摘要 目的:分析孤独症儿童生存质量、睡眠质量现状,探讨其睡眠障碍的影响因素。方法:选择2017年1月至2019年12月期间我院收治的孤独症儿童100例作为研究组,另选同期于我院进行体检的健康儿童100例作为对照组,分别应用生存质量简易量表、社会支持评定量表、儿童睡眠质量调查问卷调查两组生存质量、社会支持情况、睡眠质量。应用单因素及多因素Logistic回归分析孤独症儿童睡眠障碍的影响因素。结果:研究组儿童生存质量简易量表的主观标准评分、客观标准评分及社会支持评定量表的主观支持评分、客观支持评分、对社会支持的利用度评分、总分均显著低于对照组(P<0.05)。研究组入睡困难、睡眠不安、间断睡眠、夜惊、夜间尿床、打鼾、梦呓、梦游、张口呼吸、夜间磨牙、睡眠出汗发生率均显著高于对照组(P<0.05),两组梦魇发生率比较无统计学差异(P>0.05)。研究组患儿中存在睡眠障碍的46例,无睡眠障碍的54例。单因素分析显示,睡眠障碍组父母关系差、家族精神病史、新生儿窒息史、出生体重<2500 g比例显著高于无睡眠障碍组(P<0.05)。多因素Logistic回归分析显示,父母关系差、有家族精神疾病史、有新生儿期窒息史、出生体重<2500 g是孤独症儿童睡眠障碍的危险因素(P<0.05)。结论:与健康儿童相比,孤独症儿童生存质量、社会支持情况较差,睡眠障碍发生率也较高,父母关系差、有家族精神疾病史、有新生儿期窒息史、出生体重<2500 g是孤独症儿童睡眠障碍的危险因素。  相似文献

目的 调查学龄前儿童猛性龋的危险因素,并对诸多因素进行分析,为预防和控制猛性龋的发展提供干预措施.方法 采用Logistic分析方法,对800例2~5岁学龄前儿童进行调查和统计学分析.结果 各年龄组患病率间存在显著性差异(P<0.05),母乳喂养时间、每天喂养次数、甜食习惯、奶瓶中加甜食、每天吃零食次数、较晚建立刷牙习惯及母亲文化教育程度为学龄前儿童猛性龋患病率的危险因素.结论 学龄前儿童猛性龋的发生、发展与诸多因素有关,及早识别猛性龋的危险因素,对预防和控制学龄前儿童猛性龋的发展有重要的临床意义.  相似文献

自闭症又称孤独症。自闭症是一种儿童早期广泛性脑功能发育障碍性疾病,具有种族、地域和个体差异性特点,未经干预治疗的自闭症患儿的终身残疾率很高。近年,儿童自闭症患病率的报道呈逐年上升趋势。但目前关于儿童自闭症的病因形成的机制尚不清楚。一般认为是多种非生物因素和生物因素共同影响作用的结果。其中以脑发展、神经化学和遗传等的生物因素占多数。本文仅就目前国内外对于自闭症和自闭症谱系障碍的定义和病因形成的非生物学因素及生物学因素研究做简要概述,希望为相关研究提供有价值的参考。  相似文献

目的:了解哮喘儿童父母的对疾病知识的掌握情况以及儿童的服药依从情况和影响因素,为提高哮喘儿童的控制率提供参考依据。方法:选择2015年1月-2015年12月于上海市第十人民医院儿科门诊诊治的支气管哮喘儿童93例,调查其哮喘控制情况、哮喘服药依从性和父母基本情况与相关知识。依从性与知识知晓率的比较采用双向有序的检验,影响因素采用有序结果的累积优势Logistic回归分析。结果:本次调查93例哮喘儿童中,哮喘完全控制率为23.7%,儿童服药依从性好的比率为25.8%,哮喘儿童父母相关知识知晓率高的比率为25.8%,儿童哮喘控制率与服药依从性之间存在相关性(P=0.029),哮喘儿童服药依从性与哮喘儿童父母相关知识知晓率之间存在相关性(P=0.035)。哮喘儿童的服药依从性受到儿童性别(OR=1.153,95%CI:1.04-1.96)、家族史(OR=1.402,95%CI:1.20-2.33)、知识知晓率(OR=1.828,95%CI:1.05-3.17)和病程(OR=0.758,95%CI:0.35-0.97)等因素的影响(P0.05)。结论:哮喘儿童的服药依从性受到儿童性别、家族史、知识知晓率和病程等因素的影响,要充分发挥儿童父母的作用,从医院内干预逐渐进入家庭干预,通过对父母或者监护人的认知或用药知识的提高,切实提高哮喘儿童的用药依从性和哮喘的控制率。  相似文献

