The envelope of passive knee joint motion 总被引:5,自引:2,他引:5
The purpose of this study is to create an accurate experimental database for the passive (in vitro) freedom-of-motion characteristics of the human knee joint on a subject to subject basis, suitable for the verification and enhancement of mathematical knee-joint models. Knee-joint specimens in a six degree-of-freedom motion rig are moved through flexion under several combinations of external loads, including tibial torques, axial forces and AP-forces. Euler rotation angles and translation vectors, describing the relative, spatial motions of the joint are measured using an accurate Roentgen Stereo Photogrammetric system. Conceptually the joint is considered as a two degrees-of-freedom of motion mechanism (flexion-tibial rotation), whereby the limits of internal and external tibial rotation are defined at torques of +/- 3 Nm. The motion pathways along these limits are defined as the envelopes of passive knee joint motion. It is found that these envelope pathways are consistent and hardly influenced by additional axial forces up to 300 N and AP-forces of 30 N. Within the envelope of motion, however, the motion patterns are highly susceptible to small changes in the external load configuration. It is shown that the external tibial rotation during extension ('screw-home mechanism') is not an obligatory effect of the passive joint characteristics, but a direct result of the external loads. Anatomical differences notwithstanding, the inter-individual discrepancies in the motion patterns of the four specimens tested, showed to be relatively small in a qualitative sense. Quantitative differences can be explained by small differences in the alignment of the coordinate systems relative to the joint anatomy and by differences in rotatory laxity. 相似文献

Assessment of the functional method of hip joint center location subject to reduced range of hip motion 总被引:3,自引:0,他引:3
Motion analysis of the lower extremities usually requires determination of the location of the hip joint center. The results of several recent studies have suggested that kinematic and kinetic variables calculated from motion analysis data are highly sensitive to errors in hip joint center location. "Functional" methods in which the location of the hip joint center is determined from the relative motion of the thigh and pelvis, rather than from the locations of bony landmarks, are promising but may be ineffective when motion is limited. The aims of the present study were to determine whether the accuracy of the functional method is compromised in young and elderly subjects when limitations on hip motion are imposed and to investigate the possibility of locating the hip joint center using data collected during commonly studied motions (walking, sit-to-stand, stair ascent, stair descent) rather than using data from an ad hoc trial in which varied hip motions are performed. The results of the study suggested that functional methods would result in worst-case hip joint center location errors of 26mm (comparable to the average errors previously reported for joint center location based on bony landmarks) when available hip motion is substantially limited. Much larger errors ( approximately 70mm worst-case), however, resulted when hip joint centers were located from data collected during commonly performed motions, perhaps because these motions are, for the most part, restricted to the sagittal plane. It appears that the functional method can be successfully implemented when range of motion is limited but still requires collection of a special motion trial in which hip motion in both the sagittal and frontal planes is recorded. 相似文献

The screw-home mechanism and coupling between forces in cruciate ligaments during passive knee joint flexion were investigated for various boundary conditions, flexion axis alignments and posterior cruciate ligaments (PCL)/anterior cruciate ligament (ACL) conditions. A developed non-linear 3D finite element model was used to perform detailed elasto-static response analyses of the human tibiofemoral joint as a function of flexion angle varying from 10 degrees hyper-extension to 90 degrees flexion. The tibia rotated internally as the femur flexed and externally as the femur extended. The re-alignment of the flexion axis by +/-5 degrees rotation about the axial (distal-proximal) axis, transection of the ACL and changes in cruciate ligament initial strains substantially influenced the 'screw-home' motion. On the other hand, restraint on this coupled rotation diminished ACL forces in flexion. A remarkable coupling was predicted between ACL and PCL forces in flexion; forces in both cruciate ligaments increased as the initial strain or pretension in one of them increased whereas they both diminished as one of them was cut or became slack. This has important consequences in joint functional biomechanics following a ligament injury or replacement surgery and, hence, in the proper management of joint disorders. 相似文献

