BACKGROUND: Uptake of genetic testing remains low, even in families with known BRCA1 and BRCA2 (BRCA1/2) mutations, despite effective interventions to reduce risk. We report disclosure and uptake patterns by BRCA1/2-positive individuals to at-risk relatives, in the setting of no-cost genetic counseling and testing. METHODS: Relatives of BRCA1/2-positive individuals were offered cost-free and confidential genetic counseling and testing. If positive for a BRCA1/2 mutation, participants were eligible to complete a survey about their disclosure of mutation status and the subsequent uptake of genetic testing by at-risk family members. RESULTS: One hundred and fifteen of 142 eligible individuals responded to the survey (81%). Eighty-eight (77%) of those surveyed disclosed results to all at-risk relatives. Disclosure to first-degree relatives (FDRs) was higher than to second-degree relatives (SDRs) and third-degree relatives (TDR) (95% vs. 78%; p < 0.01). Disclosure rates to male versus female relatives were similar, but reported completion of genetic testing was higher among female versus male FDRs (73% vs. 49%; p < 0.01) and SDRs (68% vs. 43%; p < 0.01), and among members of maternal versus paternal lineages (63% vs. 0%; p < 0.01). Men were more likely than women to express general difficulty discussing positive BCRA1/2 results with at-risk family members (90% vs. 70%; p = 0.03), while women reported more emotional distress associated with disclosure than men (48% vs. 13%; p < 0.01). DISCUSSION: We report a very high rate of disclosure of genetic testing information to at-risk relatives. However, uptake of genetic testing among at-risk individuals was low despite cost-free testing services, particularly in men, SDRs, and members of paternal lineages. The complete lack of testing among paternally related at-risk individuals and the lower testing uptake among men signify a significant barrier to testing and a challenge for genetic counselors and physicians working with high-risk groups. Further research is necessary to ensure that family members understand their risk and the potential benefits of genetic counseling.  相似文献

Primates may suffer injury from both natural (fights with conspecifics, predators) and human-induced (snares, power-lines and guns) causes. Though behavioral flexibility may allow primates to compensate for injuries, permanent disabilities, such as the loss of a limb, may adversely affect both foraging and locomotory efficiency and ultimately the survival and fitness of individuals. In the Cape Peninsula, South Africa, members of the chacma baboon population (Papio ursinus) experience chronic levels of conflict with humans that manifests in high levels (15%) of disabled baboons in groups that overlap with residential areas. In this study we investigate the potential impact of such disabilities by comparing the behavior and diet of disabled baboons with uninjured baboons matched closely for age, sex, and social status from groups of a similar size and composition for 8 mo, from May to December 2005. Disabled baboons spent more time resting and traveling and less time feeding than uninjured baboons. Disabled and uninjured baboons had similar diets but the former consumed fewer food items with high handling costs and fed more on high return foods than the latter. There was no difference in the frequency of grooming or social vigilance behaviors, as might be expected if disability had compromised either competitive ability or predation risk. Further, there was no difference in the survival of disabled or uninjured individuals in each group. Together these results suggest that while permanent injury may affect the behavior and diet of Peninsula baboons, that these constraints may be offset by access to anthropogenic food sources and the lack of natural predators. Disability in baboons may lead to obligate raiding of high-return anthropogenic foods, which is an important challenge for the ongoing management of this population.  相似文献

Dental ceramics because of their translucency exemplify the most biologically realistic restorative materials for aesthetic rehabilitation and can be used to estimate dose accumulated as a result of a nuclear accident or attack. In this study, lithium disilicate ceramic obtained from Vivadent Ivoclar, Turkey was studied for its thermoluminescence (TL) properties. The lithium disilicate glass ceramic was irradiated with a 90Sr–90Y β‐source from 10 Gy to 6.9 kGy and the results read on a Harshaw 3500 reader. The TL peak of lithium disilicate ceramic showed sublinearity in the range 12 Gy to 6 kGy. The area under the TL glow curve increased by about 25% by the end of 10th measurement cycle. Fading values were also considered after irradiation. Lithium disilicate ceramic samples underwent 37% fading after 1 h and 59% fading after 1 week. In addition to the experimental study, a software‐based simulation study was also undertaken using a MATLAB system identification tool. Experimental studies are generally time consuming and some materials used for experiments are very expensive. In this study, experimental, and simulation results were compared and produced almost the same outcome with a similarity of more than 98%.  相似文献

