Background

Data on incidence of coronary heart disease (CHD) is scarce in the Middle East and little is known about the contribution of known risk factors in this area.

Methods

The incidence of CHD and the effect of modifiable risk factors were explored in 2889 men and 3803 women aged 30–74 years in the population based cohort of the Tehran Lipid and Glucose Study, during 1999–2010. Average population attributable fraction (aPAF) was calculated for any risk factor using direct method based on regression model.

Results

The crude incidence rate in men was about twice that in women (11.9 vs. 6.5 per 1000 person-years). The aPAF of hypertension, diabetes, high total cholesterol and low-HDL cholesterol was 9.4%, 6.7%, 7.3% and 6.1% in men and 17%, 16.6%, 12% and 4.6% in women respectively. This index was 7.0% for smoking in men. High risk age contributed to 42% and 22% of risk in men and women respectively.

Conclusions

The incidence in this population of Iran was comparable to those in the US in the seventies. Well known modifiable risk factors explained about 40% and 50% of CHD burden in men and women respectively. Aging, as a reflection of unmeasured or unknown risk factors, bears the most burden of CHD, especially in men; indicating more age-related health care is required.  相似文献

Aims/Hypothesis

To investigate secular trends in cardiovascular disease (CVD) risk factors during a decade of follow-up in a Middle Eastern cohort, and to compare observed trends between diabetic and non-diabetic populations.

Methods

In a population of 6181 participants (2622 males and 3559 females), diabetes status and CVD risk factors were evaluated in 4 study phases from 1999–2011. 1045 subjects had type 2 diabetes mellitus at baseline and 5136 participants were diabetes-free. To examine the trends of CVD risk factors, generalized estimation equation models were constructed. The interaction between the diabetes status and each phase of the study was checked in a separate model.

Results

During the follow-up period diabetic females significantly gained better control of their blood pressure, serum low density lipoprotein cholesterol and general and central obesity measures compared to non-diabetic counterparts, although 60% of them had high BP and 64% had high serum LDL-C levels till the end of the study. Diabetic males however, experienced significantly better control on their serum LDL-C and general and central obesity measures compared to their non-diabetic controls; but 24% of them were still smoker, 63% had high BP and 60% had high serum LDL-C levels at the end of the follow-up (all Ps interaction <0.05). Use of lipid-lowering and antihypertensive medications increased consistently in both diabetic and non-diabetic populations.

Conclusions/Interpretation

Although CVD risk factors have been controlled to some extent among diabetic population in Iran, still high numbers of people with diabetes have uncontrolled CVD risk factors that prompt more attention.  相似文献

The aim of this paper is to create a model for mapping the surface electromyogram (EMG) signals to the force that generated by human arm muscles. Because the parameters of each person's muscle are individual, the model of the muscle must have two characteristics: (1) The model must be adjustable for each subject. (2) The relationship between the input and output of model must be affected by the force-length and the force-velocity behaviors are proven through Hill's experiments. Hill's model is a kinematic mechanistic model with three elements, i.e. one contractile component and two nonlinear spring elements.In this research, fuzzy systems are applied to improve the muscle model. The advantages of using fuzzy system are as follows: they are robust to noise, they prove an adjustable nonlinear mapping, and are able to model the uncertainties of the muscle.Three fuzzy coefficients have been added to the relationships of force-length (active and passive) and force-velocity existing in Hill's model. Then, a genetic algorithm (GA) has been used as a biological search method that can adjust the parameters of the model in order to achieve the optimal possible fit.Finally, the accuracy of the fuzzy genetic implementation Hill-based muscle model (FGIHM) is invested as following: the FGIHM results have 12.4% RMS error (in worse case) in comparison to the experimental data recorded from three healthy male subjects. Moreover, the FGIHM active force-length relationship which is the key characteristics of muscles has been compared to virtual muscle (VM) and Zajac muscle model. The sensitivity of the FGIHM has been evaluated by adding a white noise with zero mean to the input and FGIHM has proved to have lower sensitivity to input noise than the traditional Hill's muscle model.  相似文献

In the present study, a sensitive and competitive chemiluminescence immunoassay (CLIA) was developed in order to detect human serum albumin (HSA) in urine specimen. The method utilizes a home-made monoclonal anti-albumin antibody conjugated to horseradish peroxidase enzyme (mAb-HRP). Sensitivity, specificity and linearity of the assay were evaluated. According to the results, the proper concentration of HSA and mAb-HRP conjugates was 800?ng/100?μl and 1:200 respectively. In optimal conditions, this method could detect HSA in a high linear range of 10–200?μg?ml?1 with the low detection limit of 0.025?μg?ml?1. No evidence of interference with presence of probable substances in the urine samples indicated its high specificity and selectivity. Moreover, high reproducibility as well as high sensitivity and specificity of the test were confirmed using diabetic and non-diabetic samples. Significant concordance was observed between CLIA and immunoturbidimetry assay regarding detection of HSA. The results of the present study can be considered in accordance with the current demands such as reliability, accuracy, convenience and high speed of performance for a precise protein detection method. Furthermore, it may be regarded as a more rapid, simpler and cheaper alternative compared to other sophisticated assays.  相似文献

Aims

Considering the key role played by the apolipoprotein E (Apo E) gene in the regulation of lipid metabolism and obesity, the current study has evaluate the association between abdominal obesity and Apo E gene polymorphism in a population of Tehran.