Hip loading affects the development of hip osteoarthritis, bone remodelling and osseointegration of implants. In this study, we analyzed the effect of subject-specific modelling of hip geometry and hip joint centre (HJC) location on the quantification of hip joint moments, muscle moments and hip contact forces during gait, using musculoskeletal modelling, inverse dynamic analysis and static optimization. For 10 subjects, hip joint moments, muscle moments and hip loading in terms of magnitude and orientation were quantified using three different model types, each including a different amount of subject-specific detail: (1) a generic scaled musculoskeletal model, (2) a generic scaled musculoskeletal model with subject-specific hip geometry (femoral anteversion, neck-length and neck-shaft angle) and (3) a generic scaled musculoskeletal model with subject-specific hip geometry including HJC location. Subject-specific geometry and HJC location were derived from CT. Significant differences were found between the three model types in HJC location, hip flexion–extension moment and inclination angle of the total contact force in the frontal plane. No model agreement was found between the three model types for the calculation of contact forces in terms of magnitude and orientations, and muscle moments. Therefore, we suggest that personalized models with individualized hip joint geometry and HJC location should be used for the quantification of hip loading. For biomechanical analyses aiming to understand modified hip joint loading, and planning hip surgery in patients with osteoarthritis, the amount of subject-specific detail, related to bone geometry and joint centre location in the musculoskeletal models used, needs to be considered. 相似文献

Introduction
The translational and predictive value of animal models highly depends on the validity of respective readout parameters. In arthritis research, there has been a shift from sole threshold testing for pain-related behavior, as well as from swelling and histology assessment for inflammation, toward an analysis of joint function as indicated, for instance, by an increasing number of studies on gait abnormalities. Clinically, the range of motion (ROM) of the affected joint plays a major role in diagnosis and the assessment of treatment benefits. Acute effects of active and passive warm-up, proprioceptive neuromuscular facilitation (PNF), and ratings of perceived exertion (RPE) were compared during hip-joint range of motion (ROM). Two active warm-up treatments included (a) achieving a respiratory exchange ratio (RER) of 1.00 and (b) achieving 60% of heart rate reserve (HRR). Hydrocollator pads (HP) served as the passive warm-up treatment. These treatments and a control were randomly assigned to increase hamstring muscle temperature of the dominant leg. Warm-up treatments were administered to 12 men (mean 25.3 years) with a minimum of 24 hours interspersed between each treatment. A timed PNF (slow-reversal-hold) technique was conducted after each warm-up treatment. Tukey tests (p < 0.05) showed ROM for RER (107.4 degrees) was greater than all other treatments. ROM for HRR (102.8 degrees) and HP (103.4 degrees) did not differ from each other but were greater than the control (98.8 degrees). Ratings of perceived exertion were lowest for RER (4.0) and highest for control (8.5). Ratings of perceived exertion for HRR (6.0) and HP (6.5) were similar. In conclusion, an active warm-up before PNF stretching appears to be the most effective treatment to increase hip ROM. Results of RPE reinforce that active warm-up reduces the resistance to stretch. In a field setting, it is estimated that a warm-up of 70% of HRR would duplicate the muscle readiness equivalent to an RER of 1.00 before PNF stretching. 相似文献

The plantarflexors of the lower limb are often assumed to act as independent actuators, but the validity of this assumption is the subject of considerable debate. This study aims to determine the degree to which passive changes in gastrocnemius muscle length, induced by knee motion, affect the tension in the adjacent soleus muscle. A second aim is to quantify the magnitude of myofascial passive force transmission between gastrocnemius and adjacent soleus. Fifteen healthy volunteers participated. Simultaneous ultrasound images of the gastrocnemius and soleus muscles were obtained during passive knee flexion (0-90°), while keeping the ankle angle fixed at either 70° or 115°. Image correlation analysis was used to quantify muscle fascicle lengths in both muscles. The data show that the soleus muscle fascicles elongate significantly during gastrocnemius shortening. The approximate change in passive soleus force as a result of the observed change in fascicle length was estimated and appears to be <5 N, but this estimate is sensitive to the assumed slack length of soleus. 相似文献

