摘要 目的:探讨耻骨后膀胱尿道悬吊术(Burch)、阴道无张力尿道悬吊术(TVT)及经闭孔经阴道尿道中段悬吊带术(TVT-O)三种不同手术方式治疗中老年女性压力性尿失禁(SUI)疗效及对患者膀胱功能和术后并发症的影响。方法:回顾性分析2019.1-2022.4收治的101例中老年女性SUI患者资料,按手术方式分为Burch组(n=30,Burch术治疗)、TVT组(n=31,TVT术治疗)和TVT-O组(n=40,TVT-O术治疗),观察三组患者临床疗效和手术情况[手术时间、出血量、住院时间、尿管留置时间],并发症发生率,治疗前后膀胱功能[24h排尿次数、膀胱容量、每次排尿量、残余尿量]及尿道功能指标[尿道长度(FUL)、最大尿道闭合压(MUCP)、Valsalva漏尿点压(VLPP)]变化。结果:Burch组、TVT组、TVT-O组治愈及改善率分别为83.34%、87.10%、87.50%,13.33%、12.90%、12.50%,三组之间比较差异无统计学意义(P>0.05);TVT组、TVT-O组患者手术时间、出血量、住院时间、尿管留置时间均显著短于Burch组(P<0.05),且TVT-O组患者手术时间显著短于TVT组(P<0.05);治疗后,三组患者24 h排尿次数、残余尿量均显著降低(P<0.05),膀胱容量、每次排尿量、FUL、MUCP、VLPP水平均显著增加(P<0.05),但三组之间比较差异无统计学意义(P>0.05);Burch组、TVT组、TVT-O组并发症总发生率分别为20.00%、12.91%、15.00%,三组之间比较差异无统计学意义(P>0.05)。结论:三种术式治疗中老年女性SUI疗效相当,均可有效改善膀胱功能及尿道指标,但TVT与TVT-O术患者康复快,TVT-O手术时间最短,TVT并发症低,可依据患者情况酌情选择。  相似文献

Mixed urinary incontinence (MUI) is a common clinical problem in the community and hospital setting. The broad definition of the term makes it difficult to diagnose, as well as determine effective treatment strategies. There are no current guidelines recommended for physicians. The estimated prevalence of this condition is approximately 30% in all women with incontinence. It has also been suggested that patients with MUI report more bothersome symptoms than either stress or urge incontinence; approximately 32% of 40- to 64-year-olds with MUI report symptoms of depression. The authors examine the diagnosis, evaluation, and treatment of patients with MUI.Key words: Mixed urinary incontinence, Detrusor overactivity, Stress incontinence, Urge incontinence, Urodynamic stress incontinence, Pelvic organ prolapse, Transvaginal tapeMixed urinary incontinence (MUI) is the leading cause of incontinence in the community and hospital setting.1 The term refers to a combination of symptoms, with the patient exhibiting features of both stress urinary incontinence (SUI) and urge urinary incontinence (UUI); it may also refer to a combination of features of urodynamic SUI and detrusor hyperactivity.1 The current International Continence Society guidelines define MUI as a complaint of the involuntary loss of urine during exertion, sneezing, or coughing, as well as leakage associated with urgency.2The term MUI is extremely broad because it may refer to equal stress and urge symptoms, stress-predominant symptoms, urge-predominant symptoms, urodynamic SUI (USUI) with detrusor overactivity (DO), or USUI with clinical urge symptoms but no DO.3 The challenge of this broad definition is that it leads to inconsistencies when evaluating treatment options and outcomes. In an attempt to validate diagnostic questions that could later be used in an epidemiological survey, Sandvik and colleagues4 defined MUI based on subjective answers to a structured questionnaire designed for their study.4 SUI was presumed if a positive answer was given to the question: “Do you lose urine during sudden physical exertion, lifting, coughing, or sneezing?” If the patient responded positively to the question: “Do you experience such a strong and sudden urge to void that you leak before reaching the toilet?” then a diagnosis of UUI was presumed. MUI was considered if a positive answer was given to both questions. In contrast, Brubaker and colleagues5 reported that strict definitions based on self-reported symptoms do not properly categorize patients as having MUI. Their group believed that patients should be broken down into MUI subgroups of SUI and UUI rather than describing it as a single entity. However, without a precise definition or understanding of the role of these stress and urge subcomponents, the assessment of an intervention for SUI or UUI is challenging.6The prevalence rates of MUI vary widely in the literature. In a secondary analysis of the Stress Incontinence Surgical Treatment Efficacy Trial (SISTEr), Brubaker and colleagues5 evaluated 655 women for the presence of incontinence and their response to treatment. They found that 50% to 93% of women fell into the category of MUI based on patient-reported answers to the Medical Epidemiologic and Social Aspects of Aging (MESA) and Urinary Distress Inventory (UDI) questionnaires. However, when objective criteria such as urodynamic findings were used, only 8% of women were categorized with MUI. Dooley and associates7 compared physical examination findings and responses to the MESA and UDI questionnaires in 551 women with a mean age of 56 ± 16 years. They estimated a prevalence rate of 30% of MUI in all women with urinary incontinence.According to Dooley and associates,7 in their cohort, MUI was more bothersome to patients than either pure SUI or UUI. In a cross-sectional population-based study across 6 European countries that included over 300 patients, the effects of overactive bladder (OAB) symptoms on employment, social interactions, and emotional well-being were evaluated by direct interview or a telephone-conducted interview. Irwin and associates8 found 32% of patients aged 40 to 64 years reported being depressed. In addition, they determined that symptoms of OAB have a statistically significant negative impact on emotional well-being both at home and at work.We sought to examine the existing literature on MUI and better understand the role urodynamic testing (UDS) plays in its diagnosis. Background

