REGIONAL ISOLATION PERFUSION IN THE TREATMENT OF ADVANCED MALIGNANT DISEASE

Regional isolation perfusion with an alkylating agent was carried out in a small community hospital, using readily available equipment, ten times in nine cases of advanced malignant disease. Visible regression of tumor, either grossly or histologically, occurred in seven patients and decided subjective improvement occurred in five patients.The long range effect of the procedure could not be determined in the present series, not enough time having elapsed.Side effects included jaundice in four patients, massive necrosis of the tumor in one patient, nausea and vomiting in three patients, postoperative edema in five patients, wound infection in two patients, Horner''s syndrome in one patient, temporary alopecia in three patients, and depression of hemoglobin level in seven patients, of the leukocyte count in four patients and of the platelet count in four patients.A feature of the procedure used in the present series was a system of monitoring the escape of alkylating agent into the systemic circulation.     

A REVIEW OF OPERATIONS ON THE TEMPORAL BONE

The majority of temporal bone operations are performed for treatment of acute or chronic middle ear and mastoid infection, otosclerosis and perforations of the tympanic membrane.Far from being a thing of the past, temporal bone surgery is an expanding field in the antibiotic age.Since treatment with antibiotics may temporarily allay the symptoms of serious disease of the ear, great care must be taken in examination of patients with a suspicious history.     

Properties of steroid sulphatase and arylsulphatase activities of human placenta

1. The properties of enzyme activities hydrolysing the sulphate esters of dehydroepiandrosterone, oestrone and p-nitrophenol are reported. The preparation studied was obtained from the microsomal fraction of human placenta by ultrasonic treatment and addition of Triton X-100. 2. The behaviour of the preparation during sedimentation at 105000g and attempts at purification indicated that the activities were particulate. Electron microscopy demonstrated the rupture of vesicular structures approx. 0.5mu in diameter concurrent with the release of activity. 3. The three activities were always associated throughout repeated attempts at separation by sucrose-density-gradient centrifugation and Sephadex-gel filtration. On the basis of kinetic studies, stability studies and treatment with butanol and ribonuclease it was concluded that a separate enzyme is responsible for each of the three activities. Widely varying plots of activity as a function of pH were consistent with this conclusion. 4. On the basis of sensitivity of the enzymes hydrolysing dehydroepiandrosterone sulphate and oestrone sulphate to ribonuclease and sensitivity of all three enzymes to lipase, it was concluded that the three enzymes are bound to a particle containing lipid and RNA. Enzymic activity is dependent on structural integrity of the particle. 5. A spectrophotometric method for the assay of oestrone sulphate hydrolysis is described. 6. Hydrolysis of nitrocatechol sulphate by human placenta under conditions described for arylsulphatases A and B is reported.     

Role of Differential Air Pressure Zones in the Control of Aerosols in a Large Animal Isolation Facility

The uncontrolled transmission of hog cholera in a large animal isolation facility, designed to control the movement of aerosols within and between individual wings of a multiunit building, indicated the need for a critical study of aerosol behavior under existing conditions of operation. Studies with aerosols of Escherichia coli B T3 bacteriophage (T3 coliphage) conclusively demonstrated the impossibility of obtaining the desired control by means of a "static" air balance relationship between adjacent areas within the facility. Modifications needed to provide the desired control of the air-handling system are outlined and discussed.     

A method for the estimation of 6-oxygenated metabolites of progesterone in urine

1. A method is described for the estimation of the 6-oxygenated metabolites of progesterone in urine. After hydrolysis the extract of urine is chromatographed on alumina to obtain a fraction containing mainly the 6-oxygenated metabolites. This fraction is oxidized to convert the metabolites into pregnane-3,6,20-triones, which are estimated as the dinitrophenylhydrazones. 2. The reliability criteria of the method are presented. Normal subjects excrete 0.1-0.6mg./day, and at the end of pregnancy values of 3.3-11.6mg./day are obtained.
