Une syringocèle révélée par un abcès périntéal

Purpose

The authors present a clinical report of a perforate syringocele diagnosed in a male patient and they review the clinical, radiological and therapeutic aspects of this disease.

Case report

A 50-year-old man attended the emergency room with perineal abscess. Computerized Tomography (CT) scan was used to establish the diagnosis. This patient also underwent urethrocystoscopy, sonography and retrograde urethrography.

Results

Treatment consisted of open surgical evacuation of the perineal abscess with bladder catheter. Urethrocystoscopy showed a large perforate syringocele in the urethra with no need for endoscopy marsupialization. No recurrence was observed at follow-up at 3 months.

Conclusion

Syringocele is a rare malformation which usually is asymptomatic. It can be congenital or acquired and can be classified into four morphological types. The most frequent treatment is endoscopic marsupialization of the syringocèle. Open surgical excision is recommended for complicated syringocèle with abscess or perineal pain.

Methyl 2-acetamido-2-deoxy-5,6-O-isopropylidene-β-D-glucofuranoside was prepared in excellent yield from methyl 2-benzamido-2-deoxy-5,6-O-isopropylidene-β-D-glucofuranoside by alkaline hydrolysis, followed by selective N-acetylation. Treatment with 60% acetic acid at room temperature gave syrupy methyl 2-acetamido-2-deoxy-β-D-glucofuranoside, characterized by a crystalline tri-O-p-nitrobenzoyl derivative. The same treatment, at 100° gave methyl 2-acetamido-2-deoxy-β-D-glucopyranoside. In an alternative procedure, the selective N-acetylation was performed after acetic acid hydrolysis of methyl 2-amino-2-deoxy-5,6-O-isopropylidene-β-D-glucofuranoside. Several derivatives of methyl 2-acetamido-2-deoxy-β-D-glucofuranoside were prepared and compared with the corresponding pyranosides. The furanoside structure was clearly demonstrated by mass spectrometry and periodate oxidation.

One hundred and twenty years after restoration of badlands by reforestation with Pinus nigra Arn. ssp. nigra, the analysis of the soils on marls of the Saignon experimental watershed (Alpes-de-Haute-Provence) demonstrates the potentialities and the driving forces behind the distribution of earthworms in the main site types. The communities are young, and density and biomass are low to normal (up to 49 individuals and 27 g·m⁻², respectively) for these low productivity pine woods. Two main site-directed trajectories would lead marls to a mixed species association: the first one is initiated by the epi-anecic behaviour of Lumbricus terrestris L. in xeric conditions (south-oriented slope, thin soil); the second is initiated by Octolasion cyaneum Sav. (north-oriented slope).     

Effets produits chez trois espèces de criquets par l'ingestion d'hydrocarbures polyaromatiques réputés cancérogènes

In a first approach to studying the effects of air pollutants, we describe some experimental results obtained after ingestion of polyaromatic hydrocarbons (HAP)^* by three species of locusts (Gomphocerus sibiricus, Gomphocerus rufus and Euchorthippus pulvinatus). We wanted to evaluate their toxicity, then to locate them in the organism. Two HAP, carcinogenic for mammals, were used. They are highly toxic for our locusts. Ingestion of 0.015 or 0.06 mg of 3 methylcholanthrene kills respectively 25 % and 50 % of them. Moreover, benzo-a-pyrene seems to have a greater toxicity, and E. pulvinatus is the most resistant species. Strongly fluorescent in UV light, 3 MC and BaP can be easily detected in the organism after dissection, and with more detail using frozen material. The microscope shows absorption activity of midgut; most of the organs are consequently invaded by intact hydrocarbon or its various derivatives. Muscles and reproductive organs show a bright fluorescence, which reaches its maximum in the nervous system. Tubes of Malpighi seem to be the principal route of elimination of these products. The remarkable susceptibility of some organs is encouraging for the progress of our study.

The four diastereoisomeric methyl 2,4,6-trideoxy-4-trifluoroacetamido-l-hexopyranosides have been synthesized. Coupling of the corresponding 1-O-acetyl-3-O-benzoyl-l-lyxo and 1-O-acetyl-3-O-p-nitrobenzoyl-l-arabino derivatives with daunomycinone in the presence of p-toluensulfonic acid as catalyst afforded two new anthracyclines.

Attempts to prepare 1,2:5,6 and 2,3:5,6 di-unsaturated sugars starting from 3,4,6-tri-O-acetyl-1,5-anhydro-1,2-dideo xy-d-arabino-hex-1-enitol or from ethyl 4,6-di-O-acetyl-1,5-anhydro-2,3-dideoxy-α-d-erythro-hex-2-enopyranoside led to 1,5-anhydro-1,2,6-trideoxy-l-threo-hex-5-enitol and its 3,4-diacetate. Hydrogenation and hydrogenolysis of the unsaturated chloro and fluoro derivatives afforded 1,5-anhydro-1,2,6-trideoxy-d-arabino-hexitol and ethyl 4-O-acetyl-2,3,6-trideoxy-α-d-erythro-hexopyranoside.     

Système sémantiquement interopérable de sélection semi-automatique des patients éligibles aux essais thérapeutiques en cancérologie

Automatic Selection of clinical Trial based on Eligibility Criteria (ASTEC) project is to automate, so as to make it systematic, the search of cancer clinical trials, by reusing the patient data contained into an oncologic electronic health record. ASTEC project tackles two major scientific challenges for medical informatics: 1) the syntactic and semantic interoperability between information systems. The oncologic electronic medical records and the recruitment decision system must be interoperable. The ASTEC project proposes a framework of syntactico-semantic interoperability based on international standards. Generic methods of mediation and reasoning based on ontologies are developed to match data from the electronic medical records to the inclusion/exclusion criteria of clinical trials; 2) a decision support system for recruitment. We have developed inference methods on the electronic medical records adapted to the data structure as well as the eligibility criteria. this paper, we present and justify our choices, concerning the medical process in oncology and the scientific and technical aspects. Furthermore the system will be evaluated in real time. The aim is to demonstrate a significative improvement of the prescreening rate of patient.

Treatment of 1,2:5,6-di-O-isopropylidene-α-d-ribo- and xylo-hexofuranos-3-uloses with (difluoromethylene)triphenylphosphorane and (chlorofluoromethylene)-triphenylphosphorane gave unsaturated, ramified halogeno sugars in good yield. Treatment of the chlorofluoromethylene derivatives with lithium aluminum hydride gave stereospecifically the corresponding fluoromethylene derivatives with inversion of configuration at the double bond. The configuration was determined by ¹h- and ¹⁹F-n.m.r. spectrometry.

Sex and cancer, pleasure and death are words very difficult and painful to associate and to evoke. The sexuality of patients with cancerous diseases is unrecognized by physicians and very little present in the practice of sexology. the sexual life of cancerous patients must be preserved. The preventions must have aims, means and time. The aims are: to allow, to keep, to find or to rediscover a sexual and relational life. The means are:

? systematic information on the clinical reality and his results,

? directed information on the sexual reality and his capacities,

? thinked information on the sense and the value of the sexuality and of the pleasure.

The time is before, during and after the treatment for cancer. The prevention is also sexological treatments if sexuals symptoms occur.
