尾加压素Ⅱ对正常及缺血-再灌注离体大鼠心脏的影响   总被引:6,自引:1,他引:5

在正常Langendorff灌流与缺血-再灌注(停灌20 min-复灌20 min)离体大鼠心脏模型,观察尾加压素Ⅱ(urotensin Ⅱ,UⅡ)对冠脉流量、心功能和心肌代谢的影响以及心肌UⅡ受体的功能,以探讨UⅡ的心脏效应。对正常心脏给予0.1、1和10 nmol/L UⅡ各5 min,然后换洗5 min,对停灌缺血-再灌注心脏在再灌注期给予1或10nmol/L UⅡ。监测心率、左室内压和左室内压升降的最大变化率等心功能指标,计算冠脉流量,测定冠脉流出液中总蛋白和肌红蛋白含量以及乳酸脱氢酶(LDH)活性。灌流结束后,测定心肌丙二醛(MDA)含量和质膜UⅡ结合位点(放射性配基结合法)。结果如下:(1)正常心脏灌流UⅡ后,冠脉流量和心功能呈浓度依赖下降,换洗后没有完全恢复。心肌蛋白、肌红蛋白和LDH漏出随UⅡ浓度的增加而增加,换洗后迅速减少。UⅡ组心肌MDA含量与对照组差异无显著性。(2)缺血-再灌注后,冠脉流量显著减少,心功能显著抑制,再灌注期心肌蛋白、肌红蛋白和LDH明显漏出;给予UⅡ后,上述变化增强,且高浓度组更强,与对照组差异有显著性(P<<0.01),再灌注后心肌MDA含量亦显著高于对照(P<0.01)。(3)缺血-再灌注心肌质膜UⅡ受体的B_(max)显著高于正常对照心肌(14.65±1.78vs20.53±1.98 fmol/mg pr,P<0.01),Kd值变化无统计学意义。上述结果表明,在正常  相似文献

目的:探讨乙醇后处理心肌保护作用是否与一氧化氮生成有关。方法:局部结扎冠状动脉左前降支30min,复灌120 min复制离体大鼠心肌缺血/复灌模型。心肌缺血末5 min,复灌初期10min给予乙醇50mmol/L,共灌流15 min进行乙醇后处理干预。实验随机分为五组,正常组,缺血/复灌组,乙醇后处理组,乙醇后处理+L-NAME组和乙醇后处理+苍术苷组。测定心室动力学指标和复灌期间冠脉流出液中乳酸脱氢酶(LDH)含量,TTC染色法测定心肌梗死面积,硝酸还原法测定心肌组织一氧化氮(NO)含量。RT-PCR检测左心室前壁心尖组织Bc-l2和BaxmRNA的表达。结果:与单纯缺血/复灌相比,乙醇后处理明显促进了左室发展压、左室做功的恢复,降低复灌期冠脉流出液中LDH的释放和心肌梗死面积,心肌组织NO释放减少,Bc-l 2/Bax mRNA比值增高。一氧化氮合酶抑制剂L-NAME和线粒体渗透性转换孔道开放剂苍术苷均抑制了乙醇后处理心室功能的恢复、LDH释放的减少和梗死面积的降低,心肌组织NO释放进一步减少,Bc-l 2/Bax mRNA比值降低。结论:乙醇后处理的心肌保护作用可能与减少NO的释放、抑制线粒体渗透性转换孔道的开放和抑制细胞凋亡的发生有关。  相似文献

目的:通过在心脏停搏液中添加适量左旋精氨酸来观察其在冠脉搭桥术中心肌保护效果。方法:选择2008年1月~2010年1月在我院行冠状动脉旁路移植术患者20例,随机分为2组,每组10例,对照组:常规心肌保护液组,不添加左旋精氨酸。实验组,心肌保护液中加入7.5g/L左旋精氨酸。测患者术前(T1)、术后6小时(T2)、术后12小时(T3)、术后24小时(T4)、术后48小时(T5)血浆中TNF-α、IL-6、IL-8及cTn I含量。记录临床观察指标。结果:两组血浆TNF-α、IL-6、IL-8、cTn I浓度术前无统计学意义(P>0.05),术后各时点TNF-α、IL-6、IL-8、cTn I浓度显著升高且实验组均低于对照组(P<0.05)。临床观察指标除血管活性药物应用情况实验组优于对照组(P<0.05)外无明显差异。结论:在心脏停搏液中加入L-精氨酸,可有效保护缺血心肌,减轻心肌再灌注损伤程度。  相似文献

