Child malnutrition is pervasive in developing countries and anthropometric measures such as weight-for-height and height-for-age have proven reliable indicators of short term malnutrition and stunting. Rather than studying these indicators separately, we look at their interaction and carve out child health dynamics. Considering height-for-age a child's health stock and weight-for-lagged height a proxy for nutritional inputs, we develop a child health production function that features self-productivity of past health stocks and contemporaneous nutritional inputs. We test the model on a Senegalese panel of 271 children between 0 and 5 years employing dynamic panel methods to control for endogeneity in the production function. In line with previous evidence, we find that children can partially catch-up from malnutrition spells. Yet, child health stocks also deplete quickly and need constant updating in the form of nutrition. This demonstrates the importance of health memory and that malnutrition cannot be fought with snapshot interventions. Consequently, sustainable nutrition interventions have to be long term and yield higher returns the earlier they reach children.  相似文献

The relationship between poverty and nutrition is a two-sided one: on the one hand, economic growth (which is generally associated with an eradication of poverty) leads to reduced malnutrition. On the other hand, nutrition is one of the key ingredients for human capital formation, which in turn represents one of the fundamental factors of growth. There are numerous studies that show the correlates of malnutrition using both household- and community-level variables. However, few of these studies allow for the potential endogeneity of community infrastructure or indicate their interplay with characteristics of the mother. The current study considers the socio-economic determinants of child malnutrition and investigates how programs compensate for the increased risks facing young mothers and their children or substitute for a low social status of the mother in the household. The empirical results show that children of mothers giving birth at a young age are disadvantaged in terms of their anthropometric status. Interaction effects of the presence of a non-governmental organization (NGO) or a health post in the village with characteristics of the mother stress the important role played by these institutions in helping disadvantaged mothers overcome their difficulties. These findings have implications for efficient program design and represent a further step towards gaining an improved understanding of the complex determinants of child (mal)nutrition.  相似文献

In this review, we discuss the effect of increased and decreased loading and nutrition deficiency on muscle and bone mass and strength (and bone length and architecture) independently and combined. Both exercise and nutrition are integral components of the mechanostat model but both have distinctly different roles. Mechanical strain imparted by muscle action is responsible for the development of the external size and shape of the bone and subsequently the bone strength. In contrast, immobilization during growth results in reduced growth in bone length and a loss of bone strength due to large losses in bone mass (a result of endosteal resorption in cortical bone and trabecular thinning) and changes in geometry (bone shafts do not develop their characteristic shape but rather develop a rounded default shape). The use of surrogate measures for peak muscle forces acting on bone (muscle strength, size, or mass) limits our ability to confirm a cause-and-effect relationship between peak muscle force acting on bone and changes in bone strength. However, the examples presented in this review support the notion that under adequate nutrition, exercise has the potential to increase peak muscle forces acting on bone and thus can lead to a proportional increase in bone strength. In contrast, nutrition alone does not influence muscle or bone in a dose-dependent manner. Muscle and bone are only influenced when there is nutritional deficiency--and in this case the effect is profound. Similar to immobilization, the immediate effect of malnutrition is a reduction in longitudinal growth. More specifically, protein and energy malnutrition results in massive bone loss due to endosteal resorption in cortical bone and trabecular thinning. Unlike loading however, there is indirect evidence that severe malnutrition when associated with menstrual dysfunction can shift the mechanostat set point upward, thus leading to less bone accrual for a given amount of bone strain.  相似文献

Because of serious consequences of malnutrition and cachexia on treatment outcome, mortality, morbidity, and quality of life, it is important to identify SCCHN patients with increased risk for the development of malnutrition and cachexia.Materials and methodsCritical review of the literature.ResultsThis review describes pathogenesis, diagnosis and treatment of malnutrition and cancer cachexia. Treatment of malnutrition and cancer cachexia includes nutritional interventions and pharmacological therapy. Advantages and disadvantages of different nutritional interventions and their effect on the nutritional status, quality of life and specific oncological treatment are presented.ConclusionsNutritional management is an essential part of care of these patients, including early screening, assessment of nutritional status and appropriate intervention.  相似文献

Background

In many developing countries nutritional, and epidemiological transitions are emerging into continuing undernutrition and escalating overnutrition, giving rise to the double burden of the malnutrition phenomenon.

