In this paper we report the clinical utility of the diagnostic guidelines for ICD‐11 mental, behavioural and neurodevelopmental disorders as assessed by 339 clinicians in 1,806 patients in 28 mental health settings in 13 countries. Clinician raters applied the guidelines for schizophrenia and other primary psychotic disorders, mood disorders (depressive and bipolar disorders), anxiety and fear‐related disorders, and disorders specifically associated with stress. Clinician ratings of the clinical utility of the proposed ICD‐11 diagnostic guidelines were very positive overall. The guidelines were perceived as easy to use, corresponding accurately to patients’ presentations (i.e., goodness of fit), clear and understandable, providing an appropriate level of detail, taking about the same or less time than clinicians’ usual practice, and providing useful guidance about distinguishing disorder from normality and from other disorders. Clinicians evaluated the guidelines as less useful for treatment selection and assessing prognosis than for communicating with other health professionals, though the former ratings were still positive overall. Field studies that assess perceived clinical utility of the proposed ICD‐11 diagnostic guidelines among their intended users have very important implications. Classification is the interface between health encounters and health information; if clinicians do not find that a new diagnostic system provides clinically useful information, they are unlikely to apply it consistently and faithfully. This would have a major impact on the validity of aggregated health encounter data used for health policy and decision making. Overall, the results of this study provide considerable reason to be optimistic about the perceived clinical utility of the ICD‐11 among global clinicians. 相似文献

Debaryomyces hansenii exhibited yeast-to-mycelium dimorphism in the continuous fermentation of xylose-containing media made from acid hydrolyzates of barley bran. The lower the dilution rate, the earlier the yeast-to-mycelia transition occurred. Within a selected range of dilution rates, the yeast morphology was reversibly affected by the dissolved O2: low aeration caused the transition from oval cells to hyphae, and further increases in dissolved O2 concentration resulted in recuperation of the oval shape. Under the operational conditions assayed, xylitol was the major fermentation product when the yeast was in both morphological forms, whereas the production of ethanol was increased when the yeast grew under hyphal morphology and oxygen limitation. The lower xylose consumption corresponded to the yeast-to-mycelia transition. In media made with commercial sugars (xylose or glucose), the yeast-to-mycelia transition was induced by adding selected amounts of acid-soluble lignin. 相似文献

A multiplex PCR-based method was designed for the simultaneous detection of the main pathogens involved in warm-water streptococcosis in fish (Streptococcus iniae, Streptococcus difficilis, Streptococcus parauberis, and Lactococcus garvieae). Each of the four pairs of oligonucleotide primers exclusively amplified the targeted gene of the specific microorganism. The sensitivity of the multiplex PCR using purified DNA was 25 pg for S. iniae, 12.5 pg for S. difficilis, 50 pg for S. parauberis, and 30 pg for L. garvieae. The multiplex PCR assay was useful for the specific detection of the four species of bacteria not only in pure culture but also in inoculated fish tissue homogenates and naturally infected fish. Therefore, this method could be a useful alternative to the culture-based method for the routine diagnosis of warm-water streptococcal infections in fish. 相似文献

MicroRNAs (miRNAs) are non-coding gene products that regulate gene expression through specific binding to target mRNAs. Cell-specific patterns of miRNAs are associated with the acquisition and maintenance of a given phenotype, such as endocrine pancreas (islets). We hypothesized that a subset of miRNAs could be differentially expressed in the islets. Using miRNA microarray technology and quantitative RT-PCR we identified a subset of miRNAs that are the most differentially expressed islet miRNAs (ratio islet/acinar > 150-fold), miR-7 being the most abundant. A similarly high ratio for miR-7 was observed in human islets. The ratio islet/acinar for miR-375, a previously described islet miRNA, was <10 and is 2.5× more abundant in the islets than miR-7. Therefore, we conclude that miR-7 is the most abundant endocrine miRNA in islets while miR-375 is the most abundant intra-islet miRNA. Our results may offer new insights into regulatory pathways of islet gene expression. 相似文献

A number of ornamental plant species produce pollen grains with allergenic properties, thus posing a potential risk to human health. A comparative study of the ecological and aerobiological characteristics of ornamental urban flora in four Andalusian cities revealed the presence of over 100 potentially allergenic ornamental plant species. Most of these were phanerophytes, and species of Asiatic origin predominated, followed by Mediterranean and American species. However, not all potentially allergenic ornamental plants were of aerobiological significance, since some produce only small amounts of pollen, while others were infrequent or were located at a considerable distance from the sampler. Even so, when designing new urban parks and gardens, every effort should be made to ensure maximum species variability avoiding the excessive use of any single species. 相似文献

