The use of teeth as tools provides clues to past subsistence patterns and cultural practices. Five Holocene period hunter‐fisher‐gatherer mortuary sites from the south‐western region of Lake Baikal, Siberia, Russian Federation, are observed for activity‐induced dental modification (AIDM) to further characterize their adaptive regimes. Grooves on the occlusal surfaces of teeth are observed in 25 out of 123 individuals (20.3%) and were most likely produced during the processing of fibers from plants and animals, for making items such as nets and cordage. Regional variation in the frequency of individuals with occlusal grooves is found in riverine versus lakeshore sites. This variation suggests that production of material culture items differed, perhaps in relation to different fishing practices. There is also variation in the distribution of grooves by sex: grooves are found predominately in females, except at the Late Neolithic‐Bronze Age river site of Ust'‐Ida I where grooves are found exclusively in males. Occlusal grooves were cast using polyvinylsiloxane and maxillary canine impressions were examined by scanning electron microscopy (SEM) to determine striation patterns. Variation in striae orientation suggests that a variety of activities, and/or different manufacturing techniques, were involved in groove production. Overall, the variability in occlusal groove frequency, sex and regional distribution, and microscopic striae patterns, points to the multiplicity of activities and ways in which people used their mouths and teeth in cultural activities. Objective: This study will compare the clinical outcomes of 139 elders residing in long‐term care (LTC) who received dental treatment with those who did not receive care under a comprehensive dental programme over 5 years. Background: Numerous studies have documented very poor oral health and limited access to dental care among frail older adults residing in LTC facilities. The University of British Columbia and Providence Healthcare developed a comprehensive dental programme to serve elderly LTC residents within seven Vancouver hospitals. Since 2002, the Geriatric Dentistry Programme has provided annual oral health assessments and access to comprehensive dental care. Material and methods: A comprehensive oral health assessment was provided using CODE (an index of Clinical Oral Disorders in Elders). A change in oral health status (improvement or worsening) was evaluated by measuring CODE scores including caries and periodontal condition, and other aspects of the dentition. Results: Eighty‐three residents received dental treatment of some form over the 5 years, while 56 did not receive any treatment beyond an annual examination. The percentage of residents initially recommended for treatment in 2002 was 97%, which declined to 70–73% after the 3rd year. The percentage of residents treated increased after the first year and remained at 56–72% thereafter. The comparison between CODE scores from baseline and 5 years later showed an improvement for those receiving care (p = 0.02, χ2 = 7.9, df = 2). Conclusion: Within the limitations of this study, residents who did consent and receive care showed an improvement in their oral health status after 5 years.  相似文献

In Europe, the fossil record of Late Cretaceous eutherian mammals is very poor, being limited to only three genera (Labes, Lainodon, and Valentinella). Labes and Lainodon are well‐supported members of Zhelestidae, a stem eutherian clade, whereas Valentinella is more problematic, being recently considered as a nomen dubium. Based on X‐ray computed microtomography scan analysis of the holotype and thanks to the discovery of new specimens from the type locality (Vitrolles‐La Plaine, south‐eastern France, late Maastrichtian), we reassessed Valentinella. This genus is unique by the association of an enlarged and rounded jaw angle with an assumed relatively elevated angular process, a bulbous protoconid and an unbasined heel on p4, a p5 with a wide molariform talonid and a hypoflexid, a robust molar morphology with a potential specialized crushing‐grinding function (bulbously constructed cusps, large talonid, and horizontal apical wear facet of the hypocone), a somewhat reduced m3 relative to m2, a premolariform ?P3 or ?P4 lacking a metacone, and a relatively large hypocone on upper molars. These characters reinstate Valentinella as a valid genus. We also describe Mistralestes arcensis gen. et sp. nov. from a newly discovered locality (La Cairanne‐Highway, south‐eastern France, late Campanian). Mistralestes is defined by a robust premolariform p5 with no cingulid, paraconid, or metaconid; molars with a transverse protocristid, a gradual compression of the trigonid from m1 to m3, and paracristid and protocristid probably confluent on m3. Based on comparisons and phylogenetic analyses, Valentinella and Mistralestes may belong to Zhelestidae but this systematic attribution remains poorly supported. © 2013 The Linnean Society of London  相似文献

Background: In ageing society, it is necessary to promote oral health in the elderly, because good oral health is a significant contributing factor to good general health. Higher‐level functional capacity has been considered a crucial factor for successful independent living in the elderly. We hypothesised that functional capacity is a significant indicator of dental health behaviours. Methods: Three hundred and thirty‐eight adults aged 65 years or older were enrolled in this study. Higher‐level functional capacity was evaluated using the Tokyo Metropolitan Institute of Gerontology Index of Competence (TMIG‐index). Univariate and multivariate models were constructed with dental health behaviours, such as regular visits to a dentist, brushing frequency and use of extra cleaning devices, as the dependent variable, and the total TMIG‐index score and its subcategory scores as the principal independent variable. Results: Univariate logistic regression analysis demonstrated a significant correlation between low TMIG‐index and ‘intellectual activity’ subcategory scores to lack of regular visits to a dentist and not using extra cleaning devices. Using a multivariate model, significant relationships remained after adjusting for a number of variables including demographics, medical status, lifestyle and number of remaining teeth. Conclusion: Intellectual activity of higher‐level functional capacity may be an accurate indicator of dental health behaviours in community‐dwelling older adults. Intellectual activity should be taken into consideration to effectively promote oral health behaviours and oral hygiene in elderly persons living independently.  相似文献

