ObjectiveTo study the relationship between bone markers and bone mineral density (BMD) in an effort to identify their utility in postmenopausal women with osteoporosis.MethodsEighty-two consecutive postmenopausal women with untreated osteoporosis were included in the study. Forearm, spinal, and femoral BMD by dual-energy x-ray absorptiometry and markers of bone formation (serum osteocalcin and bone-specific alkaline phosphatase) and bone resorption (urinary free deoxypyridinoline) were measured in all patients. Patients with low serum vitamin D levels, secondary osteoporosis, or clinically significant systemic disease were excluded from the study. The patients were classified on the basis of BMD of the lumbar spine into the following 3 groups: mild (n = 23) (T score -2.5 through -3), moderate (n = 42) (T score -3.1 through -4), or severe (n = 17) (T score ≤-4.1) osteoporosis. One-way analysis of variance and Pearson correlation were used for statistical analysis, with a P value < .05 being considered significant.ResultsSerum osteocalcin was significantly different among the 3 study groups (4.1 ± 2.7, 4.5 ± 3.1, and 6.7 ± 5.6 ng/mL, respectively; P = .0349) and had a significant negative correlation with BMD (r2 = -0.0779; P = .0168). Other bone markers such as bone-specific alkaline phosphatase and urinary free deoxypyridinoline did not correlate with the underlying BMD.ConclusionIn our study, osteocalcin was significantly correlated with BMD in postmenopausal women with osteoporosis. Other bone markers did not correlate with BMD. Further large-scale population data and analyses are needed to confirm these findings. (Endocr Pract. 2008;14:1102-1107)  相似文献

利用限制性片段长度多态性分析(RFLP)研究北京地区绝经后妇女甲状旁腺激素(PTH)基因多态性与骨密度的关系。筛选健康、无亲缘关系的绝经后妇女185例, 应用双能X射线骨密度仪(DEXA)检测腰椎等部位的骨密度,用PCR-RFLP方法检测绝经后妇女的PTH基因型。绝经后健康妇女中bb、Bb、BB三种基因型的分布频率分别为7.56%、28.11%和64.32%。方差分析显示前臂部位骨密度与PTH基因相关。除华氏三角区外,BB基因型各部位的骨密度值均高于Bb、bb基因型。Logistic回归分析结果显示,bb基因型组骨质疏松与正常妇女存在显著差异(P<0.001)。 PTH基因中B基因型可能对维持骨量具有一定的作用。  相似文献

为探讨北京地区汉族妇女雌激素受体(ER)基因XbaI多态性与骨密度的关系,采用双能X线吸收仪检测腰椎、股骨及前臂骨密度;采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)法,对179例北京地区汉族妇女ER基因XbaI多态性进行分析。北京地区汉族妇女ER基因XX、Xx和xx基因型频率分布为0.302、0.464和0.234,绝经前与绝经后妇女XbaI型基因频率分布有明显差异,绝经后妇女腰椎骨密度(0.836±0.18)g/cm2明显低于绝经前妇女(1.038±0.14);绝经后妇女骨质疏松症的发病率为54%。ER基因XbaI基因型频率分布有明显的种族差异并受绝经影响,ER基因XbaI基因型与骨密度无明显相关性。与体重和BMI有明显相关性。  相似文献

ObjectiveTo attempt to clarify the effect of simvastatin, a widely used statin, on the bone mineral density in women with type 2 diabetes.MethodsWe performed a cross-sectional, controlled study of 37 women with type 2 diabetes who were taking simvastatin. Each woman was matched with 2 control subjects who were closest in age, years since menopause (if applicable), and duration of diabetes on the date on which the examination was performed. We measured bone mineral density at the spine and the hip with a dual-energy x-ray absorptiometry scanner and compared bone density in the 2 study groups.ResultsThe mean bone mineral density values of patients in the simvastatin group were found to be slightly increased in comparison with those of the control group, both in the lumbar vertebrae and in the femoral neck, but these differences were not statistically significant (P > 0.05).ConclusionIn this cross-sectional study, we could not demonstrate a positive effect of long-term simvastatin treatment on bone mineral density in women with type 2 diabetes and hypercholesterolemia. 目的:调查分析人体计量学参数包括身高、体重及体重指数对南昌地区健康的女性腰椎、股骨颈、转子间和转子骨密度的影响。方法:在南昌地区进行随机抽样检查373例20~25岁之间的健康的女性,采用双能X线骨密度仪检测腰椎、股骨颈、转子间和转子骨密度,用SAS软件进行统计分析。结果:身高、体重及体重指数都与腰椎、股骨颈、转子间和转子骨密度显著正相关(P〈0.0001),体重与骨密度的相关系数最高(0.241~0.382),体重指数与骨密度的相关系数其次(0.171—0.320),身高与骨密度的相关系数较低(0.144~0.189)。在身高、体重和体重指数因素中,影响腰椎、股骨颈、转子间和转子骨密度的重要因素均只有体重(P〈0.0001)。结论:体重是影响南昌地区健康青年女性骨密度的主要人体测量学参数。  相似文献

