Background
Drug-Drug Interactions between Non Steroidal Anti-Inflammatory Drugs (NSAIDs) and Angiotensin Converting Enzyme Inhibitors (ACEIs), Angiotensin Receptor Blocker (ARBs) or diuretics can lead to renal failure and hyperkalemia. Thus, monitoring of serum creatinine and potassium is recommended when a first dispensing of NSAID occur in patients treated with these drugs.Methods
We conducted a pharmacoepidemiological retrospective cohort study using data from the French Health Insurance Reimbursement Database to evaluate the proportion of serum creatinine and potassium laboratory monitoring in patients treated with ACEI, ARB or diuretic and receiving a first dispensing of NSAID. We described the first dispensing of NSAID among 3,500 patients of a 4-year cohort (6,633 patients treated with antihypertensive drugs) and analyzed serum creatinine and potassium laboratory monitoring within the 3 weeks after the first NSAID dispensing.Results
General Practitioners were the most frequent prescribers of NSAIDs (85.5%, 95% CI: 84.3–86.6). The more commonly prescribed NSAIDs were ibuprofen (20%), ketoprofen (15%), diclofenac (15%) and piroxicam (12%). Serum creatinine and potassium monitoring was 10.7% (95% CI: 9.5–11.8) in patients treated by ACEIs, ARBs or diuretics. Overall, monitoring was more frequently performed to women aged over 60, treated with digoxin or glucose lowering drugs, but not to patients treated with ACEIs, ARBs or diuretics. Monitoring was more frequent when NSAIDs'' prescribers were cardiologists or anesthesiologists.Conclusion
Monitoring of serum creatinine and potassium of patients treated with ACEIs, ARBs or diuretics and receiving a first NSAID dispensing is insufficiently performed and needs to be reinforced through specific interventions. 相似文献92.
Agreement among health care professionals in diagnosing case Vignette-based surgical site infections
Objective
To assess agreement in diagnosing surgical site infection (SSI) among healthcare professionals involved in SSI surveillance.Methods
Case-vignette study done in 2009 in 140 healthcare professionals from seven specialties (20 in each specialty, Anesthesiologists, Surgeons, Public health specialists, Infection control physicians, Infection control nurses, Infectious diseases specialists, Microbiologists) in 29 University and 36 non-University hospitals in France. We developed 40 case-vignettes based on cardiac and gastrointestinal surgery patients with suspected SSI. Each participant scored six randomly assigned case-vignettes before and after reading the SSI definition on an online secure relational database. The intraclass correlation coefficient (ICC) was used to assess agreement regarding SSI diagnosis on a seven-point Likert scale and the kappa coefficient to assess agreement for superficial or deep SSI on a three-point scale.Results
Based on a consensus, SSI was present in 21 of 40 vignettes (52.5%). Intraspecialty agreement for SSI diagnosis ranged across specialties from 0.15 (95% confidence interval, 0.00–0.59) (anesthesiologists and infection control nurses) to 0.73 (0.32–0.90) (infectious diseases specialists). Reading the SSI definition improved agreement in the specialties with poor initial agreement. Intraspecialty agreement for superficial or deep SSI ranged from 0.10 (−0.19–0.38) to 0.54 (0.25–0.83) (surgeons) and increased after reading the SSI definition only among the infection control nurses from 0.10 (−0.19–0.38) to 0.41 (−0.09–0.72). Interspecialty agreement for SSI diagnosis was 0.36 (0.22–0.54) and increased to 0.47 (0.31–0.64) after reading the SSI definition.Conclusion
Among healthcare professionals evaluating case-vignettes for possible surgical site infection, there was large disagreement in diagnosis that varied both between and within specialties. 相似文献93.

