Although the mouse has become a preferred species for molecular studies on fracture healing, gait analysis after fracture fixation and during bone healing has not yet been performed in mice. Herein, we introduce a novel technique for gait analysis in mice and report the change of motion pattern after fracture and fixation. A standardized femur fracture was stabilized by a common pin. The non-fractured tibia was additionally marked with a pin, allowing continuous analysis of the tibio-femoral angle by digital video-radiography. Dynamic gait analysis was performed at day fourteen after surgery in a radio-opaque running wheel. Fracture fixation resulted in a significantly reduced range and maximum of the tibio-femoral angle compared to non-fractured controls. This was associated with a significantly reduced stride length. Because stride frequency was slightly increased and, thus, stride time diminished, stride velocity was not significantly reduced compared to controls. Thus, our study demonstrates distinct alterations of the gait of mice at 2 weeks after femur fracture and stabilization. Our results support the need of gait analysis in fracture healing studies to assess the animals’ well-being.  相似文献

PurposeThe aim of the present study was to evaluate the systemic biological effect of increased exercise on bone repair after stable fracture fixation.MethodsTwo groups of SKH-1 h mice were studied. Animals of the first group (n=36) were housed in cages supplied with a running wheel, while mice of the second group (n=37) were housed in standard cages for control. Using a closed femur fracture model, bone repair was analysed by histomorphometry and biomechanical testing at 2 and 5 weeks. At 2 weeks, we additionally evaluated the expression of the proliferation marker PCNA (proliferating cell nuclear antigen) and the angiogenic and osteogenic growth factor VEGF (vascular endothelial growth factor). To standardise the mechanical conditions in the fracture gap, we used an intramedullary compression screw for stable fracture fixation.ResultsEach mouse of the exercise group run a mean total distance of 23.5 km after 2 weeks and 104.3 km after 5 weeks. Histomorphometric analysis of the size and tissue composition of the callus could not reveal significant differences between mice undergoing exercise and controls. Accordingly, biomechanical testing showed a comparable torsional stiffness, peak rotation angle, and load at failure of the healing bones in the two groups. The expression of PCNA and VEGF did also not differ between mice of the exercise group and controls.ConclusionWe conclude that increased exercise does not affect bone repair after stable fracture fixation.  相似文献

Minimally invasive plate osteosynthesis(MIPO) has been considered as an alternative for fracture treatment. Previous study has demonstrated that MIPO technique has the advantage of less soft tissue injury compared with open reduction internal fixation (ORIF). However, the comparison of callus formation and mineralization between two plate osteosynthesis methods remains unknown. In this experiment, ulna fracture model was established in 42 beagle dogs. The fractures underwent reduction and internal fixation with MIPO or ORIF. Sequential fluorescent labeling and radiographs were applied to determine new callus formation and mineralization in two groups after operation. At 4, 8 and 12 weeks postoperatively, the animals were selected to be sacrificed and the ulna specimens were analyzed by Micro-CT. The sections were also treated with Masson staining for histological evaluation. More callus formation was observed in MIPO group in early stage of fracture healing. The fracture union rate has no significant difference between two groups. The results indicate that excessive soft tissue stripping may impact early callus formation. As MIPO technique can effectively reduce soft tissue injury with little incision, it is considered to be a promising alternative for fracture fixation.  相似文献

目的:对股骨骨折髓内钉术后1年骨愈合模型快速建模,通过有限元分析研究对比术前术后模型,通过术前判定内固定取出后骨折断端是否断裂。方法:运用Mimics、Geomagic Studio、Abaqus等软件采用快速个体化建模方法对股骨骨折髓内钉术后1年内固定取出术前后的多层螺旋CT数据进行快速建立模型,术前模型模拟剥除钢板后进行有限元分析,施加人体单腿站立时的静力载荷和约束,并将分析结果与术后模型进行对比,观察米塞斯应力分布情况、最大值及其所处部位。结果:按照材料属性进行区别显示米赛斯应力的最大值及最小值,在不同应力载荷下,手术前后各类型材料的米赛斯应力最大值及最小值部位相同,各类型材料中,最大值均没有位于骨折断端,不同方法的最大应力值部位相近,均在股骨中远端1/4交界处,手术前后应力分布基本相同。结论:采用个体化建模方法可以对骨折内固定取出前的骨愈合模型进行运算分析,快速预判术后是否导致骨折断端断裂。  相似文献

