This study was designed to determine whether significant differences in saturation levels existed among patients with aspiration and patients without and wether pulse oximetry can reliably detect aspiration in patients with dysphagia. We also examined the effects of gender and disease (neurologic versus non neurologic) on saturation levels. We studied 38 patients. They all underwent a videofluoroscopic study of swallowing (VFSS). Twenty patients aspirated on videofluoroscopic study of swallowing: ten patients were solid aspirators, ten patients were liquid aspirators. In each group (liquid aspirators, solid aspirators or non aspirators) we found no significant difference in saturation levels. We found however a significant difference in saturation levels between each group before, during and after videofluoroscopic study of swallowing. Both gender and disease had an effect on saturation levels. We conclude that pulse oximetry can not serve as a screening tool for detection of aspiration as saturation levels are dependent on many factors. Therefore one can not reliably predict aspiration with a single saturation screening.  相似文献

BACKGROUND: One method of collecting mosquitoes is to use human beings as bait. This is called human landing collection and is a reference method for evaluating mosquito density per person. The Mbita trap, described by Mathenge et al in the literature, consists of an entry-no return device whereby humans are used as bait but cannot be bitten. We compared the Mbita trap and human landing collection in field conditions to estimate mosquito density and malaria transmission. METHODS: Our study was carried out in the highlands of Madagascar in three traditional villages, for 28 nights distributed over six months, with a final comparison between 448 men-nights for human landing and 84 men-nights for Mbita trap, resulting in 6,881 and 85 collected mosquitoes, respectively. RESULTS: The number of mosquitoes collected was 15.4 per human-night and 1.0 per trap-night, i.e. an efficiency of 0.066 for Mbita trap vs. human landing. The number of anophelines was 10.30 per human-night and 0.55 per trap-night, i.e. an efficiency of 0.053. This efficiency was 0.10 for indoor Anopheles funestus, 0.24 for outdoor An. funestus, and 0.03 for Anopheles arabiensis. Large and unexplained variations in efficiency were observed between villages and months. CONCLUSION: In the highlands of Madagascar with its unique, highly zoophilic malaria vectors, Mbita trap collection was poor and unreliable compared to human landing collections, which remains the reference method for evaluating mosquito density and malaria transmission. This conclusion, however, should not be extrapolated directly to other areas such as tropical Africa, where malaria vectors are consistently endophilic.  相似文献

Background

Malaria can be eradicated from islands. To assess the prospects for eradication of malaria from the island of Príncipe in the Gulf of Guinea, we fitted a mathematical model to age-prevalence curves and thus obtained estimates of the vectorial capacity and of the basic reproductive number (R 0) for malaria.

Methods

A cross-sectional malariological survey was carried out, in mid-1999, in six communities, comprising circa 17% of the total 6,000 population of the island. All houses in these communities were registered and their mode of construction recorded. Thick and thin blood films were prepared from all consenting individuals. Each individual was asked whether they possessed a mosquito net, whether they had slept under a mosquito net the previous night, whether they were allergic to chloroquine, and whether they had visited the main island of São Tomé since the beginning of the year. Outpatient records from March 1999 until the end of December 2000 were also examined and the age and place of residence of diagnosed cases noted.

Results

203 (19.8%) of the 1,026 individuals examined were found to be infected with Plasmodium falciparum. By fitting the mathematical model of the Garki project to the age-prevalence curve we estimate that the basic reproductive number, R 0, on the island is approximately 1.6. Over a period of one year, a total of 1,792 P. falciparum cases reported to an outpatient facility at the island's hospital. Overall, 54% of the people interviewed slept under mosquito nets and were at reduced risk of infection. Conversely, people living in houses with openings between the top of the wall and the roof had higher risk of infection.

Conclusion

This high incidence suggests that most of the malaria cases on the island attend the hospital and that treatment of these cases is an important factor reducing the effective rate of transmission. Providing that clinical cases are effectively treated, endemic malaria can probably be eliminated from the island mainly by reducing exposure to the vector with simple measures such as insecticide-treated nets and mosquito-proofing of dwellings. In contrast to traditional malaria eradication strategies, this would avoid the risk of malaria epidemics because the reduction in R 0 should be sustainable.  相似文献

