Background

Hematoxylin-eosin (HE) staining of a full-thickness rectal wall fragment is classically used for the diagnosis of Hirschsprung disease (HD). However, this technique requires large fragments for a better diagnosis. Additionally, the histochemical and immunohistochemical methods of staining small fragments of rectal mucosal and submucosal biopsies are not available in all centers. Therefore, the possibility of diagnosing HD through HE staining in these biopsies could be a valuable alternative for centers that do not have more specific techniques. The objectives of the current investigation were to evaluate the concordance of the results obtained by HE staining and the calretinin method with acetylcholinesterase (AChE) activity in fragments of mucosa and submucosa in the diagnosis of HD.

Methods

For this study, 50 cases from our laboratory were selected. The tissue material was embedded in paraffin. Sixty levels of each fragment were utilized for HE, and the other 3 levels were used for calretinin. These slides were analyzed under the microscope, photographed and classified as either positive for HD when no ganglion cells were found with nerve trunks present or as negative when ganglion cells were found. The results from reading the slides were compared with those of AChE.

Results

Of the 50 cases evaluated by the HE technique, only 5 contradicted the diagnosis based on AChE, with a Kappa value of 0.800 and an accuracy of 90%. In the comparison between calretinin and AChE, 8 cases were discordant, with a Kappa value of 0.676 and an accuracy of 84%.

Conclusions

The concordance of results from AChE and HE methods was satisfactory, allowing for the potential use of the HE method for fragments of mucosa and submucosa as a valid alternative in the diagnosis of HD. The immunohistochemical technique of calretinin did not show good agreement with the AChE activity in our study.

OBJECTIVE: To correlate the subjective AgNOR counting method and DNA content with histologic diagnoses of thyroid cancer and invasion. STUDY DESIGN: Eighty-one consecutive cases of thyroid carcinoma were selected for DNA and AgNOR analysis. The diagnoses were: papillary carcinoma (n = 40), follicular carcinoma (n = 31), Hürthle cell adenocarcinoma (n = 4), and undifferentiated carcinoma (n = 6). Seven normal thyroids were used as controls. DNA quantitative measurement was performed with Vidas 2.0 software (Kontron Bildanalyse, Munich, Germany) connected to an MPM 210 photometer microscope (Carl Zeiss, Oberkochen, Germany). The DNA index was obtained using histograms. Counting the NORs was performed by subjectively counting the NORs in 200 malignant cells. RESULTS: DNA ploidy analysis showed all Hürthle cell adenocarcinomas, 21 (67%)follicular tumors, 23 (57%) papillary tumors and 4 (67%) undifferentiated carcinomas to be aneuploid. DNA analysis correlated with histologic type of the tumor (p = 0.032). There was no statistical significance to the AgNOR counting variables studied. Statistical analysis showed correlation between ploidy and histologic diagnosis, but not AgNOR counting, to have prognostic value. CONCLUSION: DNA ploidy is more useful than subjective counting of NORs as an adjunct method for thyroid lesion analysis.  相似文献

OBJECTIVE--To determine whether a booklet given to patients being discharged from hospital giving details of their admission and treatment increased their knowledge and recall when reviewed in outpatient clinics. DESIGN--Patients alternately allocated to receive a booklet or to serve as controls. Assessment by a questionnaire at first attendance at outpatient clinic after discharge. Data were collected over nine months. SETTING--One general medical and cardiological ward in a large teaching hospital and associated outpatient clinics. PATIENTS--One hundred and thirty one patients discharged taking at least one drug and scheduled to return to clinic within 12 weeks. Patients stratified by age and by the number of weeks between discharge and outpatient appointment. INTERVENTION--A booklet was given to 65 patients at discharge from the ward; 66 patients served as controls. MAIN RESULTS--Of the patients who received the booklet, 56 (86%) knew the names of their drugs, 62 (95%) the frequency of the dose, and 55 (85%) the reasons for taking each drug. The numbers in the control group were 31 (47%), 38 (58%), and 28 (42%) respectively. These differences were highly significant (p less than 0.001). Twenty six (40%) who received the booklet brought all their drugs to clinic compared with 12 (18%) control patients. Appreciably more of the first group of patients than control patients knew the reason they had been in hospital, and more of the first group indicated that they would take the correct action when their prescribed drugs ran out. Most general practitioners thought that the booklet was a good idea, that it was helpful, and that it was better than the existing interim discharge letter. CONCLUSIONS--Giving patients an information booklet at discharge from hospital appreciably increased the accuracy and thoroughness of their recall of important medical details concerning their illness and its treatment. The booklet was shown to be feasible, helpful in the outpatient clinic, and preferred by most general practitioners.  相似文献

Granulocytic sarcomas are rare tumors that can present in innumerous locations; thus there is very little clinical experience with these cases. Competition between spermatozoa of rival males to gain fertilizations has led to a wide array of modifications in sperm structure and function. Sperm cells of most muroid rodents have hook‐shaped extensions in the apical–ventral tip of the head, but the function of this structure is largely unknown. These ‘hooks’ may facilitate aggregation of spermatozoa in so‐called ‘trains’, as an adaptation to sperm competition, because sperm in trains may swim faster than free‐swimming cells. However, there is controversy regarding the role of the hook in train formation, and in relation to whether it is selected by sperm competition. We examined spermatozoa from muroid rodents with varying levels of sperm competition to assess whether (i) sperm aggregates are common in these taxa, (ii) presence of a hook relates to the formation of sperm aggregations, and (iii) formation of sperm aggregations is explained by sperm competition. Our analyses in 25 muroid species revealed that > 92% of spermatozoa swim individually in all species, with the exception of the wood mouse, Apodemus sylvaticus, which has ~50% spermatozoa swimming freely. Species with hooked spermatozoa had higher sperm competition levels and longer sperm than species whose sperm lack a hook. Neither the presence of hook nor sperm competition levels were related to the percentage of sperm in aggregations. Thus, (i) sperm aggregates in muroid rodents are an exceptional trait found only in a few species, (ii) evolution of the sperm hook is associated to sperm competition levels, but (iii) the hook is unlikely to be related to the formation of sperm aggregates. The evolutionary significance of the sperm head hook thus remains elusive, and future studies should examine potential roles of this pervasive structure in sperm's hydrodynamic efficiency and sperm–female tract interactions.  相似文献