The prevalence of obesity with aging is escalating alarmingly; and pelvic organ prolapse (POP) and stress urinary incontinence (SUI) are now becoming a growing epidemic among the elderly. Synthetic transvaginal mesh has been employed with increasing popularity in the treatment of POP and is usually highly effective in controlling the principal symptoms of prolapse. However, studies have reported that mesh operations provide fairly unfavorable SUI cure rates. Therefore, additional anti-incontinence surgical strategies are increasingly being scrutinized to achieve better postoperative continence without any significant side-effects for patients with both POP and SUI. We hypothesize that the modification with the fixing of the mesh to the mid-urethra is superior to the original transvaginal mesh operation (TVM) with regard to anti-incontinence.

Methods

One hundred and thirty patients diagnosed with POP–Q II–III and concomitant SUI requiring surgical treatment will be included in this prospective, randomized, double-blind, controlled clinical trial. Patients will be randomly allocated to receive either original TVM (TVM group, n?=?65) or modified TVM surgery (mTVM group, n?=?65). As the primary outcome parameter, we will evaluate the objective SUI and POP cure rates. Secondary endpoints include postoperative morbidity as assessed with the International Urogynaecological Association classification and subjective prolapse and incontinence cure rates reported by questionnaires.

Discussion

Recognizing the importance of an additional surgical procedure for anti-incontinence management, we aim to investigate whether a stabilizing suturing of the mesh to the mid-urethra delivers superior SUI correction compared to the original prosthesis surgery.

Stress urinary incontinence (SUI) is the most common form of urinary incontinence in women and is associated with high financial, social, and emotional costs. The history and physical examination can identify most patients with a significant stress incontinence component without the need for urodynamic testing. A variety of pharmacologic agents have been used off-label, but an evidence-based pharmacologic treatment has not been readily available. The development of a selective serotonin and norepinephrine reuptake inhibitor will add a potentially useful drug to the primary care physician's practice for treating female patients with SUI. In August 2004, a selective serotonin and norepinephrine reuptake inhibitor, duloxetine, became the first medication approved for the treatment of women with moderate to severe SUI throughout the European Union. As of November 2005, however, duloxetine has not been approved for the treatment of SUI in the United States.  相似文献