Instrumented-pointers are often used to calibrate anatomical landmarks in biomechanical analyses. However, little is known about the effect of altering the orientation of the pointer during calibration on the co-ordinates recorded. Incorrect positioning of a landmark influences the axes created, and thus the kinematic data recorded. This study aimed to investigate the reliability of the pointer method for anatomical calibration. Two points were drawn onto a fixed box to resemble knee joint epicondyles, then a custom-made pointer was used to define the positions of these landmarks in three-dimensions. Twenty different pointer-orientations were chosen, and the position of the pointer in each of these orientations was recorded 8 times. Euclidean distances between single points were calculated for both landmarks and compared statistically (α = 0.05). Average Euclidean distances between all reconstructed points were 3.2±1.4 mm (range: 0.3–7.1 mm) for one landmark and 3.3±1.5 mm (range: 0.3–7.9mm) for the other. The x- and y-co-ordinates recorded differed statistically when the pointer was moved about the X and Y axes (anterior/posterior and superior/inferior to landmark) (p < 0.05). No statistical differences were found between co-ordinates recorded when the pointer was moved around the Z axes (p > 0.05). ICC values for all co-ordinates were excellent, highlighting the reliability of the method (ICC > 0.90). These results support this method of anatomical calibration; however, we recommend that pointers be consistently held in a neutral oriented position (where the handle is not anterior, posterior, superior or inferior to the landmark) during calibration, to reduce the likelihood of calibration errors.  相似文献

BackgroundPotentially toxic elements, such as lead, can bioaccumulate and alter human physiology. Human biomonitoring is an essential tool to evaluate chemical exposures in different biological matrices (blood, urine, saliva, nails, and hair). Of these biological matrices, nails are favorable for their ease of sampling, transport and storage. The aim of this study was to investigate possible correlations between blood lead levels (BLL) and washed and non-washed fingernail lead levels (FLL) in 55 adults living in a lead-contaminated area.MethodVenous blood and fingernail (thumbs and forefingers) samples were collected. Nails from the left hand were washed with Triton X-100 (0.5 % m/v) and HNO3 solution, while nails from the right hand were not submitted to the pre-analytical procedures. Samples were analyzed by graphite furnace atomic absorption spectrometry, and pairwise correlations were used to correlate lead concentrations between BLL and FLL; nails from fingers of the same hand and between washed and unwashed fingernails. Principal component analysis was performed and scatter diagrams were plotted to investigate correlations.ResultsA non-significant positive correlation was found between BLL and washed forefinger nails lead (r = 0.219, p = 0.112) and between BLL and thumbnail lead levels (r = 0.182, p = 0.191). Comparison of fingernails from the same hand (thumb and forefinger), showed that lead concentrations of non-washed nails varied widely, even on analyses of transversal fragments from the same nail. Lead levels in non-washed forefinger nails were not correlated with non-washed thumbnails (r = 0.169, p = 0.219). Conversely, washed thumb and forefinger nails were found to be correlated (r = 0.39, p = 0.003). Washed and non-washed nails were also found to be correlated (p < 0.0001).ConclusionIn conclusion, the results showed that non-washed nails are not a reliable biomarker for lead exposure. Although washing nails before analysis may reduce external contamination, the correlation of lead concentrations between fingers is poor for fingernail lead levels to serve as an internal dose biomarker to lead exposure. In addition, levels in washed nails were not significantly correlated with blood lead levels. Fingernail lead levels seem to serve as an indicator of lead exposure sources in contact with the individual, but not as a reliable biomarker of internal dose.  相似文献

Dimethylsulfoxide (DMSO) appears to be a reliable solvent for extracting chlorophyll (Chl), however, modification of standard methods may be necessary for some species under field conditions. We found that Chl extraction of whole leaf tissue with DMSO incubated at between 25 and 40 °C was generally similar to the 80% acetone method, except for one graminoid species that required maceration. There was little effect of incubation temperature or duration of incubation beyond 7 h on extraction efficiency, but DMSO extracts were less stable than acetone extracts during one week of cold storage, especially if they thawed during this period. Since Chl extraction methods may provide variable results, particularly in the field, studies using different solvents should be compared cautiously unless specific methods have been calibrated. Common methods to assess diversity and abundance of Orthoptera are sweep netting, transect counts and box-quadrat sampling. Pitfall trapping, by contrast, is rarely used, and the value of this method is still being questioned. In 2008, we studied Orthoptera species richness and abundance in five vegetation types along a gradient of dune succession on the Baltic Sea island of Hiddensee (NE Germany) by comparing transect-count and pitfall-trapping data. Using transect counts, 12 species were detected in the study area. With pitfall traps, three chorto- and thamnobiont Ensifera species (C. dorsalis, M. roeselii and T. viridissima) were not caught at all, and it was only in low-growing and sparsely-vegetated grey dunes that all present species were detected. With pitfall traps, the proportion of present species recorded strongly declined with increasing height and density of the vegetation type. Assuming that transect counts are a good proxy for relative Orthoptera densities, densities ascertained by pitfall traps are strongly biased by vegetation structure and locomotive behaviour of the species. More than 80% of all individuals were caught in sparsely-vegetated grey dunes. Frequency patterns of the species also differed. Using pitfall traps, especially chortobiont species were significantly underrepresented. Qualitative and quantitative sampling of Orthoptera using pitfall traps seems only reasonable in habitats with low and sparse vegetation and a high proportion of geobiont species.  相似文献

