Quantitative changes in valgus/varus knee stability with different levels of muscular activity were determined for five subjects. A specially designed machine was used to measure resistance to angulation in the frontal plane. This device held the thigh stationary, the knee straight, an cycled the leg from side to side at a constant rate between present moment limits. Resistance to this forced valgus/varus motion was measured simultaneously with torque about the knee in the sagittal plane. Muscle activity was monitored by electromyography (EMG). Direct comparison of moment-rotation characteristics allowed changes in stability to be quantified as a function of extension and flexion torque. Extension torques less than 20% of the maximum increased varus stability more than valgus stability. Flexion torques of the same relative magnitude increased valgus stability more than varus stability. Comparison with the literature suggested that prevention of opening of the lateral side of the joint under varus loading was responsible for increased varus stability with increasing torque, both with extension and flexion torques.  相似文献

Malalignment is the main cause of tibial component loosening. Implants that migrate rapidly in the first two post-operative years are likely to present aseptic loosening. It has been suggested that cancellous bone stresses can be correlated with tibial component migration. A recent study has shown that patient-specific finite element (FE) models have the power to predict the short-term behavior of tibial trays. The stresses generated within the implanted tibia are dependent on the kinematics of the joint; however, previous studies have ignored the kinematics and only applied static loads. Using explicit FE, it is possible to simultaneously predict the kinematics and stresses during a gait cycle. The aim of this study was to examine the cancellous bone strains during the stance phase of the gait cycle, for varying degrees of varus/valgus eccentric loading using explicit FE. A patient-specific model of a proximal tibia was created from CT scan images, including heterogeneous bone properties. The proximal tibia was implanted with a commercial total knee replacement (TKR) model. The stance phase of gait was simulated and the applied loads and boundary conditions were based on those used for the Stanmore knee simulator. Eccentric loading was simulated. As well as examining the tibial bone strains (minimum and maximum principal strain), the kinematics of the bone-implant construct are also reported. The maximum anterior-posterior displacements and internal-external rotations were produced by the model with 20 mm offset. The peak minimum and maximum principal strain values increased as the load was shifted laterally, reaching a maximum magnitude for -20 mm offset. This suggests that when in varus, the load transferred to the bone is shifted medially, and as the bone supporting this load is stiffer, the resulting peak bone strains are lower than when the load is shifted laterally (valgus). For this particular patient, the TKR design analyzed produced the highest cancellous bone strains when in valgus. This study has provided an insight in the variations produced in bone strain distribution when the axial load is applied eccentrically. To the authors' knowledge, this is the first time that the bone strain distribution of a proximal implanted tibia has been examined, also accounting for the kinematics of the tibio-femoral joint as part of the simulation. Knee osteoarthritis occurs predominately at the medial compartment. To unload the affected compartment, valgus braces are used which induce an additional valgus moment in order to shift the load more laterally. Until now the biomechanical effect of braces was mainly evaluated by measuring changes in external knee adduction moments. The aim of this study was to investigate if and to which extent the medial compartment load is reduced in vivo when wearing valgus braces. Six components of joint contact load were measured in vivo in three subjects, using instrumented, telemeterized knee implants. From the forces and moments the medio-lateral force distribution was calculated. Two braces, MOS Genu (Bauerfeind AG) and Genu Arthro (Otto Bock) were investigated in neutral, 4° and 8° valgus adjustment during walking, stair ascending and descending. During walking with the MOS brace in 4°/8° valgus adjustment, medial forces were reduced by 24%/30% on average at terminal stance. During walking with the GA in the 8° valgus position, medial forces were reduced by only 7%. During stair ascending/descending significant reductions of 26%/24% were only observed with the MOS (8°). The load reducing ability of the two investigated valgus braces was confirmed in three subjects. However, the load reduction depends on the brace stiffness and its valgus adjustment and varies strongly inter-individually. Valgus adjustments of 8° might, especially with the MOS brace, not be tolerated by patients for a long time. Medial load reductions of more than 25% can therefore probably not be expected in clinical practise.  相似文献

