Background

The past ten years have witnessed a rising trend in the prevalence and duration of breastfeeding in Italy, but breastfeeding rates increase in an unequal way; they are higher in the North of Italy than in the South. The purpose of this study was to describe the experiences, expectations and beliefs of a sample of mothers, and to identify differences, if any, between the North and the South of Italy.

Methods

The study was conducted in two regions of Italy, Friuli Venezia Giulia in the Northeast and Basilicata in the South. Two hundred and seventy-nine mothers of infants and children 6 to 23 months of age were interviewed using an 85-item questionnaire including closed and open questions on infant feeding experiences and beliefs, sources of information and support, reasons for intended and actual choices and practices, and some demographic and social variables. Face-to-face interviews were conducted between May 2001 and September 2002. Quantitative and qualitative methods were used for data analysis.

Results

The distribution of the mothers by age, education, employment and parity did not differ from that of the general population of the two regions. The reported rates of initiation and duration of breastfeeding were also similar: 95% started breastfeeding, exclusive breastfeeding was 32% at three and 9% at six months, with 64% and 35% of any breastfeeding, respectively. Some differences were reported in the rates of full breastfeeding, reflecting different ages of introduction of non-nutritive fluids. These, as well as nutritive fluids – including infant formula – and complementary foods, were introduced far too early. Advice on infant feeding was generally provided by health professionals and often was not based on up-to-date recommendations. Mothers were generally aware of the advantages of breastfeeding, but at the same time reported problems that they were not able to solve alone or through social and health system support. Most mothers would welcome the support of a peer counsellor. More mothers in Basilicata than in Friuli Venezia Giulia reported difficulties with breastfeeding related to returning to work and were not familiar with their rights on breastfeeding and maternity leave.

Conclusion

Programmes for the protection, promotion and support of breastfeeding in these and similar regions of Italy should concentrate on better training of health professionals with regards to lactation management, communication, and counselling skills. The addition of trained peer counsellors could reinforce the work done by the health system and, through community involvement, could help change social prejudice in the mid- and long-term. The differences between regions should be taken into account in formulating these programmes to avoid increasing, and possibly to decrease, the current gaps.  相似文献

We measured thresholds for microwave-evoked skin sensations of warmth at frequencies of 2.45, 7.5, 10, 35, and 94 GHz. In the same subjects, thresholds of warmth evoked by infrared radiation (IR) were also measured for comparison. Detection thresholds were measured on the skin in the middle of the back in 15 adult male human subjects at all microwave (MW) frequencies and with IR. Long duration (10-s), large area (327-cm2) stimuli were used to minimize any differential effects of temporal or spatial summation. Sensitivity increased monotonically with frequency throughout the range of microwave frequencies tested. The threshold at 94 GHz (4.5 ± 0.6 mW/cm2) was more than an order of magnitude less than at 2.45 GHz (63.1 ± 6.7 mW/cm2), and it was comparable to the threshold for IR (5.34 ± 1.07 mW/cm2). Enhanced expression of the human SIS/PDGF-2 gene has been reported in a number of human cell lines, sarcomas, and glioblastomas. We have analyzed the SIS/PDGF-2 gene for structural alterations in fresh human tumors. DNA samples from 79 patients with solid tumors (63 mesenchymal tumors, 12 lung carcinomas, 4 breast carcinomas) were examined and compared with DNA samples from 50 leukemia patients and 14 unrelated individuals without malignant neoplasms. When DNA samples were digested with a HindIII restriction endonuclease, Southern blot analysis demonstrated two distinct bands (21kb and 18kb) after hybridization to the SIS/PDGF-2 gene probe. A pedigree analysis of a 43-member family indicated that these allelic variants segregated in a Mendelian fashion. There was, however, tumor specific allele loss in 18% of the mesenchymal tumors analyzed, which may indicate a common etiology for this tumor type.  相似文献