Objectives

This study aims to determine the prevalence of the dual form of malnutrition (overweight mother/underweight child) in the same household and its associated factors in the Gaza Strip.

Methods

A total of 357 mother-child pairs from the same households were surveyed from three different geographical locations in the Gaza Strip, namely, El Remal urban area, Jabalia refugee camp, and Al Qarara rural area. The height and weight of mothers aged 18–50 years were measured, and their body mass index (BMI) was computed. The mothers were categorized according to the criterion of the World Health Organization (WHO) for BMI as overweight if they have a BMI ≥ 25 kg/m2. Anthropometric indices were measured for children aged two to five years to classify the underweight children Z-score <-1.

Results

The results showed the prevalence of the dual form of malnutrition in the Gaza Strip was 15.7%, and its associated factors were child’s birth order (ORadj, 1.50, 95% CL, 1.22, 1.82; p = <0.001), father’s educational (low or medium) levels (ORadj, 3.19, 95% CL, 1.07, 9.5; p = 0. 036), or (ORadj, 3.4, 95% CL, 1.12, 10.37; p = 0. 031), high scores of mothers'' nutrition knowledge (ORadj, 1.23, 95% CL, 1.01, 1.52; p = 0. 048), and low monthly income (ORadj, 0.28, 95% CL, 0.09, 0.88; p = 0. 030).

Conclusions

The results from this study showed the dual form of malnutrition in the same household was prevalent in the Gaza Strip. This is a public health issue that must be understood and addressed and policy makers must implement an appropriate nutrition action plan to control dual form of malnutrition based on the underlying specific risk factors in the study population. In addition, interventions are needed to help individuals to translate their nutrition knowledge into healthy dietary behaviors.  相似文献

The mortality rate of dialysis patients is still considerably high. Beside the traditional risk factors, specific uremia-related risk factors are identified. Among them, hypoalbuminemia and malnutrition have a strong association to mortality in chronic dialysis patients. Various studies document a strong relation between reduced calorie and protein uptake and mortality in uremic patients. Several factors responsible for malnutrition in dialysis patients have been identified. These factors may be dialysis-associated, due to intercurrent illnesses or are associated with uremic complications (e.g. hyperparathyroidism, anemia, acidosis, etc.). Malnutrition is treatable and can be avoided by several means. Beside the increase in the dose of dialysis and adequate protein and calorie intake, intradialytic nutrition is an additional choice. The combination with specific drugs (e.g. growth hormone) may potentiate the success of the modified treatment modalities, particularly in patients who need nutritional support during an intercurrent illness. Further studies are required to measure the impact of for example growth hormone supplementation on mortality rate and quality of life in malnourished patients on chronic dialysis.  相似文献