Stretching exercise is effective for increasing joint range of motion (ROM). However, the Surgeon General's Report and the American College of Sports Medicine cite a lack of studies identifying strategies capable of increasing the effectiveness of stretching exercise. This investigation evaluated adding modest weight (0.45-1.35 kg) to a stretching exercise routine (Body Recall [BR]) on joint ROM. Forty-three subjects ages 55-83 years participated in 1 of 2 training groups, BR, BR with weights (BR+W), or a control group (C). ROM was evaluated at the neck, shoulder, hip, knee, and ankle before and after 10 weeks of exercise. Using ANCOVA, significant differences (p < 0.01) were observed for right and left cervical rotation, hip extension, ankle dorsiflexion, ankle plantar flexion, and shoulder flexion. Post hoc analysis revealed that cervical rotation (left and right), hip extension, and ankle dorsiflexion for BR+W subjects differed significantly from BR and C (p < 0.01). Significant differences with shoulder flexion and ankle plantar flexion were found for both BR and BR+W in comparison to C (p < 0.01). Results indicate that addition of weights enhanced the effectiveness of stretching exercise for increasing joint ROM with 4 of the 6 selected measurements. Thus, a modest intensity exercise program that is within the reach of most elderly may significantly affect joint ROM and flexibility. 相似文献

Methods to determine the hip joint centre (HJC) location are necessary in gait analysis. It has been demonstrated that the methods proposed in the literature involve large mislocation errors. The choice should be made according to the extent by which HJC location errors distort the estimates of angles and resultant moments at the hip and knee joints. This study aimed at quantifying how mislocation errors propagate to these gait analysis results. Angles and moments at the hip and knee joint were calculated for five able-bodied subjects during level walking. The nominal position of the HJC was determined as the position of the pivot point of a 3D movement of the thigh relative to the pelvis. Angles and moments were then re-calculated after having added to HJC co-ordinates errors in the range of +/-30 mm. Angles and moments at both hip and knee joints were affected by HJC mislocation. The hip moments showed the largest propagation error: a 30 mm HJC anterior mislocation resulted in a propagated error into flexion/extension component of about -22%. The hip abduction/adduction moment was found the second largest affected quantity: a 30 mm lateral HJC mislocation produced a propagated error of about -15%. Finally, a 30 mm posterior HJC mislocation produced a delay of the flexion-to-extension timing in the order of 25% of the stride duration. HJC estimation methods with minimum antero-posterior error should therefore be preferred. 相似文献

The purpose of this study was to compare a 5-minute treadmill activity at 70% maximum heart rate (MHR) and 5 to 6 minutes of ballistic stretching to a 5-minute treadmill activity at 60% of MHR and 5 to 6 minutes of static stretching. Thirty healthy college students, 7 men and 23 women, volunteered. Most volunteers were moderately active. All participants signed an informed consent. Participants received the aforementioned warm-ups in random order with 48 to 72 hours between warm-ups. The stretching exercises were a back stretch, a quadriceps stretch, and a hamstring stretch. Three trials for 30 seconds each were given. After each warm-up the participants performed the modified-modified Schober test for low back flexibility, active knee extension test for hamstring flexibility, and plantar flexion for ankle flexibility. There were no significant differences on any of the 3 range of motion (ROM) tests although the ankle ROM test was almost significantly greater (68.8 degrees ) after the warm-up with static stretching compared with 65.9 degrees after the warm-up with ballistic stretching. A more intense cardiovascular activity and ballistic stretching were similar to a less intense cardiovascular activity and static stretching on flexibility. If athletes perform a warm-up and static or ballistic stretching before their workouts, then they should continue to perform the warm-up and the stretching routine with which they are most familiar and comfortable. 相似文献

