High dose rate brachytherapy (HDR) using 192Ir sources is well accepted as an important treatment option and thus requires an accurate dosimetry standard. However, a dosimetry standard for the direct measurement of the absolute dose to water for this particular source type is currently not available. An improved standard for the absorbed dose to water based on Fricke dosimetry of HDR 192Ir brachytherapy sources is presented in this study. The main goal of this paper is to demonstrate the potential usefulness of the Fricke dosimetry technique for the standardization of the quantity absorbed dose to water for 192Ir sources. A molded, double-walled, spherical vessel for water containing the Fricke solution was constructed based on the Fricke system. The authors measured the absorbed dose to water and compared it with the doses calculated using the AAPM TG-43 report. The overall combined uncertainty associated with the measurements using Fricke dosimetry was 1.4% for k = 1, which is better than the uncertainties reported in previous studies. These results are promising; hence, the use of Fricke dosimetry to measure the absorbed dose to water as a standard for HDR 192Ir may be possible in the future.  相似文献

Alkylating agents, because of their ability to react directly with DNA either in vitro or in vivo, or following metabolic activation as in the case of the dialkylnitrosamines, have been used extensively in studying the mechanisms of mutagenicity and carcinogenicity. Their occurrence is widespread in the environment and human exposure from natural and pollutant sources is universal. Since most of these chemicals show varying degrees of both carcinogenicity and mutagenicity, and exhibit compound-specific binding patterns, they provide an excellent model for studying molecular dosimetry. Molecular dosimetry defines dose as the number of adducts bound per macromolecule and relates the binding of these adducts to the human mutagenic or carcinogenic response. This review complies DNA alkylation data for both methylating and ethylating agents in a variety of systems and discusses the role these alkylation products plays in molecular mutagenesis.  相似文献

Two different sources of ultraviolet B (UVB) radiation, an electronically controlled UVB exposure unit, containing FS40 tubes, and a hand-held Kromayer lamp, were evaluated for actual irradiance in W/m2 and spectra (physical dosimetry and biological dosimetry (skin effects in rodents)). The technical studies of the FS40 sources demonstrated that the flux intensity of the lamps could be changed electronically, without affecting the spectrum. Thus it was possible to standardize UVB exposure electronically. The biologically effective doses of these sources were analysed in RIV-Tox Wistar rats and BALB/c mice. After low doses of UVB radiation, histopathological changes such as acanthosis, hyperkeratosis and dermal inflammation were observed in the skin without the presence of major side effects such as erythema and oedema. After higher doses of UVB radiation erythema and oedema were clearly visible. Quantitative studies showed that the minimal erythema dose, as a biological parameter, correlated well to the emission in J/m2. In addition, biological parameters such as acanthosis and inflammation in the skin correlated well to the actual exposure in J/m2 and were sensitive biomarkers for UVB-induced skin toxicity. Thus, in addition to minimal erythemal doses, acanthosis and inflammation may also be applied as biologically relevant doses for studies of the biological effects of UVB radiation.  相似文献

In the 1940s and 1950s, children in Israel were treated for tinea capitis by irradiation to the scalp to induce epilation. Follow-up studies of these patients and of other radiation- exposed populations show an increased risk of malignant and benign thyroid tumors. Those analyses, however, assume that thyroid dose for individuals is estimated precisely without error. Failure to account for uncertainties in dosimetry may affect standard errors and bias dose-response estimates. For the Israeli tinea capitis study, we discuss sources of uncertainties and adjust dosimetry for uncertainties in the prediction of true dose from X-ray treatment parameters. We also account for missing ages at exposure for patients with multiple X-ray treatments, since only ages at first treatment are known, and for missing data on treatment center, which investigators use to define exposure. Our reanalysis of the dose response for thyroid cancer and benign thyroid tumors indicates that uncertainties in dosimetry have minimal effects on dose-response estimation and for inference on the modifying effects of age at first exposure, time since exposure, and other factors. Since the components of the dose uncertainties we describe are likely to be present in other epidemiological studies of patients treated with radiation, our analysis may provide a model for considering the potential role of these uncertainties.  相似文献

Radionuclide absorbed-dose dosimetry is an active area of development and has the potential to positively impact molecular radiotherapies. At present, many of the operations required to perform dosimetry calculations are unstandardized and unestablished. While the current methodology allows reasonable dosimetry estimates to be derived and published, it can be difficult to understand, and reproduce, each others’ work. To help alleviate this we have identified the collection of biodistribution information as a key step in all internal dosimetry calculations, and present a template that can be used to standardize its documentation and reporting.A generalized biodistribution template entitled the IAEA Radiotracer Biodistribution Template (IAEA RaBiT) has been built and distributed for users performing biodistribution measurements in the community. The template enables robust recording of dosimetry-relevant information through standardization of details and their format. It has been designed to be simple and easy to use, and establish a structured recording of a common reference point in dosimetry operations – biodistribution data documentation. Improved documentation procedures may benefit organization of in house data, or be used to disseminate details throughout the community – for example to supplement dosimetry related publications. The standard format information may also enable the creation of new dosimetry related tools and protocols and support robust population databases.As dosimetry in nuclear medicine becomes more routinely applied in clinical applications, we need to develop the infrastructure for robustly handling large amounts of these data. Our IAEA RaBiT can be used as a standard format structure for data collection, organization, and dissemination.  相似文献

