了解我院患者耐甲氧西林金黄色葡萄球菌(MRSA)的分子流行病学特点,为临床抗感染治疗提供依据。收集2007年1月~2008年9月我院分离的耐甲氧西林金黄色葡萄球菌共54株,采用PCR进行SCCmec基因分型、葡萄球菌A蛋白(SPA)分型,并检测杀白细胞毒素(PVL)基因,同时应用脉冲场凝胶电泳(PFGE)进行同源性分析。54株MRSA菌株SCCmec基因分型为SCCmecⅡ型17株,SCCmecⅢ型33株,SCCmecⅣ型2株,SCCmecⅤ型2株;SPA基因分型将28株归属为t030,9株为t002,8株为t037,5株为t570,2株为t437,t163和t796各1株;PVL毒素检测只有2株SCCmecⅣ型菌株阳性;PFGE证实院内MRSA感染主要为2种克隆株传播,同时还有其他型别出现。本院MRSA流行传播的SCCmec基因型主要以Ⅲ型占优势,同时发现有携带PVL毒素的CA-MRSA分离株流行,应引起密切关注。  相似文献

目的 调查本地区患者各种标本中分离耐甲氧西林金黄色葡萄球菌(MRSA)的杀白细胞素(PVL)基因携带情况,为临床MRSA的治疗及流行病学调查提供合理的依据.方法 对所分离的MRSA菌株进行药敏试验分析,同时采用PCR法检测mecA基因和PVL基因,比较社区获得性MRSA (CA-MRSA)和医院获得性MRSA (HA-MRSA)之间耐药性的比较及PVL基因携带率的比较.结果 对不同来源的9l株MRSA分离株耐药性分析,CA-MRSA对环丙沙星、利福平、庆大霉素和左旋氧氟沙星的敏感性明显高于HA-MRSA.经PCR检测发现,所有菌株均携带有mecA基因,21株携带有PVL基因,其中65株HA-MRSA有仅8株携带有PVL基因,而26株CA-MRSA中有13株携带有PVL基因,携带率差异有统计学意义.结论 本地区CA-MRSA是携带PVL基因的主要菌株,HA-MRSA对抗菌药物的耐药性明显高于CA-MRSA,尚未发现对万古霉素和替考拉宁的耐药菌株.  相似文献

近年来,耐甲氧西林金黄色葡萄球菌在全世界各地感染率和分离率不断提高,已成为目前院内感染的重要病原菌之一。运用有效、可靠、廉价的分子分型方法对分析耐甲氧西林金黄色葡萄球菌的流行病学特征及来源,对制定控制院感及流行的措施非常重要的。本研究概述了各种分子分型方法的原理及比较,如SCCmec分型、脉冲场凝胶电泳分型、多位点序列分型、葡萄球菌A蛋白分型和毒力因子分型等。脉冲场凝胶电泳仍然是暴发流行中MRSA分子分型的金标准,而其他分型方法更适合用于检测菌株的变异和建立国际监测。  相似文献

目的 应用mecA基因PCR扩增法检测耐甲氧西林金黄色葡萄球菌(methicillin resistant staphylococcus aureus,MRSA)。方法 临床分离的70株金黄色葡萄球菌,应用mecA基因PCR扩增法鉴定MRSA,并与苯唑西林纸片扩散法进行比较。结果 70株金黄色葡萄球菌用PCR扩增法和纸片扩散法有6株鉴定有差异,4株。mecA基因阳性而纸片扩散法鉴定为敏感,1株mecA基因阳性纸片扩散法鉴定为临界耐药,1株mecA基因阴性却表现为苯唑西林耐药,2种方法符合率为91.43%。结论 mecA基因PCR扩增法可以准确、快速判定MRSA,特别是对隐匿型或低水平耐药菌株的检出有重要的价值。  相似文献

目的调查大连医科大学附属二院院重症监护病房2005年至2009年耐甲氧西林金黄色葡萄球菌(MRSA)的感染状况,分析原因,探讨预防MRSA感染发生的途径和方法。方法收集2005年至2009年入住ICU病房患者各种标本中分离的179株金黄色葡萄球菌进行8种抗生素的耐药性检测并从中分离出MRSA。结果5年来MRSA感染率逐年上升,感染部位以下呼吸道,伤口(包括手术切口),泌尿道为主。结论ICU病房MRSA感染率有上升趋势,对万古霉素敏感性好,严格消毒隔离措施合理使用抗生素是控制MRSA感染的关键环节。  相似文献