目的:本研究的目的是评估绝经后女性冠心病患者心血管危险因素与骨密度的相关关系。方法:评估216例拟行冠脉造影的绝经后女性冠心病患者的危险因素,并于冠脉造影检查前日或次日行骨密度检测,依据T值将受试者分为2组:骨量正常组(T值大于-1SD)、低骨量组(T值小于-1SD)。结果:2组患者在BMI、糖尿病、高血压及吸烟等均无显著性差异。低骨量组冠心病的发生率及年龄显著高于骨量正常组。Logistic回归分析显示绝经后女性冠心病患者年龄与骨密度独立相关(OR=1.072 CI:1.036~1.11p=0.001)。结论:年龄与绝经后女性冠心病患者骨密度负相关,心血管病危险因素或冠心病与骨量不相关。  相似文献

Objective

To assess factors associated with bone mineral density (BMD) in postmenopausal women in a longitudinal study, and to examine the relative contribution of lean mass, fat mass, dietary patterns, and years since menopause to BMD.

Methods

Two hundred and eighty-two postmenopausal women were randomly selected from Hongqi Community Health Center, in Harbin City, China. All participants were followed up from 2009 to 2011. Dietary data were collected using a Food Frequency Questionnaire. BMD of the left hip, the lumbar spine, and the total body, and the body composition were measured by dual-energy X-ray absorptiometry at baseline and follow-up.

Results

Lean mass and fat mass were positively associated with BMD of the spine, hip, and the total body at both baseline and follow-up. The association between fat mass and BMD at the spine at baseline (P = 0.210) and at the spine (P = 0.116) and hip (P = 0.073) in the second year was not statistically significant when height was adjusted. Six dietary patterns were identified but only cereal grains-fruits pattern (P = 0.001 in the spine, P = 0.037 in hip) and milk-root vegetables pattern (P = 0.010 in hip) were associated with BMD of the spine and hip. The linear mixed model of follow-up data showed that lean mass, years since menopause, and age of menophania were the significant determinants of BMD of all sites. Moreover, lean mass was the best determinant of BMD (VIP = 1.936).

Conclusion

Lean mass, years since menopause, age of menophania and dietary patterns are the important determinants of BMD of the spine, hip, and the total body. Lean mass is the best determinant of BMD.  相似文献

Copper concentrations in blood plasma have been determined in 25 osteopenic females using inductively coupled plasma–mass spectrometry. A high degree of correlations has been demonstrated between the copper concentrations in plasma and the bone mineral density of the lumbar spine as measured using dual energy X-ray absorptiometry and quantitative computerized tomography. Results clearly indicate the involvement of copper in bone health and osteopenia. It is further suggested that plasma copper might be useful as a cheap and simple method indicative of bone mineral density in osteopenic postmenopausal females.  相似文献