Anthropogenic disturbances associated with urban ecosystems can create favorable conditions for populations of some invasive plant species. Light pollution is one of these disturbances, but how it affects the growth and establishment of invasive plant populations is unknown. Cheatgrass (Bromus tectorum) is a problematic invasive species where it has displaced native grassland communities in the United States, but to our knowledge, there have been no studies of the ecological factors that affect cheatgrass presence in urban ecosystems. We conducted field surveys in urban alleys in Denver, Colorado, to compare the presence of cheatgrass at sites with and without artificial light at night (hereafter artificial light) from streetlights. These streetlights are mounted on utility poles, which cause ground disturbance when installed in alleys; we were able to test the independent effect of poles on cheatgrass establishment because not all poles have streetlights on them. We found that cheatgrass was positively associated with the presence of streetlights and to a lesser extent poles. In addition to cheatgrass, we also found that other plants were positively associated with the presence of both poles and streetlights. Our results suggest that artificial light may benefit the occurrence of cheatgrass and other plant species in urban settings. While invasive populations of cheatgrass in wild habitats attract the most attention from managers, we suggest more consideration for this grass in urban environments where its growth and establishment benefit from anthropogenic changes.  相似文献

Steroid receptor RNA activator (SRA), the only known RNA coactivator, augments transactivation by nuclear receptors (NRs). We identified SLIRP (SRA stem-loop interacting RNA binding protein) binding to a functional substructure of SRA, STR7. SLIRP is expressed in normal and tumor tissues, contains an RNA recognition motif (RRM), represses NR transactivation in a SRA- and RRM-dependent manner, augments the effect of Tamoxifen, and modulates association of SRC-1 with SRA. SHARP, a RRM-containing corepressor, also binds STR7, augmenting repression with SLIRP. SLIRP colocalizes with SKIP (Chr14q24.3), another NR coregulator, and reduces SKIP-potentiated NR signaling. SLIRP is recruited to endogenous promoters (pS2 and metallothionein), the latter in a SRA-dependent manner, while NCoR promoter recruitment is dependent on SLIRP. The majority of the endogenous SLIRP resides in the mitochondria. Our data demonstrate that SLIRP modulates NR transactivation, suggest it may regulate mitochondrial function, and provide mechanistic insight into interactions between SRA, SLIRP, SRC-1, and NCoR.  相似文献

Background

The objective of this study was to assess whether sex-specific differences in fetal and infant growth exist.

Methods

This study was embedded in the Generation R Study, a population-based prospective birth cohort. In total, 8556 live singleton births were included. Fetal growth was assessed by ultrasound. During the first trimester, crown-rump-length (CRL) was measured. In the second and third trimester of pregnancy head circumference (HC), abdominal circumference (AC) and femur length (FL) were assessed. Information on infant growth during the first 2 years of life was obtained from Community Health Centers and included HC, body weight and length.

Results

In the first trimester, male CRL was larger than female CRL (0.12 SD [95% CI 0.03,0.22]). From the second trimester onwards, HC and AC were larger in males than in females (0.30 SD [95% CI 0.26,0.34] and 0.09 SD [95% CI 0.05,0.014], respectively). However, FL in males was smaller compared to female fetuses (0.21 SD [95% CI 0.17,0.26]). Repeated measurement analyses showed a different prenatal as well as postnatal HC growth pattern between males and females. A different pattern in body weight was observed with a higher body weight in males until the age of 12 months where after females have a higher body weight.

Conclusions

Sex affects both fetal as well as infant growth. Besides body size, also body proportions differ between males and females with different growth patterns. This sexual dimorphism might arise from differences in fetal programming with sex specific health differences as a consequence in later life.

