Objective
To explore external validity of randomized controlled trials (RCTs) of hypertension within China from the view of sample representation.Methods
Comprehensive literature searches were performed in Medline, Embase, Cochrane Central Register of Controlled Trials (CCTR) et al and advanced search strategies were used to locate hypertension RCTs as well as observational studies conducted in China during 1996 to 2009 synchronously. The risk of bias in RCTs and observational studies was assessed by two modified scales respectively, and then both types of studies with 3 or more grading scores were included for the purpose of evaluating of external validity. Following that the study characteristics relative to sample representation were extracted from RCTs and observational studies synchronously, and the later were taken as external references for validating sample representation of RCTs.Results
226 hypertension RCTs and 21 observational studies were included for final analysis. Comparing samples with observational studies, the mean age of samples within RCTs was 54.46 years, significantly lower than that of observational studies (66.35 years) (P=0.002). The average disease course in patients of RCTs was 3.89 years and grade III hypertensive patients accounted for 17%; both were lower than that of the observational studies (12.96 years, P<0.001; 34%, P=0.026 respectively). In addition, the proportions of patients with complications due to heart failure, stroke, diabetes, or coronary heart disease in RCTs were 8%, 5%, 12% and 11% correspondingly, all of which were significantly less than that of observational studies (11%, 18%, 17% and 29%).Conclusion
Sample characteristics within hypertension RCTs were significantly different from those in observational studies. The samples in most RCTs were under-represented. It’s feasible to take samples of observational studies as a mirror of the actual composition of hypertension patients in the real world, if the reporting of observational studies is abundant and available. 相似文献42.
Cancer marker discovery is an emerging topic in high-throughput quantitative proteomics. However, the omics technology usually generates a long list of marker candidates that requires a labor-intensive filtering process in order to screen for potentially useful markers. Specifically, various parameters, such as the level of overexpression of the marker in the cancer type of interest, which is related to sensitivity, and the specificity of the marker among cancer groups, are the most critical considerations. Protein expression profiling on the basis of immunohistochemistry (IHC) staining images is a technique commonly used during such filtering procedures. To systematically investigate the protein expression in different cancer versus normal tissues and cell types, the Human Protein Atlas is a most comprehensive resource because it includes millions of high-resolution IHC images with expert-curated annotations. To facilitate the filtering of potential biomarker candidates from large-scale omics datasets, in this study we have proposed a scoring approach for quantifying IHC annotation of paired cancerous/normal tissues and cancerous/normal cell types. We have comprehensively calculated the scores of all the 17219 tested antibodies deposited in the Human Protein Atlas based on their accumulated IHC images and obtained 457110 scores covering 20 different types of cancers. Statistical tests demonstrate the ability of the proposed scoring approach to prioritize cancer-specific proteins. Top 100 potential marker candidates were prioritized for the 20 cancer types with statistical significance. In addition, a model study was carried out of 1482 membrane proteins identified from a quantitative comparison of paired cancerous and adjacent normal tissues from patients with colorectal cancer (CRC). The proposed scoring approach demonstrated successful prioritization and identified four CRC markers, including two of the most widely used, namely CEACAM5 and CEACAM6. The structures, stabilities, and aromaticities of a series of (BCO) n (CH)4–n NH (n?=?0–4), (BCO) n (CH)4–n O (n?=?0–4), and (BCO) n (CH)4–n S (n?=?0–4) clusters were investigated at the B3LYP density functional level of theory. The most stable positional isomers of the individual clusters were obtained. All of the calculated CO binding energies were exothermic, suggesting that these BCO-substituted species are stable. Calculated differences in strain energy between the BCO-substituted structures and their corresponding hydrocarbon clusters were all exothermic, indicating that the BCO-substituted structures are less strained. The negative nucleus-independent chemical shift (NICS) values obtained show that these BCO-substituted clusters are aromatic compounds, in good agreement with the aromaticities of the corresponding hydrocarbon species. Amino acids and dipeptides that are correlated significantly with lipase optimum temperatures were searched for in 34 microbial lipase sequences by a stepwise regression method. The positive dipeptides were found to be IR, KS, NY, SA, ST and YR, whereas negative ones were DK, DY, IS, KA, WS, YS and QI. The calculated optimum temperatures from an optimal regression equation of dipeptides fitted the corresponding experimental optimum temperatures of lipases very well, and the maximal absolute difference was only 3.43°C. The spatial positions of the related dipeptides were searched for in two known crystal structures of a thermophilic and mesophilic lipase, respectively. Most of the positive dipeptides were sited in the α-helices, while the negative ones were located mainly in the β-strands or coils and about half of them existed in the N- or C-terminii of the lipases. The study's objective is to survey the attitudes of Chinese people living in Hong Kong toward genomic science and technology (GST) and their ethical and social implications. Using a 24-item questionnaire, 877 Cantonese-speaking residents between age 18 and 64 with minimum high school education are interviewed by telephone. Multiple regression analysis identifies education level as the most important demographic variable. Overall, respondents have mild agreement with genetic determinism and the use of GST for disease prevention but not for non-therapeutic genetic enhancement and production of “genetically modified” crops or meat. Respondents strongly believe that GST tampers with nature and resources should be used to solve other healthcare problems first. Respondents also show little concern that personal genetic information may be abused by their employers or schools and have only a minimal willingness to share personal genetic information with their family members. 相似文献

Interest in anthocyanins has increased immensely during the past decade. From these studies, it is clear that anthocyanins have unique properties: Anthocyanins are absorbed intact and absorption can be saturated; acylation of anthocyanins lowers their apparent absorption; anthocyanidin diglycosides in the form of sambubioside or rutinoside impart increased stability to the anthocyanin molecule; and the quantities excreted in urine are less than 0.1% of intake. However, 60–90% of the anthocyanins may disappear from the gastrointestinal tract within 4 h after a meal. What happens to the bulk of the anthocyanins that disappear is not clear. Degradation accounts for a part of this disappearance, but differs for the various aglycones and may be modified further by the nature of the aglycone glycosylation, which further complicates our understanding of this process. Anthocyanins may play an important role in health promotion in terms of obesity prevention, cardiovascular health, anti-inflammatory and anti-cancer effects. 相似文献

Cloud computing provides many kinds of application services for cloud users, but security problems have caused great impact on Software as a Service (SaaS). As a commercial model, SaaS is related among different participants who could be malicious or dishonest. This paper presents a Software Service Signature (S3) to deal with several security issues in SaaS and keep the interests and rights of all participants in safety.