Knee ligamentous injuries persist in the sport of Alpine skiing. To better understand the load mechanisms which lead to injury, pure varus/valgus and pure axial moments were applied both singly and in combination to the right knees of six human test subjects. The corresponding relative knee rotations in three degrees of freedom were measured. Knee flexion angles for each test subject were 15 and 60 degrees for the individual moments and 60 degrees for the combination moments. For both knee flexion angles the hip flexion angle was 0 degrees. Leg muscles were quiescent and axial force was minimal during all tests. Tables of data include sample statistics for each of four flexibility parameters in each loading direction. Data were analyzed statistically to test for significant differences in flexibility parameters between the test conditions. In flexing the knee from 15 to 60 degrees, the resulting knee rotations under single moments depended upon flexion angle with varus, valgus, and internal rotations increasing significantly. Also, rotations were different depending on load direction; varus rotation was significantly different and greater than valgus rotation at both flexion angles. Also external rotation was significantly different and greater than internal at 15 degrees flexion, but not at 60 degrees flexion. Coupled rotations under single moments were also observed. Applying pure varus/valgus moments resulted in coupled external/internal rotations which were inconsistent and hence not significant. Applying pure axial moments resulted in consistent and hence significant varus/valgus rotations; an external axial moment induced varus rotation and an internal axial moment induced valgus rotation. For combination moments, varus/valgus rotations decreased significantly from those rotations at similar load levels in the single moment studies. Also, a varus moment significantly increased external rotation and a valgus moment significantly decreased internal rotation. These differences indicate significant interaction between corresponding load combinations. These results suggest that load interaction is a potentially important phenomenon in knee injury mechanics.  相似文献

This study aimed to determine the influence of knee varus (VARUS) and valgus (VALGUS) on the differences in individual quadriceps muscle (QM) activity during knee extension maximum voluntary isometric contractions (MVICs) and sit/stand transitions and on the changes in individual QM activity during sit/stand transitions after QM stretching and kneeling. Ten young healthy males each with VARUS and VALGUS were included. The electromyography signals of the vastus medialis (VM), vastus lateralis, and rectus femoris were recorded during sit/stand transitions before and after rest, stretching, and kneeling and during knee extension MVICs after rest. The individual muscle-to-total muscle activity ratio was assessed. The VARUS group exhibited a significantly higher VM muscle activity ratio in the sit-to-stand and stand-to-sit tasks than in knee extension MVICs (p = 0.004 and p = 0.044, respectively) and a tendency that the VM muscle activity ratio increased in the sit-to-stand task after stretching (p = 0.051), whereas the VALGUS group exhibited no significance. Individuals with VARUS required high VM muscle activity ratios during sit/stand transitions. Future studies should be conducted to determine whether habitual sit-to-stand exercises after QM stretching are effective in preventing knee medial osteoarthritis development in individuals with VARUS.  相似文献

In this paper we studied how subjects activate their muscles in response to static varus and valgus loads at the knee. The muscles' contributions to the external moments were estimated using an EMG driven biomechanical model of the knee. The individual muscle activation and loading patterns were examined to identify the strategies that the nervous system uses to support varus and valgus knee moments. It was found that the (1) co-contraction of the hamstrings and quadriceps, and (2) activation of the gracilis and tensor fascia lata increased with the increasing magnitude of the varus and valgus moments. These 2 activation patterns provided positive support of valgus and varus loads at the knee The sartorius appears to be activated to provide positive support of valgus loads at the knee, whereas during varus moments this muscle increases the varus load on the knee, i.e. provides negative support. Generally, the hamstrings and quadriceps co-contraction contributed to most of the muscular support of the varus and valgus moments. In addition, co-contraction supported 11-14% of the external moment in pure varus and pure valgus respectively. It appears that there are activation strategies with the specific purpose to support varus and valgus moments, albeit small, which suggest dual goals of the neuromotor system during the support of varus and valgus moments.  相似文献