摘要 目的:探讨老年心力衰竭(HF)患者营养不良的影响因素及早期肠内营养对营养不良患者心功能、营养状况和肠道黏膜屏障功能的影响。方法:选取2021年2月~2022年4月期间在我院接受治疗的180例老年HF患者作为研究对象。入院后采用微型营养评价简表(MNA-SF)评估患者的营养状况。根据MNA-SF评分结果分为营养不良组(n=83)和营养正常组(n=97)。应用单因素及多因素Logistic回归分析老年HF患者营养不良的危险因素。对老年HF营养不良患者给予早期肠内营养干预,观察其治疗前、治疗一周后心功能、营养状况和肠道黏膜屏障功能的变化情况。结果:老年HF患者营养不良与性别、居住地、饮酒史、病因、职业类别、谷丙转氨酶、血肌酐、收缩压(SBP)、舒张压(DBP)无关(P>0.05),而与年龄、医保类型、病程、婚姻状况、美国纽约心脏病学会(NYHA)分级、文化程度、C反应蛋白(CRP)、家庭人均月收入、B型脑钠肽(BNP)、吸烟史、左心室射血分数(LVEF)有关(P<0.05)。Logistic回归分析结果显示:病程偏长、CRP偏高、BNP偏高、NYHA分级为IV级、年龄偏大、吸烟史是老年HF患者发生营养不良的危险因素(P<0.05)。治疗1周后,营养不良组老年HF患者的LVEF升高,BNP下降(P<0.05)。治疗1周后,营养不良组老年HF患者的前白蛋白(PA)、转铁蛋白(TRF)升高(P<0.05)。治疗1周后,营养不良组老年HF患者的D-乳酸(D-Lac)、二胺氧化酶(DAO)、肠脂肪酸结合蛋白(IFABP)下降(P<0.05)。结论:老年HF患者营养不良受到病程、CRP、BNP、NYHA分级、年龄、吸烟史等多种因素的影响,针对老年HF患者营养不良给予早期肠内营养,有助于改善患者心功能、营养状况和肠道黏膜屏障功能。  相似文献

Population aging is progressing rapidly in many industrialized countries. The United States population aged 65 and over is expected to double in size within the next 25 years. In sedentary people eating Western diets aging is associated with the development of serious chronic diseases, including type 2 diabetes mellitus, cancer and cardiovascular diseases. About 80% of adults over 65 years of age have at least one chronic disease, and 50% have at least two chronic diseases. These chronic diseases are the most important cause of illness and mortality burden, and they have become the leading driver of healthcare costs, constituting an important burden for our society. Data from epidemiological studies and clinical trials indicate that many age-associated chronic diseases can be prevented, and even reversed, with the implementation of healthy lifestyle interventions. Several recent studies suggest that more drastic interventions (i.e. calorie restriction without malnutrition and moderate protein restriction with adequate nutrition) may have additional beneficial effects on several metabolic and hormonal factors that are implicated in the biology of aging itself. Additional studies are needed to understand the complex interactions of factors that regulate aging and age-associated chronic disease.  相似文献

Soybean plants were grown in complete solution for 33 days and then transferred to medium containing inadequate sulphur (5 t M) and nitrogen at 15, 7.5, 2 or 0.25 mt M. In mature leaves (L1 and L2), and leaves that were 70% expanded at day 33 (L3), the net loss of sulphur over the ensuing 25 days was inversely related to the level of nitrogen nutrition. Leaf 5, which formed during the study period, exhibited complementary characteristics; the increase in the sulphur content was inversely related to the level of nitrogen nutrition even though low nitrogen nutrition supported less growth. L4, which was 31% expanded at day 33, exhibited intermediate characteristics. 35S-Labelled sulphate was supplied to all of the plants for 48 h at day 31 and was distributed principally to L3 at day 33. During early development, L5 became heavily labelled but, at low nitrogen nutrition, the massive import of total sulphur into L5 during the late stages of development was not accompanied by a commensurate increase in 35S-label, indicating that redistribution of soluble sulphur from mature leaves was not involved. The loss of sulphur from mature leaves was parallelled by similar changes in nitrogen at all levels of nitrogen nutrition. Collectively, the data suggest that a common mechanism, presumably proteolysis, is involved in the export of sulphur and nitrogen from mature leaves and that this process is inhibited at high levels of nitrogen nutrition, even under conditions of sulphur deficiency.  相似文献

