目的:探讨医院传染病管理制度对控制和预防感染的效果。方法:我院自2011年1月起实施医院传染病管理制度,将该年度病例列入研究组,取2010年1月至12月间病例列入对照组,分别比较两组传染病发生情况和抗生素应用情况。结果:研究组传染病发生率明显低于对照组,数据经统计学比较具有显著差异(P〈0.05)。研究组发生抗生素滥用的科室率明显低于对照组,数据经统计学比较具有显著差异(P〈0.05)。结论:传染病管理制度可有效降低院内传染病发生率,减少抗生素的临床滥用事件发生,从而提高了医疗工作的安全性,也提升了医院的管理水平。  相似文献

目的:总结解放军第302医院在防控传染病方面所践采取的措施,提高医院传染病管理相关工作。方法:完善传染病管理组织,明确责任;认真落实传染病管理相关制度;加强相关知识培训,强化医护人员责任意识,提高能动性、自觉性。总结分析以上相关措施实施后,2005年-2010年期间传染病报告情况。结果:2005年-2010年每年传染病疫情报告卡填写完整率逐渐提高,医院传染病报告漏报率呈逐年下降趋势,近两年呈现填写完整无漏报的情况。结论:提高医院传染病管理的相关措施的实施,使医护人员对的传染病防控意识有所提高,做到早发现、早报告、早隔离、早治疗,降低传染病传播风险,有效减少了医院交叉感染的发生,保障人们的生命健康和社会的发展稳定。  相似文献

目的为进一步加强医院传染病管理,评价干预措施对降低医院传染病疫情报告迟报率、漏报率的效果,提出合理化建议。方法回顾性分析医院疫情网络直报的问题,比较采取干预措施前后的传染病报告及时率、漏报率,找出影响网络直报工作质量及影响传染病疫情报告及时率、漏报率的因素,探讨提高传染病监测质量的应对对策。结果网络直报的目的不明确,漏报、迟报仍然存在,各种填写不规范及网络直报系统运行是影响网络直报的主要因素。结论采取有效的干预措施能有效地降低医院传染病疫情报告的迟报率、漏报率。从多方面不断完善疾病监测体系,提高疫情报告工作的质量。  相似文献

A new quantity called the target reproduction number is defined to measure control strategies for infectious diseases with multiple host types such as waterborne, vector-borne and zoonotic diseases. The target reproduction number includes as a special case and extends the type reproduction number to allow disease control targeting contacts between types. Relationships among the basic, type and target reproduction numbers are established. Contact tracing, followed by treatment or isolation, is a key control measure in the battle against infectious diseases. It is an extreme form of locally targeted control, and as such has the potential to be highly efficient when dealing with low numbers of cases. For this reason it is frequently used to combat sexually transmitted diseases and new invading pathogens. Accurate modelling of contact tracing requires explicit information about the disease-transmission pathways from each individual, and hence the network of contacts. Here, pairwise-approximation methods and full stochastic simulations are used to investigate the utility of contact tracing. A simple relationship is found between the efficiency of contact tracing necessary for eradication and the basic reproductive ratio of the disease. This holds for a wide variety of realistic situations including heterogeneous networks containing core-groups or super-spreaders, and asymptomatic individuals. Clustering (transitivity) within the transmission network is found to destroy the relationship, requiring lower efficiency than predicted.  相似文献

Infectious diseases are common diseases all over the world. A recent World Health Organization report indicated that infectious diseases are now the world's biggest killer of children and young adults. Infectious diseases in non-industrialized countries caused 45% in all and 63% of death in early childhood. In developed countries, the emergence of new, rare or already-forgotten infectious diseases, such as HIV/AIDS, Lyme disease and tuberculosis, has stimulated public interest and inspired commitments to surveillance and control. Recently, it is reported that infectious diseases are responsible for more than 17 million deaths worldwide each year, most of which are associated with bacterial infections. Hence, the control of infectious diseases control is still an important task in the world. The ability to control such bacterial infections is largely dependent on the ability to detect these aetiological agents in the clinical microbiology laboratory. Diagnostic medical bacteriology consists of two main components namely identification and typing. Molecular biology has the potential to revolutionise the way in which diagnostic tests are delivered in order to optimise care of the infected patient, whether they occur in hospital or in the community. Since the discovery of PCR in the late 1980s, there has been an enormous amount of research performed which has enabled the introduction of molecular tests to several areas of routine clinical microbiology. Molecular biology techniques continue to evolve rapidly, so it has been problematic for many laboratories to decide upon which test to introduce before that technology becomes outdated. However the vast majority of diagnostic clinical bacteriology laboratories do not currently employ any form of molecular diagnostics but the use such technology is becoming more widespread in both specialized regional laboratories as well as in national reference laboratories. Presently molecular biology offers a wide repertoire of techniques and permutations of these analytical tools, hence this article wishes to explore the application of these in the diagnostic laboratory setting.  相似文献