Debris-induced osteolysis due to surface wear of ultra high molecular weight polyethylene (UHMWPE) bearings is a potential long-term failure mechanism of total knee replacements (TKR). This study investigated the effect of prosthesis design, kinematics and bearing material on the wear of UHMWPE bearings using a physiological knee simulator. The use of a curved fixed bearing design with stabilised polyethylene bearings reduced wear in comparison to more flat-on-flat components which were sterilised by gamma irradiation in air. Medium levels of crosslinking further improved the wear resistance of fixed bearing TKR due to resistance to strain softening when subjected to multidirectional motion at the femoral-insert articulating interface. Backside motion was shown to be a contributing factor to the overall rate of UHMWPE wear in fixed bearing components. Wear of fixed bearing prostheses was reduced significantly when anterior-posterior displacement and internal-external rotation kinematics were reduced due to decreased cross shear on the articulating surface and a reduction in AP displacement. Rotating platform mobile bearing prostheses exhibited reduced wear rates in comparison to fixed bearing components in these simulator studies due to redistribution of knee motion to two articulating interfaces with more linear motions at each interface. This was observed in two rotating platform designs with different UHMWPE bearing materials. In knee simulator studies, wear of TKR bearings was dependent on kinematics at the articulating surfaces and the prosthesis design, as well as the type of material.  相似文献

The potential for wear in UHMWPE components for total knee replacements can be reduced by decreasing the stresses and strains arising from tibial-femoral contact. The conformity of the articular surfaces has a large effect on the resultant stresses, and components that achieve flat medial-lateral contact have been assumed to produce the lowest stresses due to their perfect conformity. We computed the stresses arising from curved and flat contact on a half-space using two-dimensional, plane strain elasticity solutions and finite element analyses to compare the performance of curved and flat indenters. These indenters were represented by a polynomial so the profiles could be continuously varied from curved to flat. Curved contact resulted in maximum stresses at the center of contact, while flat contact produced maximum stresses at the edge of contact. In addition, three contemporary tibial configurations (flat-on-flat, curved-on-flat, and curved-on-curved geometries) were analyzed using the finite element method with nonlinear material properties. The maximum contact stress, von Mises stress, and von Mises strain were lowest for the curved-on-curved model. The other configurations resulted in higher contact stresses, von Mises stresses, and von Mises strains. The perfect conformity arising from flat contact did not reduce the contact stresses in the UHMWPE component. The tensile stresses, however, were lowest for the flat-on-flat geometry compared with the other two configurations. Relating these distinct differences could prove useful in interpretation of data from simulator and retrieval studies.  相似文献

Fundamental knowledge about in vivo kinematics and contact conditions at the articulating interfaces of total knee replacements are essential for predicting and optimizing their behavior and durability. However, the prevailing motions and contact stresses in total knee replacements cannot be precisely determined using conventional in vivo measurement methods. In silico modeling, in turn, allows for a prediction of the loads, velocities, deformations, stress, and lubrication conditions across the scales during gait. Within the scope of this paper, we therefore combine musculoskeletal modeling with tribo-contact modeling. In the first step, we compute contact forces and sliding velocities by means of inverse dynamics approach and force-dependent kinematic solver based upon experimental gait data, revealing contact forces during healthy/physiological gait of young subjects. In a second step, the derived data are employed as input data for an elastohydrodynamic model based upon the finite element method full-system approach taking into account elastic deformation, the synovial fluid’s hydrodynamics as well as mixed lubrication to predict and discuss the subject-specific pressure and lubrication conditions.

Subject-specific models were developed and finite element analysis was performed to observe the effect of the frontal plane tibiofemoral angle on the normal stress, Tresca shear stress and normal strain at the surface of the knee cartilage. Finite element models were created for three subjects with different tibiofemoral angle and physiological loading conditions were defined from motion analysis and muscle force mathematical models to simulate static single-leg stance. The results showed that the greatest magnitude of the normal stress, Tresca shear stress and normal strain at the medial compartment was for the varus aligned individual. Considering the lateral knee compartment, the individual with valgus alignment had the largest stress and strain at the cartilage. The present investigation is the first known attempt to analyze the effects of tibiofemoral alignment during single-leg support on the contact variables of the cartilage at the knee joint. The method could be potentially used to help identify individuals most susceptible to osteoarthritis and to prescribe preventive measures.  相似文献

Background

The underuse of total joint arthroplasty in appropriate candidates is more than 3 times greater among women than among men. When surveyed, physicians report that the patient''s sex has no effect on their decision-making; however, what occurs in clinical practice may be different. The purpose of our study was to determine whether patients'' sex affects physicians'' decisions to refer a patient for, or to perform, total knee arthroplasty.

