La détermination sérologique de la poliomyélite asymptomatique dans la famille

Résumé Dans 4 familles où se trouvait un malade atteint de poliomyélite on a examiné les sérums de la phase aiguë et de la phase de convalescence des malades et des autres membres de famille pour chercher la présence d'anticorps neutralisant trois souches du virus poliomyélitique (Lansing, SK et MM) et le virus de la chorioméningite lymphocytaire. Dans deux familles au moins, les recherches indiquent que les anticorps contre la souche Lansing ne se forment que comme conséquence spécifique d'une infection, éventuellement asymptomatique, par un virus poliomyélitique d'une structure antigène apparentée à celle de la souche Lansing. Dans une famille le sérum de la phase aiguë d'un malade et le sérum de sa mère, contenaient des anticorps tandis que le sérum de son père et d'une soeur ne neutralisait pas la souche Lansing. Dix semaines après, les sérums du père et de la soeur avaient obtenu un pouvoir neutralisant contre cette souche, bien que ni l'un ni l'autre n'aient été malades. Ceci pourrait indiquer la possibilité d'une infection par contact direct dans la famille.     

Gestion informatisée de la biologie délocalisée

La prise en charge par le biologiste de certaines analyses délocalisées se trouve grandement facilitée par la mise en place d’un système informatique reliant l’analyseur délocalisé et le laboratorie. Elle apporte un niveau de sécurité nécessaire pour la prise de responsabilité du laboratoire dans l’acte de biologie délocalisé. L’architecture réseau de la plupart de h?pitaux permet maintenant de transmettre très facilement et de manière fiable des données entre plusieurs services voire entre établissements. Les analyseurs de gazométrie sanguine permettent cette connexion, de plus en plus d’industriels développent d’autres paramètres en plus des simples gaz du sang comme dernièrement l’urée et la créatinine. Les analyseurs permettant de doser les marqueurs cardiaques, ceux réalisant l’hémostase sont désormais fréquemment connectables. Chaque industriel du diagnostic proposant des analyseurs de glycémie a ou va prochainement commercialiser un modèle connectable. Il en est de même pour les bandelettes urinaires. Il est donc probable, et les biologistes sont les premiers demandeurs, que la connexion informatique d’analyseurs de biologie installés dans les unités de soins, soit un pré-requis pour la gestion des analyses délocalisées par le biologiste, et sa prise de responsabilité dans les résultats biologiques issus de ces analyseurs.     

Comprendre la dépression : apport de la psychologie évolutionniste

Darwinian medicine considers that many symptoms are the organism’s defence mechanisms and adaptations that have been inherited and shaped by natural selection. But in many cases, signs and symptoms of disease are of no benefit to human survival and are in fact deregulations of normal mechanisms. Evolutionary explanations help us understand how some human affective and emotional processes could have an adaptive value and enable us to cope with a wide spectrum of situations. But the wide range of intensity levels and different forms, ranging from low moods to major depression with psychotic features, suggest that these symptoms cannot always be explained by adaptation: where is the cut-off point? In this paper, we describe the evolutionary concepts related to adaptation as applied to the emotions, particularly focusing on low mood and depression, and discuss their possible functional and adaptive role.     

Optimisation du protocole d’acquisition de la TEP à la fluorocholine dans la détection des lésions parathyroïdiennes

18F-Fluorocholine (FCH) PET/CT shows very promising potential for detection of hyperfunctioning parathyroid tumors. However, the optimal time to perform imaging after FCH administration has not yet been determined and protocols are highly variable. The aim of this study was to qualitatively and quantitatively compare 5, 10, 15, 20 and 60 min post-injection acquisition times in patients with primary hyperparathyroidism and equivocal traditional imaging. Thirty-one patients were included. Two observers retrospectively analyzed the five protocols. Any focal increase in FCH uptake was localized and graded on a discrete gradation scale between 1 and 5 to assess the likelihood of hyperfunctioning parathyroid tumors. Gold standard was histopathological findings for the 11 operated patients. Regarding quantitative analysis, ratio of SUVmax of parathyroid foci on SUVmean of thyroid background (as signal-to-noise ratio) were compared between protocols. After injection of 2.5 MBq/kg, FCH PET performed 60 min post-injection (2 min) had the best sensitivity and specificity by lesion (92% and 100% respectively and the best signal-to-noise ratio (median of 2). We suggest performing PET scan 60 min after injection, associated with early acquisition so as not to miss a wash-out hyperfunctioning parathyroid tumors.     

