Thrombosis is the end result of two closely interrelated processes: the coagulation cascade and the platelet aggregation process. To determine their relative contribution, we used pharmacologic agents that selectively block each process. The specific effect of each pharmacologic agent on either fibrin deposition or platelet activity was confirmed morphologically by scanning electron microscopy and was substantiated with ADP-induced platelet aggregation and blood clotting time determinations. Forty-two rats had both femoral arteries subjected to a standardized crush-avulsion injury. A total of 84 femoral microvascular anastomoses were subsequently performed. None of the 24 control anastomoses treated with saline remained patent, whereas 6 of 24 of the anastomoses treated with dazmagrel (a selective thromboxane synthetase and platelet aggregation inhibitor), 2.5 mg/kg IV, remained patent and 18 of 24 of those treated with a single dose of heparin, 200 U/kg IV, remained patent. All 12 anastomoses treated with both drugs remained patent but developed a 33 percent hematoma rate. We conclude that in this microvascular model, fibrin mesh deposition is a more significant factor than platelet aggregation in the pathogenesis of occlusional thrombosis within traumatized arteries. Its temporary inhibition with a single dose of heparin yielded a 75 percent improvement in patency rate.  相似文献

In this experimental study, venous end-to-end and end-to-side microvascular anastomoses in similar and diameter-discrepant vessels were compared. In 50 rats, end-to-end microvascular repair of the divided epigastric vein and end-to-side repair of the epigastric vein into the femoral vein showed 5-day patency rates of 75 and 88 percent, respectively. These data are not statistically different. In 20 rats, microvascular repair of end epigastric to end femoral veins (size discrepant) and end epigastric to side femoral veins showed 5-day patency rates of 50 and 85 percent, respectively. These data are statistically different (p less than 0.05). We conclude from these experimental data that end-to-side venous repairs may be useful in lowering the anastomosis thrombosis rate seen when size-discrepant veins are repaired.  相似文献

A large percentage of arteriovenous haemodialysis angioaccess loop grafts (AVLG) fail within the first year after surgery, the occlusive lesions being found predominantly at the venous anastomosis site. This paper presents a detailed flow dynamic study of the AVLG system using three elastic, transparent bench-top flow models, which were based on the geometry of silicone rubber casts obtained at different times from a chronic animal model. Each model thus represented a different stage of the lesion development. Flow visualization and laser Doppler anemometer surveys of the flow field confirmed that the hydrodynamic factors favour lesion development near the stagnation point opposite the anastomotic toe, where the momentum of the impinging jet stream, combined with the oscillating wall shear stress generated in the vicinity of the stagnation point, acts in both directions. The accumulation of tracer particles in the region of flow separation is believed to be a combined contribution from the hydraulic forces and the inward motion of the vessel wall. As these hydrodynamic factors are enhanced upon further development of the occlusive lesion, a vicious cycle may be formed.  相似文献

Metacarpophalangeal joints in dogs were transplanted with microvascular anastomoses. Some were transplanted as half-joints, others as whole joints, and these dogs were followed for up to 5 1/2 months. When a half-joint was transplanted within the foot, the joint movement became restricted but the vascularized half-joints were well preserved with only slight damage and well-preserved epiphyseal plates--compared to severe damage of the articular cartilage and destruction of the epiphyseal plates in the non-vascularized joint transplants. Vascularized whole joint transplants within the foot were indistinguishable macroscopically and microscopically from normal joints, and they had only a slight restriction of joint movement. They took up tetracycline from the circulation, demonstrating their viability.  相似文献

Blood flow was measured in the canine saphenous artery using electromagnetic flowmetry. Significant increase in blood flow was noted after occlusion of the distal femoral artery. However, after raising a saphenous island flap there was no significant change in the blood flow before and after distal femoral artery occlusion. The flap peripheral resistance and blood flow were compared after end-to-end and end-to-side anastomosis and no statistical difference was noted.  相似文献

A new sutureless technique to successfully anastomose the abdominal aorta of rats (1.3 mm in diameter) by using a fully biodegradable, laser-activated protein solder is presented. A total of 90 rats were divided into two groups randomly. In group one, the anastomoses were performed by using conventional microsuturing technique, whereas in group two, the anastomoses were performed by using a new laser welding technique. In addition, each of the two groups were divided into five subgroups and evaluated at different follow-up periods (10 minutes, 1 hour, 1 day, 1 week, and 6 weeks). At these intervals, the anastomoses were evaluated for patency and tensile strength. Three anastomoses in each subgroup were processed for light and electron microscopy. All anastomoses were found to be patent. The mean clamp time of the anastomoses performed with conventional suturing was 20.6 minutes compared with 7.2 minutes for the laser-activated welded anastomoses (p < 0.001). The strain measurements showed a stronger mechanical bond of the sutured anastomoses in the initial phase. However, at 6 weeks the tensile strength of the laser-welded anastomoses was higher compared with the conventional suture technique. Histologic evaluations revealed a near complete resorption of the solder after 6 weeks. The junction site of the vessel ends cannot be determined on the luminal side of the artery. In conclusion, a resorbable protein used as a solder, activated by a diode laser, can provide a reliable, safe, and rapid arterial anastomosis, which could be performed by any microsurgeon faster than conventional suturing after a short learning curve.  相似文献