Implant‐supported overdenture in an elderly patient with Huntington’s disease Huntington’s disease is a hereditary, progressive, neuro‐degenerative disorder characterised by increasingly severe motor impairment, cognitive decline and behavioural manifestations leading to functional disability. Dyskinesia and hyperkinesia of the tongue and the peri‐oral musculature make it impossible for the patient to wear a conventional complete denture, despite an adequate alveolar ridge. The present paper reports on a patient with Huntington’s disease who was rehabilitated with a mandibular overdenture supported by two endosteal implants. One year follow‐up examination showed that the prosthesis was stable and there was considerable improvement in the patient’s masticatory function.  相似文献

Surface decontamination remains challenging in peri‐implant infection therapy. To investigate the bactericidal efficacy of tissue tolerable plasma, S. mitis biofilms were created in vitro on 32 microrough titanium dental implants. Biofilm imaging was performed by confocal laser scanning microscopy (CLSM) and scanning electron microscopy (SEM). The implants were either rinsed with 1% NaCl as negative control (C) or irradiated with a diode laser (DL) for 60 sec as positive control or plasma (TTP60, TTP120) for 60 or 120 sec. Subsequently, colony forming units (CFU) were counted. Post‐treatment, implants were further examined using fluorescence microscopy (FM). Median CFU counts differed significantly between TTP60, TTP120 and C (2.19 and 2.2 vs. 3.29 log CFU/ml; p = 0.012 and 0.024). No significant difference was found between TTP60 and TTP120 (p = 0.958). Logarithmic reduction factors were (TTP60) 2.21, (TTP120) 1.93 and (DL) 0.59. Prior to treatment, CLSM and SEM detected adhering bacteria. Post‐treatment FM recorded that the number of dead cells was higher using TTP compared to DL and C. In view of TTP's effectiveness, regardless of resistance patterns and absence of surface alteration, its use in peri‐implant infection therapy is promising. The results encourage conducting clinical studies to investigate its impact on relevant parameters.

Severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) is an RNA virus that causes coronavirus infection (COVID‐19). COVID‐19 is a highly contagious disease transmitted through respiratory droplets, saliva and other contact routes. Within 10 months of its outbreak, SARS‐CoV‐2 has infected more than 23 million people around the world. Evidence suggests that older adults are the most vulnerable to infection and have an increased risk of mortality. Reduced immunity and underlying medical conditions make them risk‐prone and vulnerable to critical care. Older adults affected with the SARS‐CoV‐2 virus present with distinct clinical manifestations necessitating specific treatment needs and management protocols. While it is crucial to prevent the spread of novel coronavirus (2019‐nCoV), the role of oral healthcare workers in addressing the specific needs of ageing adult patients by adopting specific guidelines and appropriate infection control protocols is timely. This paper aims to develop specific guidelines and protocols for the dental management of geriatric patients during the COVID‐19 pandemic.  相似文献

In this study, we used rat animal model to compare the efficiency of indocyanine green (ICG)‐assisted dental near‐infrared fluorescence imaging with X‐ray imaging, and we optimized the imaging window for both unerupted and erupted molars. The results show that the morphology of the dental structures was observed clearly from ICG‐assisted dental images (especially through the endoscope). A better image contrast was easily acquired at the short imaging windows (<10 minutes) for unerupted and erupted molars. For unerupted molars, there is another optimized imaging window (48‐96 hours) with a prominent glow‐in‐the‐dark effect: only the molars remain bright. This study also revealed that the laser ablation of dental follicles can disrupt the molar development, and our method is able to efficiently detect laser‐treated molars and acquire the precise morphology. Thus, ICG‐assisted dental imaging has the potential to be a safer and more efficient imaging modality for the real‐time diagnosis of dental diseases.  相似文献

Aesthetic demand of French seniors: a large‐scale study Objectives: The needs of seniors for oral health and aesthetics are growing, as are their demands for aesthetics. This large‐scale study aims to identify the demand for aesthetics for a population aged over 55, and the influence of age and gender. Methods: A 15‐item questionnaire was placed on the web in partnership with a major magazine dedicated to seniors. It reflected practitioners’ questions with regard to senior patient expectations: aesthetic demand assessment, most commonly expressed complaints, the importance given to tooth colour, knowledge of available therapeutic treatments and motivation levels for treatment. Results: The survey generated 3868 responses, 61% from women; 77% of respondents declared being satisfied to very satisfied with their smile. Their highest priority to improve their smile was tooth alignment, followed by their shape, length and shade. Although 60% of respondents were satisfied with their current shading, 53% would prefer to have them whitened. Aesthetic treatments were well‐known to seniors. Over four‐fifths of them had heard of dental implants and ceramic crowns. Two‐thirds of those who wished to improve their smile were considering dental treatment. Conclusion: The high number of collected questionnaires confirms the strong interest shown by seniors for dental aesthetics, particularly from women. Baby‐boomers seem more attentive to the appearance of their smile than their elders. However, the importance of appearance decreases with age, as it becomes less of a priority, with attention more focused on general health.  相似文献