动力加压髋螺钉对股骨上段生物力学特征性的影响   总被引:1,自引:0,他引:1  
目的:探讨股骨上端骨折,以动力加压髋螺钉进行骨固定治疗,骨折愈合后,取出动力加压髋螺钉以后的股骨上段与完整的股骨上段的生物力学特性相比较,为临床内固定取出术后功能锻炼的强度提供量化依据。方法:收集8具新鲜尸体股骨标本进行实验应力分析,分别测定完整股骨上段和动力加压髋螺钉取出后股骨上段的力学特性改变。结果:动力加压髋螺钉取出术后股骨上段的力学特性与完整股骨上段的力学特性相比有显著的差异(P<0.01)。结论:股骨上端骨折如果以动力加压髋螺钉为治疗手段,在骨折愈合取出内固定后,功能锻炼只能控制在慢速步行水平,不能进行奔跑、跳跃等活动,以防止再骨折等并发症的发生。  相似文献

目的:本研究通过观察微创锁定钢板接骨术治疗老年股骨近端粉碎骨折临床效果,旨在找出最佳治疗方式。方法:自2007年12月~2010年03月,应用股骨近端锁定加压钢板治疗老年股骨近端粉碎骨折23例。记录术中出血量、手术时间,术后并发症、骨折愈合时间及最后一次随访时功能恢复情况。结果:骨折临床愈合时间为12~28周,平均16周。除1例患者髋内翻畸形,1例锁定加压钢板断裂外,其他患者均达到骨性愈合。结论:股骨近端锁定钢板具有创伤小、固定可靠、骨折愈合快、功能恢复满意的特点,尤其适用于老年股骨近端粉碎骨折。  相似文献

Internal fixation of fractures of the proximal humerus needs a high stability of fixation to avoid secondary loss of fixation. This is especially important in osteoporotic bone. In an experimental study, the biomechanical properties of the angle-fixed Philos plate (internal fixator) and a double-plate osteosynthesis using two one-third tubular plates were assessed. The fracture model was an unstable three-part fracture (AO type B2). Eight pairs of human cadaveric humeri were submitted to axial load and torque. In the first part of the study, it was assessed to which degree the original stiffness of the humeri could be restored after the osteotomy by the osteosynthesis procedure. Subsequently, subsidence during 200 cycles of axial loading and torque was analysed. During axial loading, the Philos plate was significantly stiffer and showed less irreversible deformation. Two double-plate fixations, but none of the Philos plate osteosynthesis, failed. During torsion, there were no significant differences between the two implants. From the biomechanical point of view, the angle-fixed Philos plate represents the implant of choice for the surgical fixation of highly unstable three-part fractures of the proximal humerus, as the internal fixator system is characterised by superior biomechanical properties.  相似文献

目的:比较锁定型γ钉(Locking Gamma Nail,LGN)、普通型γ钉(Traditional Gamma Nail,TGN)和动力髋螺钉(Dynamic Hip Screw,DHS)治疗不稳定性股骨粗隆间骨折的临床疗效和安全性。方法:选择2009年1月至2011年1月在我院分别接受锁定型γ钉(A组)、普通型γ钉(B组)和动力髋(C组)内固定治疗的随访资料完整的患者共92例。记录和比较三组患者的手术时间、术中出血量、骨折愈合时间及末次髋关节功能评分等相关数据。结果:A组、B组和C组的手术时间、术中出血比较有统计学差异(均P<0.01),但A、B组之间无明显统计学差异;A组和B组、C组的骨折愈合时间、髋关节功能评分比较差异均有统计学意义(均P<0.01),B、C组之间无明显统计学意义。结论:LGN的固定效果和生物力学性能良好,操作简便,固定牢固,整体治疗效果优于TGN和DHS,可作为临床治疗不稳定性股骨粗隆间骨折的首选。  相似文献

