The amount of radiographically detectable cortical bone, as determined by measurements of the second metacarpal, was evaluated in 42 male and 45 female Guamanian Chamorros and compared with the degree of bony demineralization in U.S. Caucasians participating in the Baltimore Longitudinal Study on Aging of the Gerontology Research Center. All Chamorros were individually matched to the Caucasian participants for age, sex, and menopause status. Chamorros of both sexes showed bilateral asymmetry in bone measurements and in the amount of cortical bone. Both Chamorro and Caucasian males had longer second metacarpals and more cortical bone than females. Caucasian males, however, had longer and larger second metacarpals than Chamorro males. Despite differences in the length and total width, Chamorro and Caucasian participants generally showed no significant differences in the amount of cortical bone or percent cortical area in the second metacarpal, suggesting that larger bones may not always indicate greater cortical mass. Although cross-sectional data suggested apparent age differences in the onset and rate of bone loss between Chamorros and Caucasians, the numbers of participants were too small to allow meaningful age-by-age statistical comparisons.  相似文献

Bone mass, as assessed by measurements of total subperiosteal diameter and medullary width of the second metacarpal bone on hand-wrist radiographs, was evaluated for 31 Guamanian patients (15 males, 16 females) with amyotrophic lateral sclerosis (ALS), 67 patients (39 males, 28 females) with parkinsonism-dementia (PD), and 66 (34 males, 32 females) nonaffected Guamanian controls. Comparisons between the two disease groups and between each disease group and the nonaffected controls were made taking into account the sex, age, and disability status of each participant. At all ages, ALS patients of both sexes had significantly lower percent cortical areas (PCA) than did nonaffected controls. The ALS males also had significantly lower PCA than PD males, although no significant differences were observed between female ALS and PD patients. The PD patients of either sex had a lower PCA when compared to controls, but the differences were not statistically significant. The observed differences in PCA were due solely to increased medullary width, suggesting that the diminished cortical bone thickness resulted from greater bone resorption rather than differential bone growth. Longitudinal studies support the cross-sectional findings of accelerated bone loss among ALS patients. It is not possible to determine from the present data whether the observed differences in PCA of the second metacarpal of the ALS patients are due to atrophy of the first interosseous muscle, to a generalized resorption process inherently associated with the development and progression of ALS, or to factors not accounted for by the present analysis.  相似文献

Among 227 chronic renal disease patients, micrometer caliper radiogrammetric measurements of the second metacarpal at midshaft showed the right metacarpal to be larger (with greater bone area) and with a greater cortical area than the left second metacarpal, both in 208 right-handed individuals and in 19 left-handed individuals. This direction of asymmetry was individually characteristic of the majority of individuals, whether right-handed, left-handed or ambidextrous.  相似文献

The rate of decrease of mean metacarpal cortical width was assessed by sequential radiographic morphometry for each hand separately in a series of 46 right-handed postmenopausal women observed for 222 patient-years. For the left hand, the mean rate was 0.0331 ± 0.0053 mm/year and for the right, 0.0310 ± 0.0060 mm/year. The difference between the hands in the rate of decrease was not significant. It is concluded that the higher work load of the dominant hand has no effect on the rate of bone loss from the metacarpals of postmenopausal women.  相似文献

The sexual dimorphism in second metacarpal bone growth was investigated in 710 malnourished Guatemalan children one to seven years old to determine if the sex differences seen are only the result of differences in stature and weight. The study sample was mixed-longitudinal and consisted of 1,586 annual examinations. Boys have greater mean stature, weight, periosteal diameter, medullary diameter and cortical area than girls the same age, while girls have greater age specific mean cortical thickness and percent cortical area than boys. When the effects of stature, weight and age are removed boys still have significantly larger periosteal and medullary diameters and less cortical thickness and percent cortical area than girls. These differences between boys and girls therefore cannot be explained by sex differences in body size. However, no sex differences in cortical area remain after accounting for differences in stature, weight and age.  相似文献

Two hundred thirty–five (235) normal male participants of the Baltimore Longitudinal Study were classified as right handed, left handed, and ambidextrous on the basis of their grip–strength performance. Their left and right hands were also radiographed and the measurements of the second metacarpal bones were evaluated on the basis of hand dominance. The results indicated that, as a rule, the right hand measurements are higher than those of the left hand, regardless of hand dominance. The bilateral differences in total width, length, total area and cortical area are significant among the right hand dominant and nonsignificant among the left hand dominant. Regardless off hand dominance the bilateral differences in medullary width are nonsignificant. These results suggest an inherent tendency of the right second metacarpal to have more bone than the left regardless of hand dominance. Differential stress due to hand dominance will increase the bilateral difference in the right handed and reduce it in the left handed.  相似文献