For over 100 years, Morgan's Canon has served as the criterion for distinguishing what appears to be complex cognitive processes shown by animals from simpler associative learning processes (Pavlovian and instrumental conditioning). Morgan's ( 1894 ) canon states “In no case is an animal activity to be interpreted in terms of higher psychological processes if it can be fairly interpreted in terms of processes which stand lower in the scale of psychological evolution and development.” In the present article, several examples are provided in which complex human‐like processes are proposed to have been demonstrated but the judicious use of Morgan's Canon suggests that simpler mechanisms may be sufficient to account for the behavior. The use of Morgan's Canon is not meant to reduce behavior to its lowest common denominator but rather to challenge investigators to develop procedures that can distinguish between simple behavioral principles and the more complex processes that cannot be explained in terms of genetics or simple conditioning. Whatever the results of these experiments, they should help identify the underlying processes and mechanisms involved.  相似文献

This paper assesses the usefulness of the Difference Index (Metzger, Shapiro, Mosimann &; Vinton, 1968) for predicting, from amino acid compositions, whether two proteins are related or unrelated. It is concluded that, with a 5 % probability of false identification, Difference Index values less than 10-0 indicate relatedness and values greater than 26·8 indicate unrelatedness. A large proportion of protein pairs have Difference Index values in the region 10·0–26·8 and cannot therefore be reliably identified as related or unrelated by this criterion.  相似文献

The urgency of the tropical biodiversity crisis continues to be a major justification for wildlife research and its funding. To examine the benefits of this research for on-the-ground conservation, we focused on Borneo, where conservation has a long history and we have direct experience. We compiled, categorized and evaluated 284 publications from a broad variety of sources, 153 from peer-reviewed journals. We found that few studies address threats to species and fewer still provide input for or guidance to effective management. We consider various reasons for these shortcomings. Research is seldom judged on its relevance to pragmatic problem solving. Furthermore, many research programs lack the necessary long-term vision and organizational structure for useful applied research. We consulted conservation leaders about our conclusions and all responses suggest that our concerns are not unique to Borneo but reflect wider problems. We conclude that conservation research across most of the tropics is failing to address conservation needs. We make a number of recommendations based on our findings. Conservation biologists should place a higher priority on addressing practical conservation needs and goals.  相似文献

Results from a transmission electron microscope study of the spermiogenesis and spermatozoon of Meara stichopi (Nemertodermatida, Platyhelminthes) indicate that the sperm type of the Nemertodermatida has evolved from the primitive metazoan sperm type rather than from an aberrant biflagellar sperm type as found in many other flatworms. The spirally coiled mitochondrial derivative in the mature spermatozoon develops from two large oval mitochondria in the early spermatid stages. A single flagellum grows out from a peripheral basal body adjacent to a perpendicularly placed accessory centriole. The basal body moves to a distal depression of the nucleus, and becomes equipped with an anchoring fibre apparatus. Most of the flagellum becomes axially incorporated into the developing spermatid. No trace of a second flagellum was found in any stage of the spermiogenesis. Rounded vesicles appear around the proximal, tapering end of the elongating nucleus. Most probably these vesicles form a thin acrosomal structure in the mature spermatozoon. No dense bodies, characteristic of many other ‘turbellarian’ flatworm sperm types, were found. As the wilderness metaphor has decreased in utility due to widespread human‐driven environmental change, conservationists and restorationists have struggled to find new ways to inspire nature conservation. Some have suggested gardening as a new metaphor, but many are wary of its implications, particularly for animals viewed as threats or pests. Others, however, point out positive attributes for the metaphor including its focus on stewardship of nature which allows for positive human agency in ecosystems. We argue a gardening metaphor may also allow increased flexibility in approaches to biodiversity conservation, in part by allowing goals to be fit to communities and their specific cultural contexts. Wild gardening would seek to preserve global biodiversity while acknowledging the pivotal role humans now play in that process. Here we review the use of the garden metaphor over the last 25 years and discuss what wild gardening might mean for restoration. Consistent with a long history of environmental thought, we suggest such a metaphor will work best if it is coupled with a civic/stewardship ethic and a good dose of humility on the part of all gardeners.  相似文献

Nematode spermatozoa are highly specialized amoeboid cells that must acquire motility through the extension of a single pseudopod. Despite morphological and molecular differences with flagellated spermatozoa (including a non‐actin‐based cytoskeleton), nematode sperm must also respond to cues present in the female reproductive tract that render them motile, thereby allowing them to locate and fertilize the egg. The factors that trigger pseudopod extension in vivo are unknown, although current models suggest the activation through proteases acting on the sperm surface resulting in a myriad of biochemical, physiological, and morphological changes. Compelling evidence shows that pseudopod extension is under the regulation of physiological events also observed in other eukaryotic cells (including flagellated sperm) that involve membrane rearrangements in response to extracellular cues that initiate various signal transduction pathways. An integrative approach to the study of nonflagellated spermatozoa will shed light on the identification of unique and conserved processes during fertilization among different taxa. Mol. Reprod.
