Bioabsorbable hemostatic agents such as oxidized regenerated cellulose are widely used to control intraoperative diffuse capillary bleeding. Compared with electrocautery or ligation, oxidized regenerated cellulose has the advantage of controlling bleeding without occluding the vessel lumen or causing thermal injuries to adjacent tissue. Although the manufacturer recommends removal of the material once hemostasis is achieved, oxidized regenerated cellulose is a bioabsorbable hemostatic agent and is often left in the surgical bed to prevent subsequent bleeding after surgical closure. However, noninvasive imaging techniques have revealed granulomatous foreign-body reactions that mimic infection or tumor recurrence. We present a case report of sterile peritonitis and granuloma formation secondary to the presence of oxidized regenerated cellulose after intestinal resection to excise a colonic adenocarcinoma in an aged rhesus macaque.Bioabsorbable hemostatic agents such as oxidized regenerated cellulose (for example, Surgicel) are widely used to control intraoperative diffuse capillary bleeding. This fiber is then woven into a gauze and oxidized.17,22 Oxidized regenerated cellulose is supplied as a substrate that is flexible, malleable, and trimable.16The mechanism of hemostasis of oxidized regenerated cellulose is reportedly associated with its caustic activity.2 The oxidation of cellulose produces a low-pH organic acid that reacts with blood, thus forming an artificial clot and causing platelet aggregation.18Although the manufacturer recommends the removal of oxidized regenerated cellulose once hemostasis is achieved,8 the product, a bioabsorbable hemostatic agent, is often left in situ within the surgical bed to prevent bleeding after surgical procedures. The biodegradation and elimination of oxidized regenerated cellulose from the tissue occurs in 2 phases.14 Polyanhydroglucuronic acid, the major functional unit of oxidized regenerated cellulose, is readily soluble. This acid is degraded extracellulary and systematically cleared from the system approximately 18 h after implantation.13,14 The remaining fibrous residue, however, requires macrophage phagocytosis for clearance and can be observed within macrophages for at least 48 h after implantation.13 Unfortunately, these fibrous residues have a prolonged degradation, and their persistence for as long as 7 mo after surgery has been confirmed histologically.7Despite the biocompatibility of oxidized regenerated cellulose, granulomatous foreign-body reactions that imitate infection or tumor recurrence have been revealed by using noninvasive imaging techniques.1,11,12,15,17,18,22 Here we describe a case of peritonitis and granuloma formation secondary to the presence of oxidized regenerated cellulose after an intestinal resection to excise a colonic adenocarcinoma in an aged rhesus macaque.  相似文献

Flooding regulates the amount and quality of habitat available for fish populations in river-floodplains. Although previous studies assessed fish population responses to river hydrology, the processes by which flooding affects fish abundance and catch remain unclear. Here, we investigated whether degree of flooding affects abundance and catch of Colossoma macropomum, a long-lived, overexploited fish population of the central Amazon Basin. We computed the degree of flooding corresponding to the feeding area of young-of-the-year C. macropomum as the annual magnitude of the moving littoral zone (ML). We estimated abundance of age classes one, two, and three of C. macropomum using a modeling program based on catch, fishing effort, and fish length frequency data from the principal commercial fishery. We found that flooding positively and non-linearly affected abundance of the age-one cohort but not of older age classes. ML data corresponding to a late rising water phase in which zooplankton, seeds, and fruits dominated the diet provided the strongest effect on age-one abundance. However, flooding effects on total catch were not found, likely due to catches comprising several age classes. These results provide support to existing evidence that the magnitude of the moving littoral zone regulates abundance of juvenile fish. Because the ML quantifies food and habitat availability for various other fish species, it may constitute an important control of fish abundance in these systems. Management of these fisheries may be improved by adjusting fishing effort based on hydrology. More generally, the information also serves to assess the impacts of hydrological alterations (e.g., dams) on fish recruitment.