本文测量1607例正常儿童青少年第Ⅱ—Ⅳ蹠骨皮质厚度和皮质面积,并计算出三种相对指数。结果表明皮质厚度和皮质面积均随年龄增长逐渐增大,其发育在性别间有差异,其中前者出现两次交叉现象,符合人体发育规律;三种指数在年龄性别间无明显变化。本文认为蹠骨同掌骨一样也是骨量测量的最佳部位,建议最好用皮质厚度结合一种指数的方法判断骨量。本文为代谢性骨病的诊断和生长发育的评价提供了基础资料。  相似文献

Study of bone mass at the second metacarpal midshaft has contributed to our understanding of skeletal growth and aging within and between populations and has relied extensively on noninvasive techniques and in particular radiogrammetric data. This study reports age, sex, and side variation in size and shape data acquired from direct measurement of cross-sections obtained from a large (n = 356), homogeneous skeletal sample. Correlation analysis and three-way ANOVA of size-adjusted data confirm general impressions of patterned variation in this element: males have absolutely but not necessarily relatively larger bones than females; the right side is larger than the left, though a larger than expected proportion (approximately 25%) of left metacarpals exhibits greater values than the right; and bone mass but not strength (in males) declines with age. Contrary to the widely accepted assumption of circularity for this location, direct measurement of cross-sectional geometry confirms previous biplanar radiogrammetric conclusions regarding the noncircularity of the second metacarpal midshaft and identifies a significant difference between males and females, with the latter having a more cylindrical diaphysis. Deviation of the axes of maximum and minimum bending strength associated with noncircularity suggests a distribution of bone mass to resist bending moments perpendicular to the distal palmar arch, though this conclusion awaits more robust study of the functional anatomy of the metacarpal diaphysis. Cortical bone width measurements taken at midshaft on the second metacarpal were obtained from 156 hand X-rays of 80 karyotypically documented individuals with Turner's syndrome age 1 to 25 years. Total shaft width, medullary width, cortical width and percent cortical area were grouped by bone age and compared with normal female standards. Total width was significantly and increasingly below normal; medullary width was not consistently different from normal; cortical width was significantly lower from normal from age 14 onward, although it did rise at age 17 (adult bone age); percent cortical area was significantly below normal at ages 14 and 15, but was normal by adulthood. Values for percent cortical area did not indicate severe or widespread osteoporosis. Within the Turners sample cortical bone measurement were not significantly decreased in the presence of the XO sex chromosome constitution compared with other sex chromosome variants. Nor were the measurements decreased in the presence of positive metacarpal sign or a combination of typical Turner stigmata (web neck, low posterior hairline, shield chest). There was evidence that cortical width and percent cortical area increased significantly following estrogen treatment or spontaneous menarche.  相似文献

Heritability of skeletal maturity and bone growth is discussed on the basis of intrapair resemblances (correlation coefficients) and differences (revised percent deviations) in mono- and dizygotic twins and random pairs of unrelated children. A series of 1072 radiographs of the right hand and wrist in 63 male and 70 female pairs of monozygotic twins, and 25 male and 21 female pairs of dizygotic twins aged 12 to 18 years studied longitudinally in Tokyo were used. Skeletal maturity based on the TW2 age suggests higher heritability than bone growth based on the second metacarpal dimensions. In the latter, length has higher heritability than width and cortical thickness. A dosage effect seems to be suggested in width and some indices in the second metacarpal, but is not confirmed in skeletal maturation.  相似文献

The high prevalence of thumb carpometacarpal (CMC) joint osteoarthritis (OA) in women has been previously linked to the articular morphology of the trapezium. Studies report conflicting results on how the articular shapes of male and female trapezia compare to one another, however, mainly because their findings are based on data from older cadaveric specimens. The purpose of this in vivo study was to dissociate the effect of sex from that of aging and early OA by using cohorts of healthy young and healthy older subjects, as well as patients with early stage OA. Computed tomography scans from 68 healthy subjects and 87 arthritic subjects were used to obtain 3-D bone models. The trapezial and metacarpal articular surfaces were manually delineated on scaled bone models and compared between sex, age, and health groups by using polar histograms of curvature and average curvatures. We found no sex-related differences, but significant age-group and health-group differences, in the articular surfaces of both bones. Older healthy subjects had higher curvature in the concave and lower curvature in the convex directions of both the trapezial and metacarpal saddles than healthy young subjects. Subjects with early OA had significantly different metacarpal and trapezial articular shapes from healthy subjects of the same age group. These findings suggest that aging and OA affect the articular shape of the CMC joint, but that, in contrast to previously held beliefs, inherent sex differences are not responsible for the higher incidence of CMC OA in women.  相似文献

Introduction

The objective of this study was to assess three-dimensional bone geometry and density at the epiphysis and shaft of the third meta-carpal bone of rheumatoid arthritis (RA) patients in comparison to healthy controls with the novel method of peripheral quantitative computed tomography (pQCT).