Autophagy is the major intracellular degradation system by which cytoplasmic materials are delivered to and degraded in the lysosome. However, the purpose of autophagy is not the simple elimination of materials, but instead, autophagy serves as a dynamic recycling system that produces new building blocks and energy for cellular renovation and homeostasis. Here we provide a multidisciplinary review of our current understanding of autophagy's role in metabolic adaptation, intracellular quality control, and renovation during development and differentiation. We also explore how recent mouse models in combination with advances in human genetics are providing key insights into how the impairment or activation of autophagy contributes to pathogenesis of diverse diseases, from neurodegenerative diseases such as Parkinson disease to inflammatory disorders such as Crohn disease.  相似文献

This short review establishes the conceptual bases and discusses the principal aspects of P4-shorthand for predictive, preventive, personalized and participatory medicine-medicine, in the framework of infectious diseases. P4 medicine is a new way to approach medical care; instead of acting when the patient is sick, physicians will be able to detect early warnings of disease to take early action. Furthermore, people might even be able to adjust their lifestyles to prevent disease. P4 medicine is fuelled by systems approaches to disease, including methods for personalized genome sequencing and new computational techniques for building dynamic disease predictive networks from massive amounts of data from a variety of OMICs. An excellent example of the effectiveness of the P4 medicine approach is the change in cancer treatments. Emphasis is placed on early detection, followed by genotyping of the patient to use the most adequate treatment according to the genetic background. Cardiovascular diseases and perhaps even neurodegenerative disorders will be the next targets for P4 medicine. The application of P4 medicine to infectious diseases is still in its infancy, but is a promising field that will provide much benefit to both the patients and the health-care system.  相似文献

Capacity building in health research generally, and helminthiasis research particularly, is pivotal to the implementation of the research and development agenda for the control and elimination of human helminthiases that has been proposed thematically in the preceding reviews of this collection. Since helminth infections affect human populations particularly in marginalised and low-income regions of the world, they belong to the group of poverty-related infectious diseases, and their alleviation through research, policy, and practice is a sine qua non condition for the achievement of the United Nations Millennium Development Goals. Current efforts supporting research capacity building specifically for the control of helminthiases have been devised and funded, almost in their entirety, by international donor agencies, major funding bodies, and academic institutions from the developed world, contributing to the creation of (not always equitable) North-South "partnerships". There is an urgent need to shift this paradigm in disease-endemic countries (DECs) by refocusing political will, and harnessing unshakeable commitment by the countries' governments, towards health research and capacity building policies to ensure long-term investment in combating and sustaining the control and eventual elimination of infectious diseases of poverty. The Disease Reference Group on Helminth Infections (DRG4), established in 2009 by the Special Programme for Research and Training in Tropical Diseases (TDR), was given the mandate to review helminthiases research and identify research priorities and gaps. This paper discusses the challenges confronting capacity building for parasitic disease research in DECs, describes current capacity building strategies with particular reference to neglected tropical diseases and human helminthiases, and outlines recommendations to redress the balance of alliances and partnerships for health research between the developed countries of the "North" and the developing countries of the "South". We argue that investing in South-South collaborative research policies and capacity is as important as their North-South counterparts and is essential for scaled-up and improved control of helminthic diseases and ultimately for regional elimination.  相似文献

Synthetic biology aims to create functional devices, systems and organisms with novel and useful functions on the basis of catalogued and standardized biological building blocks. Although they were initially constructed to elucidate the dynamics of simple processes, designed devices now contribute to the understanding of disease mechanisms, provide novel diagnostic tools, enable economic production of therapeutics and allow the design of novel strategies for the treatment of cancer, immune diseases and metabolic disorders, such as diabetes and gout, as well as a range of infectious diseases. We consider the recently introduced edge-based compartmental models (EBCM) for the spread of susceptible-infected-recovered (SIR) diseases in networks. These models differ from standard infectious disease models by focusing on the status of a random partner in the population, rather than a random individual. This change in focus leads to simple analytic models for the spread of SIR diseases in random networks with heterogeneous degree. In this paper we extend this approach to handle deviations of the disease or population from the simplistic assumptions of earlier work. We allow the population to have structure due to effects such as demographic features or multiple types of risk behavior. We allow the disease to have more complicated natural history. Although we introduce these modifications in the static network context, it is straightforward to incorporate them into dynamic network models. We also consider serosorting, which requires using dynamic network models. The basic methods we use to derive these generalizations are widely applicable, and so it is straightforward to introduce many other generalizations not considered here. Our goal is twofold: to provide a number of examples generalizing the EBCM method for various different population or disease structures and to provide insight into how to derive such a model under new sets of assumptions.  相似文献

