The labeled magnitude scale (LMS) is a verbally anchored quasi-logarithmically spaced response scale with properties similar to magnitude estimation. Three experiments examined whether the LMS showed context effects similar to those found with magnitude estimation and category scales. Two versions of the LMS were used, one anchored at the high end to the strongest imaginable sweetness and the other to the strongest imaginable oral sensation. In a simple contrast experiment, subjects judged the sweetness of a 10% sucrose fruit beverage in the context of a less sweet (5%) beverage or a more sweet (20%) beverage. Consistent with previous literature, the sweetness was judged more intense in the low context and less intense in the high context, for all scaling methods. In a second experiment, this effect persisted (although was smaller) when the contextual item preceded the to-be-rated item, a so-called 'reversed-pair' design. Once again, the effect was highly significant for all scaling methods. In a third experiment, a range effect was examined using wide and narrow ranges of concentration. Psychophysical functions were flatter in a wide context and steeper in a narrow context, consistent with previous observations on range-mapping bias. This result was obtained for all scales. In three common contextual effects, the labeled magnitude scale behaved similarly to other scaling procedures. Its application to comparisons across individuals may be limited if those individuals have different experiential contexts within which they make their judgements.  相似文献

Recent work by our group has shown that an exopolysaccharide (EPS)-producing starter pair, Streptococcus thermophilus MR-1C and Lactobacillus delbrueckii subsp. bulgaricus MR-1R, can significantly increase moisture retention in low-fat mozzarella (D. B. Perry, D. J. McMahon, and C. J. Oberg, J. Dairy Sci. 80:799–805, 1997). The objectives of this study were to determine whether MR-1C, MR-1R, or both of these strains are required for enhanced moisture retention and to establish the role of EPS in this phenomenon. Analysis of low-fat mozzarella made with different combinations of MR-1C, MR-1R, and the non-EPS-producing starter culture strains S. thermophilus TA061 and Lactobacillus helveticus LH100 showed that S. thermophilus MR-1C was responsible for the increased cheese moisture level. To investigate the role of the S. thermophilus MR-1C EPS in cheese moisture retention, the epsE gene in this bacterium was inactivated by gene replacement. Low-fat mozzarella made with L. helveticus LH100 plus the non-EPS-producing mutant S. thermophilus DM10 had a significantly lower moisture content than did cheese made with strains LH100 and MR-1C, which confirmed that the MR-1C capsular EPS was responsible for the water-binding properties of this bacterium in cheese. Chemical analysis of the S. thermophilus MR-1C EPS indicated that the polymer has a novel basic repeating unit composed of d-galactose, l-rhamnose, and l-fucose in a ratio of 5:2:1.Lactic acid bacteria (LAB) are a diverse group of industrially important, gram-positive, non-spore-forming microbes that produce lactic acid as a major product of carbohydrate fermentation. Many strains of LAB produce extracellular polysaccharides which may be tightly associated with the bacterial cell wall as capsules or liberated into the growth medium as a loose slime (5). The term exopolysaccharide (EPS) has been used to refer to either type of external polysaccharide. EPSs may be homopolysaccharides, composed of a single type of sugar monomer, or heteropolysaccharides, containing several types of sugar monomers (25). Extracellular homopolysaccharides are made by such LAB as Leuconostoc mesenteroides and Streptococcus mutans, while extracellular heteropolysaccharides are produced by several other species of LAB, including Streptococcus thermophilus and Lactobacillus delbrueckii subsp. bulgaricus (6).The ability to produce EPS is unstable in LAB and may be lost following numerous transfers, prolonged periods of storage, or incubation at temperatures above that optimal for growth (6, 24). This instability of EPS production in mesophilic LAB has been attributed to the fact that the genes involved in polymer production are plasmid encoded. In contrast, genes for EPS production in thermophilic LAB, such as S. thermophilus and L. delbrueckii subsp. bulgaricus, are believed to be chromosomally encoded (6). Consequently, the unstable nature of the EPS phenotype in thermophilic strains is not understood, but it may be related to mobile genetic elements or genomic instability (24).Because of the ability of EPSs to act as viscosifying, stabilizing, or water-binding agents in various foods, these polymers can act as effective natural alternatives to commercial stabilizers (6). For example, EPS-producing (EPS+) LAB are commonly used as starter cultures for yogurt manufacture because EPS improves the viscosity and texture of yogurt and decreases its susceptibility to syneresis (loss of whey from the curd) (14, 28).Analysis of cheese microstructure has shown that in full-fat or part-skim mozzarella, the fat and a large portion of the water are located within channels that are formed by fat globules when the cheese curd is heated and stretched (18, 20). In low-fat mozzarella, however, there are very few fat globules to break up the protein matrix, resulting in less space for water retention (20). As a consequence, the cheese has a tough and rubbery texture and requires more heat for melting (19). Merrill et al. showed that procedures which increased moisture levels in reduced- and low-fat mozzarella improved the body, texture, and functional properties of the cheese (19). In addition to enhanced functionality, the ability to increase cheese moisture level (even by as little as 1%) gives processors an important economic advantage in the highly competitive mozzarella industry (27).Since EPS has the capacity to bind significant amounts of water, it was the hypothesis of our group that EPS+ LAB may be useful for the production of reduced- and low-fat mozzarella. Work by Perry et al. (21) recently showed that an EPS+ starter pair, S. thermophilus MR-1C and L. delbrueckii subsp. bulgaricus MR-1R, could be used to significantly increase moisture levels in low-fat mozzarella. The objectives of this study were to determine whether MR-1C, MR-1R, or both of these strains are required for enhanced moisture retention and to establish the role of EPS in this phenomenon. Group B and C Neisseria meningitidis are the major cause of meningococcal disease in the United States and in Europe. N . meningitidis lipooligosaccharide (LOS), a major surface antigen, can be divided into 12 immunotypes of which L1 through L8 were found among Group B and C organisms. Groups B and C but not Group A may sialylate their LOSs with N-acetylneuraminic acid (NeuNAc) at the nonreducing end because they synthesize CMP-NeuNAc. Using sialic acid-galactose binding lectins as probes in an ELISA format, six of the eight LOS immunotypes (L2, L3, L4, L5, L7, and L8) in Groups B and C bound specifically to Maackia amurensis leukoagglutinin (MAL), which recognizes NeuNAcalpha2- 3Galbeta1-4GlcNAc/Glc sequence, but not to Sambucus nigra agglutinin, which binds NeuNAcalpha2-6Gal sequence. The combination of SDS-PAGE and MAL-blot analyses revealed that these six LOSs contained only the NeuNAcalpha2-3Galbeta1-4GlcNAc trisaccharide sequence in their 4.1 kDa LOS components, which have a common terminal lacto-N-neotetraose (LNnT, Galbeta1-4GlcNAcbeta1-3Galbeta1-4Glc) structure when nonsialylated as shown by previous studies. The LOS-lectin binding was abolished when the LOSs were treated with Newcastle disease viral neuraminidase which cleaves alpha2-->3 linked sialic acid. Methylation analysis of a representative LOS (L2) confirmed that NeuNAc is 2-->3 linked to Gal. Thus, these LOSs structurally mimic certain glycolipids, i.e., paragloboside (LNnT-ceramide) and sialylparagloboside and some glycoproteins in having LNnT and N-acetyllactosamine sequences, respectively, with or without alpha2-->3 linked NeuNAc. The molecular mimicry of the LOSs may play a role in the pathogenesis of N.meningitidis by assisting the organism to evade host immune defenses in man.   相似文献

