Aim of the study

Show the added value of SPECT-CT of the trunk in the diagnosis of bone metastases, compare its results to those of whole body scintigraphy (WBS) and specify the diagnostic impact while taking into account the cost of additional irradiation attributable to the scanner.

Patients and methods

Prospective study including 150 patients presenting neoplasic pathology between June 2013 and December 2014. All patients have had WBS followed by a SPECT-CT of the axial skeleton.

Results

A total of 1375 lesions were noted, of which 15.7 % were not seen in the WBS. The rate of indeterminate lesions increased from 18.7 % in the WBS to 1.9 % in the SPECT-CT. The concordance and discordance rates between WBS and SPECT-CT in the characterization of lesions according to their nature were 63.9 % and 1.5 %, respectively. The rib cage and the thoracic spine were the first localizations of the suspicious lesions of malignancy in SPECT-CT. Patient analysis showed a reduction in the number of scintigraphies classified as indeterminate to the WBS of 69 %. It also made it possible to better specify the metastatic extension without modification of the status in 32 patients and to change the status of the patient in 6 cases. SPECT-CT did not provide additional information to the WBS in 47 patients. The effective dose of 4 mSv was due to emission imaging while the scanner delivered an average effective dose estimated at 10.4 mSv.

Conclusion

The SPECT-CT of the trunk has an indisputable contribution since it improves the quality of the report, reduces the number of undecided exams and increases the confidence level in favor of a non-significant irradiation.  相似文献

The aim of this study was to investigate microemulsion (ME) based topical delivery system for fenoprofen calcium (FPCa) to eliminate its oral gastrointestinal adverse effects. ME was prepared by the water titration method using oleic acid as oil phase, tween 80 as a surfactant and propylene glycol as a cosurfactant. Oleic acid was selected as oil phase due to its good solubilizing capacity. ME existence region was determined using pseudo-ternary phase diagrams for preparing different formulations. Six different formulations were selected with various values of oil (25–68%), water (2–3%), and the mixture of surfactant and cosurfactant (1:1) (24–67%). The selected ME formulae were characterized for optical birefringence, transmission electron microscopy (TEM), pH, % transmittance, electronic conductivity, drug content, droplet size, rheological properties and stability evaluation. In vitro release study of FPCa from ME s through the synthetic membrane and hairless rat skin were evaluated. The optimized formula ME5 consisting of 5% w/w FPCa, 60% w/w oleic acid as oil phase, 3% w/w aqueous phase, and 32% w/w of surfactant phase containing Tween 80 and propylene glycol (1:?1) showed the highest transdermal flux and highest skin permeation rate. Finally, the % inhibition of carrageenan-induced rat paw edema of the optimized formula ME5 was highly significant (p?0.001) as compared to plain gel of FPCa. Although the Gulf of Gabès is recognised as an Important Bird Area, several aspects of the ecology of waterbirds inhabiting this area still need to be investigated. We observed how waterbird foraging guilds varied among habitats. In total, 49 species belonging to 16 families were recorded. Winter visitors accounted for 73% of counts. The avifauna was dominated by shorebirds (52% of records), followed by large wading birds (25%), open-water birds (18%) and waterfowl (5%). However, the structure of local waterbird communities and their composition in terms of foraging guilds varied according to habitat type. The bird communities of sandy beaches were dominated by open-water birds, whereas large mudflats were dominated by shorebirds, and wadi estuaries showed a more even representation of foraging guilds. Locally rare species had narrow distributions, whereas locally abundant species were found widely within the gulf. For four species the 1% population level criterion of the Ramsar Convention was exceeded. Overall, our results show that the Gulf of Gabès hosts important numbers of waterbirds with different ecological requirements, which confirms the importance of this gulf as a wintering area for Palearctic waterbirds.  相似文献

Studies were conducted to determine the incidences and levels of mycotoxin contamination in household stored maize and groundnuts in Kilosa District, Tanzania. Four villages were chosen for sampling. Seventy-two samples were collected from four villages in 2010, and then tested using a high-performance liquid chromatography method. Levels of Fumonisin B1 ranged from 63.26 to 213.15 μg/kg in all collected maize samples, while levels of aflatoxin B1 ranged from 72.97 to 195.17 μg/kg in all collected groundnut samples. Significant differences in levels of fumonisins were observed over time (p < 0.0001) but not among villages (p < 0.3209). Aflatoxin levels differed significantly among villages (p < 0.0491) and over time (p < 0.0001). The association between altitude and the level of aflatoxin contaminations was also significant. This study indicates that stored maize and groundnuts were more prone to mycotoxin contamination.  相似文献

This study was carried out in 4 adjacent villages in Lower Eghypt with a combined population of 24,000. A team of social workers and physicians worked together to introduce the injectable contraceptive depomedroxyprogesterone acetate as a post-partum long-acting contraceptive to the community leaders and the villagers at several meetings and individual home visits. Post-partum women who agreed to use the drug were defined as acceptors (591) and those who did not were defined as rejectors (715). The incidence of polygamy was higher among the rejectors, and rejectors' husbands had more children from their other wives. Acceptors had more previous pregnancies and children of both sexes than rejectors. The interval between the last 2 pregnancies was shorter among the rejectors. A greater % of acceptors had previously used another contraceptive. The commonest reasons for rejection were desire for further pregnancy (69%), health problems (11%), and desire for another method of contraception (8%). Religious factors figured in only 3% of cases.  相似文献