Methods

PQCT scans were performed in 50 female RA patients and 100 healthy female controls at the distal epiphyses and shafts of the third metacarpal bone, the radius and the tibia. Reproducibility was determined by coefficient of varia-tion. Bone densitometric and geometric parameters were compared between the two groups and correlated to disease characteristics.

Results

Reproducibility of different pQCT parameters was between 0.7% and 2.5%. RA patients had 12% to 19% lower trabecular bone mineral density (BMD) (P ≤ 0.001) at the distal epiphyses of radius, tibia and metacarpal bone. At the shafts of these bones RA patients had 7% to 16% thinner cortices (P ≤ 0.03). Total cross-sectional area (CSA) at the metacarpal bone shaft of pa-tients was larger (between 5% and 7%, P < 0.02), and relative cortical area was reduced by 13%. Erosiveness by Ratingen score correlated negatively with tra-becular and total BMD at the epiphyses and shaft cortical thickness of all measured bones (P < 0.04).

Conclusions

Reduced trabecular BMD and thinner cortices at peripheral bones, and a greater bone shaft diameter at the metacarpal bone suggest RA spe-cific bone alterations. The proposed pQCT protocol is reliable and allows measuring juxta-articular trabecular BMD and shaft geometry at the metacarpal bone.  相似文献

Bilateral asymmetry in the structure of the second metacarpal was examined in relation to functional hand dominance in a large, clinically nonselected, healthy population sample from the Baltimore Longitudinal Study of Aging. Bilateral bone measurements were made from anteroposterior hand radiographs of a total of 992 individuals, 609 males and 383 females, with an age range of 19–94 years. Hand dominance was determined on the basis of personal impression. Total width and medullary width at the midshaft of the second metacarpal were measured to 0.05 mm using a Helios caliper. These two measurements were used to derive cortical thickness, cortical bone area, periosteal (total) area, medullary area, percent cortical area, and the second moment of area in the mediolateral plane. In both right and left-handed individuals, statistically significant side differences were found in the calculated bone areas and the second moment of area, with the dominant hand being larger. Cortical thickness did not show significant side-related differences for either handedness. These results show that functional handedness leads to periosteal and endosteal expansion of the second metacarpal cortex on the dominant side, increasing bone strength without increasing cortical thickness. This is the first time this pattern of asymmetry has been reported in left-handers as well as right-handers. Our results argue for the primacy of environmental (mechanical) effects in determining bilateral asymmetry of limb bone structural properties. © 1994 Wiley-Liss, Inc.  相似文献

Abstract: Bone mass and bone density were estimated in 219 pedigreed baboons (Papio hamadryas) by radiographic morphometry of the left second metacarpal. Compact bone width (total bone width – medullary canal diameter) and bone ratio (compact bone width/total bone width) decreased with increasing age squared in both sexes. The heritability of medullary canal diameter was 0.64±0.11, of compact bone width was 0.40±0.15 and of bone ratio was 0.67±0.13. The results indicate baboons are a useful model for studies of age, sex and genetic effects on bone mass.  相似文献

目的:探讨2型糖尿病患者冠脉钙化积分与骨密度的关系。方法:入选在2015年6月至2017年6月我科住院的2型糖尿病患者98例,按照性别以及糖化血红蛋白8%、低密度脂蛋白2.6 mmol/L、身高体重指数24 kg/m2为切点分为两组,进行冠脉动脉CT检查,计算冠脉动脉总钙化积分,依照钙化积分等于0为阴性组,大于0为阳性组。同时检测患者的骨密度、骨代谢指标、血钙、血磷等水平。比较冠脉钙化积分和骨代谢指标及骨密度的相关性。结果:1)男性患者中:阳性组糖化血红蛋白和颈部血管斑块形成明显高于阴性组,差异有统计学意义。女性患者中:阳性组年龄、颈部血管内膜厚度、高血压病史明显高于阴性组,差异有统计学意义。2)阳性、阴性两组之间骨代谢指标差异无统计学意义。3)男性患者中:阳性组腰椎骨密度明显高于阴性组,差异有统计学意义。女性患者中阳性组的腰椎骨密度明显低于阴性组,差异有统计学意义,在控制混杂因素以后,两组间差异无统计学意义。4)分别在男性和女性患者中按照年龄为60岁分为两组,两组间阳性组与阴性组的骨代谢指标及骨密度差异无统计学意义。结论:在2型糖尿病患者中,冠脉钙化积分和骨代谢以及骨密度之间无明显相关性。  相似文献