Protein malnutrition leads to growth retardation that can be reversed through catch‐up growth, once normative nutrition is restored. Because growth is a dynamic process, catch‐up capacity is likely influenced by the maturity of the animal and/or the duration of the insult, in addition to the type of insult experienced. We compared length of malnutrition, sexual dimorphism, body mass, and skeletal growth. Eighty Rattus norvegicus were divided into 10 treatment groups (five diets; male and female) and followed for more than 1 year. At weaning, animals were placed on either a control or low‐protein isocaloric diet. Three experimental groups were switched to the control diet at 40, 60, or 90 days. Beginning with 21 days of age, animals were weighed daily and radiographed throughout the study. To determine the presence of catch‐up growth, growth rates (GRs) were calculated (linear regression) for 20‐day time spans before and after diet changes and compared among treatment groups. Targeted growth was measured as final size or as the coefficient of variation with age. These results show that 1) protein‐restricted animals experience catch‐up growth with dietary rehabilitation; 2) for females, catch‐up GRs are proportional to GRs in control animals at the same age as the timing of dietary rehabilitation but not for males; and 3) targeted growth was observed in some, but not all, aspects of anatomy. The length of the tibia and humerus was indistinguishable from controls, regardless of length of malnutrition or gender, whereas the ulna and male body mass exceeded control sizes. Although most measures decreased in variation with ontogeny, the tibia failed to do so. These results support a complex biological regulation of catch‐up and targeted growth. The implications for selection are that flexible and responsive developmental trajectories may have an advantage over those programed into a single size. J. Average adult height is a physical measure of the biological standard of living of a population. While the biological and economic standards of living of a population are very different concepts, they are linked and may empirically move together. If this is so, then cohort heights can also be used to make inferences about the economic standard of living and health of a population when other data are not available. We investigate how informative this approach is in terms of inferring income, nutrition, and mortality using data on heights from developing countries over the last 50 years for female cohorts born 1951–1992. We find no evidence that the absolute differences in adult height across countries are associated with different economic living standards. Within countries, however, faster increases in adult cohort height over time are associated with more rapid growth of GDP per capita, life expectancy, and nutritional intake. Using our instrumental variable approach, each centimeter gain in height is associated with a 6% increase in income per capita, a reduction in infant mortality of 7 per thousand (or an 1.25 year increase in life expectancy), and an increase in nutrition of 64 calories and 2 grams of protein per person per day relative to the global trend. We find that increases in cohort height can predict increases in income even for countries not used in the estimation of the relationship. This suggests our approach has predictive power out of sample for countries where we lack income and health data.  相似文献

Individuals can compensate for poor early nutrition by accelerating their growth rates once diet improves, but if malnutrition occurs at a key stage of development only certain body structures may compensate fully. This degrees of compensation is predicted to differ between the sexes and also between species with different life history strategies. In this paper we determine how males and females in a short-lived bird species, with only slight size dimorphism, differ in their abilities to compensate for a poor start in life. Here, zebra finch Taeniopygia guttata chicks from 93 broods were reared on either a standard quality (SQ), or low quality (LQ) diet for the first 15 d of life, followed by a standard diet for the rest of their lives. Thus, the period of nutritional manipulation was relatively short compared with previous studies. Nestlings on a LQ diet showed slower body mass and tarsus growth rates than those on SQ diet but this was reversed after the diet of the LQ birds was improved. LQ birds of both sexes were able to fully compensate in terms of body mass and beak colour, but not tarsus length. Body size and beak colour are sexually selected traits in male zebra finches. By adulthood LQ females had significantly shorter wings than other birds, apparently directing resources into sex-specific structures instead of feather and skeletal growth. Thus, our experiment showed that the sexes differed in how they phenotypically compensated for a poor start in life. Males in particular invested sparse resources into structures associated with mate acquisition, as proposed by life history theory for a species that 'lives fast and dies young.' We predict that this relatively short period of malnutrition during early development will have long term, sex-specific, fitness consequences for these birds.  相似文献

This study of a Vincentian community in the Eastern Caribbean explores the impacts of income on dietary quality and food import dependency. It finds that two forms of income—total household income and the income that women control—are positively correlated with total dietary diversity as well as with frequency of consumption of numerous imported foods. However, no relation is evident between banana incomes and consumption of imported foods, thus questioning the general belief in the literature that the growth of banana production has been responsible for increasing dependence on food imports in the Windward Islands. The beneficial impacts of income on dietary quality that have been found in many other developing areas are more limited here because malnutrition is not a widespread problem. But income is closely linked to consumption of foods that contribute to overnutrition and obesity, growing concerns in the English-speaking Caribbean.  相似文献