Acute myocardial infarction is often the result of occlusion of one or more coronary arteries. Occlusion and restenosis (re-closing of the vessel) are principal reasons that percutaneous transluminal coronary angioplasty (PTCA) may fail to provide long-term benefit. PTCA has been a popular treatment, which is less invasive than surgeries involving revascularization of the myocardium, promising a better quality of life for patients. Unfortunately, the rate of restenosis after balloon angioplasty is high (approximately 30-50% in the first year after treatment). Recent data suggest that intraluminal irradiation of coronary arteries in conjunction with balloon angioplasty and/or stent implantation reduces the proliferation of smooth muscle cells and neointima formation, thereby inhibiting restenosis. In order to study radiation dosimetry in the patient and for this therapy, dose distributions for electrons and photons, with discrete energies, were simulated for blood vessels of diameter 1.5, 3 and 4.5 mm irradiated with balloon and wire sources. Electron and photon transport was performed in a simple model representing the system used for irradiation using the MCNP 4B code (Monte Carlo N-Particles). Specific calculations for balloon and wire sources were also carried out for a few radionuclides. In this work, strengths and drawbacks conceming the use of each radionuclide simulated, as well as source geometries are discussed. The dosimetry performed in this study will improve understanding of the benefit-to-risk ratio in intracoronary brachytherapy.  相似文献

SBRT for lung cancer is being rapidly adopted as a treatment option in modern radiotherapy centres. This treatment is one of the most complex in common clinical use, requiring significant expertise and resources. It delivers a high dose per fraction (typically ∼6–30 Gy/fraction) over few fractions. The complexity and high dose delivered in only a few fractions make powerful arguments for the application of in vivo dosimetry methods for these treatments to enhance patient safety. In vivo dosimetry is a group of techniques with a common objective – to estimate the dose delivered to the patient through a direct measurement of the treatment beam(s). In particular, methods employing an electronic portal imaging device have been intensely investigated over the past two decades. Treatment verification using in vivo dosimetry approaches has been shown to identify errors that would have been missed with other common quality assurance methods. With the addition of in vivo dosimetry to verify treatments, medical physicists and clinicians have a higher degree of confidence that the dose has been delivered to the patient as intended.In this review, the technical aspects and challenges of in vivo dosimetry for lung SBRT will be presented, focusing on transit dosimetry applications using electronic portal imaging devices (EPIDs). Currently available solutions will be discussed and published clinical experiences, which are very limited to date, will be highlighted.  相似文献

In an analysis of a follow-up study of a fixed population of 73,330 atomic bomb survivors in Hiroshima and Nagasaki, the slope of an estimated dose response between ionizing radiation and leukemia mortality was found to be steeper (P less than 0.002), by a factor of 2.4, among those who reported epilation within 60 days of the bombings, compared to those who did not experience this sign of acute radiation exposure. The strength of this empirical finding as evidence of biological association in individual radiosensitivity for these two end points is studied here. The major factor complicating the interpretation of this finding as evidence of such an association is the degree of imprecision of the radiation dosimetry system used in assignment of radiation doses to the A-bomb survivors. Using models recently suggested for dealing with dosimetry errors in epidemiological analysis of the A-bomb survivor data, the sensitivity of the apparent association between leukemia mortality and severe epilation to the assumed level of dosimetry error is investigated.  相似文献

The use of synchrotron X-ray sources provides innovative approaches in radiation therapy. The unique possibility to generate quasi-parallel beams promoted the development of microbeam radiation therapy (MRT), an innovative approach able to reduce damages to normal tissues while delivering considerable doses in the lesion. Accurate dosimetry in broad-beam configuration (prior to the spatial fractionation of the incident X-ray fan) is very challenging at ultra-high dose rate synchrotron sources.The available reference dosimetry protocol based on the use of a PTW PinPoint ionization chamber was compared with alanine dosimetry at the European Synchrotron Radiation Facility (ESRF) ID17 Biomedical beamline, an orthovoltage X-ray source with an average dose rate of 11.6 kGy/s. Reference dose measurements of the alanine pellets were performed at the National Centre for Radiation Research and Technology (NCRRT) 60Co facility in Egypt. All alanine dosimeters were analysed by an electron paramagnetic resonance spectrometer.We determined a relative response rESRF = 0.932 ± 0.027 (1σ) of the alanine pellets irradiated at the ESRF compared to the 60Co facility. Considering the appropriate corrections for the ESRF polychromatic spectrum and the different field size used, our result is in agreement with the previous work of Waldeland et al. for which the utilised alanine contained the same amount of binder, and it is consistent with the works of Anton et al. and Butler et al. for which the utilised alanine contained a higher amount of binder.We confirm that alanine is an appropriate dosimeter for ultra-high dose rate calibration of orthovoltage X-ray sources.  相似文献