目的:探讨半髋关节置换术治疗老年不稳定性转子间骨折的治疗效果。方法:选择我院骨科2013年1月至2015年2月入院的老年不稳定转子间骨折患者64例,随机分为半髋关节置换组和髓内钉固定组,对两组手术时间、术中出血量、术后并发症、术后髋关节功能恢复状况进行评价。结果:半髋关节置换组手术时间较髓内钉固定组短(P0.05),半髋关节置换组和髓内钉固定组术中出血量、手术后并发症、髋关节功能恢复状况的差异不具有统计学意义(P0.05)。结论:半髋关节置换术治疗老年不稳定性转子间骨折具有较好的临床效果。  相似文献

The treatment of choice for early mobilization of hip fracture is surgery, which traditionally employs side plates and screws or intramedullary nails. We examined the biomechanical properties of a new proximal femoral nail system. The new expandable Fixion proximal femur nailing (PFN) system, made of stainless-steel alloy, consists of a nail, a peg and an anti-rotation pin. Upon positioning, the nail and peg are expanded to their maximal diameter. The current biomechanical study investigated: nail bending strength and stiffness, fatigue properties and hip peg strength. A cadaveric study that determined the effect of the expandable peg on the femoral head included subsidence testing, pull and torsion testing and intra-osseous pressure (IOP) measurements before and after expansion. Biomechanical properties of the new nail met ASTM F384 guideline requirements. The cadaver study yielded equivalent results for the pullout test between the peg and the hip screw, but found the peg superior in the torsion strength test. IOP during peg insertion and expansion was substantially lower than the threshold pressure that causes avascular necrosis. The biomechanical tests found the new system to be safe and able to provide good abutment of the nail to the bone. We conclude that the Fixion PFN system proved to be an effective proximal femur fracture fixation device.  相似文献

摘要 目的:针对髓腔内固定联合低强度脉冲超声对兔股骨中段骨折愈合作用进行研究。方法:选择成年兔股骨中段骨折40只,作为本次的研究对象,将其平均分为对照组和观察组。所有兔子,在手术前禁水、禁食,对其右后肢进行备皮,称重、麻醉,对照组实施髓腔内固定治疗,观察组实施髓腔内固定联合低强度脉冲超声治疗。治疗后1、2、3、4周对兔子的骨折部位进行影像学检查确定兔子的骨折线模糊情况,并在各周采集样品进行组织学检查。治疗4周后对骨折愈合情况进行检查。结果:观察组愈合程度I级、II级、II级比例均低于对照组相应比例,差异具有统计学意义(P<0.05),观察组IV级、V级比例分别为35.0 %、55.0 %,均高于对照组的5.0 %、5.0 %,差异具有统计学意义(P<0.05);观察组兔子的在1 w、2 w、3 w、4 w骨折线模糊程度评分高于对照组相应时间评分,差异具有统计学意义(P<0.05)。结论:在兔股骨中断骨折治疗中,实施髓腔内固定联合低强度脉冲超声,可以提高骨折的愈合速度,加速骨折修复,整体治疗效果显著,可以在临床上进行推广实施。  相似文献

The purpose of this study was to analyze the impact of vanadium absorbed by Coprinus comatus (VACC) on fracture healing in streptozotocin-diabetic rats. Forty-five male Wistar rats used were divided into three groups: normal rats (control), diabetic rats, and diabetic rats treated with VACC. A standardized fracture-healing model with a stable plate fixation was established for the rat femoral fracture. After a 4-week stable fixation, callus quality was assessed by microcomputerized tomography and histological and biomechanical examinations. In addition, bone samples were obtained to evaluate the content of mineral substances in bones. Compared with the diabetic group, vanadium treatment significantly increased bone mineral content and biomechanical strength and improved microstructural properties of the callus. The ultimate load was increased by 29.1 % (P?<?0.05), and the total bone volume of callus enhanced by 11.2 % (P?<?0.05) at 4 weeks post fracture. Vanadium also promoted callus bone formation, which caused a 35.5 % increase in the total area of callus. However, VACC did not accelerate the fracture repair process in histological analysis. In conclusion, the current study suggests that systemic treatment with vanadium could promote fracture healing in streptozotocin-diabetic rats.  相似文献

