The impact of new technologies on human population studies   总被引:4,自引:0,他引:4  
Human population studies involve clinical or epidemiological observations that associate environmental exposures with health endpoints and disease. Clearly, these are the most sought after data to support assessments of human health risk from environmental exposures. However, the foundations of many health risk assessments rest on experimental studies in rodents performed at high doses that elicit adverse outcomes, such as organ toxicity or tumors. Using the results of human studies and animal data, risk assessors define the levels of environmental exposures that may lead to disease in a portion of the population. These decisions on potential health risks are frequently based on the use of default assumptions that reflect limitations in our scientific knowledge. An important immediate goal of toxicogenomics, including proteomics and metabonomics, is to offer the possibility of making decisions affecting public health and public based on detailed toxicity, mechanistic, and exposure data in which many of the uncertainties have been eliminated. Ultimately, these global technologies will dramatically impact the practice of public health and risk assessment as applied to environmental health protection. The impact is already being felt in the practice of toxicology where animal experimentation using highly controlled dose-time parameters is possible. It is also being seen in human population studies where understanding human genetic variation and genomic reactions to specific environmental exposures is enhancing our ability to uncover the causes of variations in human response to environmental exposures. These new disciplines hold the promise of reducing the costs and time lines associated with animal and human studies designed to assess both the toxicity of environmental pollutants and efficacy of therapeutic drugs. However, as with any new science, experience must be gained before the promise can be fulfilled. Given the numbers and diversity of drugs, chemicals and environmental agents; the various species in which they are studied and the time and dose factors that are critical to the induction of beneficial and adverse effects, it is only through the development of a profound knowledge base that toxicology and environmental health can rapidly advance. The National Institute of Environmental Health Sciences (NIEHS), National Center for Toxicogenomics and its university-based Toxicogenomics Research Consortium (TRC), and resource contracts, are engaged in the development, application and standardization of the science upon which to the build such a knowledge base on Chemical Effects in Biological Systems (CEBS). In addition, the NIEHS Environmental Genome Project (EGP) is working to systematically identify and characterize common sequence polymorphisms in many genes with suspected roles in determining chemical sensitivity. The rationale of the EGP is that certain genes have a greater than average influence over human susceptibility to environmental agents. If we identify and characterize the polymorphism in those genes, we will increase our understanding of human disease susceptibility. This knowledge can be used to protect susceptible individuals from disease and to reduce adverse exposure and environmentally induced disease.  相似文献

For randomly selected 50 villages in Bangladesh, an interview survey with a structured questionnaire was conducted to reveal their perception on the environmental, health and economic conditions at present and for the past 10-year change. The eight following items were analyzed in this paper: air pollution and water pollution, which represent environmental conditions with close relation to health conditions, soil degradation and deforestation, which represent environmental conditions with close relation to economic conditions, epidemic diseases and malnutrition, which represent health conditions, and poverty and jobless, which represent economic conditions. Among the 50 villages, deforestation was most frequently perceived serious at present and worsened in the past 10 years. Of the remaining seven items, those related to economic conditions were more seriously perceived than those related to health and environmental conditions. As revealed by the cluster analysis for the inter-item relations, epidemic diseases, which formed the same cluster with the environmental items, were recognized less serious whereas malnutrition, which formed the same cluster with the economic items, was recognized more serious. These findings are useful not only for rural development programs but also for mitigation programs toward health and environmental hazards in Bangladesh.  相似文献

In conducting health assessments at hazardous‐waste sites, ATSDR staff must identify environmental chemical contaminants that pose a potential public health hazard. To assist health assessors in this effort, ATSDR has developed environmental media evaluation guides (EMEGs). EMEGs have been developed for water, soil, and air. The EMEG values are expressed as a range to account for the range in exposures in different segments of the population. The use of EMEGs provides health assessors with a consistent strategy for selecting environmental contaminants to be further evaluated in the health assessment process.  相似文献

Nutrition research, like most biomedical disciplines, adopted and often uses experimental approaches based on Beadle and Tatum’s one gene—one polypeptide hypothesis, thereby reducing biological processes to single reactions or pathways. Systems thinking is needed to understand the complexity of health and disease processes requiring measurements of physiological processes, as well as environmental and social factors, which may alter the expression of genetic information. Analysis of physiological processes with omics technologies to assess systems’ responses has only become available over the past decade and remains costly. Studies of environmental and social conditions known to alter health are often not connected to biomedical research. While these facts are widely accepted, developing and conducting comprehensive research programs for health are often beyond financial and human resources of single research groups. We propose a new research program on essential nutrients for optimal underpinning of growth and health (ENOUGH) that will use systems approaches with more comprehensive measurements and biostatistical analysis of the many biological and environmental factors that influence undernutrition. Creating a knowledge base for nutrition and health is a necessary first step toward developing solutions targeted to different populations in diverse social and physical environments for the two billion undernourished people in developed and developing economies.  相似文献

