Although high-impact hemodynamic forces are thought to lead to cerebral aneurysmal change, little is known about the aneurysm formation on the inner aspect of vascular bends such as the internal carotid artery (ICA) siphon where wall shear stress (WSS) is expected to be low. This study evaluates the effect of vessel curvature and hemodynamics on aneurysm formation along the inner carotid siphon. Catheter 3D-rotational angiographic volumes of 35 ICA (10 aneurysms, 25 controls) were evaluated in 3D for radius of curvature and peak curvature of the siphon bend, followed by univariate statistical analysis. Computational fluid dynamic (CFD) simulations were performed on patient-derived models after aneurysm removal and on synthetic variants of increasing curvature. Peak focal siphon curvature was significantly higher in aneurysm bearing ICAs (0.36±0.045 vs. 0.30±0.048 mm−1, p=0.003), with no difference in global radius of curvature (p=0.36). In CFD simulations, increasing parametric curvature tightness (from 5 to 3 mm radius) resulted in dramatic increase of WSS and WSS gradient magnitude (WSSG) on the inner wall of the bend. In patient-derived data, the location of aneurysms coincided with regions of low WSS (<4 Pa) flanked by high WSS and WSSG peaks. WSS peaks correlated with the aneurysm neck. In contrast, control siphon bends displayed low, almost constant, WSS and WSSG profiles with little spatial variation. High bend curvature induces dynamically fluctuating high proximal WSS and WSSG followed by regions of flow stasis and recirculation, leading to local conditions known to induce destructive vessel wall remodeling and aneurysmal initiation. 相似文献

Limb salvage is a viable alternative to amputation in many cases of advanced sarcoma. The authors examined their experience with microvascular reconstruction of upper extremity defects after sarcoma resection, focusing on oncologic and functional outcomes. A retrospective analysis yielded 17 patients who underwent 18 free flap procedures and met the inclusion criteria. Most patients (71 percent, n = 12) had recurrent sarcoma at presentation to the authors' institution. Malignant fibrous histiocytoma was the most common pathologic subtype (n = 6). High-grade tumors were present in 94 percent of patients (n = 16). The free flap survival rate was 100 percent. The rectus abdominis flap was the most common free flap used (39 percent; n = 7). Local recurrence occurred in nine flaps (50 percent), and five patients ultimately required amputations. Six patients (35 percent) had distant recurrence. The mean Enneking score for limb function was 73 percent of the maximum (21.9 of 30). The 5-year disease-specific survival rate was 61.3 percent. In select patients with advanced upper extremity sarcoma undergoing limb salvage, microvascular flap reconstruction can provide reliable, safe coverage with reasonable preservation of function. 相似文献

Development and organization of polarity-specific segregation of primary vestibular afferent fibers in mice 总被引:1,自引:0,他引:1
A striking feature of vestibular hair cells is the polarized arrangement of their stereocilia as the basis for their directional
sensitivity. In mammals, each of the vestibular end organs is characterized by a distinct distribution of these polarized
cells. We utilized the technique of post-fixation transganglionic neuronal tracing with fluorescent lipid soluble dyes in
embryonic and postnatal mice to investigate whether these polarity characteristics correlate with the pattern of connections
between the endorgans and their central targets; the vestibular nuclei and cerebellum. We found that the cerebellar and brainstem
projections develop independently from each other and have a non-overlapping distribution of neurons and afferents from E11.5
on. In addition, we show that the vestibular fibers projecting to the cerebellum originate preferentially from the lateral
half of the utricular macula and the medial half of the saccular macula. In contrast, the brainstem vestibular afferents originate
primarily from the medial half of the utricular macula and the lateral half of the saccular macula. This indicates that the
line of hair cell polarity reversal within the striola region segregates almost mutually exclusive central projections. A
possible interpretation of this feature is that this macular organization provides an inhibitory side-loop through the cerebellum
to produce synergistic tuning effects in the vestibular nuclei. The canal cristae project to the brainstem vestibular nuclei
and cerebellum, but the projection to the vestibulocerebellum originates preferentially from the superior half of each of
the cristae. The reason for this pattern is not clear, but it may compensate for unequal activation of crista hair cells or
may be an evolutionary atavism reflecting a different polarity organization in ancestral vertebrate ears. 相似文献

