目的:探讨运动康复锻炼对冠心病经皮冠状动脉介入术(PCI)后患者心肺功能及生活质量的影响。方法:选取2016年10月-2018年4月期间我院收治的冠心病稳定性心绞痛患者80例,根据随机数字表法将患者分为对照组(n=40)和运动康复组(n=40),其中对照组给予常规康复运动,运动康复组在此基础上联合运动康复训练。比较两组患者术后12周的心肺功能指标,比较两组患者术前、术后2周、术后12周的6 min步行距离,比较两组患者术前、术后12周生活质量评分。结果:运动康复组患者术后12周峰值摄氧量、峰值通气量、峰值功率、每搏输出量、代谢当量以及摄氧效率斜率均高于对照组患者(P0.05)。两组患者术后2周、术后12周6 min步行距离均较术前增加,且运动康复组高于对照组(P0.05)。两组患者术后12周生理机能、生理职能、躯体疼痛、精神健康评分均较术前升高,且运动康复组高于对照组(P0.05);而两组患者术后12周社会功能、总体健康、精力、情感职能评分比较差异无统计学意义(P0.05)。结论:PCI术后患者给予运动康复锻炼,可显著提升患者生活质量,对患者心肺功能恢复具有较好的促进作用。  相似文献

Background

Despite its efficacy and cost-effectiveness, exercise-based cardiac rehabilitation is undertaken by less than one-third of clinically eligible cardiac patients in every country for which data is available. Reasons for non-participation include the unavailability of hospital-based rehabilitation programs, or excessive travel time and distance. For this reason, there have been calls for the development of more flexible alternatives.

Methodology and Principal Findings

We developed a system to enable walking-based cardiac rehabilitation in which the patient''s single-lead ECG, heart rate, GPS-based speed and location are transmitted by a programmed smartphone to a secure server for real-time monitoring by a qualified exercise scientist. The feasibility of this approach was evaluated in 134 remotely-monitored exercise assessment and exercise sessions in cardiac patients unable to undertake hospital-based rehabilitation. Completion rates, rates of technical problems, detection of ECG changes, pre- and post-intervention six minute walk test (6 MWT), cardiac depression and Quality of Life (QOL) were key measures. The system was rated as easy and quick to use. It allowed participants to complete six weeks of exercise-based rehabilitation near their homes, worksites, or when travelling. The majority of sessions were completed without any technical problems, although periodic signal loss in areas of poor coverage was an occasional limitation. Several exercise and post-exercise ECG changes were detected. Participants showed improvements comparable to those reported for hospital-based programs, walking significantly further on the post-intervention 6 MWT, 637 m (95% CI: 565–726), than on the pre-test, 524 m (95% CI: 420–655), and reporting significantly reduced levels of cardiac depression and significantly improved physical health-related QOL.

Conclusions and Significance

The system provided a feasible and very flexible alternative form of supervised cardiac rehabilitation for those unable to access hospital-based programs, with the potential to address a well-recognised deficiency in health care provision in many countries. Future research should assess its longer-term efficacy, cost-effectiveness and safety in larger samples representing the spectrum of cardiac morbidity and severity.  相似文献

目的:研究心脏康复运动对冠脉支架植入术后患者血脂、血糖、体重指数及生活质量的影响。方法:对实施冠脉支架植入术的 146例患者进行比较分析,根据随机原则分为试验组76 例及对照组70 例。对照组患者给予常规的健康教育及冠心病二级预防指 导,给予定期随访。试验组患者在此基础上给予规律的康复运动指导。经过6 个月随访,比较两组患者血脂、HbA1C、体重指数及 生活质量情况。结果:试验组患者通过为期6 个月的规律的心脏康复运动指导,其血脂、HbA1C等冠心病危险因素控制情况优于 对照组,差异有统计学意义(P<0.05)。同时,6个月后,试验组康复运动六月后sF 量表各项评分与对照组同期比较,差异均有统计 学意义(P<0.05)。结论:规范的心脏康复运动指导能够有效改善冠脉支架植入术后患者血脂、血糖情况,提高患者生活质量。  相似文献

