Aims
To clarify the efficacy and safety of adding sitagliptin to insulin therapy in Japanese patients with suboptimally controlled type 2 diabetes (T2DM).Study Design and Methods
This was a 24-week, prospective, randomized, open-labeled, controlled trial. Patients with T2DM who were suboptimally controlled despite receiving at least twice daily injection of insulin were enrolled in the study. The patients were randomized to continuation of insulin treatment (Insulin group) or addition of sitagliptin 50 to 100 mg daily to insulin treatment (Ins+Sita group). The primary outcome was change in HbA1c at week 24.Results
Adding sitagliptin to insulin significantly reduced HbA1c from 7.9 ± 1.0% at baseline to 7.0 ± 0.8% at week 24 (P <0.0001), while there was no significant change in HbA1c in the Insulin group (7.8 ± 0.7% vs. 7.8 ± 1.1%, P = 0.32). The difference in HbA1c reduction between the groups was 0.9% (95% confidence interval, 0.4 to 1.5, P = 0.01). There was no significant weight gain in either group. Incidence of hypoglycemia was significantly reduced in the Ins+Sita group compared with the Insulin group. Treatment satisfaction was improved in the Ins+Sita group. Baseline HbA1c level and beta cell function were associated with the magnitude of reduction in HbA1c in the Ins+Sita group.Conclusion
Adding sitagliptin to insulin reduced HbA1c without weight gain or increase in hypoglycemia, and improved treatment satisfaction in Japanese patients with T2DM who were suboptimally controlled despite at least twice daily injection of insulin.Trial Registration

In this work, six series of new energetic metal complexes were designed. Each complex contained a large, high-energy, high-nitrogen, anionic chelating ligand (either the 5,5′-bistetrazolate anion, the 5,5′-azobistetrazolate anion, or the 5,5′-(hydrazine-1,2-diyl)bis-[1H-tetrazol-1-ide] anion—each of which has a different bridging group), Cu or Ni as the metal atom, and two small complexing agent ligands (NH3 and/or NH2NO2). The molecular and electronic structures, heats of formation, densities, detonation properties, and impact sensitivities of the novel complexes were studied using density functional theory. Furthermore, the effects of varying the large chelating ligand (and thus the bridging group), the small complexing agents, and the metal atom on the structure and properties of the complex were investigated and analyzed in depth. The results show that the particular metal, bridging group, and complexing agents included in the energetic complex influence its structure and properties, but the effects of varying the constituents of the complex are complicated or unclear, and these effects are sometimes intertwined. In addition, the detonation pressures, detonation velocities, and impact sensitivities of the novel complexes ranged from 25.9 to 38.6 GPa, from 7.21 to 8.80 km s?1, and from 17 to 48 cm, respectively. Five of the complexes (B3, C3, D3, E3, and F3) appear to possess comparable performance to the famous and widely used high explosive 1,3,5-trinitro-1,3,5-triazinane, making these new complexes attractive to energetic materials experimentalists. 相似文献