大量研究表明,通过干预孤独症谱系障碍(Autism Spectrum Disorder,ASD)儿童情绪能力,可以提高其移情能力从而改善核心症状--社交交往障碍。随着ASD儿童人数逐年上升,对干预的需求日益增大。目前国内外对于ASD儿童情绪能力的系统干预研究较少,主要研究均集中在表情识别这一层面。本文结合视频分析,自动表情识别算法,Flash技术,设计交互式虚拟场景游戏,研发一个面向ASD儿童的情绪能力干预的系统。该系统可以训练儿童的表情识别能力,还可以训练儿童对表情的模仿和表达,具有实用性,方便推广。  相似文献

老年性痴呆(阿尔茨海默病,Alzheimer disease,AD)是目前严重影响老年人生存质量的疾病,且疗效不佳。据推测到2050年阿尔茨海默病的患病率将是现今的三倍。轻度认知障碍(mild cognitive impairment,MCI)是介于阿尔茨海默病和正常衰老之间的一种认知功能损害状态,是发生阿尔茨海默病的高危因素。文献报道轻度认知障碍每年以8%-25%的比例进展为阿尔茨海默病,较正常人群阿尔茨海默病发病率高10倍。与阿尔茨海默病病理损害不可逆相比,轻度认知障碍患者通过早期干预治疗,可延缓或阻止病情发展为阿尔茨海默病。因此,对阿尔茨海默病早期出现的轻度认知功能障碍诊断及干预尤为重要。本文就认知功能早期阶段,轻度认知功能障碍的历年(2000年到2014年3月)研究进展从概念及分型、临床表现、诊断标准、病理生理及其影像学研究、危险因素及其预防、干预措施(药物和非药物)等方面的最新进展进行论述。  相似文献

目的:研究危重症患者肠功能障碍的相关临床因素,为有效防治肠功能障碍提供理论依据.方法:选取31例合并肠功能障碍的危重症患者为研究对象,以无肠功能障碍的30例危重症患者为对照.研究平均动脉压(MPA)、动脉血氧分压(PaO2),血白细胞计数、钾离子、GPT/GST、白蛋白等指标与肠功能衰竭发生的相关性;分析肠功能衰竭与脏器损伤个数及死亡率的关系.结果:伴有肠功能障碍的危重症患者MPA、PaO2和血清白蛋白水平低于对照组(P<0.05);血白细胞计数和GPT含量明显高于无肠功能障碍组(P<0.05);血钾离子浓度及GST两组无差异(P>0.05);发展至肠功能障碍的患者平均脏器损伤个数多于对照组,并且有着较高的死亡率(P<0.05).结论:伴有肠功能障碍的危重症患者早期就表现出严重的低血压,低血氧,低白蛋白血症,肝功能的损害,对于这些方面的监测既可以帮助我们早期发现肠功能障碍的存在,也有助于指导早期干预.  相似文献

目的:探讨环境因素对儿童特发性矮身材的影响。方法:采用病例对照的研究方法,对100例特发性矮身材患者组和100例正常对照儿童组分别进行体格测量,问卷调查家庭基本情况、出生史、饮食习惯、生活方式、生长速率、家庭生活条件、经济状况、父母文化程度等环境因素资料进行Logistic回归分析。结果:家族矮小史、母亲文化程度、家庭经济状况、偏食、运动时间、留守儿童影响特发性矮身材的发生。结论:因素与儿童特发性矮身材有关,为特发性矮身材儿童早期干预提供临床依据。  相似文献

腹泻是小儿常见病、多发病,是一个全球性的公共卫生问题。尤以发展中国家最为突出。据WHO不完全统计,发展中国家每年约有450万5岁以下儿童死于腹泻病。由此可见。腹泻病不仅是儿童营养不良.生长发育障碍的原因.也是威胁儿童生命的主要因素。儿童腹泻病病因十分复杂,诸如社会、环境因素。个体因素.饮食与感染等。但近20年来,随着微生态学的发展,人们逐渐认识到肠道正常菌群.尤其是双歧杆菌与腹泻的关系密切。双歧杆菌等正常菌群与机体共同构成一个整体。机体为双歧杆菌提供定植的条件。双歧杆菌又为机体防治疾病起到不容忽视的作用。笔者就肠道双歧杆菌与儿童腹泻之间的相互作用和影响作一综述。  相似文献

目的:探讨早产儿发生的围产期高危因素及并发症。方法:收集我院2006.1—2009.12出生的早产儿183例的临床资料,总结其早产的相关围产期高危因素及并发症。结果:胎膜早破、多胎妊娠、妊娠高血压综合征是早产发生的主要原因;早产儿的主要并发症为肺炎和颅内出血等。结论:加强围生期保健,早期干预各种围产期高危因素,积极防治早产儿各种并发症,是提高早产儿存活率降低致残率的关键。  相似文献