The Software Defined Networking (SDN) promises exciting new networking functionality. However, there is always remains a chance of programming errors that result in unreliable data communication. The centralized programming model helps decrease bugs' probability where a single controller manages the whole network. Yet, many real-time events occur at switches and end hosts, which often affect and add a delay in the communication process. One of those events includes unannounced destination host migration after installing flow rules during receiving of data packets. Such destination host movement results in the loss of packets because controller is not aware of this recent event. Therefore, we need an efficient approach to transmit packets without any packet loss despite destination host migration. This paper proposed a design to achieve the objective as mentioned above by defining a layer named Intelligent Transmission Control Layer (ITCL). It monitors all the end hosts' connections at their specific locations and performs necessary actions whenever the connection state changes for one or multiple hosts. The controller collects information of end nodes and state change through ITCL using A star search algorithm. After that, it updates flow tables accordingly to accommodate a location-change scenario with a route-change policy. ICTL is developed on prototype-based implementation using a popular POX controller platform. By comparing ITCL with the existing solution, we conclude that our proposed approach exhibits efficient performance in terms of Packet loss, Bandwidth usage, and Network Throughput.

Genes for the enzymes that metabolize galactose in Saccharomyces cerevisiae are strongly induced by galactose and tightly repressed by glucose. Because glucose also represses mitochondrial activity, we examined if derepression of the GAL1 galactokinase gene requires physiologically active mitochondria. The effect of mitochondria on the expression of GAL1 was analyzed by a novel approach in which the activity of the organelles was altered by functional expression of URF13, a mitochondrial protein unique to the Texas-type cytoplasmic male sterility phenotype in maize. Mitochondrial targeting and functional expression of the URF13 protein in yeast result in a decrease of the mitochondrial membrane potential similar to those observed in cells treated with mitochondrial inhibitors such as antimycin A or sodium azide. Activation of URF13 in galactose-induced cells results in the inhibition of GAL1 expression in the absence of repressing concentrations of glucose. Our data reveal the existence of a regulatory pathway that connects the derepression of the GAL1 gene with mitochondrial activity.  相似文献

The filamentous fungus Trichoderma reesei is adapted to nutrient-poor environments, in which it uses extracellular cellulases to obtain glucose from the available cellulose biomass. We have isolated and characterized Trhxt1, a putative glucose transporter gene, as judged by the glucose accumulation phenotype of a DeltaTrhxt1 mutant. This gene is repressed at high glucose concentrations and expressed at micromolar levels and in the absence of glucose. The gene is also induced during the growth of T. reesei on cellulose when the glucose concentration generated from the hydrolysis of cellulose present in the culture medium is in the micromolar range. We also show that oxygen availability controls the expression of the Trxht1 gene. In this regard, the gene is down-regulated by hypoxia and also by the inhibition of the flow of electrons through the respiratory chain using antimycin A. Intriguingly, anoxia but not hypoxia strongly induces the expression of the gene in the presence of an otherwise repressive concentration of glucose. These results indicate that although the absence of repressing concentrations of glucose and an active respiratory chain are required for Trhxt1 expression under normoxic conditions these physiological processes have no effect on the expression of this gene under an anoxic state. Thus, our results highlight the presence of a novel coordinated interaction between oxygen and the regulatory circuit for glucose repression under anoxic conditions.  相似文献

Cell encapsulation in microcapsules allows the in situ delivery of secreted proteins to treat different pathological conditions. Spherical microcapsules offer optimal surface‐to‐volume ratio for protein and nutrient diffusion, and thus, cell viability. This technology permits cell survival along with protein secretion activity upon appropriate host stimuli without the deleterious effects of immunosuppressant drugs. Microcapsules can be classified in 3 categories: matrix‐core/shell microcapsules, liquid‐core/shell microcapsules, and cells‐core/shell microcapsules (or conformal coating). Many preparation techniques using natural or synthetic polymers as well as inorganic compounds have been reported. Matrix‐core/shell microcapsules in which cells are hydrogel‐embedded, exemplified by alginates capsule, is by far the most studied method. Numerous refinement of the technique have been proposed over the years such as better material characterization and purification, improvements in microbead generation methods, and new microbeads coating techniques. Other approaches, based on liquid‐core capsules showed improved protein production and increased cell survival. But aside those more traditional techniques, new techniques are emerging in response to shortcomings of existing methods. More recently, direct cell aggregate coating have been proposed to minimize membrane thickness and implants size. Microcapsule performances are largely dictated by the physicochemical properties of the materials and the preparation techniques employed. Despite numerous promising pre‐clinical results, at the present time each methods proposed need further improvements before reaching the clinical phase. © 2009 American Institute of Chemical Engineers Biotechnol. The zooplankton community was studied in four Mediterranean reservoirs to assess the relative importance of environmental factors as determinants of zooplankton dynamic in the four seasons. The water temperature and hydrology variations affect the distribution of zooplankton. A positive correlation was established between the total zooplankton and the water temperature (r=0.9, n=9, p<0.05). Fourteen zooplankton species were identified. Seasonal changes in the density (ANOVA, F=3.7, d.f=36, p<0.01) and the biomass of total zooplankton (ANOVA, F=4.4, d.f=36, p<0.001) were observed. Our results suggest that planktivorous fish may not modify the zooplankton dynamics in Beni Mtir reservoir (oligotrophic). On the contrary, in Sidi Saâd reservoir (mesotrophic), fish predation has major effects on seasonal zooplankton dynamics.  相似文献