Materials and methods

A cross-sectional study was performed on 345 men and 498 women, aged 19–86 years, selected from among participants of the Tehran Lipid and Glucose Study. The RFLP-PCR technique was employed to investigate polymorphism in the gene fragments. Based on the national survey of risk factors for non-communicable diseases of Iran, waist circumference (WC) cut off was set at 89 cm for men and 91 cm for women. The risk effect of obesity related variables and lipid profiles in two groups of WC were examined by logistic regression. For body mass index (BMI), waist to hip ratio (WHR), high-density lipoprotein-cholesterol (HDL-C), triglyceride (TG), fasting blood sugar (FBS), total cholesterol (TC), low-density lipoprotein-cholesterol (LDL-C), and blood pressure (BP), the standard risk cut-offs were applied.

Results

Frequencies of E2, E3, and E4 alleles were 9.7, 73, and 14.6%, respectively. The presence of the E3 allele was significantly associated with higher TG level in subjects with high WC, while, the presence of E4 allele decreased the plasma HDL-C (E2:52.1 ± 13.1 vs., E3:48.9 ± 11.2 vs., E4:44.6 ± 10.6 mg/dl, p < 0.05), HDL-C2 (E2:20.4 ± 9.2 vs., E3:19.1 ± 8.8 vs., E4:16.3 ± 7.9 mg/dl, p < 0.05), and HDL-C3 (E2:32.1 ± 7.4 vs., E3:30.3 ± 6.2 vs., E4:28.3 ± 6.1 mg/dl, p < 0.05) in normal WC subjects. The presence of the E3 carrier increased the risk of having higher plasma TG, compared with the E2 carrier (95% CI OR = 1.91, 1.02–3.57; p = 0.04).

Conclusion

According to the results of this study, the E3 carrier, caused an approximately 90% increase in the levels of TG in the group with abdominal obesity.  相似文献

One of the major obstacles in the design of an effective vaccine against HIV-1 is its antigenic variation, which results in viral escape from the immune system. Through a bioinformatics approach, we developed an innovative multivalent HIV-1 vaccine comprised of a pool of 176 lipidated and nonlipidated peptides representing variable regions of Env and Gag proteins. The potency and breadth of the candidate vaccine against a panel of HIV-1 subtypes was evaluated in nonhuman primate (cynomolgus macaques) and humanized mouse (HLA-A2.1) models. The results demonstrate strong immunogenicity with both breadth (humoral and cellular immunity) and depth (immune recognition of widely divergent viral sequences) against heterologous HIV-1 subtypes A-F.  相似文献

Paleontological analysis of remains from Wezmeh Cave in western Iran have yielded a Holocene Chalcolithic archeological assemblage, a rich Late Pleistocene carnivore faunal assemblage, and an isolated unerupted human maxillary premolar (P(3) or possibly P(4)). Species representation and U-series dating of faunal teeth place the carnivore assemblage during oxygen isotope stages (OIS) 3 and 2, and noninvasive gamma spectrometry dating of the human premolar places it at least as old as early OIS 2. The human premolar crown morphology is not diagnostic of late archaic versus early modern human affinities, but its buccolingual diameter places it at the upper limits of Late Pleistocene human P(3) and P(4) dimensions and separate from a terminal Pleistocene regional sample. Wezmeh Cave therefore provides additional Paleolithic human remains from the Zagros Mountains and further documents Late Pleistocene human association with otherwise carnivore-dominated cave assemblages.  相似文献

Tissue kallikrein (KLK1) is a kinin-forming serine protease synthesized in many organs including arteries and kidney. Study of the physiological role of KLK1 has benefited from the availability of mouse and human genetic models of KLK1 deficiency, through engineering of KLK1 mouse mutants and discovery of a major polymorphism in the human KLK1 gene that induces a loss of enzyme activity. Studies in KLK1-deficient mice and human subjects partially deficient in KLK1 have documented its critical role in arterial function in both species. KLK1 is also involved in the control of ionic transport in the renal tubule, an action that may not be kinin-mediated. Studies of experimental diseases in KLK1-deficient mice have revealed cardio- and nephro-protective effects of KLK1 and kinins in acute cardiac ischemia, post-ischemic heart failure, and diabetes. Potential clinical and therapeutic developments are discussed.  相似文献

We report on a simple and sensitive sulfur and nitrogen co‐doped carbon quantum dot (S,N‐CQD)‐based chemiluminescence (CL) sensor for the determination of indomethacin. S,N‐CQDs were prepared by a hydrothermal method and characterized by fluorescence spectra, Fourier transform infrared spectroscopy and transmission electron microscopy. To obtain the best CL system for determination of indomethacin, the reaction of S,N‐CQDs with some common oxidants was studied. Among the tested systems, the S,N‐CQD–KMnO4 reaction showed the highest sensitivity for the detection of indomethacin. Under optimum conditions, the calibration plot was linear over a concentration range of 0.1–1.5 mg L?1, with a limit of detection (3σ) of 65 μg L?1. The method was applied to the determination of indomethacin in environmental and biological samples with satisfactory results.  相似文献
