Small lesions of the thyroid gland--the significance of ultrasound examination in the selection of lesions for biopsy

INTRODUCTION: The aim of this study was to find the ultrasound (US) features which can be helpful in selecting small thyroid lesions for fine needle aspiration biopsy (FNAB). MATERIALS AND METHODS: The outcomes were analyzed of US examinations, FNABs, and postoperative histopathological examinations performed during the years 2000-2005. The occurrence of small lesions of the thyroid revealed by US was assessed. Then, palpability and several US features of those lesions were evaluated, like the total echogenicity, the pattern of vascularisation, the presence of microcalcifications and coexistence with other lesions in the goitre. Those features were correlated with the results of histopathological examinations. Altogether 356 lesions found in 284 patients were analyzed, of which 315 were non-neoplastic lesions, 12 - follicular neoplasms, and 29 - thyroid carcinomas. RESULTS: The small thyroid lesions were revealed in 53.8% patients examined by US. Anorexia nervosa

Anorexia nervosa is an eating disorder characterized by conscious restriction of food intake, which causes numerous metabolic and hormonal disorders. Knowledge of these changes is important due to growing morbidity and mortality of anorexia. Treatment is difficult and requires cooperation of a group of specialists, including an endocrinologist. The authors presented a clinical picture, view of etiopathogenesis and typical disorders found in patients with this illness. Furthermore, treatment methods were also discussed.     

Consensus statement of the Polish Society for Endocrinology: presurgical somatostatin analogs therapy in acromegaly

Consensus statement of the Polish Society for Endocrinology, regarding presurgical somatostatin analogs in acromegaly has been presented. It is suggested to administer depot somatostatin analog (Octreotide LAR at the dose 20 mg and then 30 mg or equivalent doses of Lanreotide Autogel 90/120 mg every 4 weeks) in order to normalize or suppress to a maximal extent GH and IGF-1 concentrations. The period of therapy in case of microadenoma would be at least 3 months (targets: biochemical improvement, reduced risk of disease's complications, perioperative risk reduction, inhibition of tumor growth). The period of therapy in case of macroadenoma would be at least 6 months, until maximal possible reduction of GH and IGF-1 concentrations (targets: tumor shrinkage, biochemical improvement, reduced risk of disease's complications, perioperative risk reduction). Using an uniform approach in a group, as numerous as possible, of treated patients would allow objective evaluation of long-term efficacy of the treatment.     

Mathematical Model of Pulsed Immunotherapy for Superficial Bladder Cancer

We present a theoretical study of superficial bladder cancer growth and its treatment via pulsed immunotherapy with Bacillus Calmette–Guérin (BCG), an attenuated strain of Mycobacterium bovis. BCG pulsed immunotherapy is a clinically established procedure for the treatment of superficial bladder cancer. In this paper, periodic BCG instillations are modeled using impulsive differential equations, which are studied using a combination of analytical and numerical techniques. In this way, we determine critical threshold values of the BCG instillation dose and rate of pulsing for successful treatment. We also identify treatment regimes in which tumor destruction occurs, but undesirable side effects are maintained at low levels by the immune system.