采用沙培法,对盐胁迫下坪山柚和福橘幼苗体内矿质元素的变化进行了研究。结果表明,随着NaCl浓度的增加,坪山柚和福橘幼苗根部及地上部Na^+、Cl-含量增加,且相同浓度下,福橘比坪山柚高。40mmol/L NaCI胁迫下,坪山柚和福橘幼苗地上部的K^+、Fe含量,根部的Ca^2+、Mg^2+、Zn含量显著下降,而根部Fe含量及地上部Zn含量显著增加。随NaCl浓度增大,坪山柚根部K^+含量,地上部Ca^2+、Mg^2+含量变化不明显,而福橘根部、地上部上述离子含量在NaCl浓度≥160mmol/L时均显著下降。因此,根部K^+含量,地上部Ca^2+、Mg^2+含量存在品种问差异,或许可作为耐盐性鉴定指标。NaCl胁迫降低坪山柚和福橘幼苗根部及地上部P、Mn含量,而Cu含量在较高浓度NaCl胁迫下显著增加。NaCl胁迫明显降低坪山柚和福橘幼苗地上部K^+/Na^+、Ca^2+/Na^+和Mg^2+/Na^+值,其中K^+/Na^+值的变化可考虑作为柑橘耐盐性鉴定的指标。  相似文献

目的:评估二种心脏停搏液不同灌注方法对心肌保护作用。方法:30例双瓣患者随机分为冷晶体停搏液间断灌注组(n=10),冷血停搏液间断灌注组(n=10),冷血停搏液持续灌注组(n=10),观察血浆心肌肌钙蛋白T(CnT)、肌酸激酶(CK)、肌酸激酶同工酶(CK—MB)。结果:体外循环后冷晶体停搏液间断灌注组血浆心肌肌钙蛋白T和肌酸激酶、肌酶激酶同工酶较其他2组明显增高;冷血停搏液间断灌注组和冷血停搏液持续灌注组血浆心肌肌钙蛋白T、肌酸激酶、肌酸激酶同工酶无明显差异。结论:冷血停搏液的心肌保护优于冷晶体停搏液,冷血停搏液间断灌注与持续灌注没有明显差异。  相似文献

目的:观察降钙素基因相关肽(CGRP)在离体大鼠心肌缺血后处理保护中的作用。方法:采用离体大鼠全心停灌心肌缺血复灌模型。测定心室动力学指标、复灌各时间点冠脉流出液中乳酸脱氢酶(LDH)含量和心肌组织formazan含量的变化。结果:与缺血/复灌组相比,缺血后处理组明显增加心脏formazan含量,降低冠脉流出液中LDH含量,促进左室发展压、左室做功和冠脉流量的恢复。CGRP受体阻断剂CGRP-(8-37)和线粒体ATP敏感性钾通道阻断剂5-HD均减弱了缺血后处理的作用,且CGRP-(8-37)阻断了线粒体ATP敏感性钾通道开放剂Diaz的心肌保护作用。结论:缺血后处理可能通过促进线粒体ATP敏感性钾通道的开放,引起内源性降钙素基因相关肽的释放发挥心肌保护作用。  相似文献

本研究通过对比冷自体血心脏停搏液、康斯特(HTK)液与改良托马斯停搏液对离体兔心室肌ATP、ADP、AMP含量及线粒体损伤程度的影响,探讨冷自体血心脏停博液对心肌的保护作用及机制。选择健康新西兰白兔60只,随机均分为3组,每组20只,建立Langendorff离体心脏灌注模型,分别灌以冷自体血停搏液、HTK液及St.Thomas液,应用高效液相色谱法测定心肌ATP、ADP、AMP含量,透射电镜下观察心肌线粒体损伤程度并评分。停跳前、复灌后3组之间冠状动脉流出液(CF)、心率(HR)及恢复百分比无统计学意义(p0.05);A组、B组心肌含水量明显低于C组(p0.05);A组、B组心肌ATP、ADP含量明显高于C组(p0.01),3组间AMP含量差异无统计学意义(p0.05);A组与B组心肌线粒体损伤程度及Flameng评分均较C组低,差异有统计学意义(p0.05)。冷自体血停搏液可提高离体兔心肌细胞ATP、ADP含量,能减轻缺血-再灌注对离体兔心肌细胞线粒体的损伤。  相似文献