The tracing of potentially infectious contacts has become an important part of the control strategy for many infectious diseases, from early cases of novel infections to endemic sexually transmitted infections. Here, we make use of mathematical models to consider the case of partner notification for sexually transmitted infection, however these models are sufficiently simple to allow more general conclusions to be drawn. We show that, when contact network structure is considered in addition to contact tracing, standard “mass action” models are generally inadequate. To consider the impact of mutual contacts (specifically clustering) we develop an improvement to existing pairwise network models, which we use to demonstrate that ceteris paribus, clustering improves the efficacy of contact tracing for a large region of parameter space. This result is sometimes reversed, however, for the case of highly effective contact tracing. We also develop stochastic simulations for comparison, using simple re-wiring methods that allow the generation of appropriate comparator networks. In this way we contribute to the general theory of network-based interventions against infectious disease.  相似文献

The tree shrew (Tupaia belangen) is a promising laboratory animal that possesses a closer genetic relationship to primates than to rodents.In addition,advantages such as small size,easy breeding,and rapid reproduction make the tree shrew an ideal subject for the study of human disease.Numerous tree shrew disease models have been generated in biological and medical studies in recent years.Here we summarize current tree shrew disease models,including models of infectious diseases,cancers,depressive disorders,drug addiction,myopia,metabolic diseases,and immune-related diseases.With the success of tree shrew transgenic technology,this species will be increasingly used in biological and medical studies in the future.  相似文献

本研究分析近年来上海市境外输入性传染病的病例特点,为上海市输入性传染病的防控提供依据。对上海市某三甲医院2012—2019年报告的所有法定及重点监测传染病中确定为境外感染的305例传染病病例进行回顾性分析。采用WPS Office 2019和 SPSS17.0软件对所收集病例的病种、数量、输入时间、病例人口学特征以及输入地区等进行整理分析。结果显示,2012—2019年该院报告境外输入性传染病11种,分别为疟疾、登革热、肺结核、麻疹、甲型肝炎、戊型肝炎、伤寒、人类免疫缺陷病毒(human immunodeficiency virus,HIV)感染、黄热病、白喉、隐性梅毒。确诊的305例病例中,疟疾226例,占74.10%;登革热68例,占22.30%;其他9种疾病共11例,占3.6%。疟疾、登革热全年均有输入,其中6月、9月和11月为疟疾输入高峰,6~8月为登革热输入高峰,肺结核、麻疹、甲型肝炎、戊型肝炎、伤寒、HIV感染、黄热病、白喉、隐性梅毒无明显季节性分布特征。输入性病例中,中国籍多于外籍,男性多于女性,以青壮年为主;主要职业为商业服务和工人;主要输入地为非洲。本研究结果提示境外输入性传染病以疟疾、登革热等虫媒传染病为主,病种呈现多样化的变化趋势,输入高峰主要在夏秋季。应加强非洲、东南亚劳务和旅游人群的健康教育,做好病例监管和防蚊、灭蚊,减少二代病例。  相似文献

目的了解鹰潭市人民医院法定传染病报告情况,分析其流行病学特点,为制定传染病防治对策及降低传染病的发生提供科学依据。方法对鹰潭市人民医院2006~2008年法定传染病疫情资料进行分析.结果共报告法定传染病19种,累计报告发病4170例,其中无甲类传染病报告。主要病种为感染性腹泻、肺结核、病毒性肝炎、流行性腮腺炎、菌痢,占报告总数的93.62%;死亡13例(5个病种).结论肺结核、乙型肝炎、肠道传染病是今后传染病防治工作的重点和难点,应加大管理力度.做好传染病的预防和控制工作.  相似文献

目的:探讨新时期提升传染病区医院管理水平的有效策略。方法:通过对传染病区医院感染因素进行探究,制定出适用于医院实际的感染预防策略。结果:制定和落实医院预防感染的规章制度,实施严格的职业防护培训,完善传染病区医院管理的诸项策略,能有效降低医院感染发生率。结论:传染病区是医院感染发生的高危地带,加强对传染病区的规范化管理,能起到良好的传染病预防效果。  相似文献
