Candidal vulvovaginitis (CVV), is the second most leading vaginal infection (global prevalence > 75%), caused due to excessive growth of Candida spp., predominantly Candida albicans (>95% cases). The current treatment regimens for CVV are marred with the challenges of fungal resistance & infection recurrence, subsequently leading to the compromised therapeutic efficacy of anti-fungal drugs, prolonged treatment and low patient compliance. The core of the present research was the fabrication & investigation of 2 T-SLN (solid lipid nanoparticles) gel carrying luliconazole for the amelioration of CVV. ‘2T’ symbolizes transvaginal & thermosensitive attributes of the present formulation. SLNs were prepared by a modified melt emulsification-ultra sonication method using a combination of solid lipids (Gelucire 50/13 & Precirol ATO 5), surfactant (Tween 80) and co-surfactant (Kolliphor). Formulation by design (FbD) approach was adopted to obtain appropriately screened and tailored SLNs. The optimized SLNs yielded a particle size, polydispersity index & entrapment efficiency of 62.18 nm, 0.263 & 81.5% respectively. To formulate the 2 T-gel, the final SLNs were loaded into Carbopol 971P-NF and Triethanolamine based gel. The 2 T-SLN gel was found to be easily spreadable and homogenous with mean extrudability (15 ± 0.4 g/cm2), viscosity (696.42 ± 2.34 Pa·s) and %drug content (93.24 ± 0.73%) values.. The pH of the prepared 2 T-SLN gel (4.5 ± 0.5) was in concordance with the vaginal pH (normal conditions). For in-vitro characterization of an optimized 2 T-SLN gel the release kinetics & anticandidal activity were assessed which offers a %cumulative drug release of 62 ± 0.5% in 72 h and 37.3 ± 1.5 mm zone of inhibition in 48 h. The visual appearance & dimensions were determined using fluorescent microscopy (spherical shape) & transmission electron microscopy (90–120 nm) respectively. The optimized 2 T-SLN gel showcases a skin-friendly profile with no significant signs of erythema and oedema and was found to be stable at room temperature for 2 months without any visual non-uniformity/cracking/breaking. In conclusion, the current research serves a new therapeutic perspective in assessing the activity of luliconazole for vaginal drug delivery using a 2 T-SLN gel system.  相似文献

Optoelectronic motion capture systems are widely employed to measure the movement of human joints. However, there can be a significant discrepancy between the data obtained by a motion capture system (MCS) and the actual movement of underlying bony structures, which is attributed to soft tissue artefact. In this paper, a computer-aided tracking and motion analysis with ultrasound (CAT & MAUS) system with an augmented globally optimal registration algorithm is presented to dynamically track the underlying bony structure during movement. The augmented registration part of CAT & MAUS was validated with a high system accuracy of 80%. The Euclidean distance between the marker-based bony landmark and the bony landmark tracked by CAT & MAUS was calculated to quantify the measurement error of an MCS caused by soft tissue artefact during movement. The average Euclidean distance between the target bony landmark measured by each of the CAT & MAUS system and the MCS alone varied from 8.32 mm to 16.87 mm in gait. This indicates the discrepancy between the MCS measured bony landmark and the actual underlying bony landmark. Moreover, Procrustes analysis was applied to demonstrate that CAT & MAUS reduces the deformation of the body segment shape modeled by markers during motion. The augmented CAT & MAUS system shows its potential to dynamically detect and locate actual underlying bony landmarks, which reduces the MCS measurement error caused by soft tissue artefact during movement.  相似文献

AimPatient setup errors were aimed to be reduced in radiotherapy (RT) of head-and-neck (H&N) cancer. Some remedies in patient setup procedure were proposed for this purpose.BackgroundRT of H&N cancer has challenges due to patient rotation and flexible anatomy. Residual position errors occurring in treatment situation and required setup margins were estimated for relevant bony landmarks after the remedies made in setup process and compared with previous results.Materials and methodsThe formation process for thermoplastic masks was improved. Also image matching was harmonized to the vertebrae in the middle of the target and a 5 mm threshold was introduced for immediate correction of systematic errors of the landmarks. After the remedies, residual position errors of bony landmarks were retrospectively determined from 748 orthogonal X-ray images of 40 H&N cancer patients. The landmarks were the vertebrae C1–2, C5–7, the occiput bone and the mandible. The errors include contributions from patient rotation, flexible anatomy and inter-observer variation in image matching. Setup margins (3D) were calculated with the Van Herk formula.ResultsSystematic residual errors of the landmarks were reduced maximally by 49.8% (p  0.05) and the margins by 3.1 mm after the remedies. With daily image guidance the setup margins of the landmarks were within 4.4 mm, but larger margins of 6.4 mm were required for the mandible.ConclusionsRemarkable decrease in the residual errors of the bony landmarks and setup margins were achieved through the remedies made in the setup process. The importance of quality assurance of the setup process was demonstrated.  相似文献