Impact d’une réduction d’activité sur la mesure de la fraction d’éjection du ventricule gauche sur caméra D-SPECT

IntroductionRadionuclide ventriculography provides a reproducible measurement of the left ventricular fraction ejection (LVEF) but with a significant body radiation (effective dose of 5,9 mSv for the injection of 850 Mbq of ^99mTc). The highly sensitive semi-conductor (CZT) cameras could allow decreasing the injected activity by a factor 3, similarly to that of myocardial perfusion imaging. Our study was aimed to determine whether the LVEF measurement provided by radionuclide ventriculography on the CZT D-SPECT camera is impacted by a 70% reduction in recorded counts.Materials and methodsAfter the in vivo labeling of red blood cells with 850 MBq of ^99mTc, 49 patients completed a conventional 2D recording (Conv-2D) on Anger camera followed by a 3D recording on the D-SPECT camera (3D-100%). The CZT recordings of all projections were subsequently shortened to 30% of their initial durations (3D-30%) in order to assess the LVEF measured with a 70% reduction in recorded counts.ResultsMean LVEF values were 62.7 ± 11.1% on Conv-2D and higher on both 3D-100% (66.8 ± 14.8%, P < 0.001) and 3D-30% (66.3 ± 15.7%, P < 0.001). The correlation coefficients with the LVEF determined with the reference Conv-2D method were equivalents for 3D-100% (r² = 0.73) and 3D-30% (r² = 0.70) and with a similar level of overestimation for the highest LEVF values.ConclusionA 70% reduction in recorded counts does not significantly impact the LVEF measured with radionuclide ventriculography on the CZT D-SPECT camera. These values are coherent with those obtained with the reference 2D method but with a clear overestimation for the highest LVEF values.

La collection d’Invertébrés fossiles d’Alcide d’Orbigny et la salle d’Orbigny

Fossil invertebrates collected or received by Alcide d’Orbigny, all his life long, have been brought together in a renowned collection that is very rich in terms of both species and specimens. The collection has resulted from a huge palaeontological work. Study of faunas from numerous French sites led Alcide d’Orbigny to do biostratigraphical comparisons and to erect geological stages, the definitions of which rest on their stratotypes. His wide knowledge of European fossil faunas was strengthened by a more global palaeobiogeographical view derived from his travel in South America. The d’Orbigny collection is housed in the ‘Muséum’ of Paris and is often visited by experts.     

Analyse de la trisomie 9p par dénaturation ménagée

Résumé L'analyse de la structure fine des chromatides obtenue par dénaturation ménagée par la chaleur permet de rapporter un nouveau cas de trisomie pour le bras court du chromosome 9.Un nouvel examen, avec cette même technique, des cas précédemment publiés a permis d'identifier les chromosomes impliqués dans les diverses translocations familiales, de localiser les points de cassure et, par là, de préciser les diverses trisomies ou monosomies associées à la trisomie 9p observée chez les patients.La confrontation des données cytologiques ainsi recueillies et des données phénotypiques observées chez dix patients permet de dégager l'ensemble malformatif correspondant à la trisomie du bras court du chromosome 9 à savoir: Un syndrome dysmorphique: microcéphalie modérée, énophtalmie, obliquité en bas et en dehors des fentes palpébrales, hypertélorisme modéré, oreilles décollées, nez charnu, coins de la bouche tombants, hypoplasie de certaines phalanges. Une particularité des dermatoglyphes: pli palmaire transverse, absence ou fusion de b et c. Une débilité mentale sévère.

Giemsa-R-banding analysis of the trisomy 9p and report of a new case

Summary Analysis of the chromatid fine structure after heat denaturation led to the identification of another case of trisomy of the short arm of chromosome 9. Reexamination of cases already published by means of this technique permitted identification of the chromosomes involved in the various familial translocations, localization of the breakage sites, and, consequently, detailed statements on the respective trisomies or monosomies occurring in connection with trisomy 9p in the patients.Comparison of the chromosomal findings and the clinical phenotypes of 10 patients reveals that a characteristic clinical entity corresponds to the trisomy of the short arm of chromosome 9 consisting of the combination of the following features: A Malformation Syndrome. Moderate microcephaliy, enophthalmus, antimongoloid slant of palpebral fissures, mild hypertelorism, protruding ears, globulous nose, downward slanting mouth hypoplasia of phalanges. A Peculiarity in the Dermal Ridge System. Transverse palamar flexion crease, absence of a digital triradius or fusion of the triradii b and c. Severe Mental Retardation.

