This paper deals with the environmental changes during two important transitional periods, namely Lower/Middle Paleolithic and Middle/Upper Paleolithic transitions, and with their impact on biological and cultural evolutions, in the context of central, eastern and northwestern Europe, where ice-sheet transgressions induced particular paleogeographical and paleoecological conditions. Introduction

Undescended testis is a frequent congenital disease, more often diagnosed and treated during childhood. However, due to ignorance or negligence, this disease can be seen even after puberty, when it raises a therapeutic problem: is orchidopexy still useful? This study was designed to evaluate the outcome of orchidopexy at adulthood in terms of improvement of fertility and prevention of malignant degeneration.

Material and methods

Retrospective study performed over a 23-year period (1983–2005). We have found 259 patients with undescended testis diagnosed and treated after the age of 18 years.

Results

The mean age of patients was 24 years (range: 18–63). In the majority of cases, undescended testis was diagnosed at a systemic medical examination in 199 patients (77% of cases), and in a context of infertility in 33 patients, testicular malignancy in 8 patients, testicular torsion in 2 patients and, in 17 cases, undescended testis had been known since birth but was neglected by the parents. Undescended testis was unilateral in 209 cases and bilateral in 50 cases. Out of 37 couples, only 4 gave birth to children (10.8% paternity rate). Sperm analysis was abnormal in all infertile patients. All patients were treated by orchidopexy, except for 36 patients in whom orchidectomy was performed due to testicular atrophy (27 cases), malignancy (8 cases) or necrosis (1 case). Biopsy of the intrascrotal testis was performed in 3 patients with unilateral cryptorchidism. Histological examination was normal in two cases and abnormal in one case. The long-term outcome was characterized by:
  1. Testicular atrophy in 6 patients (2.7% of cases).
  2. Progression to malignancy in 3 patients (1.3% of cases).
  3. Improvement of sperm parameters in 16 of 33 infertile patients (48.5%); 4 patients fathered children after treatment.

Conclusion

Orchidopexy at adulthood can lead to improvement of infertility. It can also decrease the incidence of malignancy and facilitate clinical examination looking for possible malignancy. However, the best treatment remains preventive, based on early diagnosis and orchidopexy.  相似文献

Real-time Polymerase Chain Reaction (PCR) for the study of ancient DNA. The properties of ancient DNA (aDNA) make difficult the retrieval of DNA sequence. The advantage of Real-Time PCR was exploited, for the first time, in the study of aDNA. We determined the optimal condition to amplify, in one round of PCR, aDNA, which should be directly sequenced. Beside the verification of aDNA authenticity, we compared two cleaning bone methods: scalpel and ethanol. The ethanol specimens showed the best DNA yield. The aDNA was extracted and amplified (mitochondrial hypervariable region I) from five skeletons exhumed from the archaeological site of Notre-Dame-du-Bourg (France), dated from 3rd to 17th century. Current evidence indicates that endocrine disrupters (EDs) can induce adverse effects on the male reproductive tract in various mammalian species. Recent reports indicate deterioration in male reproductive health in several human populations, but the evidence for a causal link with endocrine disruption is still weak. In addition, the experimental conditions of most of the reportedin vivo studies are not representative of environmental exposures (for example, high doses, short-term exposure, a single ED) and the mechanisms by which EDs disrupt the reproductive system are poorly understood. The objective of the present study is to develop an animal model to assess the reproductive effects and study the putative cellular and molecular mechanisms involved after exposure to genistein (phytoestrogen) and vinclozolin (fungicide with a known antiandrogenic potential) alone or in combination. The study will be performed in male Wistar rats, with administration of low and high doses of the compounds from conception to adulthood and a subset of the males in each treatment group will be mated with unexposed females. We plan to assess the level of sperm production, histology of the reproductive organs, motility and morphometry of spermatozoa and hormone levels, as well as DNA fragmentation of spermatozoa and determination of the number of germ cells, Sertoli cells and the diameters of seminiferous tubules. Estrogen, androgen, progesterone and FSH receptors will be detected and quantified and the level of testicular apoptosis and several apoptosis pathways will be studied to determine the putative cellular and molecular mechanisms involved. The preliminary results confirmed the developmental effects previously reported for high doses of vinclozolin. More interestingly, they indicated a number of deleterious effects for male rats exposed to low dosages alone or mixtures of low and high dosages compared to controls and rats exposed to high dosages alone. For example, a number of developmental anomalies of the genitalia were observed and a significant decrease of sperm motility and motion and fertilizing ability were observed. These preliminary results provide evidence that chronic exposure to environmental levels of EDs or mixtures of EDs have a detrimental impact on the male reproductive tract. The next step involves assessment of the anatomical disorders and the study of some of the cellular and molecular mechanisms possibly involved.  相似文献