Mixed urinary incontinence is estimated to affect 30% of all women who have urinary incontinence, and it has been shown to be more bothersome to women than pure stress incontinence. Given the degree of bother, many women will undergo surgical correction for incontinence. Patients have high expectations about the success of these interventions. Degree of bother is higher among women with MUI compared with those who have pure stress urinary incontinence (SUI).1 MUI can be a very challenging and costly condition to treat.2,3 Patients with MUI are often offered conservative therapy such as physical therapy, weight-loss strategies, and behavioral modification. Some patients also benefit from treatments aimed directly at urgency, frequency, and urgency incontinence (overactive bladder), which currently include pharmacologic therapy (antimuscarinic or β-3 agonists), chemodenervation (botulinum toxin), or neuromodulation (sacral or posterior tibial nerves).4 However, many patients with MUI progress to surgical therapies for treatment of SUI. We describe a new transvaginal technique for cystocoele repair. We prospectively evaluated patients with moderate and high-grade cystocoele who underwent repair with the new transvaginal repair between 2000 and June 2009. Preoperative evaluation included history and physical examination using the Pelvic Organ Prolapse Quantification, urine culture, residual urine measurement, urodinamycs and cystoscopy. We performed the repair in 76 patients with a mean age of 65.24 years (range, 36 to 84 years), wit anatomical cure in 72 (95%) patients. Four (5%) patients had recurrent cystocoele, 3 (4%) patients claimed residual sensory urgency and 4 (5%) stress urinary incontinence (SUI) after the operation. The operation is safe, simple, and provides good anatomic results with minimal complications.  相似文献

A biomechanical model of the female pelvic support system was developed to explore the contribution of pelvic floor muscle defect to the development of stress urinary incontinence (SUI). From a pool of 135 patients, clinical data of 26 patients with pelvic muscular defect were used in modelling. The model was employed to estimate the parameters that describe the stiffness properties of the vaginal wall and ligament tissues for individual patients. The parameters were then implemented into the model to evaluate for each patient the impact of pelvic muscular defect on the vaginal apex support and the bladder neck support, a factor that relates to the onset of SUI. For the modelling analysis, the compromise of pelvic muscular support was demonstrated to contribute to vaginal apex prolapse and bladder neck prolapse, a condition commonly seen in SUI patients, while simulated conditions of restored muscular support were shown to help re-establish both vaginal apex and bladder neck supports. The findings illustrate the significance of pelvic muscle strength to vaginal support and urinary continence; therefore, the clinical recommendation of pelvic muscle strengthening, such as Kegel exercises, has been shown to be an effective treatment for patients with SUI symptoms.  相似文献

Stress urinary incontinence (SUI) has an observed prevalence of between 4% and 35%. Whereas the clinical definition of SUI has been established by the International Continence Society, the epidemiologic definition has not been established, leading to a broad disparity in reported prevalence rates. Numerous risk factors for SUI have been identified. Aging, obesity, and smoking appear to have consistent causal relationships with the condition, whereas the roles of pregnancy and childbirth remain controversial. The prevalence of many of these risk factors is increasing in the adult female population of the United States. These population changes, combined with increasing physician awareness and the availability of nonsurgical therapy, will likely increase the number of women receiving care for SUI over the next 3 decades.  相似文献

摘要 目的:探讨电针阴部神经刺激疗法联合Kegel盆底康复训练对产后压力性尿失禁(SUI)患者盆底肌力、尿流动力学和生活质量的影响。方法:选取2019年6月~2021年11月期间于我院就诊的产后SUI患者109例,按照入院就诊奇偶顺序分为两组,其中对照组54例,接受Kegel盆底康复训练,研究组55例,接受电针阴部神经刺激疗法联合Kegel盆底康复训练。对比两组疗效、漏尿量、尿失禁程度、盆底肌力、尿流动力学和生活质量。结果:研究组的临床总有效率高于对照组(P<0.05)。两组治疗后盆底肌肌力各指标(手测肌力和Ⅰ类肌纤维最大值、Ⅱ类肌纤维平均值)均升高,且研究组高于对照组(P<0.05)。两组治疗后漏尿量、尿失禁程度评分均下降,且研究组低于对照组(P<0.05)。两组治疗后尿流动力学相关指标[腹压漏尿点压(AL-PP)、最大尿流率(Qmax)和最大尿道闭合压力(MUCP)]均升高,且研究组高于对照组(P<0.05)。两组治疗后尿失禁生活质量量表(I-QOL)各维度(限制性行为、心理影响、社交活动受限)评分及总分均升高,且研究组高于对照组(P<0.05)。结论:电针阴部神经刺激疗法联合Kegel盆底康复训练可有效改善产后SUI患者的盆底肌肌力和尿失禁情况,减少漏尿量,同时可促进尿流动力学恢复,进而提高患者的生活质量。  相似文献

