Poor balance control and increased fall risk have been reported in people with diabetic peripheral neuropathy (DPN). Traditional body sway measures are unable to describe underlying postural control mechanism. In the current study, we used stabilogram diffusion analysis to examine the mechanism under which balance is altered in DPN patients under local-control (postural muscle control) and central-control (postural control using sensory cueing). DPN patients and healthy age-matched adults over 55 years performed two 15-second Romberg balance trials. Center of gravity sway was measured using a motion tracker system based on wearable inertial sensors, and used to derive body sway and local/central control balance parameters. Eighteen DPN patients (age = 65.4±7.6 years; BMI = 29.3±5.3 kg/m2) and 18 age-matched healthy controls (age = 69.8±2.9; BMI = 27.0±4.1 kg/m2) with no major mobility disorder were recruited. The rate of sway within local-control was significantly higher in the DPN group by 49% (healthy local-controlslope = 1.23±1.06×10-2 cm2/sec, P<0.01), which suggests a compromised local-control balance behavior in DPN patients. Unlike local-control, the rate of sway within central-control was 60% smaller in the DPN group (healthy central-controlslope-Log = 0.39±0.23, P<0.02), which suggests an adaptation mechanism to reduce the overall body sway in DPN patients. Interestingly, significant negative correlations were observed between central-control rate of sway with neuropathy severity (rPearson = 0.65-085, P<0.05) and the history of diabetes (rPearson = 0.58-071, P<0.05). Results suggest that in the lack of sensory feedback cueing, DPN participants were highly unstable compared to controls. However, as soon as they perceived the magnitude of sway using sensory feedback, they chose a high rigid postural control strategy, probably due to high concerns for fall, which may increase the energy cost during extended period of standing; the adaptation mechanism using sensory feedback depends on the level of neuropathy and the history of diabetes. 相似文献

We studied the influence of site conditions on epiphytic orchids under a subtropical climate in the Kathmandu Valley, Nepal. We analysed 96 systematically distributed grid points situated in Kathmandu Valley across a land-use intensity gradient (national park to urbanised city area). Geographical Information System (GIS) and remote sensing were used for classification of land-use types. We identified 23 species of epiphytic orchids, within 13 genera, from 42 different host tree species. Host preference is obvious for some orchid species (e.g., Dendrobium nobile), with certain tree species (e.g., Schima wallichii, Ficus religiosa) hosting more orchid species than others. The orchid Rhynchostylis retusa was the most common species found on many different host tree species across the land-use intensity gradient. Host species and host bark characteristics (e.g., rugosity, pH and exposure to wind) played a vital role for orchid distribution, with lower abundance in areas of higher impact. Under strong human impact (urban city area), F. religiosa was the dominant host tree, with large individual trees (mean diameter in breast height, dbh?=?1.3?m) providing the habitat for considerable populations of R. retusa individuals. In general, epiphytic orchids were found on larger host trees in urban areas than in areas of lower human impact. We found that some hosts are more likely to harbour orchid species, especially native host species. Older larger trees with rougher bark, low pH, exposed to wind and reduced human impact provided better habitats for orchids. The purpose of this study was to evaluate a new method showing how custom foot orthoses (CFO) improve dynamics of plantar loading. The method is based on the probability distribution of peak pressure time series and is quantified using the Regression Factor (RF). RF is a least square regression slope between the experimentally observed plantar pressure magnitude probability distribution and a modeled Gaussian shape. Plantar pressure data from a randomized controlled trial of 154 participants with painful Pes Cavus were retrospectively re-analyzed. The participants were randomized to an active treatment group given CFO or a control group given sham orthoses. The location of 2(nd) Peak pressure as a percentage of stance time (P(Loc2)) and its magnitude (P(M2)) was also calculated. In addition, plantar pressure data were collected on 23 healthy volunteers with normal foot alignment and no foot pain. Results demonstrated Pes Cavus had a significantly lower RF than healthy participants (0.30 v. 0.51; p<10(-7)). P(M2) was reduced in both active and control groups. However, RF and the P(Loc2) were only changed in the active group (p<0.005) without any significant change in the control group (p>0.5). This study suggests that painful Pes Cavus alters the shape of probability distribution of plantar loading during walking and CFO are an effective therapeutic solution that can significantly improve it. Further use of the RF index and 2(nd) peak pressure location as an outcome measure for treatment of foot and ankle deformities is suggested. 相似文献

Indole-3-acetic acid (IAA) strongly enhanced rooting of etiolated pea epicotyl cuttings while gibberellic acid (GA3) enhanced rooting only slightly. The promoting effects of the hormones appeared not until 14 d after the onset of treatment. When GA3 and IAA were applied together, the initiation of rooting started already after 6 d after onset of treatment.