Background

The Multi-Ethnic Study of Atherosclerosis (MESA) showed that the addition of coronary artery calcium (CAC) to traditional risk factors improves risk classification, particularly in intermediate risk asymptomatic patients with LDL cholesterol levels <160 mg/dL. However, the cost-effectiveness of incorporating CAC into treatment decision rules has yet to be clearly delineated.

Objective

To model the cost-effectiveness of CAC for cardiovascular risk stratification in asymptomatic, intermediate risk patients not taking a statin. Treatment based on CAC was compared to (1) treatment of all intermediate-risk patients, and (2) treatment on the basis of United States guidelines.

Methods

We developed a Markov model of first coronary heart disease (CHD) and cardiovascular disease (CVD) events. We modeled statin treatment in intermediate risk patients with CAC≥1 and CAC≥100, with different intensities of statins based on the CAC score. We compared these CAC-based treatment strategies to a “treat all” strategy and to treatment according to the Adult Treatment Panel III (ATP III) guidelines. Clinical and economic outcomes were modeled over both five- and ten-year time horizons. Outcomes consisted of CHD and CVD events and Quality-Adjusted Life Years (QALYs). Sensitivity analyses considered the effect of higher event rates, different CAC and statin costs, indirect costs, and re-scanning patients with incidentalomas.

Results

We project that it is both cost-saving and more effective to scan intermediate-risk patients for CAC and to treat those with CAC≥1, compared to treatment based on established risk-assessment guidelines. Treating patients with CAC≥100 is also preferred to existing guidelines when we account for statin side effects and the disutility of statin use.

Conclusion

Compared to the alternatives we assessed, CAC testing is both effective and cost saving as a risk-stratification tool, particularly if there are adverse effects of long-term statin use. CAC may enable providers to better tailor preventive therapy to patients'' risks of CVD.  相似文献

Background

This paper presents the study protocol for a pragmatic randomised controlled trial to evaluate the impact of a school based program developed to prevent teenage pregnancy. The program includes students taking care of an Infant Simulator; despite growing popularity and an increasing global presence of such programs, there is no published evidence of their long-term impact. The aim of this trial is to evaluate the Virtual Infant Parenting (VIP) program by investigating pre-conceptual health and risk behaviours, teen pregnancy and the resultant birth outcomes, early child health and maternal health.