Invertebrates show considerable potential as sentinel organisms for the monitoring of the health status of aquatic systems. They are generally small, abundant, relatively sessile, and may readily bioaccumulate toxins. Cascade-like stress responses can occur following acute or chronic exposures to contaminated environments and as such, the overall health status of individuals within those environments, both in terms of histopathological lesions and the presence of infecting organisms, may ultimately reflect the general health status of these sites. The current study provides baseline multi-organ histopathological data for two common crustacean species, the shore crab (Carcinus maenas) and the brown shrimp (Crangon crangon) collected from six UK estuarine sites. Changes in the metabolic condition of crustaceans from these sites (measured in terms of connective tissue storage cell status) were interpreted in relation to other health measures (including parasite load and the presence of microbial pathogens). The relative ease at which a holistic assessment of health can be made using histopathology and the suitability of these species as environmental sentinels provide support for the inclusion of crustaceans as indicators of aquatic environmental health. Studies linking disease status to burdens of industrial contamination in these environments are now required.  相似文献

Health state parameters in natural populations can be used as bioindicators of environmental health if they respond to environmental cues. We studied to which extent health state parameters (morphological, biochemical, and haematological) were determined by the environment in which nestling birds were reared or by their genotype, using a cross-fostering experiment in great tits Parus major . Morphological parameters and plasma protein, had a higher environmental than genetic component of variance which could be related with food availability differing between rearing nests. Glutathione peroxidase, an enzyme involved in antioxidant protection, also had a relatively high environmental component of variance, suggesting that nestlings in different rearing nests were exposed to different degrees of oxidative stress. Plasma cholinesterases were more influenced by the birds' genotype than by the rearing environment. The parameters with the lowest residual variance and the highest environmental component of variance such as glutathione peroxidase, plasma protein and morphological parameters (tarsus length and weight) would be more adequate as bioindicators.  相似文献

1. There is a mismatch between broad holistic questions typically posed in policy formation and narrow reductionist questions that are susceptible to scientific method. This inhibits the two-way flow of information at the science-policy interface and weakens the impact of applied ecology on environmental policy.
2. We investigate the approaches to building policy in the health services as a model to help establish a framework in applied ecology and environmental management by which reductionist science can underpin decision making at the policy level.
3. A comparison of policy documents in the health and environmental sectors reveals many similarities in identifying approaches and specific interventions that might achieve policy objectives. The difference is that in the health services, information on the effectiveness of potential interventions is far more readily available through the collaborative process of systematic review.
4. Synthesis and applications . Decision makers are increasingly looking to produce policies that are shaped by evidence through evidence-based policy making. The approach that we outline here provides a framework for structuring systematic reviews to deliver the evidence on key policy issues in a way that will see a faster return and provide better use of the systematic review methodology in environmental management.  相似文献

The lysosomal-autophagic system appears to be a common target for many environmental pollutants as lysosomes accumulate many toxic metals and organic xenobiotics, which perturb normal function and damage the lysosomal membrane. In fact, lysosomal membrane integrity or stability appears to be an effective generic indicator of cellular well-being in eukaryotes: in bivalve molluscs and fish, stability is correlated with many toxicological responses and pathological reactions. Prognostic use of adverse lysosomal and autophagic reactions to environmental pollutants has been explored in relation to predicting cellular dysfunction and health in marine mussels, which are extensively used as sensitive bioindicators in monitoring ecosystem health. Derivation of explanatory frameworks for prediction of pollutant impact on health is a major goal; and we have developed a conceptual mechanistic model linking lysosomal damage and autophagic dysfunction with injury to cells and tissues. This model has also complemented the creation of a cell-based computational model for molluscan hepatopancreatic cells that simulates lysosomal, autophagic and other cellular reactions to pollutants. Experimental and simulated results have also indicated that nutritional deprivation-induced autophagy has a protective function against toxic effects mediated by reactive oxygen species (ROS). Finally, coupled measurement of lysosomal-autophagic reactions and modelling is proposed as a practical toolbox for predicting toxic environmental risk.  相似文献