OBJECTIVE: To determine the prognostic role of thallium-201 imaging compared with that of exercise electrocardiography in patients with acute myocardial infarction treated by thrombolysis. DESIGN: Patients who remained free of adverse cardiac events six weeks after myocardial infarction had stress and rest 201TI imaging and exercise electrocardiography and were followed up for 8-32 months. Adverse cardiac events (death, reinfarction, unstable angina, and congestive heart failure) were documented. SETTING: Large district general hospital, Middlesex. SUBJECTS: 100 consecutive male and female patients who were stable six weeks after thrombolysis for myocardial infarction. MAIN OUTCOME MEASURES: Prediction of occurrence of adverse cardiac events after myocardial infarction by exercise cardiography and 201TI myocardial perfusion imaging. RESULTS: Reversible ischaemia on 201TI imaging predicted adverse cardiac events in 33 out of 37 patients with such events during follow up (hazard ratio 8.1 (95% confidence interval 2.7 to 23.8), P < 0.001). Exercise electrocardiography showed reversible ischaemia in 33 patients, of whom 13 had subsequent events, and failed to predict events in 24 patients (hazard ratio 1.1 (0.56 to 2.2), P = 0.8). CONCLUSION: 201TI imaging is a sensitive predictor of subsequent adverse cardiac events in patients who have received thrombolysis after acute myocardial infarction, whereas exercise electrocardiography fails to predict outcome.  相似文献

OBJECTIVE: To evaluate rehabilitation after myocardial infarction. DESIGN: Randomised controlled trial of rehabilitation in unselected myocardial infarction patients in six centres, baseline data being collected on admission and by structured interview (of patients and spouses) shortly after discharge and outcome being assessed by structured interview at six months and clinical examination at 12 months. SETTING: Six district general hospitals. SUBJECTS: All 2328 eligible patients admitted over two years with confirmed myocardial infarction and discharged home within 28 days. INTERVENTIONS: Rehabilitation programmes comprising psychological therapy, counselling, relaxation training, and stress management training over seven weekly group outpatient sessions for patients and spouses. MAIN OUTCOME MEASURES: Anxiety, depression, quality of life, morbidity, use of medication, and mortality. RESULTS: At six months there were no significant differences between rehabilitation patients and controls in reported anxiety (prevalence 33%) or depression (19%). Rehabilitation patients reported a lower frequency of angina (median three versus four episodes a week), medication, and physical activity. At 12 months there were no differences in clinical complications, clinical sequelae, or mortality. CONCLUSIONS: Rehabilitation programmes based on psychological therapy, counselling, relaxation training, and stress management seem to offer little objective benefit to patients who have experienced myocardial infarction compared with previous reports of smaller trials.  相似文献

Background

Cardiac rehabilitation following myocardial infarction reduces subsequent mortality, but uptake and adherence to rehabilitation programmes remains poor, particularly among women, the elderly and ethnic minority groups. Evidence of the effectiveness of home-based cardiac rehabilitation remains limited. This trial evaluates the effectiveness and cost-effectiveness of home-based compared to hospital-based cardiac rehabilitation.

Methods/design

A pragmatic randomised controlled trial of home-based compared with hospital-based cardiac rehabilitation in four hospitals serving a multi-ethnic inner city population in the United Kingdom was designed. The home programme is nurse-facilitated, manual-based using the Heart Manual. The hospital programmes offer comprehensive cardiac rehabilitation in an out-patient setting.

Patients

We will randomise 650 adult, English or Punjabi-speaking patients of low-medium risk following myocardial infarction, coronary angioplasty or coronary artery bypass graft who have been referred for cardiac rehabilitation.

Main outcome measures

Serum cholesterol, smoking cessation, blood pressure, Hospital Anxiety and Depression Score, distance walked on Shuttle walk-test measured at 6, 12 and 24 months. Adherence to the programmes will be estimated using patient self-reports of activity. In-depth interviews with non-attendees and non-adherers will ascertain patient views and the acceptability of the programmes and provide insights about non-attendance and aims to generate a theory of attendance at cardiac rehabilitation. The economic analysis will measure National Health Service costs using resource inputs. Patient costs will be established from the qualitative research, in particular how they affect adherence.

Discussion

More data are needed on the role of home-based versus hospital-based cardiac rehabilitation for patients following myocardial infarction and revascularisation, which would be provided by the Birmingham Rehabilitation Uptake Maximisation Study (BRUM) study and has implications for the clinical management of these patients. A novel feature of this study is the inclusion of non-English Punjabi speakers.  相似文献

