As a consequence of habitat loss, breeding in man-made habitats has become increasingly common for many coastal breeding bird species. While artificial sites provide valuable substitutes, they may also be more attractive, and importantly, differ in quality from natural sites. Therefore, information on habitat specific breeding success and their potential for supporting stable populations are needed. We compared little tern (Sternula albifrons) breeding success (nest and hatching success) between natural habitat (sandy beaches) and artificial port habitat at Bothnian Bay, Finland from 2006 to 2011. We further reviewed published estimates on pre-fledging and adult survival for little terns and least terns (Sternula antillarum), and used these ranges to estimate plausible parameter spaces for population growth rates given our estimates of breeding success. Nest success was among the highest reported for little terns in the artificial habitat (82 %) while being lower in the natural habitat (58 %). This difference may have resulted from differences in colony sizes and levels of disturbance. Hatching success did not differ significantly, but the percentage of successful nests containing unhatched eggs was twice as high in the natural habitat. The parameter spaces for population growth rates indicated that the artificial habitat has good potential to sustain stable populations (66 % positive growth rate) while for the natural habitat this potential was lower (37 % positive growth rate). While our results suggest that artificial habitats can be very productive breeding sites for habitat deprived tern populations, management should concentrate on improving both habitats with emphasis on natural sites.  相似文献

One of the few examples of explicit rationing in the National Health Service is provided by in vitro fertilisation. Of six purchasing authorities examined three have decided against buying in vitro fertilisation while three have decided in favour. The decisions reflect local factors such as the absence or presence of local providers and the views of the public and health professionals. But in vitro fertilisation also illustrates some of the wider issues involved in all decisions about purchasing: questions about what should be provided by the National Health Service, about what procedures should be compared when weighing up value for money, and whether equity demands national decisions about what to provide.  相似文献

Objective: To perform an oral biopsy survey focused on the incidence of oral lesions in Brazilian elderly patients. Methods: A total of 17 329 oral biopsy records were analysed and divided into two age groups: elderly patients, ≥60 years old; and non‐elderly patients, <60 years old. Information about sex, race, age and histopathological diagnosis (categorised in non‐neoplastic and neoplastic lesions) was collected. Differences of diagnosis incidence were tested by comparison between two proportions (binomial test). Results: The incidence of epithelial malignant neoplasms and pre‐malignant lesions in the elderly group was higher than non‐elderly group, as well as autoimmune diseases and salivary gland tumours. The three most prevalent lesions in the elderly group were inflammatory fibrous hyperplasia, squamous cell carcinoma, and fibroma. Conclusion: The distribution of oral diseases using biopsies allows greater accuracy in data about oral health of elderly patients, especially when considering malignant and pre‐malignant lesions.  相似文献

A fertilisation cohort of 244 pregnancies resulting from in vitro fertilisation was reported to a national register by eight units specialising in in vitro fertilisation. Early pregnancy losses were high, with 5% tubal ectopic pregnancies, 18% biochemical pregnancies, and an incidence of spontaneous abortion of 27%. Among pregnancies of at least 20 weeks'' gestation 22% were multiple, with 26 pairs of twins and four sets of triplets. The incidence of preterm births was more than three times higher than in the general population. Low birthweight rates were also higher, owing both to preterm births and to multiple pregnancies. The sex ratio and the incidence of major congenital malformations were similar to those in naturally conceived pregnancies. In this series the high fetal losses at all stages of pregnancy suggested maternal reproductive causes and should not be interpreted as being due to factors peculiar to in vitro fertilisation. Further analysis will be necessary when larger numbers are available.  相似文献

Test day milk yields of three lactations in Sfakia sheep were analyzed fitting a random regression (RR) model, regressing on orthogonal polynomials of the stage of the lactation period, i.e. days in milk. Univariate (UV) and multivariate (MV) analyses were also performed for four stages of the lactation period, represented by average days in milk, i.e. 15, 45, 70 and 105 days, to compare estimates obtained from RR models with estimates from UV and MV analyses. The total number of test day records were 790, 1314 and 1041 obtained from 214, 342 and 303 ewes in the first, second and third lactation, respectively. Error variances and covariances between regression coefficients were estimated by restricted maximum likelihood. Models were compared using likelihood ratio tests (LRTs). Log likelihoods were not significantly reduced when the rank of the orthogonal Legendre polynomials (LPs) of lactation stage was reduced from 4 to 2 and homogenous variances for lactation stages within lactations were considered. Mean weighted heritability estimates with RR models were 0.19, 0.09 and 0.08 for first, second and third lactation, respectively. The respective estimates obtained from UV analyses were 0.14, 0.12 and 0.08, respectively. Mean permanent environmental variance, as a proportion of the total, was high at all stages and lactations ranging from 0.54 to 0.71. Within lactations, genetic and permanent environmental correlations between lactation stages were in the range from 0.36 to 0.99 and 0.76 to 0.99, respectively. Genetic parameters for additive genetic and permanent environmental effects obtained from RR models were different from those obtained from UV and MV analyses.  相似文献

Background

This study assessed the cost-effectiveness of indacaterol/glycopyrronium (IND/GLY) versus salmeterol/fluticasone (SFC) in chronic obstructive pulmonary disease (COPD) patients with moderate to very severe airflow limitation and ≥1 exacerbation in the preceding year.

Methods

A previously published and validated patient-level simulation model was adapted using clinical data from the FLAME trial and real-world cost data from the ARCTIC study. Costs (total monetary costs comprising drug, maintenance, exacerbation, and pneumonia costs) and health outcomes (life-years (LYs), quality-adjusted life-years (QALYs)) were projected over various time horizons (1, 5, 10 years, and lifetime) from the Swedish payer’s perspective and were discounted at 3% annually. Uncertainty in model input values was studied through one-way and probabilistic sensitivity analyses. Subgroup analyses were also performed.