Approximately one-third of patients with stress urinary incontinence (SUI) also suffer from urgency incontinence, which is one of the major symptoms of overactive bladder (OAB) syndrome. Pudendal nerve injury has been recognized as a possible cause for both SUI and OAB. Therefore, we investigated the effects of pudendal nerve ligation (PNL) on bladder function and urinary continence in female Sprague-Dawley rats. Conscious cystometry with or without capsaicin pretreatment (125 mg/kg sc), leak point pressures (LPPs), contractile responses of bladder muscle strips to carbachol or phenylephrine, and levels of nerve growth factor (NGF) protein and mRNA in the bladder were compared in sham and PNL rats 4 wk after the injury. Urinary frequency detected by a reduction in intercontraction intervals and voided volume was observed in PNL rats compared with sham rats, but it was not seen in PNL rats with capsaicin pretreatment that desensitizes C-fiber-afferent pathways. LPPs in PNL rats were significantly decreased compared with sham rats. The contractile responses of detrusor muscle strips to phenylephrine, but not to carbachol, were significantly increased in PNL rats. The levels of NGF protein and mRNA in the bladder of PNL rats were significantly increased compared with sham rats. These results suggest that pudendal nerve neuropathy induced by PNL may be one of the potential risk factors for OAB, as well as SUI. Somato-visceral cross sensitization between somatic (pudendal) and visceral (bladder) sensory pathways that increases NGF expression and alpha(1)-adrenoceptor-mediated contractility in the bladder may be involved in this pathophysiological mechanism.  相似文献

目的:探讨经阴道放置网片的全盆底重建术治疗重度子宫脱垂的临床疗效和安全性。方法:选择2010年7月至2015年6月在青岛大学附属医院因重度盆腔器官脱垂接受全盆底重建术的患者126例,回顾性分析患者的临床资料、生活质量评分及并发症等相关资料。结果:126例患者平均手术时间(67.18±18.00)min,出血量(56.27±26.47)m L,留置尿管(3.47±0.94)天,住院天数(4.21±0.93)天。术后尿潴留的发生率为3.96%、阴道壁血肿0.79%、术后肢体疼痛7.14%、下肢静脉血栓0.79%,所有患者术中均无膀胱或直肠损伤等严重并发症的发生。126例患者中,111例完成随访(88.09%,111/126),随访时间为6~48个月,中位随访时间为27个月。术后发生网片暴露1例(0.9%,1/111),网片挛缩1例(0.9%,1/111),慢性盆腔疼痛2例(1.80%,2/111)、新发尿失禁6例(5.4%,6/111)。其中3例为急迫性尿失禁(2.7%,3/111),3例压力性尿失禁(2.7%,3/111)。盆底重建术后共有3例患者出现复发,其中2例因症状明显再次行手术治疗,客观治愈率的为97.29%(108/111),主观治愈率为98.19%(109/111)。与术前相比,术后6个月、12个月生活质量评分(PFDI-20)均较前均明显降低(P0.05)。术后21例患者恢复性生活,性生活疼痛者3例(14.28%),性生活不适者4例(19.04%),总体性生活满意度为85.71%(18/21)。结论:全盆重建术治疗重度盆腔器官脱垂的疗效较好,且网片相关并发症的发生率较低。  相似文献

Objectives

To revisit the diagnostic roles of cystoscopic hydrodistention and the potassium sensitivity test (PST) for the diagnosis of interstitial cystitis (IC).