The pathogenesis of anxiety disorders is multifactorial, involving complex interactions between biological factors, environmental influences and psychological mechanisms. Recent advances have highlighted the role of epigenetics in bridging the gap between multiple contributing risk factors toward an increased understanding of the pathomechanisms underlying anxiety. In this review, we present an overview of the current state of knowledge regarding putative risk mechanisms in the pathogenesis of anxiety disorders, placing a particular focus on the role of protective factors serving to buffer a risk factor constellation and the role of epigenetic processes functioning as a potent turnstile changing passage direction toward disorder risk or resilience. We discuss promising future directions in epigenetic research regarding the prediction, prevention and personalized treatment of anxiety disorders.  相似文献

Women's health is affected by breast cancer worldwide and Saudi Arabia (SA) is no exception. Malignancy has enormous consequences for social, psychological and public health. The aim of this study was to examine the risk factors for Saudi women from breast cancer using logistic regression models. In 135 patient cases for different stages of breast cancer was used to study case management, 270 healthy women from King Abd Alla Medical City, Mecca, SA were taken to predict the probability of women developing breast cancer, logistic regression was analyzed taking factors such as age, marital status, family history, parity, age at first full-term pregnancy, menopausal status, body mass index (BMI) and breast feeding. The logistic regression model showed that there are important risk factors (age, marital status, family history, parity, age at first full-term pregnancy, menopausal status, body mass index, and breast feeding) in development of breast cancer. Fewer cases were observed in unmarried women, age ≤30, BMI ≤20. In contrast, more cases were found with women age 41–50 married, BMI > 30, a smaller number of children, not breast feeding, age of first pregnancy ≥30, menopausal status age at 46–50. Based on our data there is role of risk factors in developing breast cancer, less BMI, the increase number of children, breast feeding, which are playing as protective factor for breast cancer.  相似文献

Background

Many children can be exposed to multiple adversities in low and middle-income countries (LMICs) placing them at potential risk of psychological problems. However, there is a paucity of research using large representative cohorts examining the psychological adjustment of children in school settings in these countries. Children’s psychological adjustment has been shown to affect educational progress which is critical for their future. This study, based in a rural, socio-economically disadvantaged area of South Africa, aimed to examine the prevalence of children’s psychological problems as well as possible risk and protective factors.

Methods

Rates of psychological problems in 10–12 year olds were examined using teacher- and child-report questionnaires. Data on children from 10 rural primary schools, selected by stratified random sampling, were linked to individual and household data from the Agincourt health and socio-demographic surveillance system collected from households over 15 years.

Results

A total of 1,025 children were assessed. Teachers identified high levels of behavioural and emotional problems (41%). Children reported lower, but substantial rates of anxiety/depression (14%), and significant post-traumatic stress symptoms (24%); almost a quarter felt unsafe in school. Risk factors included being a second-generation former refugee and being from a large household. Protective factors highlight the importance of maternal factors, such as being more educated and in a stable partnership.

Conclusion

The high levels of psychological problems identified by teachers are a serious public health concern, as they are likely to impact negatively on children’s education, particularly given the large class sizes and limited resources in rural LMIC settings. Despite the high levels of risk, a proportion of children were managing well and research to understand resilience could inform interventions.  相似文献

We investigate whether the profile of factors protecting psychosocial functioning of high risk exposed Australian Aboriginal youth are the same as those promoting psychosocial functioning in low risk exposed youth. Data on 1,021 youth aged 12–17 years were drawn from the Western Australian Aboriginal Child Health Survey (WAACHS 2000–2002), a population representative survey of the health and well-being of Aboriginal children, their families and community contexts. A person-centered approach was used to define four groups of youth cross-classified according to level of risk exposure (high/low) and psychosocial functioning (good/poor). Multivariate logistic regression was used to model the influence of individual, family, cultural and community factors on psychosocial outcomes separately for youth in high and low family-risk contexts. Results showed that in high family risk contexts, prosocial friendship and low area-level socioeconomic status uniquely protected psychosocial functioning. However, in low family risk contexts the perception of racism increased the likelihood of poor psychosocial functioning. For youth in both high and low risk contexts, higher self-esteem and self-regulation were associated with good psychosocial functioning although the relationship was non-linear. These findings demonstrate that an empirical resilience framework of analysis can identify potent protective processes operating uniquely in contexts of high risk and is the first to describe distinct profiles of risk, protective and promotive factors within high and low risk exposed Australian Aboriginal youth.  相似文献

Background

Symptoms of maternal postnatal depression are associated with an increased risk of adverse effects on child development. However, some children exposed to postnatal depression have outcomes similar to unexposed children, and can be referred to as resilient. This study aimed to determine the mechanisms of resilience in children exposed to depressive symptoms postnatally.