The purpose of this study was to analyze the effect of 3 different exercise interventions plus a control group on passive hip range of motion (ROM). Previous research studies into the methods of improving passive hip mobility have focused on stretching protocols aimed specifically at the hip joint. The effect of core stabilization, motor training, and myofascial stretching techniques on hip mobility in a selected asymptomatic group with limited hip mobility is unclear. In this study, 24 young men with limited hip mobility (<50th percentile) were randomly assigned to 4 groups: stretching, stretching with motor control exercises for the hip and trunk, core endurance with motor control exercises, and the control group. Six-week home exercise programs were individually prescribed based on the assigned group, hip ROM, movement patterns, and timed core endurance. Two-way analyses of variances were conducted to analyze the effect of group assignment on hip ROM improvements. Both stretching groups demonstrated significant improvements in hip ROM (p < 0.05), attaining hip mobility levels at or above the 75th percentile, with rotation improving as much as 56%. The group receiving core endurance and motor control exercises with no stretching also demonstrated a moderate increase in ROM but only significantly so in rotation. Average core endurance holding times improved 38-53%. These results indicate that stretches aimed at the myofascial components of the upper body, in addition to the hip joint, resulted in dramatic increases in hip ROM in a group of young men with limited hip mobility. Hip ROM also improved in the group that did no active stretching, highlighting the potential role of including stabilization or "proximal stiffening training" when rehabilitating the extremities. 相似文献

The reduced range of motion (ROM) resulting from total hip replacement (THR) leads to frequent prosthetic impingement, which may restrict activities of daily living and cause subluxation and dislocation. Therefore, to know the ROM of THR is very important in clinical situations and in the design of prostheses. THR involves a pure ball and socket joint. We created a mathematical formula to calculate the theoretical ROM of THR limited by the prosthetic impingement. The ROM of THR is governed by the following five factors, (1) The prosthetic ROM (oscillation angle: obtained from company data), (2) cup abduction (3) cup anterior opening, (4) the angle of the femoral neck component from the horizontal plane, and (5) the femoral neck anteversion. The last 4 factors are able to be obtained from anterior-posterior, axial X-rays and CT of the patient's THR. The objective was to create mathematical formulas that could accurately and quickly calculate the ROM of THR. By entering the five values into a computer programmed with the formulas, one could obtain the ROM for the THR. This reveals the effect on ROM of the oscillation angle and the interaction of ROM with cup abduction, anterior opening and neck anteversion. Furthermore this readily would enable a clinical evaluation of the possibility of postoperative dislocation and help in postoperative rehabilitation. The calculated numerical values of ROM by these mathematical formulas were successfully compared with the ROMs obtained from 3-dimensional computer graphics (3D-CG). 相似文献

In morphological analysis of the femur, the hip joint centre (HJC) is generally determined using a 3D model of the femoral head based on medical images. However, the portion of the image selected to represent the femoral head may influence the HJC. We determined if this influence invalidates the results of three HJC calculation methods, one of which we introduce here. To isolate femoral heads in cadaver CT images, thresholds were applied to the distance between femur and acetabulum models. The sensitivity of the HJC to these thresholds and the differences between methods were quantified. For thresholds between 6 and 9?mm and healthy hips, differences between methods were below 1?mm and all methods were insensitive to threshold changes. For higher thresholds, the fovea capitis femoris disturbed the HJC. In two deformed hips, the new method performed superiorly. We conclude that for normal hips all methods produce valid results. 相似文献

