Acute pulmonary embolism (PE) is a common disease which frequently results in life-threatening right ventricular (RV) failure. High-risk PE, presenting with hypotension, shock, RV dysfunction or right heart thrombus is associated with a high mortality, particularly during the first few hours. Accordingly, it is important to commence effective therapy as soon as possible. In the case described in this report, a 49-year-old woman with myotonic dystrophy type 1 presented with acute respiratory failure and hypotension. Transthoracic echocardiography showed signs of right heart failure and a mobile right heart mass highly suspicious of a thrombus. Based on echocardiographic findings, acute thrombolysis was performed resulting in hemodynamic stabilization of the patient and complete resolution of the right heart thrombus. This case underscores the important role of transthoracic echocardiography for the diagnosis, management and monitoring of PE and underlines the efficacy and safety of thrombolysis in the treatment of PE associated with right heart thrombus.  相似文献

The standard collagen triple‐helix requires a perfect (Gly‐Xaa‐Yaa)n sequence, yet all nonfibrillar collagens contain interruptions in this tripeptide repeating pattern. Defining the structural consequences of disruptions in the sequence pattern may shed light on the biological role of sequence interruptions, which have been suggested to play a role in molecular flexibility, collagen degradation, and ligand binding. Previous studies on model peptides with 1‐ and 4‐residue interruptions showed a localized perturbation within the triple‐helix, and this work is extended to introduce natural collagen interruptions up to nine residue in length within a fixed (Gly‐Pro‐Hyp)n peptide context. All peptides in this set show decreases in triple‐helix content and stability, with greater conformational perturbations for the interruptions longer than five residue. The most stable and least perturbed structure is seen for the 5‐residue interruption peptide, whose sequence corresponds to a Gly to Ala missense mutation, such as those leading to collagen genetic diseases. The triple‐helix peptides containing 8‐ and 9‐residue interruptions exhibit a strong propensity for self‐association to fibrous structures. In addition, a small peptide modeling only the 9‐residue sequence within the interruption aggregates to form amyloid‐like fibrils with antiparallel β‐sheet structure. The 8‐ and 9‐residue interruption sequences studied here are predicted to have significant cross‐β aggregation potential, and a similar propensity is reported for ~10% of other naturally occurring interruptions. The presence of amyloidogenic sequences within or between triple‐helix domains may play a role in molecular association to normal tissue structures and could participate in observed interactions between collagen and amyloid.  相似文献

The single transmembrane domain serine/threonine kinase activin receptor type IIB (ActRIIB) has been proposed to bind key regulators of skeletal muscle mass development, including the ligands GDF-8 (myostatin) and GDF-11 (BMP-11). Here we provide a detailed kinetic characterization of ActRIIB binding to several low and high affinity ligands using a soluble activin receptor type IIB-Fc chimera (ActRIIB.Fc). We show that both GDF-8 and GDF-11 bind the extracellular domain of ActRIIB with affinities comparable with those of activin A, a known high affinity ActRIIB ligand, whereas BMP-2 and BMP-7 affinities for ActRIIB are at least 100-fold lower. Using site-directed mutagenesis, we demonstrate that ActRIIB binds GDF-11 and activin A in different ways such as, for example, substitutions in ActRIIB Leu79 effectively abolish ActRIIB binding to activin A yet not to GDF-11. Native ActRIIB has four isoforms that differ in the length of the C-terminal portion of their extracellular domains. We demonstrate that the C terminus of the ActRIIB extracellular domain is crucial for maintaining biological activity of the ActRIIB.Fc receptor chimera. In addition, we show that glycosylation of ActRIIB is not required for binding to activin A or GDF-11. Together, our findings reveal binding specificity and activity determinants of the ActRIIB receptor that combine to effect specificity in the activation of distinct signaling pathways.  相似文献

The chemotherapy of lymphatic filariasis relies upon drugs such as diethylcarbamazine and ivermectin that largely target the microfilarial stages of the parasite, necessitating continued treatment over the long reproductive life span of the adult worm. The identification of compounds that target adult worms has been a long-term goal of WHO. Here we describe a fluorescence polarization assay for the identification of compounds that target Hsp90 in adult filarial worms. The assay was originally developed to identify inhibitors of Hsp90 in tumor cells, and relies upon the ability of small molecules to inhibit the binding of fluorescently labelled geldanamycin to Hsp90. We demonstrate that the assay works well with soluble extracts of Brugia, while extracts of the free-living nematode C. elegans fail to bind the probe, in agreement with data from other experiments. The assay was validated using known inhibitors of Hsp90 that compete with geldanamycin for binding to Hsp90, including members of the synthetic purine-scaffold series of compounds. The efficacy of some of these compounds against adult worms was confirmed in vitro. Moreover, the assay is sufficiently sensitive to differentiate between binding of purine-scaffold compounds to human and Brugia Hsp90. The assay is suitable for high-throughput screening and provides the first example of a format with the potential to identify novel inhibitors of Hsp90 in filarial worms and in other parasitic species where Hsp90 may be a target.  相似文献

An important goal of native plant restorations was to reconstitute populations that are genetically similar to native ones, thereby increasing the probably of successful establishment and persistence. We examined the extent to which this goal has been accomplished in Great Lakes restorations of Ammophila breviligulata Fern., a beachgrass species that is widely used for habitat restoration and is considered threatened in the study areas. In parallel studies on Lake Michigan and Lake Superior, we used polymorphic Intersimple Sequence Repeat markers to assess genetic similarity between well‐established and new native populations, restored populations, and restoration propagules obtained from two commercial suppliers. Native populations were generally more diverse than expected for a clonal species, whereas the commercially cultivated releases were monotypic. One of the commercial releases used in Minnesota was exclusively found in restored populations and did not occur in any other native population at this site. The propagules used in the newly planted restoration in Illinois were derived from a release that commercial suppliers maintain was derived from a native Michigan population, as opposed to a selected release. Diversity in this restoration was equivalent to that native Illinois’ populations; however, many of the genotypes were not of local origin. Overall, study underscores the importance of obtaining baseline genetic surveys of remnant native populations and restoration propagules before restoration efforts are initiated, especially when the populations are threatened or endangered.  相似文献