Results

IND/GLY was associated with lower costs and better outcomes compared with SFC over all the analysed time horizons. Use of IND/GLY resulted in additional 0.192 LYs and 0.134 QALYs with cost savings of €1211 compared with SFC over lifetime. The net monetary benefit (NMB) was estimated to be €8560 based on a willingness-to-pay threshold of €55,000/QALY. The NMB was higher in the following subgroups: severe (GOLD 3), high risk and more symptoms (GOLD D), females, and current smokers.

Conclusion

IND/GLY is a cost-effective treatment compared with SFC in COPD patients with mMRC dyspnea grade?≥?2, moderate to very severe airflow limitation, and ≥1 exacerbation in the preceding year.

Objective: To determine the size of reduction in homocysteine concentrations produced by dietary supplementation with folic acid and with vitamins B-12 or B-6. Design: Meta-analysis of randomised controlled trials that assessed the effects of folic acid based supplements on blood homocysteine concentrations. Multivariate regression analysis was used to determine the effects on homocysteine concentrations of different doses of folic acid and of the addition of vitamin B-12 or B-6. Subjects: Individual data on 1114 people included in 12 trials. Findings: The proportional and absolute reductions in blood homocysteine produced by folic acid supplements were greater at higher pretreatment blood homocysteine concentrations (P<0.001) and at lower pretreatment blood folate concentrations (P<0.001). After standardisation to pretreatment blood concentrations of homocysteine of 12 μmol/l and of folate of 12 nmol/l (approximate average concentrations for Western populations), dietary folic acid reduced blood homocysteine concentrations by 25% (95% confidence interval 23% to 28%; P<0.001), with similar effects in the range of 0.5-5 mg folic acid daily. Vitamin B-12 (mean 0.5 mg daily) produced an additional 7% (3% to 10%) reduction in blood homocysteine. Vitamin B-6 (mean 16.5 mg daily) did not have a significant additional effect. Conclusions: Typically in Western populations, daily supplementation with both 0.5-5 mg folic acid and about 0.5 mg vitamin B-12 would be expected to reduce blood homocysteine concentrations by about a quarter to a third (for example, from about 12 μmol/l to 8-9 μmol/l). Large scale randomised trials of such regimens in high risk populations are now needed to determine whether lowering blood homocysteine concentrations reduces the risk of vascular disease.

BACKGROUND: Recent studies reported no reduction in the frequency of lipomeningomyelocele (LMMC) in Hawaii and Nova Scotia after the implementation of a folic acid food fortification policy in 1998, while a marked reduction in the prevalence of other NTDs was observed. This study was performed to assess the prevalence of LMMC in Canada in relation to the timing of food fortification. METHODS: The study population included livebirths, stillbirths, and terminations of pregnancies because of fetal anomaly to women residing in seven Canadian provinces, from 1993 to 2002. In each province, the ascertainment of NTD cases relied on multiple sources, and in addition all medical charts were reviewed. The study period was divided into pre‐, partial, and full fortification periods, based on results of red cell folate tests published in the literature. RESULTS: A total of 86 LMMC cases were recorded among approximately 1.9 million live births. The average birth prevalence rate was 0.05/1,000, ranging from a minimum of 0.01/1,000 in 2002 to a maximum of 0.08/1,000 in 1999. There was statistical heterogeneity between years (p = .01), but no pattern compatible with a decrease following fortification. Comparing the full fortification period with the prefortification period, there was a slight but not statistically significant decrease in LMMC birth prevalence. CONCLUSIONS: LMMC seems to be pathogenically distinct from myelomeningocele and more studies are needed to understand the embryologic mechanisms leading to this condition, and the environmental and genetic factors involved in its etiology. Arachidonic acid (AA) metabolism in the non-pregnant sheep uterus was studied in vitro using conventional chromatographic and HPLC techniques. High expression of both lipoxygenase (LOX) as well as cyclooxygenase (COX) enzymes and their activities was found in the uterine tissues. On incubation of uterine enymes with AA, the LOX products formed were identified as 5-hydroperoxyeicosatetraenoic acid (5-HPETE), 12- and 15-hydroxyeicosatetraenoic acids (12- and 15-HETEs), based on their separation on TLC and HPLC. By employing differential salt precipitation techniques, the LOXs generating products 5-HPETE (5-LOX), 12-HETE and 15-HETE (12- and 15-dual LOX) were isolated. Based on their analysis on TLC, the COX products formed were identified as prostaglandins - PGF2alpha and prostacyclin derivative 6-keto PGF1alpha. The study forms the first report on the comprehensive analysis on the metabolism of AA in sheep uterus in vitro via the LOX and COX pathways.  相似文献

For the first time, we applied cell-population based mass spectrometry (CP-MS) for biosensing intact eukaryotic cells of Chlamydomonas reinhardtii and Saccharomyces cerevisiae. Cell counts ranging from 1 × 10(7) to 1.28 × 10(2) were analyzed using MALDI-MS to obtain the threshold detection sensitivity. Platinum nanodots (Pt NDs) were used to enhance the detection sensitivity of CP-MS. Pt NDs were able to improve the detection sensitivity of CP-MS from 3200 cells/mL to 640 cells/mL (5-fold) for Chlamydomonas. For yeast cells, the detection sensitivity was also increased from 400,000 cells/mL to 3200 cells/mL (125-fold) when Pt NDs were used. Using the Clin Pro tool, the obtained results from MALDI-MS data were validated. Statistical analysis of the mass data was performed using MYSTAT software.  相似文献