Poly(ADP-ribosyl)ation (PARylation), a protein post-translational modification that was originally connected to the DNA damage response, is now known to engage in a continuously increasing number of biological processes. Despite extensive research and ceaseless, important findings about its role and mode of action, poly(ADP-ribose) remains an enigma regarding its structural complexity and diversity. The recent identification and structural characterization of four different poly(ADP-ribose) binding motifs represents a quantum leap in the comprehension of how this molecule can be decoded. Moreover, the recent discovery of a direct connection between PARylation and poly-ubiquitylation in targeting proteins for degradation by the proteasome has paved the way for a new interpretation of this protein modification. Peripheral T lymphocytes recirculating back into the thymus can mediate thymocyte positive selection

The thymus continuously produces T lymphocytes that contribute to the maintenance of the peripheral T cell pool. Since peripheral recirculating T cells represent a very minor population among total thymocytes in normal animals, the relationship between the thymus and secondary lymphoid organs is generally considered unidirectional. Recently, several reports have described the presence of recirculating T cells in the thymus, raising issues regarding their possible function. In this article, we show that the niche for recirculating T cells in the thymus, i.e., their absolute number, is the same in lymphopenic and normal mice. Using a novel combination of TCR-transgenic mice in which the ligand necessary for positive selection of host T cells is only expressed by transferred donor T cells, we show that mature T cells recirculating back to the thymus can mediate positive selection. 相似文献

Swarming without positioning information is interesting in application-oriented systems because it alleviates the need for sensors which are dependent on the environment, expensive in terms of energy, cost, size and weight, or unusable at useful ranges for real-life scenarios. This principle is applied to the development of a swarm of micro air vehicles (SMAVs) for the deployment of ad hoc wireless communication networks (SMAVNETs) between ground users in disaster areas. Rather than relying on positioning information, MAVs rely on local communication with immediate neighbors and proprioceptive sensors which provide heading, speed and altitude. To solve the challenging task of designing agent controllers to achieve the swarm behavior of the SMAVNET, inspiration is taken from army ants which are capable of laying and maintaining pheromone paths leading from their nest to food sources in nature. This is analogous to the deployment of communication pathways between multiple ground users. However, instead of being physically deposited in the air or on a map, pheromone is virtually deposited on the MAVs using local communication. The aim of this work was to investigate the effect of supplementation of a maternal alcohol diet with a grape extract on glial
cell development. Glial cells were cultured during 4 weeks from cortical brain cells of the new born offspring in DMEM medium
supplemented with fetal calf serum. Enzymatic markers of nerve cell development were measured (enolase isoenzymes and glutamine
synthetase). Since alcohol consumption produces free radicals the antioxidant system superoxide dismutase was also investigated.
Compared to the decrease found in only alcohol treated animals, all parameters except neuron-specific enolase were antagonized
and even stimulated after grape extract supplementation. The effect was more important after only 1 month than 3 months of
treatment. Also in the total brain an alcohol antagonizing effect and a glutamine synthetase activation were found. Our data
demonstrate that addition of a grape extract to the maternal alcohol diet may partially or completely overcome the alcohol

A rapid method was developed for detecting in soil Desulfitobacterium frappieri strain PCP-1, an anaerobic gram-positive bacterium, isolated from a methanogenic consortium degrading pentachlorophenol.
The method involved the establishment of a protocol for extracting total DNA from soil with the least contamination, and the
use of the polymerase chain reaction (PCR) to detect strain PCP-1 with primers targeted with PCP-1 16S rRNA. To optimize the
DNA extraction conditions, a glass mill homogenizer and a low-salt buffer containing polyvinylpolypyrrolidone were used on
a black soil rich in organic matter. Recovered DNA was further purified with phenol/chloroform extractions, ammonium acetate
precipitation and a G200 Sephadex gel-filtration column. DNA was extracted from soil supplemented with different concentrations
of PCP-1 cells. Detection of PCP-1 was by PCR. The limit of detection was 800 added PCP-1 cells/g dry soil. This level of
detection was achieved when the T4 gene-32 protein and 1 μg soil DNA were added to the PCR mixture followed by a nested PCR.
This method is quick, sensitive, and can process several samples at the same time.