Methods

Seventy-one physicians (38 family physicians and 33 orthopedic surgeons) in Ontario performed blinded assessments of 2 standardized patients (1 man and 1 woman) with moderate knee osteoarthritis who differed only by sex. The standardized patients recorded the physicians'' final recommendations about total knee arthroplasty. Four surgeons did not consent to the inclusion of their data. After detecting an overall main effect, we tested for an interaction with physician type (family physician v. orthopedic surgeon). We used a binary logistic regression analysis with a generalized estimating equation approach to assess the effect of patients'' sex on physicians'' recommendations for total knee arthroplasty.

Results

In total, 42% of physicians recommended total knee arthroplasty to the male but not the female standardized patient, and 8% of physicians recommended total knee arthroplasty to the female but not the male standardized patient (odds ratio [OR] 4.2, 95% confidence interval [CI] 2.4–7.3, p < 0.001; risk ratio [RR] 2.1, 95% CI 1.5–2.8, p < 0.001). The odds of an orthopedic surgeon recommending total knee arthroplasty to a male patient was 22 times (95% CI 6.4–76.0, p < 0.001) that for a female patient. The odds of a family physician recommending total knee arthroplasty to a male patient was 2 times (95% CI 1.04–4.71, p = 0.04) that for a female patient.

Interpretation

Physicians were more likely to recommend total knee arthroplasty to a male patient than to a female patient, suggesting that gender bias may contribute to the sex-based disparity in the rates of use of total knee arthroplasty.Disparity in the use of medical or surgical interventions based on patient characteristics, such as sex, ethnic background or socioeconomic status, is an important health care issue.1 Women are less likely than men to receive lipid-lowering medication after a myocardial infarction,2 receive kidney dialysis,3 be admitted to an intensive care unit,4 or undergo cardiac catheterization,5 renal transplantation6 or total joint arthroplasty.7 Although women''s preferences for surgery or the information needed to make an informed decision may differ from men and explain sex-based differences in care,8,9 subtle or overt gender bias may inappropriately influence physicians'' clinical decision-making.2,5,7 A more pronounced gender bias might be expected when the clinical decision involves an elective surgical procedure such as total joint arthroplasty.Total hip and knee arthroplasty is the definitive treatment for relieving pain and restoring function in people with moderate to severe osteoarthritis for whom medical therapy has failed.10 Although age-adjusted rates of total joint arthroplasty are higher among women than among men,11 based on a population-based epidemiologic survey, underuse of arthroplasty is 3 times greater in women.7 In prior opinion surveys, more than 93% of referring physicians and orthopedic surgeons have reported that patients'' sex has no effect on their decision to refer a patient for, or perform, total knee arthroplasty.12,13 However, there may be a difference between what is reported in a survey and what occurs in clinical practice. The purpose of our study was to determine whether physicians would provide the same recommendation about total knee arthroplasty to a male and a female standardized patient presenting to their offices with identical clinical scenarios that differed only by sex.  相似文献

Excessive wear in total knee arthroplasty is detected by measuring the minimum joint space width (mJSW) in anterioposterior radiographs. The accuracy of conventional measurement methods is limited and can be improved using model-based techniques. In this study, the model-based wear measurement (MBWM) is introduced. Its accuracy and reproducibility are assessed and compared to the conventional measurement. Forty anterioposterior radiographs were obtained of a knee prosthesis using a phantom setup. Both measurement methods were applied and the accuracy and precision were compared. The reproducibility was calculated with inter- and intra-observer experiments. Three observers measured the mJSW in 30 clinical radiographs with both the conventional measurement and the MBWM and repeated this after 6 weeks. The experiments were conducted with a NexGen mobile bearing and fixed bearing prostheses. In the phantom experiment, the accuracy (mean of the absolute error) was significantly higher (t-test, p<0.01) for the MBWM as for the conventional measurement (0.15 mm versus 0.43 mm, 0.14 mm versus 0.35 mm for the mobile and fixed bearing, respectively). The standard deviation of the measurements is the smallest for the MBWM measurement for both prosthesis types (0.16 mm versus 0.47 mm, Levene's test, p<0.01). In the reproducibility experiment, both the intra- and inter-observer agreements were higher for the MBWM than for the conventional method. The results show that the MBWM is superior to the conventional measurement in both accuracy and reproducibility. Although the use of a phantom experiment poses some limitations in conveying the findings to clinical practice, this improved mJSW measurement can lead to better wear detection for surgery decisions and research purposes.  相似文献