Zusammenfassung Die Analyse der Feinstruktur der Chromatiden nach Hitzedenaturierung erlaubte die Identifizierung eines neuen Falles einer Trisomie des kurzen Armes von Chromosom 9. Die Nachuntersuchung bereits publizierter Fälle mit dieser Technik ermöglichte die Identifizierung der an den verschiedenen familiären Translokationen beteiligten Chromosomen, die Lokalisierung der Bruchstellen und damit auch genauere Angaben über die unterschiedlichen Trisomien oder Monosomien, die im Zusammenhang mit der bei den Patienten beobachteten Trisomie 9p auftraten.Die Gegenüberstellung der Chromosomenbefunde und der phänotypischen Merkmale bei 10 Patienten erlaubt die Angrenzung der folgenden Kombination klinischer Merkmale, die der Trisomie des kurzen Armes von Chromosom 9 entspricht: Ein Mißbildungssyndrom. Mäßige Mikrocephalie, Enophthalmie, schräg nach außen und unten gerichtete Lidachsenstellung, mäßiger Hypertelorismus, abstehende Ohren, fleischige Nase, abfallende Mundwinkel, Hypoplasie einiger Phalangen. Eine Eigentümlichkeit im Hautleisten- und-furchensystem. Transverse palmare Beugefurche, Fehlen eines digitalen Triradius oder Fusion der Triradien b und c. Schwere geistige Retardierung.

Délocalisation de la biologie d’urgence et Assurance Qualité

La biologie délocalisée prend une importance croissante dans le fonctionnement des services traitant les urgences à l’h?pital. Elle devrait représenter une réele évolution pour la prise en charge du patient tout en améliorant l’organisation interne des unités de soins. Le biologiste occupe un r?le clé pour mener un tel projet et la mise en place d’un système d’assurance qualité adapté à cette pratique est indispensable.     

Performances diagnostiques actuelles du coroscanner dans la détection de la maladie coronaire

Recent advances in multislice computed tomography (MSCT), which might represent a minimally invasive alternative to coronary angiography (CA), have allowed the ability to analyze coronary arteries. In this review, we present the current diagnostic performance of MSCT, as compared with the reference standard (CA), for the assessment of native coronary arteries, coronary in-stent restenosis and coronary artery bypass graft.     

Perles rectangulaires gravettiennes : apport de la démarche expérimentale

The cultural facies called Final Gravettian is rare, restricted only to four reference sites located in the south half of France. Three of them yielded rectangular beads, which constitute a category of ornaments that seems to be characteristic of this late phase of the Gravettian. The series of 85 rectangular beads from the level 2 of the Abri Pataud (Dordogne) was the object of a targeted study aiming first at determining in which raw material these beads were made that turned out to be mammoth ivory for the great part. Therefore, the question of what are the techniques used to make these beads. In order to answer an experimental approach was managed with a constant dialogue with the archaeological context. This allow us to contribute to an enlarged discussion about the lifestyles of people who lived during the Final Gravettian and more precisely about the role played by the mammoth within a nomadic hunter-gatherer society with an economy essentially based on the exploitation of the reindeer.     

La dépression et le traitement antidépresseur: de la neurotoxicité à la neurogenèse

Major depressive disorder is characterized by structural and neurochemical changes in limbic structures, including the hippocampus that regulates mood and cognitive functions. Hippocampal atrophy is observed in patients with depression: structural changes in the hippocampus associated with depression include dendritic atrophy, decreased adult neurogenesis and reduced volume. Impairment of neuroplasticity in the hippocampus, amygdala and cortex is hypothesized to be the mechanism by which cognitive function, episodic verbal memory and emotions are altered in depression. Chronic stress exposure and depression leads to hippocampal atrophy and cell loss as well as to decreased expression of neurotrophic growth factors. All types of antidepressant drugs reverse or block the effects of stress. Chronic antidepressant administration upregulates neurogenesis and neuroplasticity in the adult hippocampus and these cellular responses are required for the effects of antidepressants in animal models of depression.     