目的总结和分析同期施行冠状动脉搭桥和心脏瓣膜手术的体外循环方法。方法125例患者分为3组:M组(冠脉病变及二尖瓣病变)75例,A组(冠脉病变及主动脉瓣病变)34例,D组(冠脉病变及二尖瓣和主动脉瓣病变)16例。心肌保护采用4:1冷含血停搏液,应用单纯顺灌、顺灌逆灌结合、顺灌桥灌结合、顺逆灌和桥灌结合技术。结果术中转流平稳,血流动力学稳定,监测指标均在正常范围,无手术死亡。结论同期施行冠状动脉搭桥和心脏瓣膜手术,术中良好的心肌保护方法和合理的体外循环灌注是保证手术顺利成功的重要因素。  相似文献

本文拟探讨缺血预处理(ischemic preconditioning,IP)合并低温及晶体停搏液对幼兔的离体心脏是否具有心肌保护作用.采用Langendorff离体心脏灌注模型,灌注液为Krebs-Henseleit液(K-H液).取3~4周龄幼兔心脏,在第一部分实验中分为Con、IP1、IP2、IP3组(n=6),分别给予0、1、2、3次IP,其后各组心脏均在20℃低温下停灌2 h,37℃常温下再灌注30 min.在第二部分实验中分为SConI、SCon2、SCon3、SIPl、SIP2、SIP3组(n=8),其中SIPl、SIP2、SIP3组给予2次IP后灌注St.Tho-mas Ⅱ晶体停搏液(CCS)使心脏停搏,然后分别使心脏在32℃、25℃、20℃下停灌30、90和120min,其后各组均在37℃再灌注30 min.SConl,SCon2,SCon3三组则不给予IP,继续灌注20min后灌注CCS使心脏停搏,然后分别在32℃、25℃、20℃下停灌30、90和120 min,其后各组均在37℃再灌注30 min.以Maclab/4 s生理实验系统记录平衡末、缺血前、再灌注后1、3、5、10、20、30 min时心率(HR)、左心室发展压(LVDP)以及左心室内压上升及下降最大速率(±dp/dtmax),测定再灌注末心肌组织中ATP和丙二醛(MDA)的含量,以及超氧化物歧化酶(SOD)的活性.在20℃低温停灌且停灌期间不给予CCS时,再灌注末IP2组LVDP×HR、+dp/dtmax和-dp/dtmax的恢复率分别为96%±21%、101%±19%和84%±15%,显著高于Con组和IP3组(P<0.01,P<0.05);心肌组织的ATP含量亦高于Con组(P<0.01).在不同低温停灌且停灌期间给予CCS时,再灌注末SIP1、SIP2组的LVDP×HR、+dp/dtmax分别恢复到87%±14%、99%±26%(P<0.05,vs SConl group)和87%±16%、102%±20%(P<0.05,vs SCon2 group);心肌组织的ATP含量均分别显著高于SCon1组和SCon2组(P<0.05),心肌组织MDA含量亦分别低于SCon1组和SCon2组(P<0.05).上述结果提示,IP对在20℃低温停灌的兔未成熟心脏具有一定的心肌保护作用,2次IP的保护效应优于1次或3次IP.在停灌期间应用CCS,IP的心肌保护作用随停灌期间低温程度的升高而减弱.  相似文献