Disparities in the prevalence of obesity in youth place minority and low socioeconomic status youth at increased risk for the development of chronic disease, such as metabolic syndrome and type 2 diabetes. Contributing factors to the increases in obesity include a decline in positive health behaviors, such as making healthy dietary choices, engaging in physical activity, and limiting sedentary behaviors. Family and physical environmental contextual factors related to health behaviors are increasingly the focus of health behavior interventions in line with the bioecological model that encourages a system-focused perspective on understanding health behavior influences. Physical environmental characteristics, such as home and neighborhood characteristics and resources, provide the tangible means to support health behaviors and are important contextual variables to consider that may increase intervention effectiveness. Therefore, the current review seeks to highlight the importance of investigating influences of behavior beyond individual characteristics in understanding factors related to the risk of developing metabolic syndrome and type 2 diabetes in youth at high risk for developing chronic disease. The current study reviews the non-intervention literature on family and physical environmental factors related to health behaviors (i.e., diet, physical activity, and sedentary behavior) in youth who are considered to be at-risk for developing metabolic syndrome and type 2 diabetes. Results on 38 published articles of diet, physical activity, and sedentary behaviors showed support for the role of parenting and physical environmental factors, particularly parental monitoring and neighborhood context, such as social cohesion, as they relate to health behaviors in high-risk youth. Implications and recommendations for future research are discussed.  相似文献

Environmental change has negatively affected most biological systems on our planet and is becoming of increasing concern for the well-being and survival of many species. At an organism level, effects encompass not only endocrine disruptions, sex-ratio changes and decreased reproductive parameters, but also include teratogenic and genotoxic effects, immunosuppression and other immune-system impairments that can lead directly to disease or increase the risk of acquiring disease. Living organisms will strive to maintain health by recognizing and resolving abnormal situations, such as the presence of invading microorganisms or harmful peptides, abnormal cell replication and deleterious mutations. However, fast-paced environmental changes may pose additional pressure on immunocompetence and health maintenance, which may seriously impact population viability and persistence. Here, we outline the importance of a functional immune system for survival and examine the effects that exposure to a rapidly changing environment might exert on immunocompetence. We then address the various levels at which anthropogenic environmental change might affect wildlife health and identify potential deficits in reproductive parameters that might arise owing to new immune challenges in the context of a rapidly changing environment. Throughout the paper, a series of examples and case studies are used to illustrate the impact of environmental change on wildlife health.  相似文献

The history of environmental decision-making in the United States is primarily one of antagonism, conflict, and litigation. Four sectors of American society are typically at odds over why and how to solve environmental problems—government regulators, businesses proponents, environmental advocates, and members of affected communities. Dissimilar worldviews are at the heart of most environmental disputes, and people in the four sectors tend to have diverse perspectives and philosophies that affect how they interpret and respond to environmental issues. To promote integrated, cost-effective decisions, the public health paradigm (prevention first, intervention second, treatment third), as embodied in the Precautionary Principle and the concept of Sustainable Development, should be mutually adopted as a joint framework for prioritizing solutions to pressing environmental problems. The core public health principle of prevention first is a simple yet powerful tenet that can help foster better environmental choices that are more effective, efficient, and equitable.  相似文献

Advances in research concerning the mental health implications of dietary patterns and select nutrients have been remarkable. At the same time, there have been rapid increases in the understanding of the ways in which non-pathogenic microbes can potentially influence many aspects of human health, including those in the mental realm. Discussions of nutrition and microbiota are often overlapping. A separate, yet equally connected, avenue of research is that related to natural (for example, green space) and built environments, and in particular, how they are connected to human cognition and behaviors. It is argued here that in Western industrial nations a ‘disparity of microbiota’ might be expected among the socioeconomically disadvantaged, those whom face more profound environmental forces. Many of the environmental forces pushing against the vulnerable are at the neighborhood level. Matching the developing microbiome research with existing environmental justice research suggests that grey space may promote dysbiosis by default. In addition, the influence of Westernized lifestyle patterns, and the marketing forces that drive unhealthy behaviors in deprived communities, might allow dysbiosis to be the norm rather than the exception in those already at high risk of depression, subthreshold (subsyndromal) conditions, and subpar mental health. If microbiota are indeed at the intersection of nutrition, environmental health, and lifestyle medicine (as these avenues pertain to mental health), then perhaps the rapidly evolving gut-brain-microbiota conversation needs to operate through a wider lens. In contrast to the more narrowly defined psychobiotic, the term eco-psychotropic is introduced.  相似文献

There is ongoing debate concerning the possible environmental and human health impacts of growing genetically modified (GM) crops. Here, we report the results of a life-cycle assessment (LCA) comparing the environmental and human health impacts of conventional sugar beet growing regimes in the UK and Germany with those that might be expected if GM herbicide-tolerant (to glyphosate) sugar beet is commercialized. The results presented for a number of environmental and human health impact categories suggest that growing the GM herbicide-tolerant crop would be less harmful to the environment and human health than growing the conventional crop, largely due to lower emissions from herbicide manufacture, transport and field operations. Emissions contributing to negative environmental impacts, such as global warming, ozone depletion, ecotoxicity of water and acidification and nutrification of soil and water, were much lower for the herbicide-tolerant crop than for the conventional crop. Emissions contributing to summer smog, toxic particulate matter and carcinogenicity, which have negative human health impacts, were also substantially lower for the herbicide-tolerant crop. The environmental and human health impacts of growing GM crops need to be assessed on a case-by-case basis using a holistic approach. LCA is a valuable technique for helping to undertake such assessments.  相似文献