Methods

We prospectively enrolled 214 patients clinically diagnosed with IC, 125 non-IC patients who underwent video urodynamic studies and PST, and another 144 non-IC patients who underwent cystoscopic hydrodistention before transurethral surgery. The sensitivity, specificity, and positive and negative predictive values were calculated for the PST and glomerulations after cystoscopic hydrodistention.

Results

After cystoscopic hydrodistention, glomerulations developed in 211/214 (98.6%) IC patients and 61/144 (42.4%) of the non-IC patients including patients with stones (45/67, 67%), hematuria (2/5, 40%), and stress urinary incontinence (SUI) (6/17, 35%). When positive glomerulation was defined as grade 2 or more, the sensitivity was 61.7%. The PST was positive in 183/214 (85.5%) IC patients and 7/17 (41%) with hypersensitive bladder, 7/32 (22%) with detrusor overactivity, 5/27 (18%) with SUI, 2/21 (10%) with lower urinary tract symptoms, and 2/25 (8%) with bladder outlet obstruction. The PST had a sensitivity of 85.5% and a specificity of 81.6% for diagnosis of IC. IC patients with a positive PST had a significantly smaller urgency sensation capacity, smaller voided volume, and greater bladder pain score.

Conclusions

Both the PST and glomerulations after hydrodistention are sensitive indicators of IC, but the specificity of glomerulations in the diagnosis of IC is lower than that of the PST. A positive PST is associated with a more hypersensitive bladder and bladder pain, but not the grade of glomerulations in IC patients. Neither test provided 100% diagnostic accuracy for IC, we might select patients into different subgroups based on different PST and hydrodistention results, not for making a diagnosis of IC but for guidance of different treatments.  相似文献

Stress urinary incontinence (SUI), as an isolated symptom, is not a life threatening condition. However, the fear of unexpected urine leakage contributes to a significant decline in quality of life parameters for afflicted patients. Compared to other forms of incontinence, SUI cannot be easily treated with pharmacotherapy since it is inherently an anatomic problem. Treatment options include the use of bio-injectable materials to enhance closing pressures, and the placement of slings to bolster fascial support to the urethra. However, histologic findings of degeneration in the incontinent urethral sphincter invite the use of tissues engineering strategies to regenerate structures that aid in promoting continence. In this review, we will assess the role of stem cells in restoring multiple anatomic and physiological aspects of the sphincter. In particular, mesenchymal stem cells and CD34+ cells have shown great promise to differentiate into muscular and vascular components, respectively. Evidence supporting the use of cytokines and growth factors such as hypoxia-inducible factor 1-alpha, vascular endothelial growth factor, basic fibroblast growth factor, hepatocyte growth factor and insulin-like growth factor further enhance the viability and direction of differentiation. Bridging the benefits of stem cells and growth factors involves the use of synthetic scaffolds like poly (1,8-octanediol-co-citrate) (POC) thin films. POC scaffolds are synthetic, elastomeric polymers that serve as substrates for cell growth, and upon degradation, release growth factors to the microenvironment in a controlled, predictable fashion. The combination of cellular, cytokine and scaffold elements aims to address the pathologic deficits to urinary incontinence, with a goal to improve patient symptoms and overall quality of life.  相似文献

To determine the prevalence of urinary incontinence and other urinary symptoms a questionnaire was sent to all women aged 25 and over and to women under 21 taking oral contraceptives registered with a rural practice (n=937); the questionnaire was completed by 833 women (89%). The overall prevalence of urinary incontinence was 41% (343/833); rates were lower in nulliparous and postmenopausal women (30/181 (17%) and 120/344 (35%) respectively) than parous and premenopausal women (313/652 (48%) and 225/479 (47%) respectively). Incontinence was significantly associated with perineal suturing after childbirth, being present in 201 of 376 (53%) women with sutures compared with 113 of 270 (42%) without. Of the 166 women with a history of minor gynaecological surgery, 100 had symptoms of incontinence, compared with 263 of the 657 (37%) without such a history. Incontinence was not related to type of delivery, and postnatal exercises for the pelvic floor were not beneficial.Inappropriate leakage of urine is perceived by many women as common and therefore not serious; thus it is often not reported to the doctor. Nevertheless, the 6% of women who always require protection against leakage could be helped by treatment.  相似文献