Myocardial ischemia-reperfusion (IR) injury is a major contributor to the morbidity and mortality associated with coronary artery disease. Muscular exercise is a countermeasure to protect against IR-induced cardiac injury in both young and old animals. Specifically, regular bouts of endurance exercise protect the heart against all levels of IR-induced injury. Proposed mechanisms to explain the cardioprotective effects of exercise include alterations in coronary circulation, expression of endoplasmic reticulum stress proteins, increased cyclooxygenase-2 activity, induction of myocardial heat shock proteins, improved cardiac antioxidant capacity, and/or elevation of ATP-sensitive potassium channels on both the sarcolemmal and the mitochondrial inner membranes. Moreover, it seems possible that other, yet to be defined, mechanisms of exercise-induced cardioprotection may also exist. Of the known putative cardioprotective mechanisms, current evidence suggests that elevated myocardial levels of antioxidants and increased expression of sarcolemmal ATP-sensitive potassium channels are both contributors to exercise-induced cardioprotection against IR injury. At present, it is unclear if these two protective mediators act independently or interact to contribute to exercise-induced cardioprotection. Understanding the molecular basis for exercise-induced cardioprotection will provide the required knowledge base to develop therapeutic approaches to protect the heart during an IR insult.  相似文献

Respiratory and cardiocirculatory response to rehabilitation calisthenics in 30 patients, aged 39-66 years, with recent myocardial infarction was studied. Respiratory exchange (Douglas-Haldane method), heart rate, blood pressure, oxygen pulse and electrocardiogram during exercise were investigated. Rehabilitation was performed between the 4th and 21st day of myocardial infarction, in 4 periods with gradually increasing effort, according to the model A designed at the Cardiology Institute in Warsaw. The time of exercises was 10-18 min, depending on the period of rehabilitation. It was found that lung ventilation, tidal volume, oxygen uptake and carbon dioxide output were increased by 20-40% during exercise of the I and II periods of rehabilitation and by 60-100% in the III and IV periods of mobilization. Energy cost of calisthenics rehabilitation was in the lightest case 13 kJ/min. The most increase in respiratory exchange caused exercises performed in sitting position, walking and stair climbing. The last type produced also the highest rise in heart rate (mean 20%) and systolic blood pressure (mean 17% of resting values).  相似文献

Cardiovascular related diseases are the major cause of death in the United States. The primary goal of the physician should be prevention of these diseases; however, once problems occur, a definitive rehabilitation program must be instituted in addition to the medical and surgical treatment. Although risk factors for the development of cardiovascular disease are numerous, statistical reports suggest that control of these factors might delay or prevent such development. This presentation describes the recommended Physical Medicine and Rehabilitation Program for high-risk patients or those with existing cardiovascular disease. The basis of the physical fitness and rehabilitation program is controlled progression-of-energy-expenditure physical activity and exercise in metabolic equivalents (METS). Education plays a key role in decreasing the incidence of cardiovascular disease, and should start at an early age. Adults at high risk of developing cardiovascular disease should be offered medically supervised physical fitness exercise programs, with monitoring where necessary.  相似文献

摘要 目的:探讨坐式八段锦联合运动康复治疗对急性冠脉综合征(ACS)经皮冠状动脉介入治疗术(PCI)后患者运动耐力、血清心肌损伤标志物和生活质量的影响。方法:根据随机数字表法,将我院2020年4月~2021年5月期间收治的ACS行PCI术的患者(n=120)分为对照组(运动康复治疗,n=60)和研究组(坐式八段锦联合运动康复治疗,n=60)。对比两组心功能指标[左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、氨基末端B型脑利钠肽前体(NT-proBNP)、左心室舒张末期容积(LVEDV)]、心肌损伤标志物[肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)]、运动耐力指标[无氧阈(AT)、最大摄氧量(VO2max )、运动持续时间(ED)]和生活质量[西雅图心绞痛量表(SAQ)评分:躯体活动受限程度(PL)、心绞痛稳定情况(AS)、疾病认知程度(DS)、心绞痛发作状况(AF)和治疗满意程度(TS)],并纪录两组不良心血管事件发生情况。结果:研究组的心血管不良事件发生率低于对照组(P<0.05)。研究组治疗3个月后LVEF高于对照组,LVEDD、LVEDV、NT-proBNP低于对照组(P<0.05)。研究组治疗3个月后CK-MB、cTnI低于对照组(P<0.05)。研究组治疗3个月后AT、VO2max 、ED高于对照组(P<0.05)。研究组治疗3个月后PL、AS、DS、AF、TS评分高于对照组(P<0.05)。结论:坐式八段锦联合运动康复治疗可促进ACS患者PCI术后心功能改善,减轻心肌损伤,进而提高运动耐力和生活质量。  相似文献