Markers of oxidative stress were measured in blood samples of 338 subjects (965 observations): Alzheimer’s, vascular dementia,
diabetes (type II) superimposed to dementias, Parkinson’s disease and controls. Patients showed increased thiobarbituric acid
reactive substances (+21%; P < 0.05), copper-zinc superoxide dismutase (+64%; P < 0.001) and decreased antioxidant capacity (−28%; P < 0.001); pairs of variables resulted linearly related across groups (P < 0.001). Catalase and glutathione peroxidase, involved in discrimination between diseases, resulted non-significant. When
diabetes is superimposed with dementias, changes resulted less marked but significant. Also, superoxide dismutase resulted
not linearly correlated with any other variable or age-related (pure Alzheimer’s peaks at 70 years, P < 0.001). Systemic oxidative stress was significantly associated (P ≪ 0.001) with all diseases indicating a disbalance in peripheral/adaptive responses to oxidative disorders through different

2-Phenyl propionic acid, ibuprofen and trans-2-phenyl-1-cyclohexanol were resolved using commercial Rhizomucor miehei lipase (Lipozyme IM20) and Candida rugosa lipase produced in our laboratory immobilised on EP100 polypropylene powder. Important differences were found on the enantioselectivity of both lipases in esterification reactions. Candida rugosa lipase was more enantioselective in the resolution of the tested substrates, especially with trans-2-phenyl-1-cyclohexanol, whereas the lipase from Rhizomucor miehei did not show catalytic activity with this substrate. © Rapid Science Ltd. 1998 相似文献

effect of oral creatine supplementation (CR; 5 g/day) in conjunction with exercise training on physical fitness was investigated in men between 55 and 75 yr of age (n = 46). A double-blind randomized placebo-controlled (PL) trial was performed over a 6-mo period. Furthermore, a subgroup (n = 20) completed a 1-yr follow-up. The training program consisted of cardiorespiratory endurance training as well as moderate resistance training (2-3 sessions/wk). Endurance capacity was evaluated during a maximal incremental bicycle ergometer test, maximal isometric strength of the knee-extensor muscles was assessed by an isokinetic dynamometer, and body composition was assessed by hydrostatic weighing. Furthermore, in a subgroup (PL: n = 13; CR: n = 12) biopsies were taken from m. vastus lateralis to determine total creatine (TCr) content. In PL, 6 mo of training increased peak oxygen uptake rate (+16%; P < 0.05). Fat-free mass slightly increased (+0.3 kg; P < 0.05), whereas percent body fat slightly decreased (-1.2%; P < 0.05). The training intervention did not significantly change either maximal isometric strength or body weight. The responses were independent of CR. Still, compared with PL, TCr was increased by approximately 5% in CR, and this increase was closely correlated with initial muscle creatine content (r = -0.78; P < 0.05). After a 1-yr follow-up, muscle TCr was not higher in CR than in PL. Furthermore, the other measurements were not affected by CR. It is concluded that long-term creatine intake (5 g/day) in conjunction with exercise training does not beneficially impact physical fitness in men between 55 and 75 yr of age. 相似文献

The influence of meteorological parameters on the dispersion of airborne pollen has been studied by several authors. Olive pollen is the major cause of allergy in southern Spain, where a large part of the arable surface area is given over to olive cultivation. Daily pollen forecasts provide important information both for pollen-allergy sufferers and for agronomists trying to achieve a better biological understanding of variations in airborne olive pollen levels. The main purpose of this paper is to study, by means of short-term statistical analysis, the effect of meteorological parameters on airborne olive pollen concentrations in the city of Cordoba (south-western Spain). Twenty-one-year (1982–2002) aerobiological and meteorological databases were used. Correlation and multiple regression analyses were used to study the relationships between olive pollen levels and several meteorological parameters. Statistical analysis was applied both to the whole pollen season and to the pre-peak period. Daily meteorological parameters, such as accumulated mean temperature, accumulated sunlight hours, and accumulated rainfall were used as independent variables in both statistical analyses. Respiration is a rhythmic motor behavior that appears in the fetus and acquires a vital importance at birth. It is generated within central pattern-generating neuronal networks of the hindbrain. This region of the brain is of particular interest since it is the most understood part with respect to the cellular and molecular mechanisms that underlie its development. Hox paralogs and Hox-regulating genes kreisler/mafB and Krox20 are required for the normal formation of rhombomeres in vertebrate embryos. From studies of rhombomeres r3 and r4, the authors review mechanisms whereby these developmental genes may govern the early embryonic development of para-facial neuronal networks and specify patterns of motor activities operating throughout life. A model whereby the regional identity of progenitor cells can be abnormally specified in r3 and r4 after a mutation of these genes is proposed. Novel neuronal circuits may develop from some of these misspecified progenitors while others are eliminated, eventually affecting respiration and survival after birth. 相似文献