The aim of this study is to evaluate the fracture union or non-union for a specific patient that presented oblique fractures in tibia and fibula, using a mechanistic-based bone healing model. Normally, this kind of fractures can be treated through an intramedullary nail using two possible configurations that depends on the mechanical stabilisation: static and dynamic. Both cases are simulated under different fracture geometries in order to understand the effect of the mechanical stabilisation on the fracture healing outcome. The results of both simulations are in good agreement with previous clinical experience. From the results, it is demonstrated that the dynamization of the fracture improves healing in comparison with a static or rigid fixation of the fracture. This work shows the versatility and potential of a mechanistic-based bone healing model to predict the final outcome (union, non-union, delayed union) of realistic 3D fractures where even more than one bone is involved.  相似文献

目的:比较动力髋螺钉与股骨近端髓内钉治疗老年股骨近端骨折的临床疗效和安全性。方法:收集我院收治的老年股骨近端骨折患者64例,随机分为DHS组和PFN组,每组各32例。DHS组患者给予动力髋螺钉的固定方式,PFN组给予股骨近端髓内钉的固定方式。手术后对患者的手术切口长度、术中出血量、手术时间、骨折愈合时间、术后并发症以及患者临床疗效进行检测并比较。结果:与治疗前相比,两组患者治疗后的Harris评分均显著下降(P0.05);与DHS组相比,PFN组患者的手术切口长度、术中出血量、手术时间、骨折愈合时间、术后并发症的发生率以及Harris评分均较低(P0.05)。结论:股骨近端髓内钉的固定治疗老年股骨近端骨折的临床疗效较好,安全性更高。  相似文献

This study aimed to investigate the impact of organic gallium (OG) on osteoporotic fracture healing in ovariectomized female Sprague-Dawley rats, as well as study the mechanisms of OG on osteoporotic fracture healing. Forty-five female Sprague-Dawley rats were divided into three groups: sham operation group (Sxas control group), ovariectomized group (Ovx), and Ovx treated with OG group (Ovx + OG). Rat femoral fractures were studied using a standardized fracture-healing model utilizing bone fixation with an intramedullary pin. Six weeks later, analyses of micro-CT, histomorphometric, RNA extraction, RT-qPCR, and serum were performed following sacrifice of all mice. In comparison with Ovx group, OG can significantly increase bone volume (BV), tissue volume (TV), BV/TV radio, bone strength, callus bony area, and as similar to BMP-2 expression. OG treatment elevated OPG messenger RNA (mRNA) and inhibited RANKL mRNA, and showed an effect on OPG/RANKL ratio. OG treatment can inhibit the expression of TNF-α and IL-6. In conclusion, current study results indicate that organic OG can positively affect the OPG/RANKL ratio and inhibit the expression of serum inflammatory cytokines; thus, it can improve osteoporotic fracture healing.  相似文献

The purpose of this study was to analyze the impact of yeast-incorporated gallium on fracture healing in ovariectomized osteopenic rats. Forty Wistar female rats used were divided into three groups: sham-operated rats (SHAM), ovariectomized (OVX) rats, and ovx rats treated with yeast-bound gallium (YG). A standardized fracture-healing model with stable plate fixation was established for rat femoral. After 4-week stable fixation, animals were killed to prepare bones for Micro-CT, biomechanical testing, and histomorphometry. In addition, bone samples were obtained to evaluate the content of mineral substances in bones. Quantitative analysis of the bones from animals in the organic gallium group revealed significantly increased mineral contents compared to bones from OVX and SHAM groups. Micro-CT showed that treatment with yeast-incorporated gallium increased BV/TV and trabecular thickness and decreased trabecular separation in ovx animals. Histomorphometric evaluation demonstrated that YG increased callus area and callus bone formation. Yeast-bound gallium also improved the biomechanical properties of bone healing. In conclusion, this study suggests that yeast-incorporated gallium could promote fracture healing in ovariectomized rats.  相似文献