Multiple myeloma (MM) is a cancer of antibody-making plasma cells. It frequently harbors alterations in DNA and chromosome copy numbers, and can be divided into two major subtypes, hyperdiploid (HMM) and non-hyperdiploid multiple myeloma (NHMM). The two subtypes have different survival prognosis, possibly due to different but converging paths to oncogenesis. Existing methods for identifying the two subtypes are fluorescence in situ hybridization (FISH) and copy number microarrays, with increased cost and sample requirements. We hypothesize that chromosome alterations have their imprint in gene expression through dosage effect. Using five MM expression datasets that have HMM status measured by FISH and copy number microarrays, we have developed and validated a K-nearest-neighbor method to classify MM into HMM and NHMM based on gene expression profiles. Classification accuracy for test datasets ranges from 0.83 to 0.88. This classification will enable researchers to study differences and commonalities of the two MM subtypes in disease biology and prognosis using expression datasets without need for additional subtype measurements. Our study also supports the advantages of using cancer specific characteristics in feature design and pooling multiple rounds of classification results to improve accuracy. 观察3月龄SD雌鼠切除双侧卵巢29周内模型组和雌二醇组动物第三腰椎及股骨骨密度的变化,同时与假手术组比较。结果表明,SD雌性大鼠切除卵巢9周股骨骨密度明显低于假手术组(P<001),13周后腰椎骨骨密度也有显著差异(P<001),29周内模型稳定。皮下注射苯甲酸雌二醇40μgkg×2次周,在12周能够明显提高切卵巢SD大鼠的股骨骨密度并使其接近正常,16周后第三腰椎的骨密度也能够明显提高。切卵巢骨质疏松症大鼠模型治疗性给药需要大约9~13周左右模型才能成功,切除卵巢后29周内模型仍然稳定。  相似文献

HIV infection is associated with high rates of osteopenia and osteoporosis, but the mechanisms involved are unclear. We recently reported that bone loss in the HIV transgenic rat model was associated with upregulation of B cell expression of the key osteoclastogenic cytokine receptor-activator of NF-κB ligand (RANKL), compounded by a simultaneous decline in expression of its physiological moderator, osteoprotegerin (OPG). To clinically translate these findings we performed cross-sectional immuno-skeletal profiling of HIV-uninfected and antiretroviral therapy-naïve HIV-infected individuals. Bone resorption and osteopenia were significantly higher in HIV-infected individuals. B cell expression of RANKL was significantly increased, while B cell expression of OPG was significantly diminished, conditions favoring osteoclastic bone resorption. The B cell RANKL/OPG ratio correlated significantly with total hip and femoral neck bone mineral density (BMD), T- and/or Z-scores in HIV infected subjects, but revealed no association at the lumbar spine. B cell subset analyses revealed significant HIV-related increases in RANKL-expressing naïve, resting memory and exhausted tissue-like memory B cells. By contrast, the net B cell OPG decrease in HIV-infected individuals resulted from a significant decline in resting memory B cells, a population containing a high frequency of OPG-expressing cells, concurrent with a significant increase in exhausted tissue-like memory B cells, a population with a lower frequency of OPG-expressing cells. These data validate our pre-clinical findings of an immuno-centric mechanism for accelerated HIV-induced bone loss, aligned with B cell dysfunction.  相似文献

The development of quantitative ultrasound (QUS) technologies to measure bone is motivated by the need to overcome the limitations of X-ray based methods, measuring bone mineral density (BMD) which is the gold standard to date for the diagnosis of osteoporosis. Because it uses mechanical waves, the ultrasound modality is a particularly relevant means to probe bone mechanical resistance. The vast majority of QUS technologies commercialized to date merely aim to provide surrogate markers for BMD. During the past decade, innovative QUS approaches have emerged to assess bone beyond BMD. This may be achieved by (1) specifically assessing the cortical bone compartment, independently of trabecular bone, and (2) providing intrinsic bone properties such as cortical bone thickness and material properties. One specific motivation is to estimate intracortical porosity, a quantity reflected in material properties. This article aims at an overview of recent QUS developments to measure cortical bone properties. We also draw a picture of the current knowledge on bone material properties of interest for bone QUS. We discuss the potential of ultrasound to provide novel biomarkers of bone health through the assessment of material properties.  相似文献

研究组前期的全基因组关联研究发现PHACTR3基因与骨折关联,为了检测该基因与骨密度的关联关系,采用精细定位关联研究来检测PHACTR3基因内及其附近的SNPs与骨密度的关系。首先在中国样本(1627个不相关的汉族样本)和美国样本(2286个不相关高加索样本)中对PHACTR3基因的140个SNPs进行基因分型,然后采用Plink软件检测PHACTR3基因与腰椎和髋部骨密度的关联关系。发现研究组以前报道的与骨折关联的SNPs rs1555364和rs6064822与腰椎和髋部骨密度关联(P=4.89×10^-2-1.26×10^-2)。另外还发现位于PHACTR3基因内含子中3个SNPs位点(rs6027138,rs1182531和rs1182532)与中国人群和白人腰椎骨密度均显著关联,将中国人与白人样本合并起来进行荟萃分析(Meta—analysis),得到合并P值为1.40×10^-3到4.00×10^-4,另外发现rs6064820与髋部BMD相关联,合并P值为6.70×10^-3。本研究进一步证实了PHACTR3基因在骨密度变异中的作用,对骨质疏松发病机制的认识提供了新的理论依据。  相似文献