Enjeux de la biologie délocalisée en milieu hospitalier et contractualisation

La réussite d’une délocalisation des analyses de biologie médicale est la résultante d’une volonté de coopération entre les trois partenaires que sont le clinicien, le biologiste et les services administratifs. Elle s’appuie sur la répartition clairement définie des responsabilités en termes d’utilisation de résultats et de management, la mise en place de procédures de fonctionnement et un suivi continu de la totalité du processus. Elle ne peut se passer d’une liaison informatique en temps réel avec le laboratoire qui permet de respecter les règles de l’assurance qualité et du GBEA. Bien ma?trisée, et appliquée à bon escient, elle peut représenter une solution pour améliorer la prise en charge du patient.     

Valeur de la TEP-FDG dans la détection précoce des métastases osseuses : étude d’une série de 20 cas

FDG-PET is useful for metabolic analysis of metastatic cells. Based on a retrospective study relating to 20 cases of osseous metastases (MO), and a literature review, we will try to specify the main characteristics of bone metastases detected with FDG-PET. In our study, the primary malignant findings included lymphoma (n = 7), lung cancer (n = 5), breast cancer (n = 4), colon cancer (n = 2), melanoma (n = 1), and nasopharyngeal cancer (n = 1). On FDG-PET, 4 patients had a solitary metastatic focus (20%), 1 patient had two metastatic focuses (5%) and 15 patients had multiple lesions (75%). Most of the lesions showed intense abnormal uptake with SUVmax > 4.0 (14 patients, 70%); 6 patients had both intense and moderate FDG uptake with SUVmax > 2.1. Only 6 patients had MO diagnosed with standard diagnostic procedures (SDP) before FDG-PET. However, FDG-PET consistently revealed more metastatic foci missed by SDP. We conclude that FDG-PET is better than SDP in detecting all bone metastases at all sites of the skeleton according to the data of the literature.     

Pathologie de la mémoire: du déficit lésionnel au trouble fonctionnel

The purpose of this presentation of pathological data is to demonstrate how our ideas have developed from a chronological point of view to our present interest in elementary cognitive processes. The present relative rarity of significant data in brain imagery, compared with what is observed in other fields of neuropsychology, is probably due to the complexity of this cognitive processes.     

Polyéthisme et défense de la société chez la fourmi champignonnisteAtta laevigata (Fr. Smith)

Résumé L'étude du comportement de défense du nid chezAtta laevigata permet de classer les 4 castes physiques d'individus stériles étudiés en deux groupes. Le premier groupe, constitué par les ouvrièresminor etmedia, rassemble des individus pouvant reconnaître avec précision des intrus de même espèce. Menacés et mordus, ces derniers sont alors mutilés ou transportés puis abandonnés par les fourmis résidentes aux frontières de leur territoire. Les ouvrièresmajor et les soldats, formant le second groupe, nécessitent quant à eux le marquage de l'ennemi par la phéromone d'alarme de leurs congénères pour pouvoir prendre part aux affrontements intraspécifiques avec efficacité. Les soldats se montrent en revanche très performants dans la défense du nid contre des envahisseurs appartenant à d'autres espèces, développant dans ce cas une agressivité supérieure à celle observée chez les ouvrières. L'âge de ces dernières ou le type de tâche qu'elles assument ne semble pas influencer leur comportement agonistique qui est comparable dans les deux castes temporelles de récolteuses et d'architectes.

Polyethism and nest-defense in the leaf-cutting ant Atta laevigata (Fr. Smith)

Summary The study of the nest-defense behavior inAtta laevigata makes out the four physical castes of sterile individuals studied in two groups.Minor andmedia workers (first group) are able to recognize precisely the intruders of the same species that they threat, bite and mutilate or carry outside the territorial limits of the nest. As formajor workers and soldiers (second group), they need their ennemy to be impregnated by the alarm pheromone of their nestmates before they can take part with efficiency in the intraspecific fights. Soldiers though appear to be quite performent in the nest-defense against invaders from other species, showing in this case an aggressiveness superior to the one observed amongst workers. Age, for these, or the sort of work they assume don't seem to interfere with the agonsitic behavior that can be observed in both architects and foragers temporal castes.