Geometric properties of the third metacarpal bone were compared between the young and adult Standardbred and Thoroughbred racehorse. The change in shape during growth and superimposed training was dramatic in both breeds but the Thoroughbred showed the greatest difference in the minimum moment of inertia as the animal matured. Males had larger moments of inertia throughout the length of the diaphysis than did females. The differences in geometric properties of the third metacarpal bone between the Thoroughbred and Standardbred were related to the incidence of fatigue fractures which are common in the racing Thoroughbred but uncommon in the Standardbred racehorse.  相似文献

Changes in metacarpal cortical bone mineral density (BMD) using digital x-ray radiogrammetry were studied in patients with early rheumatoid arthritis. After 1, 2, and 5 years, large BMD losses were found: -1.7%, -2.8%, and -5.6%, respectively. Elevated erythrocyte sedimentation rate and anti-cyclic citrullinated peptide levels were independent predictors of bone loss, indicating that the largest amount of bone loss was found in patients with severe inflammation and high production of auto-antibodies, who are known to be at the highest risk of developing radiological bone damage. Studies are needed about the spatial and time relationship between erosions and juxta-articular and metacarpal bone loss.  相似文献

Additional and extended data on 2726 subjects from Costa Rica, Honduras, Nicaragua and Panama provide confirming evidence for continuing bone expansion of the second metacarpal at midshaft. As shown in 4924 subjects from eight populations, subperiosteal apposition is more rapid in the female than in the male, and accounts for a net increase of 10% in cross-sectional area from age 25 to age 85. Unlike endosteal resorption, which is precipitous in onset, adult subperiosteal gain is continuous from the third through the ninth decades.  相似文献

Insulin-like growth factor-binding protein 2 (IGFBP-2) is a member of a family of six highly conserved IGFBPs that are carriers for the insulin-like growth factors (IGFs). IGFBP-2 levels rise during rapid neonatal growth and at the time of peak bone acquisition. In contrast, Igfbp2(-/-) mice have low bone mass accompanied by reduced osteoblast numbers, low bone formation rates, and increased PTEN expression. In the current study, we postulated that IGFBP-2 increased bone mass partly through the activity of its heparin-binding domain (HBD). We synthesized a HBD peptide specific for IGFBP-2 and demonstrated in vitro that it rescued the mineralization phenotype of Igfbp2(-/-) bone marrow stromal cells and calvarial osteoblasts. Consistent with its cellular actions, the HBD peptide ex vivo stimulated metacarpal periosteal expansion. Furthermore, administration of HBD peptide to Igfbp2(-/-) mice increased osteoblast number, suppressed marrow adipogenesis, restored trabecular bone mass, and reduced bone resorption. Skeletal rescue in the Igfbp2(-/-) mice was characterized by reduced PTEN expression followed by enhanced Akt phosphorylation in response to IGF-I and increased β-catenin signaling through two mechanisms: 1) stimulation of its cytosolic accumulation and 2) increased phosphorylation of serine 552. We conclude that the HBD peptide of IGFBP-2 has anabolic activity by activating IGF-I/Akt and β-catenin signaling pathways. These data support a growing body of evidence that IGFBP-2 is not just a transport protein but rather that it functions coordinately with IGF-I to stimulate growth and skeletal acquisition.  相似文献

为了了解成年太行山猕猴5根掌骨的性差大小。本文对太行山猕猴掌骨标本40例(雄:10, 雌:30)进行观察并选择掌骨的9项形态变量进行测量。数据用SPSS19.0软件进行多变量判别分析。结果表明: 5根掌骨长度等变量在性别之间有明显差异。用全模型法5根掌骨的性别正确判别率范围为94.1%~100.0%, 用逐步判别法掌骨性别正确判别率范围为93.8%~97.5%。有关掌骨的9个形态变量, 长度变量首先被挑选出来, 说明在性别判别中起重要作用。分别用左右侧掌骨的判别函数来判别性别时侧别差异不明显。结论: 用每根掌骨的形态变量建立判别函数可以有效地区分性别, 对非人灵长类掌骨标本的性别鉴定有一定的理论意义和应用价值。  相似文献