In the spring of 1986 the Radiation Effects Research Foundation (RERF) received a new atomic bomb dosimetry system. This report presents the comparisons of leukemia and nonleukemia cancer mortality risk estimates under the old and new dosimetries. In terms of total kerma (essentially whole-body gamma plus neutron exposure), risk estimates for both classes of cancer are 75-85% higher with the new dosimetry. This and other summary comparisons allow for possible nonlinearity at high estimated doses. Changes are also considered in relation to organ doses and assumptions about the relative biological effectiveness (RBE) of neutrons. Without regard to RBE, the risk estimates for total organ dose are essentially unchanged by the dosimetry revision. However, with increasing assumed values of RBE, the estimated low-LET risk decreases much less rapidly under the new dosimetry, due to the smaller neutron component. Thus at an assumed constant RBE of 10, for example, the effect of the dosimetry revision is to increase organ dose risk estimates, relative to those based on the old dosimetry, by 30% for nonleukemia and 80% for leukemia. At an RBE of 20 these increases are 72 and 136%, respectively. A number of other issues are discussed. The city difference in dose is no longer statistically significant, even at an RBE of one. Estimation of RBE is even less feasible with new dosimetry. There is substantial question of the linearity in dose response, in the sense of a leveling off at higher doses. Finally, some indication is given of how risks estimated from this dosimetry and the current data may compare to widely used estimates based largely on the RERF data with the previous dosimetry.  相似文献

Precise positioning of source and dosimeters is essential in the experimental determination of dosimetric characteristics of brachytherapy sources. Various near-water equivalent solid phantoms have been used to achieve the necessary precision in the positioning. However, the uncertainties in their chemical compositions may lead to non-negligible uncertainties in the determined doses. It is proposed here that ice may be used as an alternative to the conventional solid phantoms, since its chemical composition is identical to water while the positioning advantage associated with solid phantoms is retained. In this work, the feasibility of using ice as a solid phantom for brachytherapy dosimetry is investigated. Ice-to-water conversion factors are calculated at distances of 0.2–10 cm from the source, for six high- and low-energy photon-emitting brachytherapy sources and mono-energetic photons between 10 keV to 2.0 MeV. Practical issues and challenges associated with measuring dose in an ice phantom are discussed.  相似文献

Changes of environmental UV radiation as part of global atmospheric changes will influence the biosphere substantially. The determination of the biological effects of these changes requires accurate and reliable UV monitoring systems that weight the spectral irradiance according to the biological responses under consideration. Biological UV dosimeters, which directly weight the incident UV components of sunlight in relation to the effectiveness of the different wavelengths and the potential interactions between them, can complement weighted physical UV measurements. Up to now several UV-dependent endpoints in biomolecules (e.g. uracil, DNA, provitamin D3), bacteriophages (e.g. T7), bacteria (e.g.E. coli, B. subtilis) and cultured eukaryotic cells have been suggested as sensing elements in biological UV dosimeters. One example is the DLR-biofilm consisting of immobilised spores of the bacterium B. subtilis as a UV sensor. It weights per se the incident UV radiation according to its DNA-damaging effectiveness. In several examples the applicability of the DLR-biofilm technique for personal UV dosimetry as well as for the measurement of the biologically weighted irradiance of the sun and of artificial UV sources is demonstrated.  相似文献

Boron Neutron Capture Therapy (BNCT) is a binary radiotherapy method developed to treat patients with certain malignant tumours. To date, over 300 treatments have been carried out at the Finnish BNCT facility in various on-going and past clinical trials. In this technical review, we discuss our research work in the field of medical physics to form the groundwork for the Finnish BNCT patient treatments, as well as the possibilities to further develop and optimize the method in the future. Accordingly, the following aspects are described: neutron sources, beam dosimetry, treatment planning, boron imaging and determination, and finally the possibilities to detect the efficacy and effects of BNCT on patients.  相似文献

