新型冠状病毒肺炎(Coronavirus disease 2019,COVID-19)是由新型冠状病毒(Severe acute respiratory syndrome coronavirus 2,SARS-CoV-2)引起的一种新的传染病。阻止COVID-19流行最有效的方法就是研制安全有效的疫苗,目前国内外多家机构开展了COVID-19疫苗的研究,包括核酸疫苗、病毒载体疫苗、灭活疫苗、重组蛋白疫苗、减毒流感病毒载体疫苗等不同类型,并取得了快速进展。本文对COVID-19疫苗的类型、研究进展、存在的问题等进行综述。  相似文献

The coronavirus disease 2019 (COVID-19) has been a global pandemic for more than 2 years and it still impacts our daily lifestyle and quality in unprecedented ways. A better understanding of immunity and its regulation in response to SARS-CoV-2 infection is urgently needed. Based on the current literature, we review here the various virus mutations and the evolving disease manifestations along with the alterations of immune responses with specific focuses on the innate immune response, neutrophil extracellular traps, humoral immunity, and cellular immunity. Different types of vaccines were compared and analyzed based on their unique properties to elicit specific immunity. Various therapeutic strategies such as antibody, anti-viral medications and inflammation control were discussed. We predict that with the available and continuously emerging new technologies, more powerful vaccines and administration schedules, more effective medications and better public health measures, the COVID-19 pandemic will be under control in the near future. The newly emerged coronavirus (severe acute respiratory syndrome coronavirus 2 SARS-CoV-2) and the disease that it causes coronavirus disease 2019 (COVID-19) have changed the world we know. Yet, the origin and evolution of SARS-CoV-2 remain mostly vague. Many virulence factors and immune mechanisms contribute to the deteriorating effects on the organism during SARS-CoV-2 infection. Both humoral and cellular immune responses are involved in the pathophysiology of the disease, where the principal and effective immune response towards viral infection is the cell-mediated immunity. The clinical picture of COVID-19, which includes immune memory and reinfection, remains unclear and unpredictable. However, many hopes are put in developing an effective vaccine against the virus, and different therapeutic options have been implemented to find effective, even though not specific, treatment to the disease. We can assume that the interaction between the SARS-CoV-2 virus and the individual's immune system determines the onset and development of the disease significantly.  相似文献

The etiopathogenesis of COVID-19 and its differential geographic spread suggest some populations are apparently ‘less affected’ through many host-related factors that involve angiotensin-converting enzyme 2 (ACE2) protein, which is also the entry receptor for SARS-CoV-2. The role of ACE2 has been well studied in COVID-19 but not in the context of malaria and COVID-19. We have previously suggested how malaria might intersect with COVID-19 through ACE2 mutation and here we evaluate the currently available data that could provide a link between the two diseases. Based on the existing global and Indian data on malaria, COVID-19 and the suggested ACE2 mutation, the association could not be examined robustly, neither accepting nor refuting the suggested hypothesis. We strongly recommend targeted evaluation of this hypothesis through carefully designed robust molecular epidemiological studies.  相似文献

2019年12月,武汉市发现多例不明原因肺炎病例,随后被证实由一种新型冠状病毒引起。2020年2月11日WHO将该疾病命名为COVID-19(Coronavirus disease 2019)。此后,疫情蔓延至全国。全球多个国家和地区也出现了COVID-19。COVID-19主要经呼吸道飞沫和密切接触传播,潜伏期1~14d,多为3~7d,大部分患者症状较轻,少数患者出现呼吸困难和/或低氧血症,甚至进展为急性呼吸窘迫综合征、多器官功能衰竭等。目前尚无针对该疾病的疫苗和特异性治疗药物。本文将从病原学、流行病学、致病机制、预防与治疗等方面对COVID-19进行综述,希望对疫情防控、药物和疫苗研究提供参考。  相似文献

Critically ill COVID-19 patients have higher pro-inflammatory (IL-1, IL-2, IL-6, tumor necrosis alpha) and anti-inflammatory (IL-4, IL-10) cytokine levels, less CD4 interferon-gamma expression, and fewer CD4 and CD8 cells. This severe clinical situation increases the risk of serious fungal infections, such as invasive pulmonary aspergillosis, invasive candidiasis or Pneumocystis jirovecii pneumonia. However, few studies have investigated fungal coinfections in this population. We describe an update on published reports on fungal coinfections and our personal experience in three Spanish hospitals. We can conclude that despite the serious disease caused by SARS-CoV-2 in many patients, the scarcity of invasive mycoses is probably due to the few bronchoscopies and necropsies performed in these patients because of the high risk in aerosol generation. However, the presence of fungal markers in clinically relevant specimens, with the exception of bronchopulmonary colonization by Candida, should make it advisable to early implement antifungal therapy.  相似文献

ObjectiveCase reports of postvaccine early-onset Graves’ hyperthyroidism (PVGD) after the administration of COVID-19 vaccination have emerged. Our aim was to investigate whether the incidence of Graves’ hyperthyroidism (GD) has increased after the introduction of COVID-19 vaccination.MethodsWe compared the incidence of new-onset GD at a single academic center during 2 periods: December 2017 to October 2019 and December 2020 to October 2022, ie, before and after the implementation of COVID-19 vaccinations. We defined PVGD as laboratory-confirmed hyperthyroidism and GD within 4 weeks after the vaccination or clear onset of symptoms of thyrotoxicosis within 4 weeks of vaccination with evidence of hyperthyroidism and GD within 3 months.ResultsDuring the prevaccination period, 803 patients carried diagnoses of GD, and of these, 131 were new. During the postvaccination period, 901 patients carried diagnoses of GD, and of these, 138 were new. There was no statistically significant difference in the incidence of GD (P = .52), age at onset, gender, or race between the 2 groups. Twenty-four of 138 newly diagnosed patients in the post–COVID-19 group met the criteria for PVGD. The median free T4 was higher, but this was not statistically significant (3.9 vs 2.5 ng/dL, P = .05). There were no differences in age, gender, race, antibody titers, or type of vaccination between PVGD and controls.ConclusionThere was no increase in new-onset GD after COVID-19 vaccination. Median free T4 was higher in patients with PVGD, but this was not statistically significant.  相似文献

2019年12月,湖北省武汉市暴发了一种由新型冠状病毒引起的肺炎(COVID-19)随后蔓延到中国所有省区以及多个国家和地区,WHO宣布其为全球关注的突发公共卫生事件。及时了解和表征该病毒对抗击疫情至关重要。通过收集和梳理该疫情暴发之后2个月内全球多领域研究人员关于COVID-19的论文,发现这些研究主要集中在溯源分析、检测手段、病患治疗、临床表现、药物研发、致病机理、传播途径、流行特征等方面。该冠状病毒与蝙蝠冠状病毒以及穿山甲冠状病毒相关,其利用与SARS-CoV相同的人受体ACE2,感染途径证实为呼吸系统和消化系统。该病毒具有人际传播能力,并且出现无症状传播。COVID-19确诊病例多数与武汉有关,大多数为轻症,年老者病死率较高。快速灵敏核酸检测通常作为确诊依据,目前已经筛选出有价值的候选药物如瑞德西韦进行临床试验。中国采取的旅行禁令和隔离等干预措施有效减轻了疫情蔓延。  相似文献