Environmental toxicants are ubiquitous,and many are known to cause harmful health effects.However,much of what we know or think we know concerning the targets and long-term effects of exposure to environmental stressors is sadly lacking.Toxicant exposure may have health effects that are currently mischaracterized or at least mechanistically incompletely understood.While much of the recent excitement about stem cells(SCs)focuses on their potential as therapeutic agents,they also offer a valuable resource to give us insight into the mechanisms and risks of toxicant effects.Not only as a response to the increasing ethical pressure to reduce animal testing,SC studies allow us valuable insight into the true effects of human exposure to environmental stressors under controlled conditions.We present a review of the history of publications on the effects of environmental stressors on SCs,followed by a consolidation of the literature over the past five years on a subset of key environmental stressors of importance to human health and their effects on both embryonic and tissue SCs.The review will make constructive suggestions as to areas of toxicant research where further studies are needed,as well as making indications of the potential utility for advancing knowledge and directing research on environmental toxicology.  相似文献

The human skin is one of the major human organs in perpetual interaction with environmental factors. Permanent micro-aggressions by light, mechanical factors or chemical factors are useful for the maintenance of healthy skin. For example light is necessary for vitamin D synthesis and for the control of skin inflammation. If the aggression is too strong, repair mechanisms are involved such as wound healing or DNA repair. Accumulation of aggressions could induce acute or chronic diseases such as sunburn, photoageing or skin carcinoma. On specific individuals with genetic predisposition environmental aggressions can induce frequent diseases such as atopic dermatitis, present in 20% of children or psoriasis, present in 2-3% of the European population. Chemical aggressions are responsible for irritant dermatitis or allergic dermatitis, which are among the most frequent professional diseases. Aggression by UV light is one of the best examples of the interaction between environment and health. At a low dose UV light is very good for the health. At a high dose it is responsible for photoageing and skin carcinoma. To improve the control of environmental factors important for health, two approaches are essential: 1) research into epidemiology to discover the causal relationships and into biology to discover the mechanisms involved; 2) education from childhood to explain the results of research and to propose effective behaviours. The general aim of this study was to assess the possibility of including elements of environmental health education within the curriculum of a school located in a shantytown of Buenos Aires city, Argentina. An environmental health education campaign was designed to introduce school-aged children to the problems posed by the lack of environmental sanitation, by using rodents as indicators of environmental disorder. The methodology implemented consisted of a lecture and two practical activities where the participating children were the evaluators of their neighborhood environment, recording the environmental factors that indicate direct or indirect presence of rodents and carrying out a survey about rodents among their neighbors. To assess the impact of the activities, an anonymous questionnaire was performed with the students before and after the campaign. The results showed that students were able to identify the man-made factors which favor the presence of rodents and were encouraged to propose strategies related to environmental sanitation to reduce rodent proliferation and the transmission of their parasites. This study demonstrated the feasibility of performing environmental health education campaigns in school-aged children by using practical activities to stimulate observation, participation, and comprehensive understanding of the problems posed by urban pests.  相似文献

Stachybotrys chartarum is one of several species of filamentous fungi capable of producing mycotoxins under certain environmental conditions. In some observational studies, the growth of this toxigenic mold in the indoor environment has been implicated as a cause of building-related illness. Following reports of a cluster of cases of pulmonary hemosiderosis and hemorrhage associated with exposure to Stachybotrys, public health measures have been recommended which have far-reaching implications. Although the hazards associated with exposure to some mycotoxins have been well studied, the health risks from environmental exposure to Stachybotrys remain poorly defined. The purpose of this review is to critically evaluate the current body of epidemiologic knowledge regarding Stachybotrys and to increase physician awareness regarding this emerging environmental health issue.  相似文献

Issues of spatial scale and resolution are intrinsic to efforts aimed at protecting and improving environmental health. Deciding on an appropriate policy or selecting a suitable research design implies a decision, either implicit or explicit, about spatial scale and resolution. This article looks at issues in the context of environmental health, reviews crucial problems and questions, and examines examples of spatial effects on analytical results related to causal inference, disease clustering, and analysis and interpretation of census data. The discussion focuses on the need to consider spatial issues as a key component of informed, well- reasoned decisions about safeguarding environmental health.  相似文献
