of patients:
a diverse pattern, in which synovial fluid responses were greater than peripheral blood responses for all antigens tested;
and a restricted pattern, in which peripheral blood responses to some antigens were more vigorous than those in the synovial
fluid compartment. The diverse pattern was generally found in patients with a high acute phase response, whereas patients
without elevated acute phase proteins were more likely to demonstrate a restricted pattern. We propose that an association
between the synovial fluid T cell repertoire and the acute phase response suggests that proinflammatory cytokines may influence
recruitment of memory T cells to an inflammatory site, independent of their antigen specificity. Additionally, increased responses
to enteric bacteria and the presence of αEβ7 T cells in synovial fluid may reflect accumulation of gut associated T cells
in the synovial compartment, even in the absence of an elevated acute phase response. This is the first report of an association

This study examined the transcultural robustness of a screening instrument
for hypomania, the Hypomania Checklist-32, first revised version (HCL-32 R1).
It was carried out in 2606 patients from twelve countries in five geographic
regions (Northern, Southern and Eastern Europe, South America and East Asia).
In addition, GAMIAN Europe contributed data from its members. Exploratory
and confirmatory factor analyses were used to examine the transregional stability
of the measurement properties of the HCL-32 R1, including the influence of
sex and age as covariates. Across cultures, a two-factor structure was confirmed:
the first factor (F1) reflected the more positive aspects of hypomania (being
more active, elated, self-confident, and cogni-tively enhanced); the second
factor (F2) reflected the more negative aspects (being irritable, impulsive,
careless, more substance use). The measurement properties of the HCL-32 R1
were largely invariant across cultures. Only few items showed transcultural
differences in their relation to hypomania as measured by the test. F2 was
higher among men and in more severe manic syndromes; F1 was highest in North
and East Europe and lowest in South America. The scores decreased slightly
with age. The frequency of the 32 items showed remarkable similarities across
geographic areas, with two excep-tions: South Europeans had lower symptom
frequencies in general and East Europeans higher rates of substance use. These
findings support the interna-tional applicability of the HCL-32 R1 as a screening

Abstract: Common reed (Phragmites australis) forms dense stands with deep layers of residual organic matter that negatively affects plant diversity and possibly habitat use by wetland birds. We sought to determine whether seasonal relative abundance and species richness of birds varied among 3 habitat types in Great Lakes coastal wetland complexes recently invaded by common reed. We used fixed-distance point counts to determine species relative abundances and species richness in edge and interior locales within common reed, cattail (Typha spp.), and meadow marsh habitats of various sizes during 2 summers (2001 and 2002) and 1 autumn (2001) at Long Point, Lake Erie, Ontario, Canada. We found that total relative abundance and species richness of birds were greater in common reed habitat compared to cattail or meadow marsh habitats. However, we also found that relative abundance of marsh-nesting birds was greater in meadow marsh habitat than in cattail and common reed during summer. Lastly, we found that, irrespective of habitat type, habitat edges had higher total relative abundance and species richness of birds than did habitat interiors. Our results show that common reed provides suitable habitat for a diversity of landbirds during summer and autumn but only limited habitat for many marsh-nesting birds during summer. Based on these results, we recommend restoration of meadow marsh habitat through reduction of common reed in Great Lakes wetlands where providing habitat for breeding marsh-nesting birds is an objective. The sea acts as an effective dispersal barrier for most terrestrial animal species. Narrow sea straits, therefore, often represent areas where species are able to disperse from one land mass to another. In the Mediterranean Sea, the narrowest connecting points between North Africa and Europe are the Strait of Gibraltar and the Strait of Sicily. In the past, climatic oscillations caused changing sea levels and thus influenced the permeability of these sea straits. We analysed the genetic structure of four butterfly species that all occur on both sides of the Strait of Sicily. In all four species, we observed a lack of genetic differentiation between the populations of North Africa and those of Italy. Species distribution models support the strong cohesiveness in that they show a largely continuous glacial distribution over Italy and North Africa. The data obtained reveal that there was a large exchange of individuals between Italy and the eastern Maghreb during the last ice age. 1. Rates of whole‐system metabolism (production and respiration) are fundamental indicators of ecosystem structure and function. Although first‐order, proximal controls are well understood, assessments of the interactions between proximal controls and distal controls, such as land use and geographic region, are lacking. Thus, the influence of land use on stream metabolism across geographic regions is unknown. Further, there is limited understanding of how land use may alter variability in ecosystem metabolism across regions. 2. Stream metabolism was measured in nine streams in each of eight regions (n = 72) across the United States and Puerto Rico. In each region, three streams were selected from a range of three land uses: agriculturally influenced, urban‐influenced, and reference streams. Stream metabolism was estimated from diel changes in dissolved oxygen concentrations in each stream reach with correction for reaeration and groundwater input. 3. Gross primary production (GPP) was highest in regions with little riparian vegetation (sagebrush steppe in Wyoming, desert shrub in Arizona/New Mexico) and lowest in forested regions (North Carolina, Oregon). In contrast, ecosystem respiration (ER) varied both within and among regions. Reference streams had significantly lower rates of GPP than urban or agriculturally influenced streams. 4. GPP was positively correlated with photosynthetically active radiation and autotrophic biomass. Multiple regression models compared using Akaike’s information criterion (AIC) indicated GPP increased with water column ammonium and the fraction of the catchment in urban and reference land‐use categories. Multiple regression models also identified velocity, temperature, nitrate, ammonium, dissolved organic carbon, GPP, coarse benthic organic matter, fine benthic organic matter and the fraction of all land‐use categories in the catchment as regulators of ER. 5. Structural equation modelling indicated significant distal as well as proximal control pathways including a direct effect of land‐use on GPP as well as SRP, DIN, and PAR effects on GPP; GPP effects on autotrophic biomass, organic matter, and ER; and organic matter effects on ER. 6. Overall, consideration of the data separated by land‐use categories showed reduced inter‐regional variability in rates of metabolism, indicating that the influence of agricultural and urban land use can obscure regional differences in stream metabolism. 相似文献