目的:探讨运动康复训练对冠心病多支病变患者活动耐量及血脂的影响。方法:选取冠心病多支病变患者,所有患者均进行不完全血运重建术,分为运动康复训练组及对照组,运动康复训练组自术后第三日起治疗组进行运动康复训练,对照组对运动量无要求。3月后,测量两组患者血脂水平及6分钟步行试验,比较两组患者步行距离、心绞痛发作次数、运动至心绞痛出现时间、6分钟内累积运动时间及血脂水平。结果:经运动康复训练治疗3月后,治疗组6分钟步行距离、运动至心绞痛出现时间及运动累积时间较对照组均明显增加,发生心绞痛次数较对照组明显减少。运动康复训练组治疗后甘油三酯、总胆固醇及低密度脂蛋白水平较治疗前明显降低,对照组仅总胆固醇有所降低,甘油三酯及低密度脂蛋白水平较治疗前无统计学差异。结论:运动康复训练可减少冠心病多支病变患者心绞痛发作次数,增加患者运动耐量并调节血脂代谢。  相似文献

目的:探讨有氧康复运动对慢性心力衰竭(chronic cardiac failure,CHF)患者心室重构及血管内皮功能的影响。方法:78例CHF患者随机分为运动组(n=39)、对照组(n=39)。对照组给予常规内科药物治疗、日常活动能力训练,运动组在此基础上根据美国心脏病学会(American Heart Association,AHA)的《三阶段康复运动方案》进行有氧康复运动,共持续12周。比较两组治疗前后心脏结构和功能、血管内皮功能及生活质量的改善情况。结果:干预后,两组左室收缩末期内径(left ventricular end systolic diameter,LVESD)、左室舒张末期内径(left ventricular end-diastolic diameter,LVEDD)、明尼苏达心衰生活质量(Minnesota Living With Heart Failure,MLHF)评分、血清内皮素-1(Endothelin-1,ET-1)、血管紧张素Ⅱ(angiotensin Ⅱ,Ang Ⅱ)水平均明显减小,左室射血分数(left ventricular ejection fraction,LVEF)、6分钟步行试验(6 minute walking test,6MWT)、血清一氧化氮(nitric oxide,NO)、降钙素基因相关肽(calcitonin gene-related peptide,CGRP)水平均明显升高,且运动组LVESD、LVEDD、MLHF评分、血清ET-1、Ang Ⅱ水平明显低于对照组,LVEF、6MWT、血清NO、CGRP水平明显高于对照组,差异均有统计学意义(P0.05)。运动组干预期间心衰再入院率显著低于对照组,差异均有统计学意义(P0.05)。结论:有氧运动康复训练有助于改善CHF患者的血管内皮功能,延缓或逆转心室重构,提高生活质量,改善预后。  相似文献

目的:探讨诺欣妥联合心脏运动康复对射血分数降低(HFr EF)的心力衰竭(HF)的临床疗效。方法:将我院心内科于2018年1月~2019年4月收治的70例HFr EF患者随机分为两组,各35例。对照组均给予诺欣妥规范治疗,实验组在此基础上根据心肺运动测试(CPET)测得代谢当量制定个性化心脏运动康复,包括院内、院外心脏康复干预及定期随访,为期6个月。采用彩色心脏超声诊断仪、心肺运动测试(CPET)分析两组治疗前后心肺功能变化,同时观察住院及随访期间的预后情况。结果:治疗6个月后,两组左心室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左心室射血分数(LVEF)均明显改善,且实验组显著优于对照组(P0.05)。治疗6个月后,实验组AT明显升高,峰值VO2/kg、峰值VO2水平均有一定程度上升,且明显优于对照组(P0.05)。与对照组比较,实验组90d内HF再住院率(8.6%vs.28.6%)、随访期间MACEs发生率(17.1%vs.40.0%)均显著降低(P0.05)。结论:诺欣妥联合心脏运动康复治疗可使HFr EF患者显著获益,在改善心肺功能、运动能力及近期预后方面疗效显著,可作为HFr EF患者的一线治疗方案。  相似文献