目的:观察线粒体钙单向转运体在心肌低氧/复氧损伤中的作用并探讨其机制。方法:应用Langendorff大鼠心脏灌流模型,低氧/复氧(H/R)采用冠脉前降支结扎30 min、复灌120 min的方法。用生物信号采集系统记录左室发展压(LVDP)、左室压最大上升/下降速率(±dP/dtmax)、左室舒张末压(LVEDP);分光光度法分别检测冠脉流出液中乳酸脱氢酶(LDH)的含量和线粒体活性氧(ROS);TTC染色法检测心肌梗死面积。结果:与单纯低氧/复氧组相比,复氧起始给予线粒体钙单向转运体抑制剂钌红(5μmol/L)明显改善左心室各项功能指标,减小心肌梗死面积,降低线粒体ROS和冠脉流出液中LDH含量;而在复氧期起始给予线粒体钙单向转运体激动剂精胺(20μmol/L),显著升高了线粒体ROS活性,冠脉流出液中LDH含量在复氧5 min、20 min、30 min时显著增多,左心室各项功能指标与心肌梗死面积与单纯低氧/复氧组相比无显著差异。ROS清除剂MPG(1 mmol/L)与精胺联合应用则取消了精胺的作用。结论:抑制线粒体钙单向转运体可能通过减少线粒体ROS的生成减轻心脏低氧/复氧损伤。  相似文献

目的探讨体外循环手术中温血心肌保护液对老年患者心肌的保护作用.资料与方法 30例在我院接受瓣膜置换术的老年患者随机分为研究组和对照组.对照组患者主动脉阻断后,应用冷血心肌保护液灌注心肌.若患者心脏不能自动复跳,应用电击除颤.研究组患者心肌保护液灌注遵循温-冷-温顺序.若心脏复跳不满意,可再次阻断主动脉,灌注温血心肌保护液或应用温血灌注心肌至心肌出现较有规律的心电活动时,开放主动脉阻断钳.结果研究组患者的心肌保护效果优于对照组.结论:此技术的对老年患者的心肌保护效果好,患者术后心功能恢复好.  相似文献

The general anesthetic propofol has been shown to be cardioprotective. However, its benefits when used in cardioplegia during cardiac surgery have not been demonstrated. In this study, we investigated the effects of propofol on metabolic stress, cardiac function, and injury in a clinically relevant model of normothermic cardioplegic arrest and cardiopulmonary bypass. Twenty anesthetized pigs, randomized to propofol treatment (n = 8) and control (n = 12) groups, were surgically prepared for cardiopulmonary bypass (CPB) and cardioplegic arrest. Doses of warm blood cardioplegia were delivered at 15-min intervals during a 60-min aortic cross-clamped period. Propofol was continuously infused for the duration of CPB and was therefore present in blood cardioplegia. Myocardial biopsies were collected before, at the end of cardioplegic arrest, and 20 mins after the release of the aortic cross-clamp. Hemodynamic parameters were monitored and blood samples collected for cardiac troponin I measurements. Propofol infusion during CPB and before ischemia did not alter cardiac function or myocardial metabolism. Propofol treatment attenuated the changes in myocardial tissue levels of adenine nucleotides, lactate, and amino acids during ischemia and reduced cardiac troponin I release on reperfusion. Propofol treatment reduced measurable hemodynamic dysfunction after cardioplegic arrest when compared to untreated controls. In conclusion, propofol protects the heart from ischemia-reperfusion injury in a clinically relevant experimental model. Propofol may therefore be a useful adjunct to cardioplegic solutions as well as being an appropriate anesthetic for cardiac surgery.  相似文献

本文通过两种不同的模型来探讨竹叶提取物对心肌缺血再灌注(缺氧/复氧)损伤的保护作用。在体内模型中不同剂量的竹叶提取物均提高了模型组室内压最大上升和下降速率,降低了左室末期舒张期压力,提高了心输出量。同时降低了模型组肌酸磷酸激酶及乳酸脱氢酶的活性,减少了丙二醛的含量,增加了超氧化物歧化酶的活性,通过电压依赖性钙通道及受体依赖性钙通道降低了钙的含量。说明竹叶提取物对心肌缺血再灌注(缺氧复氧)损伤具有保护作用,其作用是通过电压依赖性钙通道及受体依赖性钙通道降低了钙的含量而对心肌缺血再灌注(缺氧复氧)损伤产生保护作用。  相似文献