In the natural knee, the femoral tibial contacts move posteriorly as the knee is flexed, guided primarily by the cruciate ligaments. This kinematic behaviour is important regarding muscle lever arms and in achieving a high flexion range. Most contemporary total knee designs use either posterior cruciate preservation or a cam system to produce posterior displacement with flexion, but there is no specific provision for anterior displacement. In this study, a method for the design of cams is described where the cams would guide the motion in both posterior and anterior directions, without requiring cruciate ligaments. The cams consist of a femoral Guide Surface interacting with a tibial Guide Surface while the main lateral and medial bearing surfaces carry the forces across the knee. It is shown that Guide Surfaces can be designed which provide the required motion, but with some laxity at different flexion ranges. It is then demonstrated that the Guide Surfaces can be applied to a range of possible knee designs including mobile-bearing types, rotating-platform types, and fixed-bearing types. The relative advantages of the different possibilities are discussed.  相似文献

The determination of biomechanical force systems of implanted femurs to obtain adequate strain measurements has been neglected in many published studies. Due to geometric alterations induced by surgery and those inherent to the design of the prosthesis, the loading system changes because the lever arms are modified. This paper discusses the determination of adequate loading of the implanted femur based on the intact femur-loading configuration. Four reconstructions with Lubinus SPII, Charnley Roundback, Muller Straight and Stanmore prostheses were used in the study. Pseudophysiologic and nonphysiologic implanted system forces were generated and assessed with finite element analysis. Using an equilibrium system of forces composed by the Fx (medially direction) component of the hip contact force and the bending moments Mx (median plane) and My (coronal plane) allowed adequate, pseudo-physiological loading of the implanted femur. We suggest that at least the bending moment at the coronal plane must be restored in the implanted femur-loading configuration.  相似文献

Background

Osteoarthritis constitutes a major musculoskeletal burden for the aged Australians. Hip and knee replacement surgeries are effective interventions once all conservative therapies to manage the symptoms have been exhausted. This study aims to evaluate the cost-effectiveness of hip and knee replacements in Australia. To our best knowledge, the study is the first attempt to account for the dual nature of hip and knee osteoarthritis in modelling the severities of right and left joints separately.

Methodology/Principal Findings

We developed a discrete-event simulation model that follows up the individuals with osteoarthritis over their lifetimes. The model defines separate attributes for right and left joints and accounts for several repeat replacements. The Australian population with osteoarthritis who were 40 years of age or older in 2003 were followed up until extinct. Intervention effects were modelled by means of disability-adjusted life-years (DALYs) averted. Both hip and knee replacements are highly cost effective (AUD 5,000 per DALY and AUD 12,000 per DALY respectively) under an AUD 50,000/DALY threshold level. The exclusion of cost offsets, and inclusion of future unrelated health care costs in extended years of life, did not change the findings that the interventions are cost-effective (AUD 17,000 per DALY and AUD 26,000 per DALY respectively). However, there was a substantial difference between hip and knee replacements where surgeries administered for hips were more cost-effective than for knees.

Conclusions/Significance

Both hip and knee replacements are cost-effective interventions to improve the quality of life of people with osteoarthritis. It was also shown that the dual nature of hip and knee OA should be taken into account to provide more accurate estimation on the cost-effectiveness of hip and knee replacements.  相似文献