Women's ovulation is perceivable with different senses. Already subtle face shape differences are enough to trigger men's preference for the ovulatory female. The aim of the present study is to investigate if men's testosterone level can be linked to their preference for the ovulatory female. Thirty-nine heterosexual participants were shown face pairs of which one of them was transformed to the shape of a prototype face of a woman in her luteal cycle phase and the other was transformed to the shape of a prototype face of an ovulatory woman. Participants were asked to choose the face which they perceived as being more attractive (attractiveness task), or the woman with whom they would have better chances to get a date (dating task). In both tasks, the ovulatory female was chosen more often. Testosterone was not predictive for the chosen face; regardless of testosterone level men preferred the ovulatory woman. However testosterone predicted how confident the men were with their choice. Men with lower testosterone levels were more confident with their choice than men with higher testosterone levels.  相似文献

An autism spectrum disorder (ASD) diagnosis is based on clinical behaviours as there are no validated biological diagnostic tools. Indolyl-3-acryloylglycine (IAG) is a chemical produced by gut microflora and there are conflicting reports as to whether urinary levels are elevated in children with ASD compared with controls. Urinary IAG levels in morning urine samples were statistically significantly higher in children with ASD whose caregivers reported the presence of chronic gastrointestinal (GI) disturbance than children with ASD without chronic GI disturbance. Urinary IAG, however, was not statistically significantly higher in children with ASD, compared with siblings or unrelated controls without ASD.  相似文献

Decreased nutrition has been proposed as a potential mechanism leading to intervertebral disc degeneration. A method to investigate it in vivo is the MRI evaluation of the transport of a paramagnetic contrast agent, which is assumed to diffuse through the endplate to the disc using the same mechanisms as the cell nutrients. However, previous numerical studies questioned the value of this method as a model to investigate disc nutrition. To assess its validity, a parametric osmoporoelastic finite element model of a lumbar intervertebral disc incorporating diffusion and convection of a solute (representing the contrast agent) was developed. A Taguchi sensitivity analysis was performed in order to assess the relevance of various parameters which influence the solute transport. Subsequently, a full-factorial sensitivity analysis was used to investigate specifically the diffusion coefficients of the contrast agent. The most important parameters in determining the results were the disc height, the diffusion coefficients and the pharmacokinetic of the contrast agent. However, diffusion coefficients values as measured in in vitro studies would lead to insubstantial enhancement of the MRI signal. Thus, transport mechanisms other than pure diffusion should be active in in vivo transport of the contrast agent. In conclusion, the study showed that post-contrast MRI may not be suited for a quantitative analysis, but only for a qualitative examination aimed for example to detect endplate lesions. Open questions remain on the use of post-contrast MRI for the investigation of the relevance of reduced nutrition as a trigger to disc degeneration.  相似文献

Free Cu species in soils is a key issue to its bioavailability. However, predictive models for Cu speciation across a wide range of soils were still unavailable. In this study, Cu speciation in 34 contaminated soil samples were investigated via analytical technique and predictive models. The results showed that most of free Cu2+ was underestimated when using default log KCuFA and 65% active fulvic acid as inputs in models of WHAM VI and NICA-Donnan. The best prediction was found when using either adjusted active fulvic acid from 10% to 125% for WHAM VI or from 15% to 65% for NICA-Donnan model with the RMSE < 0.32 and r2 > 0.96. In contrast, NICA-Donnan demonstrated a slightly stronger binding for Cu than WHAM VI due to extra 26% of samples was underestimated. This work presents a comprehensive database of Cu speciation and an effective attempt of free Cu2+ prediction in a wide range of Chinese soils.  相似文献

Background and Aims

Despite their importance for plant production, estimations of below-ground biomass and its distribution in the soil are still difficult and time consuming, and no single reliable methodology is available for different root types. To identify the best method for root biomass estimations, four different methods, with labour requirements, were tested at the same location.

Methods

The four methods, applied in a 6-year-old Eucalyptus plantation in Congo, were based on different soil sampling volumes: auger (8 cm in diameter), monolith (25 × 25 cm quadrate), half Voronoi trench (1·5 m3) and a full Voronoi trench (3 m3), chosen as the reference method.