A propitiously timed household survey carried out in Mozambique over the period 2008/2009 permits us to study the relationship between shifts in food prices and child nutrition status in a low income setting. We focus on weight-for-height and weight-for-age in different survey quarters characterized by very different food price inflation rates. Using propensity score matching techniques, we find that these nutrition measures, which are sensitive in the short run, improve significantly in the fourth quarter of the survey, when the inflation rate for basic food products is low, compared to the first semester or three quarters, when food price inflation was generally high. The prevalence of underweight, in particular, falls by about 40 percent. We conclude that the best available evidence points to food penury, driven by the food and fuel price crisis combined with a short agricultural production year, as substantially increasing malnutrition amongst under-five children in Mozambique.  相似文献

BACKGROUND: Hypernatremic dehydration in neonates is a potentially devastating condition. Recent reports have identified breast-feeding malnutrition as a key factor in its pathophysiology. METHODS: Using a theoretical framework for breast-feeding kinetics, a retrospective chart review of all neonates less than 28 days of age who were seen at either British Columbia''s Children''s Hospital or the Vancouver Breastfeeding Centre between 1991-1994 was conducted to identify and classify possible causes of breast-feeding malnutrition among neonates who developed hypernatremic dehydration. RESULTS: Twenty-one cases hypernatremic dehydration were identified. Infant weight loss ranged from 8% to 30% of birth weight, and serum sodium levels ranged from 146 mmol/L to 207 mmol/L. In each case, maternal or infant factors (e.g., poor breast-feeding technique, lactation failure following postpartum hemorrhage and infant suckling disorders associated with cleft palate or ankyloglossia) that could interfere with either lactation or breast-feeding dynamics and account for insufficient breast milk intake were identified. INTERPRETATION: Prenatal and in-hospital screening for maternal and infant risk factors for breast-feeding malnutrition combined with early postpartum follow-up to detect excessive infant weight loss are important for the prevention of neonatal hypernatremic dehydration.  相似文献

BACKGROUND: Malnutrition in elderly patients in institutions has become an issue of clinical concern, but it remains largely unrecognized in acute care hospitals. The demonstrated benefits of intervention emphasize the need for routine nutritional assessment. The objectives of this study were to determine the prevalence of malnutrition in elderly patients admitted to a tertiary care centre and to test the sensitivity and specificity of 3 nutrition screening tools. METHODS: Between July and November 1996 patients 65 years and older were consecutively recruited from the general medicine, orthopedics, general surgery and neurosciences services of The Ottawa Hospital--General Campus within 72 hours of admission. They were interviewed using 3 nutritional screening tools: one developed by Chandra and colleagues (Chandra), the Nutrition Screening Initiative (NSI) and the Mini Nutritional Assessment (MNA). A detailed nutrition assessment was then undertaken, which included anthropometric assessment, laboratory tests, determination of risk factors and assessment of dietary intake. A dietitian blinded to the screening results classified each patient as being well nourished, at risk for malnutrition or malnourished. The prevalence of malnutrition was assessed, and screening results were compared with the results of the detailed nutrition assessment for sensitivity and specificity. RESULTS: In total, 160 patients (86 women) were recruited. Detailed nutrition assessments were completed for 152 patients, of which 62 (40.8%) were found to be well nourished, 67 (44.1%) at moderate risk for malnutrition and 23 (15.1%) malnourished. Matched comparisons showed that, of the 23 malnourished patients, 1 was found to be at high risk for malnutrition using the Chandra screening tool, 9 using the NSI and 4 using the MNA, giving sensitivities of 32%, 54% and 57%, and specificities of 85%, 61% and 69%, respectively. INTERPRETATION: Given the high rate of malnutrition or risk of malnutrition in this study, admitting physicians need to be aware of this problem and its scope. The 3 screening tools tested performed poorly in comparison with the detailed nutrition assessment. This may have been because the score thresholds for the screening tools were set for screening purposes and because the screening tools were designed for different settings and a wider population.  相似文献