Le protoplasme de l'uf fraîchement pondu remplit la membrane vitelline; bientôt, il se produit une contraction de volume, et il se forme un ellipsoïde. Le protoplasme continue à sa contraction pendant quelque temps, puis il se produit à une augmentation de volume. La projection des cellules polaires se fait immédiatement après cette augmentation. Celle-ci est élevée après la formation des cellules polaires et le volume atteint alors un maximum. Pendant cette période, la formation du blastoderme s'accomplit. Ensuite, pendant la période de la formation de la gouttière ventrale, le volume de l'embryon se réduit à nouveau; mais cela la contraction se limite à la partie céphalique seulement. Dans l'embryon létal nullo X, la contraction et l'augmentation de volume se font comme dans l'embryon sain, bien que les noyaux n'exécutent pas leur migration centrifuge. European and US drug regulatory authorities have recently warned against the use of selective serotonin reuptake inhibitors (SSRIs) in childhood and adolescent depression. Through an extensive literature review on the use of SSRIs to treat depression in children and adolescents — including psychiatric clinical trials, pharmacology, and drug safety data — we will: i) describe the main arguments justifying various regulatory decisions; ii) present published data and subsequent meta-analyses; iii) discuss how to integrate the recommendations into daily clinical practice, freeing professionals from a Manichean approach. Chlamydia trachomatis is responsible for the most common sexually transmitted disease, up to 40% of the non gonococcal urethritis. It is a compulsary intracellular parasite. Its peculiar metabolism makes the bacteria interfere with the host cell immunity. Infection can become chronic and induce inflammation, if not properly treated, by permanent emission of antigens and heat shock proteins, and stimulation of Tcells. Autoimmunization against sperm can occur and impair fertilization and embryo implantation. Thus, Chlamydial infection can be associated with male infertility because of tissue damage, altered movement and antisperm immunization. PCR and LCR are to date the most sensitive detection techniques for c.trachomatis. Our study aim was to determine the prevalence of Chlamydial infection by PCR in 6 groups of men in andrologic practice. The highest prevalences have been found in the infertile men group and in the symptomatic group. No Chlamydia culture was found positive in any group. The association of PCR with anti C. trachomatis seminal IgA and seric IgG are useful tools for andrologic exploration.  相似文献

The identification of ischemia-related coronary artery by stress myocardial perfusion SPECT (SPECT-MPS) is difficult, but it is likely that this identification could be easier by fusionning the SPECT images with images from coronary CT angiography (CTA).ObjectiveTo assess the usefulness of the fusion of SPECT-MPS images with CTA images in the identification of ischemia-related coronary arteries.MethodSPECT-MPS and CTA, which had been recorded in a delay ≤ 3 months, were reanalyzed blindly to identify the coronary arterial vessels with (1) stress ischemia at SPECT-MPS and (2) significant coronary stenosis (> 70 %) at CTA. In case of discordance between the two techniques, an analysis was performed while fusioning the SPECT-MPS and CTA images.ResultsThe population included 38 patients, of whom 22 (58 %) had an ischemia at SPECT-MPS. The identification of the ischemia-related vessels was initially discordant between SPECT-MPS and CTA for 17 patients (45 %). After the fusion's analysis, nine out of these 17 patients (53 %) became totally concordant because of modifications in the CTA analysis in six patients and in the SPECT-MPS analysis in only three. A low severity of ischemia was the main correlate of persisting discordances after fusion: 58 % (7/12) of such low severity ischemia were observed in coronary vessels with persisting discordance, but only 24 % (5/27) with concordant coronary vessels (p = 0.02).ConclusionThe identification with SPECT-MPS of the ischemia-related coronary artery is rarely modified by a fusion analysis with CTA images. Discordances between the two techniques remain frequent, even after fusion, but especially in case of low severity SPECT-MPS ischemias.  相似文献