The purpose of this study was to analyse the effect of a valgus knee orthosis designed for patients with knee osteoarthritis on the electromyographic activity (EMG) of seven muscles of the lower limb during gait. Twelve patients with medial knee osteoarthritis walked on a treadmill in three different conditions: without orthosis, with a knee orthosis in 4° valgus adjustment and with an orthosis in a neutral flexible adjustment. Root-mean-square (RMS) was analysed in each condition during a 150 ms pre-activation phase and during the stance phase of gait, which was divided in four sub-phases. In addition, co-contraction ratios (CCRs) were calculated between extensor/flexor, medial/lateral muscles and between agonist and antagonist muscle pairs. Significant decreases in muscle activity and CCRs were observed with the use of the knee orthosis in both adjustments compared to the condition without orthosis. Using the valgus brace, medial/lateral CCR decreased significantly during the late stance and the flexor/extensor CCR decreased significantly during the loading phase and late stance. Decreases of muscle pairs CCRs were observed with the neutral flexible adjustment. The results support the theory of a possible beneficial effect of knee braces in reducing knee loading by decreasing muscle activation and co-contraction levels, which could contribute to decelerate disease progression in patients with knee osteoarthritis.  相似文献

Sixty-five Holstein-Friesian calves were randomly allocated to one of eight nutritional treatments at 4 days of age. In this factorial design study, the treatments comprised of four levels of milk replacer (MR) mixed in 6 l of water (500, 750, 1000 and 1250 g/day) × two crude protein (CP) concentrations (230 and 270 g CP/kg dry matter (DM)). MR was fed via automatic teat feeders and concentrates were offered via automated dispensers during the pre-wean period. MR and calf starter concentrate intake were recorded until weaning with live weight and body measurements recorded throughout the rearing period until heifers entered the dairy herd at a targeted 24 months of age. There was no effect of MR protein concentration on concentrate or MR intake, and no effect on body size or live weight at any stage of development. During the pre-weaning period, for every 100 g increase in MR allowance, concentrate consumption was reduced by 39 g/day. While, for every 100 g increase in the amount of MR offered, live weight at days 28 and 270 increased by 0.76 and 2.61 kg, respectively (P < 0.05). Increasing MR feed levels increased (P < 0.05) heart girth and body condition score at recordings during the first year of life, but these effects disappeared thereafter. Increasing MR feeding level tended to reduce both age at first observed oestrus and age at first service but no significant effect on age at first calving was observed. Neither MR feeding level nor MR CP content affected post-calving live weight or subsequent milk production. Balance measurements conducted using 44 male calves during the pre-weaning period showed that increasing milk allowance increased energy and nitrogen (N) intake, diet DM digestibility, true N digestibility and the biological value of the dietary protein. Increasing the MR protein content had no significant effect on the apparent digestibility of N or DM.  相似文献

The cost and time efficiency of computational polyethylene wear simulations may enable the optimization of total knee replacements for the reduction of polyethylene wear. The present study proposes an energy dissipation wear model for polyethylene which considers the time dependent molecular behavior of polyethylene, aspects of tractive rolling and contact pressure. This time dependent – energy dissipation wear model was evaluated, along with several other wear models, by comparison to pin-on-disk results, knee simulator wear test results under various kinematic conditions and knee simulator wear test results that were performed following the ISO 14243-3 standard. The proposed time dependent – energy dissipation wear model resulted in improved accuracy for the prediction of pin-on-disk and knee simulator wear test results compared with several previously published wear models.  相似文献

Durability and kinematics are two critical factors which must be considered during total knee replacement (TKR) implant design. It is hypothesized, however, that there exists a competing relationship between these two performance measures, such that improvement of one requires sacrifice with respect to the other. No previous studies have used rigorous and systematic methods to quantify this relationship. During this study, multiobjective design optimization (MOO) using the adaptive weighted sum (AWS) method is used to determine a set of Pareto-optimal implant designs considering durability and kinematics simultaneously. Previously validated numerical simulations and a parametric modeller are used in conjunction with the AWS method in order to generate a durability-versus-kinematics Pareto curve. In terms of kinematics, a design optimized for kinematics alone outperformed a design optimized for durability by 61.8%. In terms of durability, the design optimized for durability outperformed the kinematics-optimized design by 70.6%. Considering the entire Pareto curve, a balanced (1:1) trade-off could be obtained when equal weighting was placed on both performance measures; however improvement of one performance measure required greater sacrifices with respect to the other when the weighting was extremized. For the first time, the competing relationship between durability and kinematics was confirmed and quantified using optimization methods. This information can aid future developments in TKR design and can be expanded to other total joint replacement designs.  相似文献