目的通过分析住院患儿分离出的耐甲氧西林金黄色葡萄球菌(MRSA)菌株的分布情况及临床特点,了解MRSA感染的危险因素,为预防、控制MRSA的传播提供参考。方法分析临床分离出的445株MRSA,对其季节分布、年龄特点、科室分布及标本类型等进行分析。结果 MRSA的检出率为30.44%(445/1462),其中男287例(检出率64.49%),女158例(检出率35.51%),男性高于女性。从季节分布看:6月份最多,12月份最少;从科室分布看,呼吸科最多,其次为骨科、新生儿科、心内科感染MRSA患儿居多;从标本类型来看,痰液中MRSA检出率最高。结论患儿由于自身因素,年龄小、体内产生抗体的器官尚未发育完善,全身抵抗力差,较易感染。因此,医院应加强侵入性操作的无菌管理,避免不必要的侵入性操作,实行对导管插管等相关医院感染危险因素的预防与控制,缩短住院时间,及时隔离MRSA感染者,并做好监测。  相似文献

目的通过对ICU病区耐甲氧西林金黄色葡萄球菌(MRSA)感染进行流行病学调查,并经过耐药菌谱的分析,探讨临床分离菌株的同源性,为预防和控制医院感染提供参考.方法对2005年3月7日~3月29日ICU病区感染MRSA的10例患者及医院环境进行了流行病学调查分析.结果ICU病区MRSA的感染率为47.6%.且环境中空气、陪护人员手、医务人员等亦培养出MRSA,通过耐药谱分析显示细菌具有高度同源性.结论该次MRSA感染为局部暴发流行.医院必须加强室内外环境和空气监控,防止交叉感染,严格无菌侵入性操作和抗生素的使用原则,从而有效减少MRSA院内感染的发生.  相似文献

Staphylococcus aureus is a commensal gram positive bacteria which causes severe and non severe infections in humans and livestock. In India, ST772 is a dominant and ST672 is an emerging clone of Staphylococcus aureus. Both cause serious human diseases, and carry type V SCCmec elements. The objective of this study was to characterize SCCmec type V elements of ST772 and ST672 because the usual PCR methods did not amplify all primers specific to the type. Whole genome sequencing analysis of seven ST772 and one ST672 S. aureus isolates revealed that the SCCmec elements of six of the ST772 isolates were the smallest of the extant type V elements and in addition have several other novel features. Only one ST772 isolate and the ST672 isolate carried bigger SCCmec cassettes which were composites carrying multiple ccrC genes. These cassettes had some similarities to type V SCCmec element from M013 isolate (ST59) from Taiwan in certain aspects. SCCmec elements of all Indian isolates had an inversion of the mec complex, similar to the bovine SCCmec type X. This study reveals that six out of seven ST772 S. aureus isolates have a novel type V (5C2) SCCmec element while one each of ST772 and ST672 isolates have a composite SCCmec type V element (5C2&5) formed by the integration of type V SCCmec into a MSSA carrying a SCC element, in addition to the mec gene complex inversions and extensive recombinations.  相似文献

"超级细菌"耐甲氧西林金黄色葡萄球菌(methicillin resistant Staphylococcus aureus,MRSA)是诱发连续腹膜透析患者腹膜炎的常见细菌,且治疗困难。目前缺少MRSA腹膜炎动物模型。腹腔注射2×109~2×1010CFU/m L 7组不同浓度的MRSA感染小鼠,观察小鼠死亡时间,测定肝脏与脾脏细菌定植量,进行肝、脾病理分析,确定适宜的建模浓度。研究发现,小鼠感染细菌浓度最小致死剂量为每只2×109CFU,最适建模浓度为每只1.4×109CFU。结果表明建立了耐甲氧西林金黄色葡萄球菌小鼠腹膜炎模型,为MRSA致腹膜炎的致病机制研究、疫苗的研制提供实验基础。  相似文献

Background

Patient isolation using contact precautions has gained widespread use to halt MRSA transmission, however supportive data is scarce and concerns regarding patient safety and satisfaction have been raised. At our institution, MRSA patients are isolated on a dedicated ward (cohort isolation), rather than in separate rooms. Our objectives were (1) to determine the proportion of bedside medical visits to patients on an isolation ward, (2) to quantify complications in those patients and (3) to determine if those complications are related to isolation and if they can be prevented.