High Ag load in chronic viral infections has been associated with impairment of Ag-specific T cell responses; however, the relationship between Ag load in chronic Mycobacterium tuberculosis infection and functional capacity of M. tuberculosis-specific T cells in humans is not clear. We compared M. tuberculosis-specific T cell-associated cytokine production and proliferative capacity in peripheral blood from adults with progressively higher mycobacterial loads-that is, persons with latent M. tuberculosis infection (LTBI), with smear-negative pulmonary tuberculosis (TB), and smear-positive TB. Patients with smear-positive TB had decreased polyfunctional IFN-γ(+)IL-2(+)TNF-α(+) and IL-2-producing specific CD4 T cells and increased TNF-α single-positive cells, when compared with smear-negative TB and LTBI. TB patients also had increased frequencies of M. tuberculosis-specific CD8 T cells, compared with LTBI. M. tuberculosis-specific CD4 and CD8 T cell proliferative capacity was profoundly impaired in individuals with smear-positive TB, and correlated positively with ex vivo IFN-γ(+)IL-2(+)TNF-α(+) CD4 T cells, and inversely with TNF-α single-positive CD4 T cells. During 6 mo of anti-TB treatment, specific IFN-γ(+)IL-2(+)TNF-α(+) CD4 and CD8 T cells increased, whereas TNF-α and IFN-γ single-positive T cells decreased. These results suggest progressive impairment of M. tuberculosis-specific T cell responses with increasing mycobacterial load and recovery of responses during therapy. Furthermore, these data provide a link between specific cytokine-producing subsets and functional capacity of M. tuberculosis-specific T cells, and between the presence of specific CD8 T cells ex vivo and active TB disease. These data have potentially significant applications for the diagnosis of TB and for the identification of T cell correlates of TB disease progression.  相似文献

Antarctic marine organisms are considered to have extremely limited ability to respond to environmental temperature change. However, here we show that the Antarctic notothenioid fish Pagothenia borchgrevinki is an exception to this theory. P. borchgrevinki was able to acclimate its resting metabolic rate and resting ventilation frequency after a 5°C rise in temperature. Acute exposure to 4°C resulted in an elevation in metabolic rate (57.8 ± 4.79 mg O2 kg−1 h−1) and resting ventilation rate (40.38 ± 1.61 breaths min−1) compared with fish at −1°C (metabolic rate 34.45 ± 3.12 mg O2 kg−1 h−1; ventilation rate 29.88 ± 3.72 breaths min−1). However, after a 1-month acclimation period, there was no significant difference in the metabolic rate (cold fish 29.52 ± 3.01; warm fish 31.13 ± 2.30 mg O2 kg−1 h−1), or the resting ventilation rate (cold fish 28.75 ± 0.98; warm fish 34.25 ± 2.28 breaths min−1) of cold and warm acclimated fish. Acclimation changes to the rate of oxygen consumption following exhaustive exercise were complex. The pattern of oxygen consumption during recovery from exhaustive exercise was not significantly different in either cold or warm acclimated fish.  相似文献

Climate change is a major threat to species worldwide, yet it remains uncertain whether tropical or temperate species are more vulnerable to changing temperatures. To further our understanding of this, we used a standardised field protocol to (1) study the buffering ability (ability to regulate body temperature relative to surrounding air temperature) of neotropical (Panama) and temperate (the United Kingdom, Czech Republic and Austria) butterflies at the assemblage and family level, (2) determine if any differences in buffering ability were driven by morphological characteristics and (3) used ecologically relevant temperature measurements to investigate how butterflies use microclimates and behaviour to thermoregulate. We hypothesised that temperate butterflies would be better at buffering than neotropical butterflies as temperate species naturally experience a wider range of temperatures than their tropical counterparts. Contrary to our hypothesis, at the assemblage level, neotropical species (especially Nymphalidae) were better at buffering than temperate species, driven primarily by neotropical individuals cooling themselves more at higher air temperatures. Morphology was the main driver of differences in buffering ability between neotropical and temperate species as opposed to the thermal environment butterflies experienced. Temperate butterflies used postural thermoregulation to raise their body temperature more than neotropical butterflies, probably as an adaptation to temperate climates, but the selection of microclimates did not differ between regions. Our findings demonstrate that butterfly species have unique thermoregulatory strategies driven by behaviour and morphology, and that neotropical species are not likely to be more inherently vulnerable to warming than temperate species.  相似文献

We previously reported the case of a patient who became ill with Salmonella serotype Blockley after eating a chicken meal and who was still suffering symptoms of salmonella-reactive arthritis 10 years after the initial infection. The immunological consequences of infection were of concern and prompted a literature review because of the possibility that Salmonella Blockley may be more likely to cause long-term illness in certain patients than more common serovars. The review revealed the additional cause for concern of plasmid-borne extended spectrum beta-lactamases and the high frequency of integron-associated multidrug resistance in some isolates. The serovar was described in 1955 in the USA and Europe. It has become common in South East Asia and recently has been reported more often in Europe. A review of reactive arthritis outbreaks shows that information on the incidence and duration of the condition caused by serovars other than Salmonella serotype Enteritidis and Salmonella serotype Typhimurium is very limited. Rheumatologists can assist microbiologists and epidemiologists by reporting relevant cases of long-term reactive arthritis.  相似文献