To provide direct estimates of cancer risk after low-dose protracted exposure to ionizing radiation, a large-scale epidemiological study of nuclear industry workers was conducted in 15 countries. As part of this study, identification and quantification of errors in historical recorded doses was conducted based on a review of dosimetric practices and technologies in participating facilities. The main sources of errors on doses from "high-energy" photons (100-3000 keV) were identified as the response of dosimeters in workplace exposure conditions and historical calibration practices. Errors related to dosimetry technology and radiation fields were quantified to derive period- and facility-specific estimates of bias and uncertainties in recorded doses. This was based on (1) an evaluation of predominant workplace radiation from measurement studies and dosimetry expert assessment and (2) an estimation of the energy and geometry response of dosimeters used historically in study facilities. Coefficients were derived to convert recorded doses to H(p) (10) and organ dose, taking into account different aspects of the calibration procedures. A parametric, lognormal error structure model was developed to describe errors in doses as a function of facility and time period. Doses from other radiation types, particularly neutrons and radionuclide intake, could not be adequately reconstructed in the framework of the 15-Country Study. Workers with substantial doses from these radiation types were therefore identified and excluded from analyses. Doses from "lower-energy" photons (<100 keV) and from "higher-energy" photons (>3 MeV) were estimated to be small.  相似文献

PurposeTo analyze the changes in the polarization state of the flatbed scanner light caused by the EBT3 films and to propose a new method for correcting the lateral effects.Methods and materialsThe polarization changes induced by radiochromic films are analyzed using linear polarizing film. Based on the results, the linear polarizing films are used in the scanning process of the EBT3 films. This method is tested against the conventional EBT3 dosimetry using a series of simple regular beams and 21 cases of IMRT.ResultsThe mean results are statically different from the conventional dosimetry with EBT3. Depending on the transmission axis of the polarizing sheet, the results are better or worse compared to conventional dosimetry EBT3 film. When the transmission axis of the polarizing sheet is parallel to the coating direction, the dosimetry results are better and its variability is smaller. However, when the polarizer transmission axis is perpendicular to the coating direction, results are worse as well as its variability.ConclusionUsing a polarized film with the polarization axis parallel to the coating direction of the radiochromic film, and preferably above it, significantly improves the dosimetry results and is an easy and inexpensive way to correct the lateral artifacts of the conventional EBT3 dosimetry.  相似文献

Cobalt 60 source is generally available on high dose rate (HDR) afterloading equipment especially for treatment of gynecological lesions. The GZP6 remote afterloader (Nuclear Power Institute of China) utilizes 60Co sources for treatment of intracavitary and intraluminal malignancies. In this study, the AAPM TG-43 dosimetric parameters of three sources in GZP6 system have been studied using MCNP4C Monte Carlo (MC) code; and the results are compared with other available 60Co HDR sources. The presented parameters consist of air kerma strength, dose rate constant, radial dose function and anisotropy function. They show less than 1% uncertainty. The TG-43 based dosimetry data can be used not only to validate the dedicated treatment planning software (TPS), but also to introduce new complementary software to enhance the system performance in gynecological treatments.  相似文献

The Gafchromic XR type R film is a suitable dosimeter to determine the map of the skin dose in patients undergone complex interventional radiological procedures, such as cardiology ones. The need of preventing or locating possible skin injuries due to high doses administered to patients—as recommended by international organizations—wants the introduction in patient dosimetry of a dosimeter easy to handle, with low dependence of the response on energy in the typical radiological range, and extended measurable dose range. XR type R films fulfil all these requirements and moreover may be quickly analyzed by cheap commercial scanners. In order to determine skin-dose values by XR-R, a film calibration curve is required.In this work, validation of the XR-R dosimetry has been performed for the determination of the skin dose: maximum skin-dose values in 14 patients undergone radiofrequency ablation and pacemaker implant procedures have been determined by XR-R calibrated films. A comparison between skin-dose values determined by XR-R films and retrospective ionometric measurements has pointed out some discrepancies in the results, due to difficulties in retrospectively reproducing the real procedure settings, where XR-R film dosimetry is related to the specific patient procedure, even, in very complex interventional settings.  相似文献

Children are more and more using wireless communication systems. This growth has strengthened public concern and has highlighted the need to assess the radio frequency (RF) exposure of children. In dosimetry, taking advantage of the improvement of High Performance Calculation systems, great efforts have been carried out to improve the numerical tools and human models used to assess the Specific Absorption Rate (SAR). This paper analyses progress in building child and foetus models for numerical dosimetry purpose. The simulation results, in terms of Specific Absorption Rate over 1 and 10 g of tissues, in specific organs such as brain and averaged over the whole body, are reported and analysed. The results show that compliance methods used nowadays to certify phones are valid for children. The studies also show that specific tissues such as peripheral brain tissues can have higher exposure with children than with adults. Studies performed with plane waves as sources and whole body children models show that the whole body SAR of children can be higher than the WBSAR of adults and that the compliance to ICNIRP reference levels does not guarantee the compliance to ICNIRP basic restrictions. Dealing with the foetus models and dielectric properties great efforts have been made. Preliminary results show that the foetus exposure is often lower than the mother exposure, with an important influencing parameter: the foetus position in the uterus.  相似文献