Leptin is an adipokine that acts in the central nervous system and regulates energy balance. Animal models and human observational studies have suggested that leptin surge in the perinatal period has a critical role in programming long-term risk of obesity. In utero exposure to maternal hyperglycemia has been associated with increased risk of obesity later in life. Epigenetic mechanisms are suspected to be involved in fetal programming of long term metabolic diseases. We investigated whether DNA methylation levels near LEP locus mediate the relation between maternal glycemia and neonatal leptin levels using the 2-step epigenetic Mendelian randomization approach. We used data and samples from up to 485 mother-child dyads from Gen3G, a large prospective population-based cohort. First, we built a genetic risk score to capture maternal glycemia based on 10 known glycemic genetic variants (GRS10) and showed it was an adequate instrumental variable (β = 0.046 mmol/L of maternal fasting glucose per additional risk allele; SE = 0.007; P = 7.8 × 10−11; N = 467). A higher GRS10 was associated with lower methylation levels at cg12083122 located near LEP (β = −0.072 unit per additional risk allele; SE = 0.04; P = 0.05; N = 166). Direction and effect size of association between the instrumental variable GRS10 and methylation at cg12083122 were consistent with the negative association we observed using measured maternal glycemia. Lower DNA methylation levels at cg12083122 were associated with higher cord blood leptin levels (β = −0.17 log of cord blood leptin per unit; SE = 0.07; P = 0.01; N = 170). Our study supports that maternal glycemia is part of causal pathways influencing offspring leptin epigenetic regulation.  相似文献

Defined as operational units, the estuarine and marine fishing production systems (FPS) from the two southernmost states of Brazil, Santa Catarina and Rio Grande do Sul were analyzed using the RAPFISH technique with respect to sustainability of fisheries. The FPS were defined by 49 attributes divided into five fields: economic, social, ecological, technological and managerial. Data were obtained from the literature and interviews with fishers, skippers, and ship owners in 2003 and 2004. Overall, 26 FPS were identified: nine small-scale; three medium-scale; ten large-scale operating on the shelf, upper slope or nearby oceanic waters; and four large-scale fishing systems on the upper slope or neighboring oceanic waters. The latter included only foreign vessels, which produced frozen products exclusively for export. Analyses of 15 selected attributes more closely associated with sustainability in the five fields of evaluation showed all FPS in an intermediate position between the extremes of ideal and bad, reinforcing the perception that no FPS in southern Brazil can be considered truly sustainable; evidence of clearly unsustainable FPS was also not found. Thus RAPFISH fell short of producing a clear pattern that could result in sound management, probably due to disagreement among the different dimensions of sustainability. All five fields of evaluation were equally weighted; however, it could be argued that the biological and ecological status of fisheries were not given enough emphasis in relation to the other dimensions. Dissimilarities within FPS and differences regarding sustainability levels must all be taken into consideration for sound management measures.  相似文献

Arapaima are listed as endangered fishes according to the Convention on International Trade of Endangered Species of Wild Fauna and Flora (CITES), thus their international trade is regulated by non-detriment finding (NDF) procedures. The authors critically assessed Brazil's regulations for NDF procedures for Arapaima using IUCN's checklist for making NDFs, and found that those regulations cannot ensure the sustainability of Arapaima populations. Arapaima are among the largest fishes in the world, migrate short distances among several floodplain habitats, and are very vulnerable to fishing during spawning. They are threatened mainly by overfishing. The fishery is largely unregulated because government regulations on size, season, and even moratoriums on capture have been very poorly enforced. Arapaima remain poorly understood and the taxonomy and geographical distribution of the genus remain uncertain. There are no data on catch levels or status of wild populations, although available information suggests they are in decline. Brazil's NDF procedures for specimens originating in the wild are inadequate as they rely on 'technical opinion reports', which do not necessarily require scientific evidence. Furthermore, Brazil's NDF procedures exempt the need for NDF reports on 'captive' specimens; however, 'captive' specimens originating in the wild and raised in captivity can be exported because regulations do not specify that they must be 'captive-bred'. Six suggestions are offered to improve the reliability of NDF procedures for Arapaima in Brazil, emphasizing the utility of participatory monitoring and adaptive harvesting to strengthen much needed harvest control capacity in other tropical fisheries.  相似文献

Goblet cell carcinoid of the large intestine is a rare neoplasm, usually located in ascending colon and rectum. A 60-year-old male patient underwent surgery after the diagnosis of acute abdomen. Exploratory laparotomy revealed perforation with a diameter of 1 cm at the site of the previously performed gastroenterostomy and dilatation of the right colic flexure, secondary to a solid obstructive mass located in the mid-portion of transverse colon. Histopathological investigation of the biopsies, taken from the gastroenterostomy site and the tumor, revealed mixed carcinoid-adenocarcinoma with carcinoid component, predominantly composed of goblet cells. Three cycles of FOLFOX-4 protocol was administered. Following respiratory distress secondary to pulmonary metastasis, the patient's condition deteriorated and subsequently died in the fourth postoperative month. Our aim with this paper is to point out that more cases should be reported for more effective diagnosis, histopathological study, clinical investigation, treatment and prognosis of this specific neoplasm.  相似文献