A comparative proteomic approach was used to assess the protein expression profile in the liver of 34 days old pikeperch larvae fed from day 10 post hatching, with three isoproteic and isolipidic formulated diets varying by their phospholipid (PL) contents (% dry diet weight): 1.4% (PL1), 4.7% (PL5) and 9.5% (PL9). Using 2D-DIGE minimal labelling of liver extracts, we were able to show 56 protein spots with a differential intensity (p < 0.05) depending on the dietary PL content. Among these spots, 11 proteins were unambiguously identified using nanoLC-MS/MS tandem mass spectrometry. In the PL9 larvae, our results indicate that the glycolytic pathway could be down-regulated due to the under-expression of the fructose biphosphate aldolase B and the phosphoglucomutase 1. Meanwhile, propionyl coenzyme A carboxylase (a gluconeogenic enzyme) was under-expressed. In addition, another gluconeogenic and lipogenic enzyme, pyruvate carboxylase, was identified in 3 different spots as being under-expressed in fish fed with the intermediate PL level (PL5). A high PL content increased the expression of sarcosine dehydrogenase, an enzyme involved in methionine metabolism, along with vinculin, a structural protein. Moreover, several stress proteins (glutathione S-transferase M, glucose regulated protein 75 and peroxiredoxin-1) were modulated in response to the dietary PL level and fatty acid composition. In the larvae fed with the lowest dietary PL content (PL1), over-expression of both GSTM and GRP75 might indicate a cellular stress in this experimental treatment, while the under-expression of Prx1 might indicate a lower defence against oxidative stress. In conclusion, this nutriproteomic approach showed significant modifications of protein expression in the liver of pikeperch larvae fed different PL contents, highlighting the importance of these nutrients and their influence on metabolism processes and on stress response. 相似文献

The purpose of this investigation was to study the anti-inflammatory activities of both glycerrhitinic acid (GA) and the aqueous
licorice extract (ALE) in comparison with diclofenac sodium (DS) (10 mg/kg), using the carrageenan-induced paw edema model
in male albino rats. In addition, the anti-ulcer activities of ALE, famotidine (FT), and a combination of ALE and FT using
indomethacin-induced ulceration technique in rat stomach were investigated. Conventional DS tablets containing GA, as well
as DS chewable tablets containing either GA or ALE with different tastes were prepared. Also, rapidly disintegrating FT tablets
were prepared using direct compression and camphor sublimation methods. ALE or GA produced significant anti-inflammatory activity
similar to DS, and when taken concomitantly, there is no possible antagonism. The anti-ulcer activity of licorice was found
to be similar to that of FT in indomethacin-induced ulceration technique in rat stomach. Combination therapy of both FT and
licorice showed higher anti-ulcer activity than either of them alone. Generally, tablets containing the crosslinked sodium
carboxymethyl cellulose (AcDisol) showed more rapidly disintegrating effect than those including Sodium starch glycolate (Primojel).
The oral disintegration was very rapid for all the tested formulations. Also, the amount of FT absorbed from the oral cavity
was nearly 9 from 10 mg theoretically present in each formula. It could be concluded that both GA and ALE have anti-inflammatory
activity comparable with DS. It may be recommended to add ALE to either FT or diclofinac for more effective anti-inflammatory

Kindler syndrome is an autosomal recessive disorder characterized by neonatal blistering, sun sensitivity, atrophy, abnormal pigmentation, and fragility of the skin. Linkage and homozygosity analysis in an isolated Panamanian cohort and in additional inbred families mapped the gene to 20p12.3. Loss-of-function mutations were identified in the FLJ20116 gene (renamed “KIND1” [encoding kindlin-1]). Kindlin-1 is a human homolog of the Caenorhabditis elegans protein UNC-112, a membrane-associated structural/signaling protein that has been implicated in linking the actin cytoskeleton to the extracellular matrix (ECM). Citrus Tristeza Virus (CTV), usually occurs in nature as a mixture of genotypes. Six naturally infected citrus
(Citrus sinensis) trees grafted on sour orange rootstock were collected from three citrus growing governorates
in Egypt (Sharqia, Qalyubia and Garbia). In this study, RT-PCR, Single-Strand Conformation Polymorphism
(SSCP) and nucleotide sequence analysis were used for four independent CTV genomic regions (p65, p18,
p20, and p23) to detect and assess the sequence and genetic variabilities among CTV Egyptian isolates. RTPCR products (650 bp) for the CTV p23 gene obtained from the selected isolates were used for the SSCP analysis
and DNA sequencing. SSCP patterns of p23 gene for individual isolates yielded different complex haplotype patterns. Nucleotide sequence analysis of p23 region amplified from six isolates under study revealed that p23 shared
high nucleotide identity 98.7% with T36 isolate from USA, Florida. Phylogenetic analysis of p23 gene indicated a
close evolutionary relationship between all examined isolates and Qaha isolate (T36 isolate group), suggesting that
they may have originated from closely related ancestors. Nucleotide sequence analysis of the three genes located
on CTV 3′-coterminal overhang, p18, p20 and p65, amplified from isolate A3, Sharqia governorate, revealed that
the p18, p65, and p20 genes were related to the T3-KB isolate from South Africa with 99%–100% sequence
homology. Phylogenetic relationship analysis for p65, p18 and p20 ORFs clustered the current A3 isolate with
T3 genotype group. The recombination analysis identified three of six isolates from Sharqia, and Garbia as potential recombinant for p23 gene. The isolates T36 and T3 were identified as major donors for recombination events
in isolate A3. Our results concluded that p23 ORF likely to be as a hotspot region for recombination and originated through recombination event. The current study indicated that recombination is an important factor for the
origin of CTV strains in Egypt. 相似文献