Network analyses provide a unified framework to evaluate different types of species interactions. We used a network approach to comparatively evaluate three types of arboreal plant metacommunities. Interactions between mistletoes, lianas and epiphytes and their host trees were quantified in two New Zealand forests and individual-based null models were used to test for non-random patterns in network degree, nestedness and negative co-occurrence patterns. Arboreal plants were simulated to randomly occur on individual host trees to derive ‘null’ interaction matrices, which were then compared to the observed matrix. Results showed that mistletoes, lianas and epiphytes had very different network properties. Mistletoe and liana degree distributions exhibited fewer links than expected under the null model, indicating strong host preferences. Conversely, degree distributions for epiphytes were consistent with randomised expectations. Mistletoes and lianas were less nested than null model expectations and instead showed support for negative co-occurrence patterns, meaning mistletoe and liana species tended to have mutually exclusive host preferences. Conversely, epiphytes were more nested than expected by chance and showed positive co-occurrence patterns. Overall results indicate that plant–plant interactions exhibited by different types of arboreal plants have very different network properties. We hypothesize that these differences result from (1) differences in coevolutionary dynamics between arboreal plants and their hosts, which range from parasitic (mistletoes) to commensal (epiphytes), and (2) biotic interactions among arboreal plant species for access to host trees.  相似文献

Since 1994, our research has demonstrated how thermophysiological responses are mobilized in human volunteers exposed to three radio frequencies, 100, 450, and 2450 MHz. A significant gap in this frequency range is now filled by the present study, conducted at 220 MHz. Thermoregulatory responses of heat loss and heat production were measured in six adult volunteers (five males, one female, aged 24-63 years) during 45 min whole body dorsal exposures to 220 MHz radio frequency (RF) energy. Three power densities (PD = 9, 12, and 15 mW/cm(2) [1 mW/cm(2) = 10 W/m(2)], whole body average normalized specific absorption rate [SAR] = 0.045 [W/kg]/[mW/cm(2)] = 0.0045 [W/kg]/[W/m(2)]) were tested at each of three ambient temperatures (T(a) = 24, 28, and 31 degrees C) plus T(a) controls (no RF). Measured responses included esophageal (T(esoph)) and seven skin temperatures (T(sk)), metabolic rate (M), local sweat rate, and local skin blood flow (SkBF). Derived measures included heart rate (HR), respiration rate, and total evaporative water loss (EWL). Finite difference-time domain (FDTD) modeling of a seated 70 kg human exposed to 220 MHz predicted six localized "hot spots" at which local temperatures were also measured. No changes in M occurred under any test condition, while T(esoph) showed small changes (< or =0.35 degrees C) but never exceeded 37.3 degrees C. As with similar exposures at 100 MHz, local T(sk) changed little and modest increases in SkBF were recorded. At 220 MHz, vigorous sweating occurred at PD = 12 and 15 mW/cm(2), with sweating levels higher than those observed for equivalent PD at 100 MHz. Predicted "hot spots" were confirmed by local temperature measurements. The FDTD model showed the local SAR in deep neural tissues that harbor temperature-sensitive neurons (e.g., brainstem, spinal cord) to be greater at 220 than at 100 MHz. Human exposure at both 220 and 100 MHz results in far less skin heating than occurs during exposure at 450 MHz. However, the exposed subjects thermoregulate efficiently because of increased heat loss responses, particularly sweating. It is clear that these responses are controlled by neural signals from thermosensors deep in the brainstem and spinal cord, rather than those in the skin.  相似文献

The combination of inorganic semiconductor nanocrystals, also called quantum dots (QDs), with biological materials has recently attracted considerable interest since the QDs can be used as superior fluorescent labels. Here, we report on CdSe QD initiated current bursts in lipid bilayer membranes on application of a bias voltage. The current bursts observed resemble those produced by the peptaibol class of antibiotics such as alamethicin and trichorzins. The current fluctuations are dependent on the bias voltage and on the concentration of the QD applied to the membrane. Our data suggest that QDs with intrinsic dipole moments similar to alamethicin can be controlled by an external electric field, which creates a torque resulting in the insertion into the lipid membrane.  相似文献