The aim of this study was to develop a model which describes the mechanical spinal response to small alternating pelvic stimulation induced by an active rotational movement of a normal chair. The rotary continuous passive motion (RCPM) of the seat about a vertical axis of only 0.6 degrees resulted in an increased in spinal length as opposed to the normal daily shrinkage, and back patients experienced pain relief. Passive and active exercies have been broadly applied for treating and healing spinal disorders. A rigid body package (ADAMS Android) was used to translate the stimulation of the ischial tubersoity in caudo-cranial handing-over visualisation. The parameters of the model were set so that the values of the global stiffness and geometry of the intervertebral discs could be changed. In vivo validation of the model was based on force and moment measurements using an internal AO fixator. The predicitons of the model concerning natural frequency (4.5Hz) in vertical direction and the axial torsion response on small pelvic torsion are comparable with experimental data. 相似文献

Hip joint centre (HJC) localization is used in several biomedical applications, such as movement analysis and computer-assisted orthopaedic surgery.The purpose of this study was to validate in vitro a new algorithm (MC-pivoting) for HJC computation and to compare its performances with the state-of-the-art (least square approach–LSA). The MC-pivoting algorithm iteratively searches for the 3D coordinates of the point belonging to the femoral bone that, during the circumduction of the femur around the hip joint (pivoting), runs the minimum length trajectory. The algorithm was initialized with a point distribution that can be considered close to a Monte Carlo simulation sampling all around the LSA estimate.The performances of the MC-pivoting algorithm, compared with LSA, were evaluated with tests on cadavers. Dynamic reference frames were applied on both the femur and the pelvis and were tracked by an optical localizer.Results proved the algorithm accuracy (1.7 mm±1.6, 2.3—median value±quartiles), reliability (smaller upper quartiles of the errors distribution with respect to LSA) and robustness (reduction of the errors also in case of large pelvis displacements). 相似文献

The aryl hydrocarbon receptor (AhR) mediates a variety of biological responses to ubiquitous environmental pollutants. In this study, the effects of administration of β-naphthoflavone (BNF), a potent AhR ligand, on the expression of AhR-dependent genes were examined by microarray and qPCR analysis in both, differentiated and undifferentiated HepaRG cell lines. To prove that BNF-induced changes of investigated genes were indeed AhR-dependent, we knock down the expression of AhR by stable transfection of HepaRG cells with shRNA. Regardless of genetical identity, our results clearly demonstrate different expression profiles of AhR-dependent genes between differentiated and undifferentiated HepaRG cells. Genes involved in metabolism of xenobiotics constitute only minute fraction of all genes regulated by AhR in HepaRG cells. Participation of AhR in induction of expression of genes associated with regulation of apoptosis or involved in cell proliferation as well as AhR-dependent inhibition of genes connected to cell adhesion could support suggestion of involvement of AhR not only in initiation but also in progression of carcinogenesis. Among the AhR-dependent genes known to be involved in metabolism of xenobiotics, cytochromes P4501A1 and 1B1 belong to the most inducible by BNF. On the contrary, expression of GSTA1 and GSTA2 was significantly inhibited after BNF treatment of HepaRG cells. Among the AhR-dependent genes that are not involved in metabolism of xenobiotics SERPINB2, STC2, ARL4C, and TIPARP belong to the most inducible by BNF. Our results imply involvement of Ah receptor in regulation of CYP19A1, the gene-encoding aromatase, and an enzyme responsible for a key step in the biosynthesis of estrogens. 相似文献

The objective of the present work is to make use of the limited existing information on human joint impedance and to supplement and refine it synthetically, by means of simulation. A modular concept was applied to the construction of a joint model. The basic module was an individual revolute joint which was constrained by four resistive elements. The module was adapted to represent the different anatomical joints by specifying their corresponding mechanical parameters. The modular joint model was tested by incorporating it into a two dimensional whole body model and subjecting it to known impulsive conditions such as those reported in literature with regard to vehicular collision. Fine tuning was done by examining the sensitivity of the human model kinematic response to variation of the joint parameters. The whole body model was simulated with the aid of the ADAMS computer code. A comparison of the existing experimental data related to the sled test performed on a Hybrid III dummy with synthetic collision results showed good agreement. 相似文献