Physiol. 83(3): 927-935, 1997.A four-compartment (4C) model of body composition was used as acriterion to determine the accuracy of three-compartment (3C) andtwo-compartment (2C) models to estimate percent body fat (%BF) inprepubertal and pubertal boys (genital I & II,n = 17; genital III & IV,n = 7) and girls (breast I & II, n = 8; breast III & IV,n = 15). The 3C water-density (3C-H2O) and 3C mineral-densitymodels, dual-energy X-ray absorptiometry, the Lohman age-adjustedequations, the Slaughter et al. skinfold equations, and the Houtkooperet al. and Boileau bioelectrical impedance equations wereevaluated. Agreement with the 4C model increased with thenumber of compartments (i.e., body water, bone mineral) measured.Except for the 3C-H2O model, thelimits of agreement were large and did not perform well forindividuals. The mean %BF by dual-energy X-ray absorptiometry (23.6%)was greater than that of the criterion 4C method (21.7%).For the field methods, the Slaughter et al. skinfold equationsperformed better than did the Houtkooper et al. and Boileaubioimpedance equations. The hydration of the fat-free mass decreased(genital I & II = 75.7%, genital III & IV = 74.8%, breast I & II = 75.5%, breast III & IV = 74.4%) and the mineral content increased(genital I & II = 4.9%, genital III & IV = 5.0%, breast I & II = 5.1%, breast III & IV = 5.7%) with maturation. The densityof the fat-free mass also increased (genital I & II = 1.084 g/ml,genital III & IV = 1.087 g/ml, breast I & II = 1.086 g/ml, breast III & IV = 1.091 g/ml) with maturation. All of the models reduced the %BF overprediction of the Siri 2C model, but only the 4C and3C-H2O models should be used ascriterion methods for body composition validation in children andadolescents.

PurposeThe aim of this study is to investigate the effect of beam interruptions during delivery of volumetric modulated arc therapy (VMAT) on delivered dose distributions.MethodsTen prostate and ten head and neck (H&N) VMAT plans were retrospectively selected. Each VMAT plan was delivered using Trilogy™ without beam interruption, and with 4 and 8 intentional beam interruptions per a single arc. Two-dimensional global and local gamma evaluations with a diode array were performed with gamma criteria of 3%/3 mm, 2%/2 mm, 1%/2 mm and 2%/1 mm for each VMAT plan with and without beam interruptions. The VMAT plans were reconstructed with log files recorded during delivery and the dose-volumetric parameters were calculated for each reconstructed plan. The differences among dose-volumetric parameters due to the beam interruptions were calculated.ResultsThe changes in global gamma passing rates with various gamma criteria were less than 1.6% on average, while the changes in local gamma passing rates were less than 5.3% on average. The dose-volumetric parameter changes for the target volumes of prostate and H&N VMAT plans due to beam interruptions were less than 0.72% and 1.5% on average, respectively.ConclusionThe delivered dose distributions with up to 8 beam interruptions per an arc were clinically acceptable, showing minimal changes in both gamma passing rates and dose-volumetric parameters.  相似文献