Effective oral therapy for genuine stress urinary incontinence (SUI) in women has, to date, been an unattainable goal. Although oral pharmacologic agents have been used for this condition, none has ultimately been successful, because of side effects, lack of efficacy, or problematic compliance with drug ingestion. The availability of an effective oral agent for SUI would increase the range of therapeutic options for symptom management and possibly make treatment accessible to more women who otherwise feel that surgical therapy is not an option because of social, personal, or medical reasons. Duloxetine is a selective serotonin (5-HT) and norepinephrine reuptake inhibitor that has been shown to increase rhabdosphincter activity. Rhabdosphincter contractility changes are thought to occur as the result of increased stimulation of alpha(1)-adrenergic and 5-HT(2) receptors in the sacral spinal cord, resulting in increased efferent pudendal nerve activity, producing increased pelvic floor tonus. Two large-scale studies have been completed employing subjective and objective outcomes to assess the therapeutic index of duloxetine as a therapy for SUI.  相似文献

OBJECTIVES--To determine the symptomatic and urodynamic outcome of elective prostatectomy and to establish whether the outcome is influenced or can be predicted by preoperative urodynamic measurements. DESIGN--Prospective non-randomised study with follow up at a mean of 11 months after operation. Most men were assessed jointly by a urologist and a general practitioner. SETTING--Department of urology in a teaching hospital serving a large district population. PATIENTS--253 Men listed for elective prostatectomy because of symptoms and low urinary flow rates (less than 15 ml/s) and excluding those already on a waiting list or with acute urinary retention, clinically apparent prostatic cancer, and neurological or cerebrovascular disease; 217 (86%) were followed up. INTERVENTION--Elective prostatectomy. MAIN OUTCOME MEASURE--Classification on the basis of relief of symptoms assessed by patients and urologist and general practitioner and of symptom scores obtained by questionnaire. RESULTS--Of the 217 men followed up, 171 (79%) had a satisfactory subjective review and 155 (72%) had a satisfactory review and also low symptom scores. An unsatisfactory outcome was associated with preoperative symptoms of urge incontinence, small prostatic size and resected weight, low voiding pressures, and low urethral resistance. Preoperative maximum urinary flow rates did not predict outcome. Men with poor outcome could be classified into two groups: those with irritative symptoms who were more likely before operation to have had urge incontinence and detrusor instability and men with symptoms of poor urinary flow who were more likely before operation to have had a small prostate, low voiding pressures, and low urethral resistance. In patients in the second group flow rates or voiding pressures improved little after operation. Men with stable detrusors and either low urethral resistance or low voiding pressures were less likely to do well after prostatectomy, but despite these associations preoperative urodynamic measurements were unable to predict outcome accurately. CONCLUSIONS--Prostatectomy was satisfactory in relieving symptoms and improving urodynamic measurements in most men, but even in those with classic symptoms and low urinary flow rates a substantial minority experienced little improvement afterwards and urodynamic measurements did not accurately predict outcome in individual patients.  相似文献