The relation between the nutrition of the mother and that of her baby was assessed in a south Indian community where malnutrition is common and women do not smoke. Unselected mothers and their infants of over 37 weeks'' gestation were studied in two groups: those who paid for their care (150) and a poorer group who did not (172). There were significnat differences between the paying and non-paying groups in maternal triceps skinfold thickness, infant weight, and infant length. Overall there was a significant positive correlation between maternal triceps thickness and infant weight, length, and triceps and subscapular skinfold thickness. The correlation with the infant head circumference was less significant. These findings are further evidence that the nutrition of the mother has an important effect on the nutrition of her baby and that malnutrition is an important reason why Indian babies are lighter than European ones.  相似文献

Metabolic syndrome (MetS) is defined as a constellation of individual metabolic disturbances, including central obesity, hypertension, dyslipidemia, and insulin resistance. The established pathogenesis of MetS varies extensively with gender, age, ethnic background, and nutritional status. In terms of nutritional status, micronutrients are more likely to be discounted as essential components of required nutrition than macronutrients due to the small amount required. Numerous observational studies have shown that pregnant women frequently experience malnutrition, especially in developing and low-income countries, resulting in chronic MetS in the offspring due to the urgent and increasing demands for micronutrients during gestation and lactation. Over the past few decades, scientific developments have revolutionized our understanding of the association between balanced maternal micronutrients and MetS in the offspring. Examples of successful individual, dual, or multiple maternal micronutrient interventions on the offspring include iron for hypertension, selenium for type 2 diabetes, and a combination of folate and vitamin D for adiposity. In this review, we aim to elucidate the effects of maternal micronutrient intake on offspring metabolic homeostasis and discuss potential perspectives and challenges in the field of maternal micronutrient interventions.  相似文献

National Nutrition Surveillance includes nutritional assessment surveys to ascertain the extent of malnutrition in populations, to identify possible causes, to establish baseline data for monitoring nutrition, and to select mechanisms for nutrition surveillance (in a restricted sense). An example of the results from a recent nutritional assessment survey in the United States is the negative association of obesity with energy intake, exercise and socioeconomic status, which has implications for public nutritional policy. Nutritional monitoring measures changes in population nutrition over time. An example of the results from nutritional monitoring is the unexpected and presently unexplained decrease in serum cholesterol levels of middle-aged women in the United States over the past decade. Nutritional surveillance in the restricted sense not only identifies malnutrition but is administratively organized to intervene rapidly. National Nutrition Surveillance depends on metabolic and clinical research to decide on its priorities. This research indicates that malnutrition involves more than under-nutrition, and greater emphasis should be given in National Nutrition Surveillance to this wider context of malnutrition. These results will in turn help set priorities for basic and applied research in nutrition. It is important that the research community participate in the review presently under way of the role of the National Center for Health Statistics in National Nutrition Surveillance.  相似文献

A field experiment was conducted to investigate the effect ofK nutrition under water stress conditions on cell membrane stabilitymeasured by the polyethylene glycol test, plant growth, internalplant water relations and solute and mineral concentrationsin maize (Zea mays L.). Water-stressed plants showed greateradaptation to water deficits at higher K levels. Cell membranestability increased, leaf water potential and osmotic potentialdecreased, turgor potential increased and stomatal resistancedecreased with increasing K nutrition. Osmotic adjustment wasevident and it may have been influenced by increased K+ concentrationsin leaf tissues with increasing K nutrition. Higher leaf thicknessand higher leaf water content were observed at higher K levels.Results suggested that higher supplies of K nutrition may increaseplant production during periods of water stress.