目的:观察个体化康复运动训练联合八段锦运动对冠心病患者经皮冠状动脉介入术(PCI)术后心功能、心境状态和生活质量的影响。方法:选取2017年9月-2018年9月期间来我院接受治疗的100例冠心病PCI术后患者,根据随机数字表法分为对照组和研究组,各50例。对照组患者在常规治疗的基础上接受个体化康复运动训练,研究组患者在对照组的基础上联合八段锦运动。观察两组心功能、生活质量和心境状态变化情况。统计两组6个月内心血管不良事件发生率。结果:干预后,两组患者6 min步行试验(6MWT)距离、左心室射血分数(LVEF)均较干预前升高,且研究组的变化程度优于对照组(P<0.05)。干预后,两组患者紧张-焦虑、抑郁-沮丧、愤怒-敌意、疲乏-迟钝、迷惑-混乱评分均较干预前下降,精力-活力、与自我有关的情绪评分较干预前升高,且研究组的变化程度优于对照组(P<0.05)。研究组6个月内的心血管不良事件发生率低于对照组,但是两组组间对比无统计学差异(P>0.05)。干预后,两组总体/精神健康、精力、情感/生理职能、躯体疼痛、生理/社会功能各维度评分升高,且研究组较对照组高(P<0.05)。结论:八段锦运动联合个体化康复运动训练可促进冠心病PCI术后患者心功能、生活质量和心境状态改善,同时还可控制心血管不良事件发生风险。  相似文献

Earlier concepts that the heart is spared in malnutrition have been shown to be incorrect. Inadequate intake of protein and energy results in proportional loss of skeletal and myocardial muscle. As myocardial mass decreases, so does the ability to generate cardiac output; however, various compensatory factors come into play. Nutritional supplementation for malnourished patients reverses the compensatory factors and may increase the short-term potential for heart failure. Severe cardiac debility results in poor nutrition, which may in turn produce unsuspected but clinically significant myocardial atrophy. Nutritional support may play a role in improving cardiac function in selected patients with cardiac cachexia who are being prepared for cardiac surgery and in patients with rapid weight loss who are at risk for sudden death due to arrhythmias. Malnutrition is common in hospitalized patients, and many patients in hospital now receive nutritional supplementation; both facts have important cardiac implications.  相似文献

It is well documented that children with a Fontan circulation have a reduced exercise capacity. One of the modalities to improve exercise capacity might be exercise training. We performed a systematic literature review on the effects of exercise training in patients with a Fontan circulation. Six published studies were included that reported on the effects of exercise training in 40 patients. All studies had a small sample size and/or did not include a control group. Based on the six published studies we can conclude that children who have undergone a Fontan operation and who are in a stable haemodynamic condition can safely participate in an exercise training programme and that exercise training results in an improved exercise capacity. However, more research is needed to establish the optimal exercise mode, dose-response relation, and the effects of exercise training on cardiac function, peripheral muscle function, physical activity, and health-related quality of life. Sudden cardiac death (SCD) is one of the leading causes of mortality in industrialized countries. Thus, identifying patients at high risk of SCD is an important goal. T wave alternans (TWA) is a new method for identifying patients with lethal ventricular tachyarrhythmias, and is dependent on heart rate. The maximal predictive accuracy is achieved at heart rates between 100 and 120 bpm, so that TWA is usually measured during exercise, pharmacological stress, or atrial pacing. It has been shown that TWA has high sensitivity and negative predictive value for predicting SCD after myocardial infarction and is also useful for predicting SCD in patients with nonischemic cardiomyopathy. Although the implantable cardioverter defibrillator (ICD) is now the primary therapy for preventing SCD, it is difficult to identify those patients who are susceptible to lethal ventricular tachyarrhythmias for primary prevention. In the prediction of SCD, TWA can be used as a screening test of appropriate patients for further electrophysiological examination and therapy.  相似文献

Although ventricular tachycardia is a well-known complication of myocardial ischaemia and may be provoked by exercise, many patients may appreciate only the angina and be unaware of the unduly rapid heart rate that precipitates it. Exercise testing is needed to show this arrhythmia and to enable treatment to be started.Twenty-three patients were found to have chronic ischaemic heart disease complicated by ventricular tachycardia. Six patients with old myocardial infarction had ventricular tachycardia at rest which required conversion to sinus rhythm; 17 patients developed ventricular tachycardia only when they exercised. In 12 of these 17 patients coronary angiography showed disease of the anterior descending branch of the left coronary artery; other vessels were usually also affected. Although beta-adrenergic blocking drugs increased exercise tolerance, ventricular tachycardia still occurred when the heart rate on exercise reached a level similar to that before treatment. In five patients coronary artery bypass surgery was performed because of angina and exercise-induced ventricular tachycardia. Exercise tolerance was increased in all three patients who underwent exercise tests after operation, and in two of these patients, both of whom were known to have patent grafts, ventricular tachycardia was abolished.If part of the beneficial effect of coronary bypass surgery is preventing life-threatening ventricular arrhythmias it is essential to detect these, and ambulatory monitoring and stress testing have a complementary role.  相似文献