目的:探究近端膨胀髓内钉与近端螺旋刀片抗旋髓内钉治疗老年股骨转子间骨折疗效及躁动发生率比较。方法:病例来源为2012年5月至2013年10月收入我院的老年股骨转子间骨折患者60例,采用随机数字表随机分为两组,膨胀钉组30例,采用近端膨胀髓内钉的手术方法,PFNA组30例,采用近端螺旋刀片抗旋髓内钉的手术方法。手术结束后,对两组患者进行X射线片、手术时间及出血量、髋关节功能的比较,同时对比躁动发生率。结果:X线检查结果显示两组患者经手术后骨折处均复位良好,无并发症及内固定物的脱落及松动,两组比较无显著差异(P0.05);与PFNA组相比,膨胀组患者的手术时间及出血量明显降低,差异具有统计学意义(P0.05);两组患者术后3个月、6个月的Harris评分显示,与PFNA组相比,膨胀钉组的优良率明显升高,髋关节优于PFNA组,差异具有统计学意义(P0.05);膨胀钉组患者躁动率为10.00%低于PFNA组患者躁动率为33.33%,具有统计学意义(P0.05)。结论:可膨胀髓内钉具有手术时间较短、出血量少,更利于骨折的愈合,且躁动发生率较低,对股骨转子间骨折的疗效优于较近端螺旋刀片抗旋髓内钉,对临床有指导意义,值得临床推广。  相似文献

目的:观察并比较弹性髓内钉(ESIN)与钢板固定(PF)治疗儿童股骨干中段骨折的临床疗效。方法:选择2013年2月至2016年12月我院收治的股骨干中段骨折患儿90例,依据治疗方法不同分为ESIN组和PF组。ESIN组(n=45)采用弹性髓内钉固定,PF组(n=45)采用钢板固定,比较两组患者手术时间、手术切口、术中失血量、术后切口引流量等手术指标,随访评估患者住院时间、完全负重时间、骨折愈合时间;按Flynn评定标准比较两组患者的最终治疗结果。结果:两组患者术中失血量、术后引流量比较差异无统计学意义(P0.05),ESIN组患者手术切口显著小于PF组(P0.05),手术时间、透视时间、完全负重时间均显著短于PF组(P0.05),临床疗效明显优于PF组(P0.05)。结论:与钢板固定比较,弹性髓内钉在手术时间、透视时间显著短于钢板固定,临床疗效明显优于钢板固定,可作为儿童股骨干骨折内固定的首选材料。  相似文献

目的:分析对比股骨近端防旋髓内钉(PFNA)与股骨近端锁定钢板(PFLP)治疗老年股骨转子间不稳定骨折的临床疗效。方法:回顾性分析我院骨外科2009年4月至2013年4月收治的老年股骨转子间不稳定骨折患者90例,根据患者手术方式的不同,将其分为PFNA组及PFLP组,各45例。对比分析两组患者术中出血量、手术持续时间及骨折愈合时间术后髋关节功能及内固定并发症情况。结果:PFNA组髋关节功能Harris评分优良率91.11%明显高于PFLP组的71.11%(P0.05);PFNA组比PFLP组骨折愈合时间短、内固定并发症少,比较差异有统计学意义(P0.05)。结论:应用PFNA治疗老年股骨转子间不稳定骨折患者具有骨折愈合时间短、髋关节功能恢复好、术后内固定并发症少的特点,治疗效果较PFLP更满意。  相似文献

目的:评价扩髓更换髓内钉治疗髓内钉固定后股骨干肥大性骨不连的临床疗效及手术适应症。方法:自1998年4月至2009年6月采用扩髓更换髓内钉治疗11例髓内钉固定后股骨干肥大性骨不连,其中男9例,女2例,年龄23-61岁。平均36.2岁,骨折部位在股骨上1/3者2例;中1/3者6例;下1/3者3例,原始骨折Winquist-Hansen分型:I型2例,II型3例,III型4例,IV型2例。结果:11例患者均获随访,时间:11~56个月,平均27.4个月,2例患肢短缩1 cm。另4例未获得骨性愈合,3例再次采用附加钢板合并自体髂骨植骨,1例远端锁钉动力化,再次干预后获得骨性愈合,愈合时间6~16个月,平均8.3个月。结论:扩髓更换髓内钉是治疗髓内钉固定后股骨肥大性骨不连的传统方法,基于力学稳定和临床观察,更适用于股骨中段骨不连。  相似文献