Existing animal models of stress urinary incontinence (SUI) are limited because of the low rate of incontinence seen in the animals and to their relatively low durability. In addition, most methods described to measure incontinence are operator-dependent. The aim of this study was to develop a new durable animal model of SUI and establish objective measures to quantify SUI. We subjected female rats to transabdominal urethrolysis. At baseline and at 1, 4, 8, 12, and 24 wk after intervention, animals underwent cystometry and evaluation with abdominal leak point pressure (ALPP). Urethral resistance was evaluated by retrograde urethral perfusion pressure (RUPP). Tissues were obtained for histology and immunohistochemistry. Normal female rats had an average ALPP of 19.4 cmH2O and RUPP of 22.6 cmH2O at baseline. More than 93% of the animals had significantly decreased ALPP and RUPP after the procedure. The mean ALPP and RUPP decreased to 9.8 cmH2O and 11.2 cmH2O, respectively, by 1 wk after urethrolysis. These changes were maintained for up to 24 wk. Changes seen in urethral resistance and ALPP appear to be mediated by apoptosis, decreased neuronal mass, and smooth muscle atrophy. These results indicate that transabdominal urethrolysis is a reliable method of achieving durable decreased urethral resistance in a SUI model. RUPP and ALPP are objective and reproducible methods of assessing urethral resistance. Changes in continence and urethral resistance appear to be mediated by denervation and smooth muscle atrophy, which are seen in both elderly incontinent patients and in patients with intrinsic sphincter dysfunction.  相似文献

Background

Stress urinary incontinence (SUI) is a relatively common disorder that significantly affects the quality of life. Many conservative and surgical treatment methods have been recommended for SUI, but they have major limitations.

Aims

To assess the use of the CO2 fractional laser in the treatment of SUI.

Methods

This clinical trial included 55 patients with confirmed SUI. Patients underwent fractional CO2 laser treatment 3 times at 30-day intervals. Data on age, smoking history, sexual activity, menopause, and history of hormone replacement therapy (HRT) were collected. Response to treatment was assessed by SUI severity and the level of sexual satisfaction was assessed using the visual analog scale (VAS). Patients were evaluated at 3 different time points: before treatment, and 45 days and 6 months after the last laser treatment.

Results

The mean patient age was 44.4±11.4 years (range: 28 to 68 years). Smoking history was positive in 6 patients (9.1%); 19 (54.3%) were menopausal on HRT. The SUI severity score at baseline (before treatment) was 8.56±0.62 and decreased to 2.28 6 months after treatment (p<0.0001). The sexual satisfaction score was 3±0.94 at baseline and increased to 7.87±0.93 6 months after treatment (day 180) (p<0.0001, slope = + 2.2)

Conclusion

Our findings are in line with a previous study that showed the value of transvaginal CO2 fractional laser treatment for alleviation of SUI symptoms and its potential as an alternative treatment. We also observed improved sexual satisfaction in SUI patients.

Stress urinary incontinence (SUI) involves involuntary leakage of urine in response to abdominal pressure caused by activities such as sneezing and coughing. The condition affects millions of women worldwide, causing physical discomfort as well as social distress and even social isolation. Until recently, SUI was approached by clinicians as a purely anatomic problem requiring behavioral or surgical therapy. Over the past several years, extensive basic and clinical research in the field of neurourology has enhanced our understanding of the complex neural circuitry regulating normal function of the lower urinary tract. As a result, novel concepts have emerged regarding possible neurologic dysfunctions that might underlie the development of SUI, as well as potential novel strategies for pharmacologic therapy. This article reviews the normal neurophysiologic control of lower urinary tract function and considers potential pharmacologic approaches to correcting SUI.  相似文献

The decline of estrogen production after menopause is contributing factor to urinary incontinence (UI), and particularly stress urinary incontinence (SUI). We determined the preventive effects of herbal extract mixture (HEM) on UI in ovariectomized Sprague Dawley rats. Female 9-weeks old rats were ovariectomized and treated with HEM (2.2, 11, or 55 mg/kg/day) for 8 weeks. The index of urinary bladder weight to body weight in the HEM and non-ovariectomized and non-treated (SHAM) groups were slightly higher than the ovariectomized, non-treated group (OVX). The contraction index of acetylcholine to KCl on detrusor smooth muscle strips in the HEM groups showed a dose-dependent recovery. HEM treatment also significantly improved collagen levels, as shown by Masson trichrome staining, as well as hydroxyproline levels in the urinary bladder. Serum estradiol levels in the HEM groups were higher than the OVX group. In conclusion, HEM increased estradiol levels in serum and improved factors related to urinary incontinence. The improvements in estradiol levels were related to changes in urinary incontinence.  相似文献