Method

Data are from a prospective cohort study, the Avon Longitudinal Study of Parents and Children. Self-report questionnaire data were collected during pregnancy and the child’s first 2 years regarding maternal views of parenting and her perception of the child. The Edinburgh Postnatal Depression Scale (EPDS) was completed postnatally at 8 months and the Strengths and Difficulties Questionnaire (SDQ) at age 11 years. Exposed children who scored above the median score of non-exposed children were defined as resilient. Structural equation modeling was used to investigate the development of resilience.

Results

From the core ALSPAC cohort, 1,009 children (6.9%) were exposed to maternal depression at 8 months postnatally. The SDQ total difficulties scores at 11 years of age indicated that 325 (32.2%) were resilient, 684 were non-resilient. Maternal positive feelings about parenting and child non-verbal communication at 15 months increased the likelihood of later resilience.

Conclusions

In this study, resilience was associated with two factors: the child’s nonverbal communication at 15 months and by maternal positive feelings about parenting. Early intervention to support mother-child interaction and foster child development in women identified with postnatal depressive symptoms may benefit later child resilience.  相似文献

Most studies of the association between family structure and risky sexual behaviour among adolescents and young adults have employed a risk perspective which assumes that, compared with other types, two-parent families are protective. Drawing from a positive-oriented approach in this study, it is hypothesized that within each family type some influential factors may mitigate such anticipated deleterious effects of non-intact families and decrease sexual risk-taking. The paper examines specifically the effects of risk and protective factors with an emphasis on family processes associated with resilience, using data from a pooled sample of 1025 females and males aged 12-24 years from Bandjoun (West Cameroon). Findings show that the quality of parent/guardian-youth relationships significantly decreases the odds of risky sexual behaviour by 36%, 65% and 50% in neither-, one- and two-parent families, respectively. For two-parent families only, parental control acts as a significant protective factor; it decreased by 41% the odds of risky sexual behaviour. Programmatically, protective family factors such as parent/guardian-youth interactions need to be promoted to improve the efficiency of reproductive health and HIV interventions in sub-Saharan Africa.  相似文献

For restoration to be an effective strategy to reverse large‐scale habitat loss and land degradation, funding programs need policies that promote selection of and commitment to projects that can reasonably be expected to succeed. Programmatic project selection practices have received minimal formal evaluation, despite their importance. In this study, we considered the extent to which a program needs to consider both ecological and organizational factors during project selection in order to minimize the incidence of project failure. Our assessment of a long‐term program that funds ecological restoration efforts across Minnesota (U.S.A.), based on project records, manager surveys, and field surveys, yielded several broadly relevant insights. First, factors well understood to confer ecological resilience (level of landscape alteration and starting condition) were clearly associated with restoration outcomes, regardless of time‐since‐initiation of restoration. Second, restoration of low‐resilience ecosystems is typically a labor‐ and skill‐intensive enterprise for organizations that undertake them. Our analysis revealed four organizational limitations, in addition to insufficient funds, that hindered capacity to keep projects on‐track: lack of planning and goal‐setting, inadequate staffing, leadership change, and incomplete records. Third, to reduce risk, programs do not necessarily need to avoid challenging projects, but do need to consider whether organizations proposing restorations have adequate internal capacity to competently plan and to sustain actions for a duration sufficient to restore ecological resilience. If a restoration is degraded enough to require human intervention to recover, the outcome of a project is as likely to reflect its organizational reality as much as its ecological circumstances.  相似文献

Background

Previous studies have examined physical risk factors in relation to functional health, but less work has focused on the protective role of psychological and social factors. We examined the individual and joint protective contribution of control beliefs, social support and physical exercise to changes in functional health, beyond the influence of health status and physical risk factors in middle-aged and older adults. Given that functional health typically declines throughout adulthood, it is important to identify modifiable factors that can be implemented to maintain functioning, improve quality of life, and reduce disability.

Methodology/Principal Findings

We conducted a national longitudinal study, Midlife in the United States (MIDUS), with assessments in 1995–1996 and 2004–2006, and 3,626 community-residing adults, aged 32 to 84, were included in the analyses. Functional health (Physical Functioning subscale of the SF-36) and protective factors were measured at both occasions. While controlling for socio-demographic, health status, and physical risk factors (large waist circumference, smoking, and alcohol or drug problems), a composite of the three protective variables (control beliefs, social support, and physical exercise) at Time 1 was significantly related to functional health change. The more of these factors at Time 1, the better the health maintenance over 10 years. Among middle-aged and older adults, declines in health were significantly reduced with an increased number of protective factors.

Conclusion/Significance

Age-related declines in health were reduced among those with more protective factors up to a decade earlier in life. Modifiable psychological, social, and physical protective factors, individually and in the aggregate, are associated with maintenance of functional health, beyond the damaging effects of physical risk factors. The results are encouraging for the prospect of developing interventions to promote functional health and for reducing public health expenditures for physical disability in later life.  相似文献