Methods and Design

Fifty-seven schools (86% of 66 eligible secondary schools) in Perth, Australia were recruited to the clustered (by school) randomised trial, with even randomisation to the intervention and control arms. Between 2003 and 2006, the VIP program was administered to 1,267 participants in the intervention schools, while 1,567 participants in the non-intervention schools received standard curriculum. Participants were all female and aged between 13-15 years upon recruitment. Pre and post-intervention questionnaires measured short-term impact and participants are now being followed through their teenage years via data linkage to hospital medical records, abortion clinics and education records. Participants who have a live birth are interviewed by face-to-face interview. Kaplan-Meier survival analysis and proportional hazards regression will test for differences in pregnancy, birth and abortion rates during the teenage years between the study arms.

Discussion

This protocol paper provides a detailed overview of the trial design as well as initial results in the form of participant flow. The authors describe the intervention and its delivery within the natural school setting and discuss the practical issues in the conduct of the trial, including recruitment. The trial is pragmatic and will directly inform those who provide Infant Simulator based programs in school settings.

Crown rot of wheat (Triticum aestivum), predominantly caused by the fungus Fusarium pseudograminearum, has become an increasingly important disease constraint in many winter cereal production regions in Australia. Our group has previously identified a range of quantitative trait loci (QTL) for partial resistance to crown rot in various bread wheat sources. Here, we report on work that has assessed the effectiveness of pyramiding QTL to improve resistance to crown rot. Two doubled haploid populations were analysed—one from a cross between two previously characterised sources of partial seedling resistance (2-49 and W21MMT70; n = 208) and one from a cross between 2-49 and the commercial variety Sunco, a source of adult field resistance (n = 134). Both populations were phenotyped for seedling resistance to crown rot. Microsatellite and DArT markers were used to construct whole genome linkage maps for use in composite interval mapping (CIM) to identify QTL. Three QTL were detected in both trials conducted on the 2-49/W21MMT70 population. These were located on chromosomes 1D (QCr.usq-1D.1), 3B (QCr.usq-3B.1) and 7A. QCr.usq-1D.1 and the previously undetected 7A QTL were inherited from 2-49. QCr.usq-3B.1, inherited from W21MMT70, was the most significant of the QTL, explaining up to 40.5% of the phenotypic variance. Three QTL were identified in multiple trials of the Sunco/2-49 population. These were located on chromosomes 1D (QCr.usq-1D.1), 2B (QCr.usq-2B.2) and 4B (QCr.usq-4B.1). Only QCr.usq-2B.2 was inherited from Sunco. QCr.usq-4B.1 was the most significant of these QTL, explaining up to 19.1% of the phenotypic variance. In the 2-49/W21MMT70 population, several DH lines performed significantly better than either parent, with the best recording an average disease severity rating of only 3.8% of that scored by the susceptible check cultivar Puseas. These lines represent a new level of seedling crown rot resistance in wheat.  相似文献

Background  

Campylobacter jejuni is the most common bacterial cause of human gastroenteritis worldwide. Due to the sporadic nature of infection, sources often remain unknown. Multilocus sequence typing (MLST) has been successfully applied to population genetics of Campylobacter jejuni and mathematical modelling can be applied to the sequence data. Here, we analysed the population structure of a total of 250 Finnish C. jejuni isolates from bovines, poultry meat and humans collected in 2003 using a combination of Bayesian clustering (BAPS software) and phylogenetic analysis.  相似文献

Although osteoporosis and its related fractures are common in patients with COPD, patients at high risk of fracture are poorly identified, and consequently, undertreated. Since there are no fracture prevention guidelines available that focus on COPD patients, we developed a clinical approach to improve the identification and treatment of COPD patients at high risk of fracture. We organised a round-table discussion with 8 clinical experts in the field of COPD and fracture prevention in the Netherlands in December 2013. The clinical experts presented a review of the literature on COPD, osteoporosis and fracture prevention. Based on the Dutch fracture prevention guideline, they developed a 5-step clinical approach for fracture prevention in COPD. Thereby, they took into account both classical risk factors for fracture (low body mass index, older age, personal and family history of fracture, immobility, smoking, alcohol intake, use of glucocorticoids and increased fall risk) and COPD-specific risk factors for fracture (severe airflow obstruction, pulmonary exacerbations and oxygen therapy). Severe COPD (defined as postbronchodilator FEV1 < 50% predicted) was added as COPD-specific risk factor to the list of classical risk factors for fracture. The 5-step clinical approach starts with case finding using clinical risk factors, followed by risk evaluation (dual energy X-ray absorptiometry and imaging of the spine), differential diagnosis, treatment and follow-up. This systematic clinical approach, which is evidence-based and easy-to-use in daily practice by pulmonologists, should contribute to optimise fracture prevention in COPD patients at high risk of fracture.  相似文献