Cardioplegic arrest for bypass surgery imposes global ischemia on the myocardium, which generates oxyradicals and depletes myocardial high-energy phosphates. The glycolytic metabolite pyruvate, but not its reduced congener lactate, increases phosphorylation potential and detoxifies oxyradicals in ischemic and postischemic myocardium. This study tested the hypothesis that pyruvate mitigates oxidative stress and preserves the energy state in cardioplegically arrested myocardium. In situ swine hearts were arrested for 60 min with a 4:1 mixture of blood and crystalloid cardioplegia solution containing 188 mM glucose alone (control) or with additional 23.8 mM lactate or 23.8 mM pyruvate and then reperfused for 3 min with cardioplegia-free blood. Glutathione (GSH), glutathione disulfide (GSSG), and energy metabolites [phosphocreatine (PCr), creatine (Cr), P(i)] were measured in myocardium, which was snap frozen at 45 min arrest and 3 min reperfusion to determine antioxidant GSH redox state (GSH/GSSG) and PCr phosphorylation potential {[PCr]/([Cr][P(i)])}. Coronary sinus 8-isoprostane indexed oxidative stress. Pyruvate cardioplegia lowered 8-isoprostane release approximately 40% during arrest versus control and lactate cardioplegia. Lactate and pyruvate cardioplegia dampened (P < 0.05 vs. control) the surge of 8-isoprostane release following reperfusion. Pyruvate doubled GSH/GSSG versus lactate cardioplegia during arrest, but GSH/GSSG fell in all three groups after reperfusion. Myocardial [PCr]/([Cr][P(i)]) was maintained in all three groups during arrest. Pyruvate cardioplegia doubled [PCr]/([Cr][P(i)]) versus control and lactate cardioplegia after reperfusion. Pyruvate cardioplegia mitigates oxidative stress during cardioplegic arrest and enhances myocardial energy state on reperfusion.  相似文献

N-n-butyl haloperidol iodide (F2), a novel compound derived from haloperidol, was synthesized by our drugs research lab. The present study aims to evaluate the protective effects of F2 on myocardial ischemia-reperfusion injury in vivo, and to try to find the protective mechanism of F2. The animal model of myocardial ischemia-reperfusion injury was established by ligaturing rabbit's left ventricular branch of coronary artery for 40 min and removing the ligation later to reperfuse for 40 min. Different doses of F2 were intravenously injected before the onset of ischemia. The changes of hemodynamics were recorded during the experiment, and the activities of superoxide dismutase (SOD), creatine kinase (CK), Ca2+-ATPase, Na+,K+-ATPase and the level of malondialdehyde (MDA) of myocardial tissue were detected after reperfusion. Administration of F2 could dose-dependently ameliorate the hemodynamics of ischemia-reperfusion injured myocardium. During the course of reperfusion, MAP, LVSP, +/-dP/dt(max) in all F2 groups were obviously higher than those in the ischemia-reperfusion control group, and LVEDP were lower. F2 could also reduce the production of MDA, and maintain the activities of SOD, Ca2+-ATPase, Na+,K+-ATPase, and minimize the leakage of CK out of myocardial cells in a dose-dependent manner. These results suggested that F2 had apparent protective effects against myocardial ischemia-reperfusion injury.  相似文献

Myocardial ischemia-reperfusion injury is a medical problem occurring as damage to the myocardium following blood flow restoration after a critical period of coronary occlusion. Oxygen free radicals (OFR) are implicated in reperfusion injury after myocardial ischemia. The antioxidant enzyme, Cu, Zn-superoxide dismutase (Cu, Zn-SOD, also called SOD1) is one of the major means by which cells counteract the deleterious effects of OFR after ischemia. Recently, we reported that a PEP-1-SOD1 fusion protein was efficiently delivered into cultured cells and isolated rat hearts with ischemia-reperfusion injury. In the present study, we investigated the protective effects of the PEP-1-SOD1 fusion protein after ischemic insult. Immunofluorescecnce analysis revealed that the expressed and purified PEP-1-SOD1 fusion protein injected into rat tail veins was efficiently transduced into the myocardium with its native protein structure intact. When injected into Sprague-Dawley rat tail veins, the PEP-1- SOD1 fusion protein significantly attenuated myocardial ischemia-reperfusion damage; characterized by improving cardiac function of the left ventricle, decreasing infarct size, reducing the level of malondialdehyde (MDA), decreasing the release of creatine kinase (CK) and lactate dehydrogenase (LDH), and relieving cardiomyocyte apoptosis. These results suggest that the biologically active intact forms of PEP-1-SOD1 fusion protein will provide an efficient strategy for therapeutic delivery in various diseases related to SOD1 or to OFR.  相似文献