Methods

This retrospective case-control study was performed on the two sites of a tertiary teaching hospital in Sherbrooke, QC, Canada. We matched MRSA patients with an admission diagnosis of heart failure or chronic obstructive pulmonary disease to similar non-isolated controls. The proportion of bedside visits was ascertained through the number of progress notes with subjective elements or with a physical examination. Complications were sought through an extensive file review, and events were analysed according to Baker’s CAES causality and preventability scales.

Results

Overall, 111 patient pairs were analysed (35 with heart failure and 76 with COPD). Isolated patients received less bedside visits (subjective notes/1,000 patient-days: 849.6 vs. 983.3, p = 0,001). Attending physicians (454.5 vs. 451.4, p = 0,02) and residents (347.0 vs. 416.9, p = 0.01) are responsible for this discrepancy, while medical students appear to visit isolated and non-isolated patients equally (116.5 vs. 114.9, p = 0.90). Isolated patients showed a tendency towards longer stay and more preventable complications, although no difference in the total number of complications was observed.

Conclusion

Isolated patients have less documented care that suggests less bedside visits from the medical staff, which could hamper the therapeutical relationship. Further studies are needed to explain this finding.  相似文献

Community acquired methicillin resistant Staphylococcus aureus (CA-MRSA), and the USA300 strain of CA-MRSA in particular, are known for their rapid community transmission, and propensity to cause aggressive skin and soft tissue infections. To assess factors that contribute to these hallmark traits of CA-MRSA, we evaluated how growth of USA300 and production of secreted virulence factors was influenced on exposure to physiologic levels of unsaturated free fatty acids that would be encountered on the skin or anterior nares, which represent the first sites of contact with healthy human hosts. There was a sharp threshold between sub-inhibitory and inhibitory concentrations, such that 100 µM sapienic acid (C16∶1) and linoleic acid (C18∶1) were sufficient to prevent growth after 24 h incubation, while 25 µM allowed unrestricted growth, and 50 µM caused an approximate 10–12 h lag, followed by unimpeded exponential growth. Conversely, saturated palmitic or stearic acids did not affect growth at 100 µM. Although growth was not affected by 25 µM sapienic or linoleic acid, these and other unsaturated C16 and C18 fatty acids, but not their saturated counterparts, promoted robust production of secreted proteases comprising the Staphylococcal proteolytic cascade. This trait was also manifested to varying degrees in other CA-MRSA, and in genetically diverse methicillin susceptible S. aureus strains. Therefore, induction of the Staphylococcal proteolytic cascade by unsaturated fatty acids is another feature that should now be evaluated as a potential contributing factor in the aggressive nature of skin and soft tissue infections caused by USA300, and as a general virulence mechanism of S. aureus.  相似文献

Methicillin sensitive Staphylococcus aureus is an important bacterial pathogen associated with hospital- and community-acquired infections leading to endocarditis, skin tissue infection and pneumonia. The objective of this study was to determine both the genetic characteristics of methicillin-sensitive S. aureus (MSSA) strains, and the occurrence of virulence factors produced by S. aureus strains isolated from UMMC and healthy students in the University from year 2009. Out of 429 nasal swab samples, 67 were MSSA. The prevalence of 21 different virulence genes among 67 Malaysian clinical and community MSSA strains was determined by PCR, and their genetic features were assessed by PCR-RFLP of coa gene, agr types, spa typing and PFGE. The five predominant virulence genes were ica (79 %), efb and fnbA (61 % each), sdrE (57 %) and hlg (45 %). Toxin genes (enterotoxin, etd and pvl) were significantly more common (P < 0.05) in clinical strains compared to community strains. Three agr genotypes were observed: agr type I (45 %), agr type III (25 %) and agr type II (19 %). All 67 MSSA strains were distinguished into 26 profiles by PCR-RFLP of coa, 55 pulsotypes and 21 spa types. Four novel spa types (t7312, t7581, t7582 and t7583) were observed. In conclusion, different virulence profiles were observed in MSSA strains in Malaysia where toxin genes were more prevalent among clinical strains. No correlation between DNA profiles (coa-RFLP, PFGE and spa) and virulotypes was observed. The Malaysian MSSA strains from clinical and community sources were genetically diverse and heterogeneous.  相似文献