最近的全基因组关联研究发现ZBTB40(zinc finger and BTB domain containing 40)基因是一个潜在的调节骨密度的新基因,为了可靠地验证该基因与骨密度的关联关系,采用精细定位关联研究来检测ZBTB40基因内及其附近的SNPs与骨密度的关系.首先在中国样本(1 627个不相关的汉族样本)和美国样本(2 286个不相关高加索样本)中对ZBTB40基因的50个SNPs进行基因分型,然后采用Plink软件检测ZBTB40基因与腰椎和髋部骨密度的关联关系.证实了以前报道的SNPs rs7524102与腰椎和髋部骨密度的关联关系.另外还发现5个SNPs(rs10917209、rs6426748、rs4433361、rs3856183和rs7550872)与腰椎和髋部骨密度相关联.其中最显著的SNP是位于ZBTB40基因上游区域的rs6426748,其与腰椎骨密度的关联P值为9.82×10-4,多重检验校正后仍然显著.连锁不平衡分析表明rs6426748与rs10917209,rs7524102和rs7550872呈现高度连锁关系,位于长度为27 kb的单体型块内,因此ZBTB40基因的功能致病位点可能是rs6426748或该单体型块内某一未分型的SNP.  相似文献

目的:研究老年男性血清脂联素与骨密度和骨转化指标之间的关系。方法:对165例男性老年患者采用双能量X线吸收测量仪测定骨密度、肌肉及脂肪量,同时测定患者血清脂联素、骨碱性磷酸酶、甲状旁腺素、25羟维生素D和I型胶原β羧基端肽水平。结果:165例年龄超过58岁男性患者(平均年龄69.4±6.4岁,体重指数24.9±3.1 kg/m2),脂联素与股骨颈骨密度相关系数为-0.31(P〈0.05)、与全髋骨密度相关系数为-0.23(P〈0.05),年龄、BMI和脂肪量校正后,脂联素仅与股骨颈骨密度有显著相关(r=-0.25,P〈0.05);脂联素与骨碱性磷酸酶正相关(r=0.28,P〈0.01),混杂因素校正后,相关仍具有显著性(r=0.19,P〈0.05);脂联素与I型胶原β羧基端肽呈正相关(r=0.15,P〈0.05)。结论:老年男性血清脂联素与股骨颈骨密度和骨ALP密切相关。  相似文献

Background

A recent meta-analysis found that secreted phosphoprotein-1 (SPP1) can predict the risk of both osteoporosis and fracture. No study has explored the association of SPP1 haplotype-tagging single nucleotide polymorphisms (htSNPs) and haplotypes with bone mineral density (BMD).

Methods

This is a cross-sectional study. A total of 1,313 healthy Taiwanese women aged 40 to 55 years were recruited from MJ Health Management Institute from 2009 to 2010. BMD was dichotomized into high and low BMD groups. Three common (allele frequency ≥5%) htSNPs were selected to examine the association between sequence variants of SPP1 and BMD.

Results

Homozygosity for the T allele of rs4754 were protective from low BMD [TT vs. CC: adjusted OR (AOR)  = 0.58, 95% confidence interval (CI)  = 0.83–0.89]. A protective effect was also found for women carrying 2 copies of Hap3 TCT (AOR  = 0.57, 95% CI  = 0.34–0.95). Menopausal status marginally interacted with SPP1 rs6839524 on BMD (p = 0.049). Postmenopausal women carrying variant rs6839524 (GG+GC vs. CC: AOR  = 2.35, 95% CI  = 1.06–5.20) or Hap1 TGC (AOR  = 2.36, 95% CI  = 1.06–5.24) were associated with 2.4-fold risk of low BMD. For women with low BMI (<18.5 kg/m2), variant rs6839524 (AOR  = 7.64) and Hap1 (AOR  = 6.42) were associated with increased risk of low BMD. These findings did not reach statistical significance after correction for multiple tests.

Conclusions

SPP1 htSNP protected against low BMD in middle-aged women. SPP1 genetic markers may be important for the prediction of osteoporosis at an early age.  相似文献