As stem cells undergo differentiation, mitochondrial DNA (mtDNA) copy number is strictly regulated in order that specialized cells can generate appropriate levels of adenosine triphosphate (ATP) through oxidative phosphorylation (OXPHOS) to undertake their specific functions. It is not understood whether tumor-initiating cells regulate their mtDNA in a similar manner or whether mtDNA is essential for tumorigenesis. We show that human neural stem cells (hNSCs) increased their mtDNA content during differentiation in a process that was mediated by a synergistic relationship between the nuclear and mitochondrial genomes and results in increased respiratory capacity. Differentiating multipotent glioblastoma cells failed to match the expansion in mtDNA copy number, patterns of gene expression and increased respiratory capacity observed in hNSCs. Partial depletion of glioblastoma cell mtDNA rescued mtDNA replication events and enhanced cell differentiation. However, prolonged depletion resulted in impaired mtDNA replication, reduced proliferation and induced the expression of early developmental and pro-survival markers including POU class 5 homeobox 1 (OCT4) and sonic hedgehog (SHH). The transfer of glioblastoma cells depleted to varying degrees of their mtDNA content into immunocompromised mice resulted in tumors requiring significantly longer to form compared with non-depleted cells. The number of tumors formed and the time to tumor formation was relative to the degree of mtDNA depletion. The tumors derived from mtDNA depleted glioblastoma cells recovered their mtDNA copy number as part of the tumor formation process. These outcomes demonstrate the importance of mtDNA to the initiation and maintenance of tumorigenesis in glioblastoma multiforme.  相似文献

Flowery field margins and intersowing of crops with flowers are used as management practices to promote arthropod biodiversity as well as biocontrol agents. Positive effects of enhancement (in abundance and species richness) of hymenopteran parasitoids on control of Lepidoptera pests have previously been demonstrated. However, effects on predatory arthropods, which may also serve as pest control agents, remain unclear. In an experimental study in cabbage fields we tested how sown flower strips on field margins and intersowing with cornflower affected the species richness, abundance and community composition of ground beetles and spiders. Furthermore, we investigated whether effects of flower margins are dependent on the distance from the field margins. We found that field margins generally harboured higher species richness, whereas effects on abundance were weaker. Intersown cornflower had positive effects on spider and ground beetle abundance, but affected species richness only weakly. Our results do not provide evidence for effects of distance from the flowery field margins on predator richness or abundance. Species composition was strongly affected by the habitat management actions. We conclude that habitat management practices like flower strips on field margins and intersowing with flowers, which are primarily added to attract and enhance parasitoids for pest control, also benefit biodiversity conservation in spiders and ground beetles. They also positively affect the abundance of these primarily predatory taxa, which adds to the biocontrol potential of non-crop flowering plants.  相似文献

We investigated the effects of the matrix metalloproteinase 13 (MMP13)-selective inhibitor, 5-(4-{4-[4-(4-fluorophenyl)-1,3-oxazol-2-yl]phenoxy}phenoxy)-5-(2-methoxyethyl) pyrimidine-2,4,6(1H,3H,5H)-trione (Cmpd-1), on the primary tumor growth and breast cancer-associated bone remodeling using xenograft and syngeneic mouse models. We used human breast cancer MDA-MB-231 cells inoculated into the mammary fat pad and left ventricle of BALB/c Nu/Nu mice, respectively, and spontaneously metastasizing 4T1.2-Luc mouse mammary cells inoculated into mammary fat pad of BALB/c mice. In a prevention setting, treatment with Cmpd-1 markedly delayed the growth of primary tumors in both models, and reduced the onset and severity of osteolytic lesions in the MDA-MB-231 intracardiac model. Intervention treatment with Cmpd-1 on established MDA-MB-231 primary tumors also significantly inhibited subsequent growth. In contrast, no effects of Cmpd-1 were observed on soft organ metastatic burden following intracardiac or mammary fat pad inoculations of MDA-MB-231 and 4T1.2-Luc cells respectively. MMP13 immunostaining of clinical primary breast tumors and experimental mice tumors revealed intra-tumoral and stromal expression in most tumors, and vasculature expression in all. MMP13 was also detected in osteoblasts in clinical samples of breast-to-bone metastases. The data suggest that MMP13-selective inhibitors, which lack musculoskeletal side effects, may have therapeutic potential both in primary breast cancer and cancer-induced bone osteolysis.  相似文献