As genotyping of S. aureus is important for epidemiologic research and for hygiene management, methods are required for standardized fast and easily applicable evaluation of closely related epidemic strains with high prevalence in hospitals. In this single centre matched control study we compared a new commercially available DNA microarray (IdentiBAC) with standard spa-typing for S. aureus genotyping. Included in the study was a subgroup of 46 MRSA and matched 46 MSSA nasal isolates of the Saarland University Medical Center collected during a state-wide admission prevalence screening. Microarray (MA) and also spa-typing could easily differentiate the genetically diverse MSSA group. However, due to the predominance of CC5/t003 in the MRSA group a sufficient subtyping required analysis of more complex genetic profiles as was shown here by the MA comprising a total number of 334 different hybridization probes. The genetic repertoire of the MRSA group was characterized by more virulence genes as compared to the MSSA group. The standard evaluation of MA results by the original software into CCs, agr-, SCCmec- and capsule-types was substituted in the present study by implementation of multivariate subtyping of closely related CC5 isolates using three different bioinformatic methods (splits graph, cluster dendrogram, and principal component analysis). Each method used was applicable for standardized and highly discriminative subtyping with high concordance. We propose that the identified S. aureus subtypes with characteristic virulence gene profiles are presumably associated also with virulence and pathogenicity in vivo; however, this remains to be analyzed in future studies. MA was superior to spa-typing for epidemiologic and presumably also provide functional respectively virulence associated characterization of S. aureus isolates. This is of specific importance for the hospital setting. In future, MA could become a new standard test for S. aureus typing in combination with multivariate bioinformatic analysis.  相似文献

Introduction

Recently a specific MRSA sequence type, ST398, emerged in food production animals and farmers. Risk factors for carrying MRSA ST398 in both animals and humans have not been fully evaluated. In this cross-sectional study, we investigated factors associated with MRSA colonization in veal calves and humans working and living on these farms.

Methods

A sample of 102 veal calf farms were randomly selected and visited from March 2007–February 2008. Participating farmers were asked to fill in a questionnaire (n = 390) to identify potential risk factors. A nasal swab was taken from each participant. Furthermore, nasal swabs were taken from calves (n = 2151). Swabs were analysed for MRSA by selective enrichment and suspected colonies were confirmed as MRSA by using slide coagulase test and PCR for presence of the mecA-gene. Spa types were identified and a random selection of each spa type was tested with ST398 specific PCR. The Sequence Type of non ST398 strains was determined. Data were analyzed using logistic regression analysis.

Results

Human MRSA carriage was strongly associated with intensity of animal contact and with the number of MRSA positive animals on the farm. Calves were more often carrier when treated with antibiotics, while farm hygiene was associated with a lower prevalence of MRSA.

Conclusion

This is the first study showing direct associations between animal and human carriage of ST398. The direct associations between animal and human MRSA carriage and the association between MRSA and antimicrobial use in calves implicate prudent use of antibiotics in farm animals.  相似文献

Understanding the pathology resulting from Staphylococcus aureus and Pseudomonas aeruginosa polymicrobial wound infections is of great importance due to their ubiquitous nature, increasing prevalence, growing resistance to antimicrobial agents, and ability to delay healing. Methicillin-resistant S. aureus USA300 is the leading cause of community-associated bacterial infections resulting in increased morbidity and mortality. We utilized a well-established porcine partial thickness wound healing model to study the synergistic effects of USA300 and P. aeruginosa on wound healing. Wound re-epithelialization was significantly delayed by mixed-species biofilms through suppression of keratinocyte growth factor 1. Pseudomonas showed an inhibitory effect on USA300 growth in vitro while both species co-existed in cutaneous wounds in vivo. Polymicrobial wound infection in the presence of P. aeruginosa resulted in induced expression of USA300 virulence factors Panton-Valentine leukocidin and α-hemolysin. These results provide evidence for the interaction of bacterial species within mixed-species biofilms in vivo and for the first time, the contribution of virulence factors to the severity of polymicrobial wound infections.  相似文献