Commensal behavior of jack mackerel Trachurus japonicus (Temminck & Schlegel) with jellyfishes has been widely observed but its ecological function is still unclear. The goal of the present research is to examine the function of association behavior with jellyfish in the laboratory and in field observations with an emphasis on ontogenetic changes. In the laboratory, jack mackerel juveniles (mean standard length (SL) = 11, 19, 38, and 55 mm) were placed in 500-l polycarbonate tanks with two live moon jellyfish, Aurelia aurita (Linné), and one artificial jellyfish made of silicon. Association behavior with either live or artificial jellyfish was visually observed under the following conditions: control, presence of a predator model, before and after feeding live Artemia, 1 h and 3 h after feeding, and at night. Jack mackerel at 11 mm SL associated with both the moon jellyfish and artificial jellyfish, unrelated to the presence of a predator model or feeding. Juveniles at 19 mm associated with moon jellyfish only in the presence of a predator model. Larger juveniles associated with moon jellyfish at 1 h and 3 h after feeding. Thus the ecological function of association was proposed to develop first from school formation, next as a hiding place from predators, and then as a food source. Underwater observations of jack mackerel associating with giant jellyfish Nemopilema nomurai (Kishinouye) in two different areas in the Sea of Japan supported this hypothesis. High predation pressure from benthic piscivorous fishes in the southern area (Tsushima) may encourage association with jellyfish, whereas pressure from pelagic predators in the northern area (Maizuru) may encourage settlement to rocky reef habitats in temperate waters. We hypothesized that central fatigue may have a role in limitingthe endurance capacity of horses. Therefore, we tested the effect ofinfusing tryptophan and/or glucose on endurance time and plasmaconcentrations of free tryptophan and other substrates thought toaffect tryptophan uptake into the brain of seven mares (3-4 yr ofage, 353-435 kg) that ran on a treadmill at 50% of maximalO2 consumption to fatigue. Withuse of a counterbalanced crossover design, the horses were infused withtryptophan (100 mg/kg in saline solution) or a similar volume of salinesolution (placebo) before exercise. During exercise, horses receivedinfusions of glucose (2 g/min, 50% wt/vol) or a similar volume ofsaline. Thus the treatments were 1)tryptophan and glucose (T & G), 2) tryptophan and placebo (T & P), 3)placebo and glucose (P & G), and 4)placebo and placebo (P & P). Mean heart rate, hematocrit, andconcentration of plasma total solids before and during exercise weresimilar for all trials. Mean time to exhaustion was reduced (P < 0.05) for T & P and T & Gcompared with P & P [86.1 ± 6.9 and 87.1 ± 6.8 vs. 102.3 ± 10.3 (SE) min], whereas endurance for P & G(122.4 ± 11.9 min) was greater than for all other trials (P < 0.05). Compared withnontryptophan trials, during the tryptophan trials plasma prolactinincreased (P < 0.05) nearlythreefold before exercise and almost twofold early in exercise. Muscleglycogen concentrations were reduced(P < 0.05) below preexercise values in the P & G and P & P trials only. However, glucose infusions (P & G)did not affect (P > 0.05)concentrations of plasma free fatty acids or ratios of branched-chainamino acids to free tryptophan. In conclusion, tryptophan infusionreduced endurance time, which was consistent with the central fatiguehypothesis. The failure of glucose infusion to alleviate the effects oftryptophan and the absence of significant muscle glycogen reduction inthe tryptophan trials suggest that the early onset of fatigue in thetryptophan trials is not due to a lack of readily available substrate.

PurposeStatic beam intensity-modulated-radiation-therapy (IMRT) and/or Volumetric-Modulated-Arc-Therapy (VMAT) are now available in many regional radiotherapy departments. The aim of this multi-institutional audit was to design a new methodology based on radiochromic films to perform an independent quality control.MethodsA set of data were sent to all participating centres for two clinical localizations: prostate and Head and Neck (H&N) cancers. The agreement between calculations and measurements was verified in the Octavius phantom (PTW) by point measurements using ionization chambers and by 2D measurements using EBT3 radiochromic films. Due to uncertainties in the whole procedure, criteria were set to 5% and 3% in local dose and 3 mm in distance excluding doses lower than 10% of the maximum doses. No normalization point or area was used for the quantitative analysis.Results13 radiotherapy centres participated in this audit involving 28 plans (12 IMRT, 16 VMAT). For point measurements, mean errors were −0.18 ± 1.54% and 0.00 ± 1.58% for prostate and H&N cases respectively. For 2D measurements with 5%/3 mm criteria, gamma map analysis showed a pixel pass rate higher than 95% for prostate and H&N. Mean gamma index was lower than 0.4 for prostate and 0.5 for H&N. Both techniques yielded similar results.ConclusionThis study showed the feasibility of an independent quality control by peers for conventional IMRT and VMAT. Results from all participating centres were found to be in good agreement. This regional study demonstrated the feasibility of our new methodology based on radiochromic films without dose normalization on a specific point.  相似文献