Key Results

With the reference method (0–1m deep), fine-root biomass (FRB, diameter <2 mm) was estimated at 1·8 t ha−1, medium-root biomass (MRB diameter 2–10 mm) at 2·0 t ha−1, coarse-root biomass (CRB, diameter >10 mm) at 5·6 t ha−1 and stump biomass at 6·8 t ha−1. Total below-ground biomass was estimated at 16·2 t ha−1 (root : shoot ratio equal to 0·23) for this 800 tree ha−1 eucalypt plantation density. The density of FRB was very high (0·56 t ha−1) in the top soil horizon (0–3 cm layer) and decreased greatly (0·3 t ha−1) with depth (50–100 cm). Without labour requirement considerations, no significant differences were found between the four methods for FRB and MRB; however, CRB was better estimated by the half and full Voronoi trenches. When labour requirements were considered, the most effective method was auger coring for FRB, whereas the half and full Voronoi trenches were the most appropriate methods for MRB and CRB, respectively.

Conclusions

As CRB combined with stumps amounted to 78 % of total below-ground biomass, a full Voronoi trench is strongly recommended when estimating total standing root biomass. Conversely, for FRB estimation, auger coring is recommended with a design pattern accounting for the spatial variability of fine-root distribution.  相似文献

Material properties of tissue in vivo present an opportunity for clinical analysis of healing progression and pathologies as well as provide an excellent research tool yielding quantified data for longitudinal and cross population studies. Echogenicity is a material?s ability to reflect sound and, using ultrasound, it has been shown to increase with tendon tension in vitro, though this non-invasive measurement technique for determining mechanical properties has not been tested in vivo. The aim of this study was to establish if echogenicity, seen by the increase in image brightness, could be correlated to stress within a tissue. 18 Achilles tendons were imaged in the sagittal and transverse planes while producing a series of isometric contractions starting from rest and producing the torque equivalent of 0.5, 1.0, 1.5, and 2.0× body weights. Manual tracing identified the tendon in each of the images. The cross-sectional area determined from the transverse plane images in conjunction with the tendon force yielded the tendon stress. The echogenicity of the tendon was determined from the mean brightness change from rest to each of the contraction cases, measured from the sagittal plane images. A weak correlation existed between the echogenicity and stress (R=0.25) but it was found that there was no significant change in axial area during contraction (p=0.683) establishing the tendon as incompressible. Echogenicity proved to be non-functional for measuring the mechanical properties of the Achilles tendon due to the additional factors included with in vivo testing e.g. tendon twist and multi-axial loading.  相似文献

Soil microbial biomass (SMB), the living part of soil organic matter, is used to quantify the total biomass of microorganisms present in the soil. The importance of studies about SMB has emphasized the need to identify methods which can measure the size of SMB. Among the methods currently available, chloroform-fumigation extraction and incubation are the most commonly used for estimation of SMB. However, several studies have proposed the microwave (MW) irradiation as a quick, simple and safe alternate method. There are different opinions about suitability of this method for measuring SMB. There is a question to do “Is the microwave irradiation a suitable method for measuring soil microbial biomass?” Most of the published papers comparing MW and chloroform-fumigation showed strong relationship between both methods. Therefore, we consider MW a suitable method for measuring SMB; however, it is necessary to calibrate the MW methods in different soils with a range of properties, such as clay content, to find an appropriate conversion factor in order to generate correct values for SMB with MW method.  相似文献

Sperm concentration assessment is a key point to insure appropriate sperm number per dose in species subjected to artificial insemination (AI). The aim of the present study was to evaluate the accuracy and reliability of two commercially available photometers, AccuCell™ and AccuRead™ pre-calibrated for boar semen in comparison to UltiMate™ boar version 12.3D, NucleoCounter SP100 and Thoma hemacytometer. For each type of instrument, concentration was measured on 34 boar semen samples in quadruplicate and agreement between measurements and instruments were evaluated. Accuracy for both photometers was illustrated by mean of percentage differences to the general mean. It was −0.6% and 0.5% for Accucell™ and Accuread™ respectively, no significant differences were found between instrument and mean of measurement among all equipment. Repeatability for both photometers was 1.8% and 3.2% for AccuCell™ and AccuRead™ respectively. Low differences were observed between instruments (confidence interval 3%) except when hemacytometer was used as a reference. Even though hemacytometer is considered worldwide as the gold standard, it is the more variable instrument (confidence interval 7.1%).The conclusion is that routine photometry measures of raw semen concentration are reliable, accurate and precise using AccuRead™ or AccuCell™. There are multiple steps in semen processing that can induce sperm loss and therefore increase differences between theoretical and real sperm numbers in doses. Potential biases that depend on the workflow but not on the initial photometric measure of semen concentration are discussed.  相似文献