In this study, the complete mitochondrial (mt) genome sequence of the South China deep‐sea giant isopod Bathynomus sp. was determined, and this study is the first to explore in detail the mt genome of a deep‐sea member of the order Isopoda. This species belongs to the genus Bathynomus, the members of which are saprophagous residents of the deep‐sea benthic environment; based on their large size, Bathynomus is included in the “supergiant group” of isopods. The mt genome of Bathynomus sp. is 14,965 bp in length and consists of 13 protein‐coding genes, two ribosomal RNA genes, only 18 transfer RNA genes, and a noncoding control region 362 bp in length, which is the smallest control region discovered in Isopoda to date. Although the overall genome organization is typical for metazoans, the mt genome of Bathynomus sp. shows a number of derived characters, such as an inversion of 10 genes when compared to the pancrustacean ground pattern. Rearrangements in some genes (e.g., cob, trnT, nad5, and trnF) are shared by nearly all isopod mt genomes analyzed thus far, and when compared to the putative isopod ground pattern, five rearrangements were found in Bathynomus sp. Two tRNAs exhibit modified secondary structures: The TΨC arm is absent from trnQ, and trnC lacks the DHU. Within the class Malacostraca, trnC arm loss is only found in other isopods. Phylogenetic analysis revealed that Bathynomus sp. (Cymothoida) and Sphaeroma serratum (Sphaeromatidea) form a single clade, although it is unclear whether Cymothoida is monophyletic or paraphyletic. Moreover, the evolutionary rate of Bathynomus sp. (dN/dS [nonsynonymous mutational rate/synonymous mutational rate] = 0.0705) is the slowest measured to date among Cymothoida, which may be associated with its relatively constant deep‐sea environment. The bioactivity-guided isolation on the Scutellaria barbata extract resulted in the purification of four undescribed neo-clerodane diterpenoids, scuttenlines A–D ( 1 – 4 ), alone with 20 known diterpenoids ( 5 – 24 ). The chemical structures of them were elaborated by extensive spectroscopic means, including 1D, 2D-NMR and HR-MS. The anti-inflammatory potential ability of 1 – 24 was screened in lipopolysaccharide-stimulated mouse RAW 264.7 cells. A comparative study between EHVF (eel herpesvirus in Formosa) and HVA ( Herpesvirus anguillae ) was performed. Similar syncytia were produced by each virus in cell lines infected with the viruses and similar viral yields were obtained. Significant cross-reactivity was observed in neutralization tests between HVA and EHVF but HVA and EFVF were distinctly different from channel catfish virus (CCV). An electrophoretic analysis of the structural proteins of EHVF and HVA produced similar electrophoretic patterns. Western blotting analysis of the viral proteins, using antiserum against EHVF, supported the results obtained in the cross-neutralization test. In conclusion, the isolates of HVA and EHVF are tightly clustered. 相似文献

The current study was conducted with the hypothesis that failure of maintenance of the vascular tone may be central to failure of the peripheral circulation and spiralling down of blood pressure in sepsis. Namely, we examined the balance between expression of myosin light chain (MLC) phosphatase and kinase, enzymes that regulate MLCs dephosphorylation and phosphorylation with a direct effect on pharmacomechanical coupling for smooth muscle relaxation and contraction respectively. Mechanical recordings and enzyme immunoassays of vascular smooth muscle lysates were used as the major methods to examine arterial biopsy samples from terminally ill sepsis patients. The results of the present study provide evidence that genomic alteration of expression of key regulatory proteins in vascular smooth muscles may be responsible for the relentless downhill course in sepsis. Down-regulation of myosin light chain kinase (MLCK) and up-regulation of MLCK may explain the loss of tone and failure to mount contractile response in vivo during circulation. The mechanical studies demonstrated the inability of the arteries to develop tone when stimulated by phenylephrine in vitro. The results of our study provide indirect hint that control of inflammation is a major therapeutic approach in sepsis, and may facilitate to ameliorate the progressive cardiovascular collapse. 相似文献

The self‐standing electrode nanomaterials with highly effective bifunctional electrocatalysis for oxygen reduction and evolution reactions (ORR/OER) are important for practical applications in metal–air batteries. Herein, a defect‐enriched and pyridinic‐N (PN) dominated bifunctional electrocatalyst with novel core–shell architecture (DN‐CP@G) is successfully fabricated by in situ exfoliating graphene from carbon paper followed by high temperature ammonia treatment. Benefitting from its strongly coupled core–shell structure, abundant defective sites and high‐content PN dopants, the DN‐CP@G displays an excellent electrocatalytic (ORR and OER) activity and stability in alkaline media, which are comparable to commercial Pt/C and Ir/C catalysts. The experiment, and theoretical calculations demonstrate that the electrocatalytic activities of carbon materials strongly depend on their defective sites and PN dopants. By directly using DN‐CP@G as a self‐standing electrode, the assembled zinc–air battery demonstrates a high discharge performance and outstanding long‐term cycle stability with at least 250 cycles, which is much superior to the mixed Pt/C and Ir/C electrodes. Remarkably, the DN‐CP@G based all‐solid‐state battery also reveals a good discharge and cycle performance. A facile and cost‐efficient approach to prepare highly effective bifunctional self‐standing electrode is provided by in situ generation of active sites on carbon support for metal–air batteries. 相似文献

A new species, Chenia zanderi C.Feng & J.Kou, from Xishan Forest Park in Yunnan Province, China is described and illustrated. Particularly distinctive features of the new species are the fragile stems and leaves fragile along their insertions, narrow leaves usually constricted near the middle, broadly rounded leaf apices with a long apiculus, large distal laminal cells with numerous solid and bifurcate papillae, a narrow groove on the distal ventral surface of the costa, and absence of a costal stereid band.