The paper studies electrical activity of the heart during the anti-ischemic protection by cool high-potassium cardioplegic solution based on the blood. Parametres characterising automatism function and the myocardial state of the ventricles practically did not differ from the control data obtained during the experiments without ischemia. Atrioventricular conduction and the atrium myocardium were worse preserved. The positive effects of the papaverine addition to the cardioplegic perfusate has been also shown. The author came to the conclusion that it is necessary to pay special attention to the electrical activity of the supraventricular area of the heart for the adequate evaluation of the cardioplegia.  相似文献

Previous studies have shown improvement of preservation with cardioplegia by calcitonin gene-related peptide (CGRP)-induced preconditioning. Therefore we examined the hypothesis that endogenous CGRP may be involved in the protection of heat stress against myocardial damages after prolonged cardioplegic arrest in isolated rat heart. Reperfusion after 4 h of hypothermic ischemia caused a decline of cardiac function and an increase of creatine kinase (CK) release. Heat stress induced by pretreatment with whole body hyperthermia (rectal 42 degrees C) for 15 min produced a significant increase in the plasma content of CGRP, an improvement of cardiac function and a decrease in the release of CK. However, after pretreatment with capsaicin (50 mg/kg, s.c.) to deplete CGRP in cardiac sensory nerves, the plasma concentration of CGRP was no longer increased and the cardioprotection afforded by heat stress was abolished. These findings suggest that improvement of preservation with cardioplegia by heat stress may be mediated by endogenous CGRP in the rat.  相似文献

The aims of the present study were to establish if myocardial ischemia/reperfusion is associated with altered eNOS activity and if myocardial eNOS detection depends on its activity. We determined detectable eNOS in (1) myocardium of isolated perfused rat hearts subjected to either global or regional ischemia and (2) in left ventricular biopsies from patients undergoing two different methods of myocardial protection (i.e., intermittent cold blood cardioplegia and continuous coronary perfusion with warm, beta-blocker-enriched blood) during coronary artery surgery. NOS detection was performed by NADPH-d staining and three eNOS-antibodies against different eNOS epitopes. In addition, activity dependent alteration of detectable eNOS was proofed by bradykinin treatment for 2 to 10 min. Ischemic and receptor mediated eNOS activation increased NADPH-d reactivity and eNOS immunoreaction as measured by antibodies against either amino acids of a central bovine eNOS domain or the human eNOS N-terminal end. In contrast, the antibody against the human eNOS C-terminal end exhibited no alteration of eNOS immunoreaction. The transient eNOS activation was associated with increased cGMP content. In human myocardium subjected to ischemia during cardiac surgery we found that early reperfusion increases eNOS activity. These data demonstrate a strong association between myocardial ischemia/reperfusion and increased eNOS activity as measured by immunocytochemical staining against specific eNOS epitopes. It appears that eNOS activation and subsequent NO release may act as a regulatory system to counter balance the potentially deleterious effects of myocardial ischemia/reperfusion.  相似文献

为研究桑叶总黄酮预处理对缺血再灌注损伤心肌的抗氧化作用,采用结扎左冠状动脉前降支30min,再灌注2h的方法制备大鼠心肌缺血再灌注损伤模型。将50只大鼠随机分为假手术组、缺血再灌注损伤模型组和桑叶总黄酮高、中、低剂量预处理组,每组10只。实验结束后,取动脉血和心脏。测定各组血清生化指标肌酸激酶(CK)和乳酸脱氢酶(LDH)的含量;测定心肌生化指标超氧化物歧化酶(SOD)的活性和丙二醛(MDA)的含量。结果显示,与模型组相比,桑叶总黄酮预处理组使血清中的CK、LDH含量明显降低,同时使心肌组织中的SOD活性提高,MDA含量降低。结果表明,桑叶总黄酮预处理对缺血再灌注损伤心肌有明显的保护作用,其机制可能与提高心肌SOD活性、清除自由基、增强抗氧化能力有关。  相似文献
