摘要 目的:探讨抑郁症患者的脑CT灌注成像特征与认知功能的相关性。方法:选取我院2020年1月到2023年1月收治的90例抑郁症患者作为研究对象,将其分为观察组,另选取同期来我院体检的90名健康志愿者作为对照组。收集所有受检者脑CT灌注成像检查数据,分析抑郁症患者的脑CT灌注成像特征,并建立受试者工作特征(ROC)曲线分析脑CT灌注成像对抑郁症的诊断效能。随后对观察组和对照组受检者均进行认知功能评估,其中包括连线检测(TMT)、视觉再生测验(VRT)、言语流畅性测验(VF)、数字广度测验(DST)以及数字符号测验(SDMT),并分析脑CT灌注成像与抑郁症认知功能的相关性。结果:观察组与对照组受检者rCBV、rCBF、MTT、TIP、右枕叶、左枕叶、右颞叶、左颞叶、右顶叶、左顶叶CT值对比无明显差异(P>0.05),观察组与对照组受检者右额叶、左额叶CT值对比差异显著,观察组明显低于对照组(P<0.05);90例抑郁症患者经过汉密尔顿抑郁量表(HAMD)评估后分数均>20分,确定存在抑郁症状,脑CT灌注成像与HAMD评分诊断抑郁症的准确性、灵敏度、特异性、阳性预测值和阴性预测值对比无明显差异(P>0.05),脑CT灌注成像的曲线下面积为83.89,最佳诊断着色界限值为82.53%,HAMD评分的曲线下面积为84.26,最佳诊断着色界限值为87.57%;观察组与对照组受检者连线提笔数、连线错误数、视觉再生检测结果对比无明显差异(P>0.05),观察组与对照组受检者连线、言语流畅性、数字广度、数字符号检测结果对比差异显著(P<0.05);Spearman相关分析结果表明:连线提笔数、连线错误数、视觉再生与脑CT灌注参数均无明显相关性(P>0.05),连线、言语流畅性、数字广度、数字符号与rCBV、rCBF、MTT、TIP、右枕叶、左枕叶、右颞叶、左颞叶、右顶叶、左顶叶CT值无明显相关性(P>0.05),连线与右额叶、左额叶CT值呈负相关(P<0.05),言语流畅性、数字广度、数字符号与右额叶、左额叶CT值呈正相关(P<0.05)。结论:抑郁症患者的脑CT灌注成像与健康群体呈现差异,其中右额叶、左额叶差异情况最为显著,提示抑郁症患者可能存在大脑额叶功能改变,另外,抑郁症患者的大脑额叶功能与认知功能变化具有明显相关性。  相似文献

Since early neuropathological findings, temporal lobe has been related to the pathophysiology of some disorders with psychotic features. The aim of this study was to compare temporal lobe volumes and asymmetry differences in patients with schizophrenia, schizoaffective and bipolar disorders, disorders that cover the whole psychotic spectrum. Temporal lobe volumes were estimated using high resolution magnetic resonance imaging in 60 subjects, 15 subjects in each patient and one healthy volunteer (control) group. There are no statistically significant differences in temporal lobe volumes among patient and control groups. Comparison of left and right temporal lobes shows that left temporal lobes are smaller than right temporal lobes, however this difference reaches statistical significance only in groups of patients with schizoaffective and bipolar disorders. Overall temporal lobe volume may be less informative in respect to neuropathology of disorders with psychotic features than volumes of specific temporal lobe structures, in particular medial temporal lobe structures.  相似文献

Functional magnetic resonance imaging (fMRI) was used to demonstrate the brain activation during volitional control of breathing in nine healthy human subjects. This type of breathing was induced by acoustic stimuli dictating the respiratory frequency. During the period of dictated breathing not only the frontal and temporal lobes of the brain, but also the parietal lobes were bilaterally activated. The frontal lobe was activated bilaterally in all subjects, with frequent activation of Brodmann areas 4 and 6. In the parietal lobe, activation could mostly be demonstrated in gyrus postcentralis and the same was true for area 22 in the temporal lobe.  相似文献

ABSTRACT: BACKGROUND: Post-stroke mania is an infrequent complication after stroke, and the mechanisms underlying this disorder remain unclear. Although a contralesional release phenomenon has been implicated in post-stroke mania, empirical findings are lacking. Case Presentation: we preset a case report of post stroke mania. Single photon emission tomography (SPECT) was performed twice, during the manic state and during the remitted euthymic state.The first SPECT study performed during the manic state demonstrated hypoperfusion in the right temporal and frontal regions due to right putaminal hemorrhage. It also showed hyperperfusion in the inferior lateral prefrontal lobe, the temporal lobe, and the medial and lateral parts of the parietal lobe in the left hemisphere. The second SPECT study performed during the euthymic state demonstrated moderate improvement in the hypoperfusion in the right fronto-temporal regions. Furthermore, compared to the findings on the first SPECT study, the second study showed that the focal hyperperfusion in the anterior insular cortex, inferior lateral prefrontal lobes, and superior-middle temporal gyrus in the left hemisphere had vanished. CONCLUSION: Increased left inferior prefrontal and anterior insular activity and reduced extensive right fronto-temporal lobe activity are involved in the development of post-stroke mania.  相似文献

Background

The functional architecture of the human brain has been extensively described in terms of functional connectivity networks, detected from the low–frequency coherent neuronal fluctuations that can be observed in a resting state condition. Little is known, so far, about the changes in functional connectivity and in the topological properties of functional networks, associated with different brain diseases.

Methodology/Principal Findings

In this study, we investigated alterations related to mesial temporal lobe epilepsy (mTLE), using resting state functional magnetic resonance imaging on 18 mTLE patients and 27 healthy controls. Functional connectivity among 90 cortical and subcortical regions was measured by temporal correlation. The related values were analyzed to construct a set of undirected graphs. Compared to controls, mTLE patients showed significantly increased connectivity within the medial temporal lobes, but also significantly decreased connectivity within the frontal and parietal lobes, and between frontal and parietal lobes. Our findings demonstrated that a large number of areas in the default-mode network of mTLE patients showed a significantly decreased number of connections to other regions. Furthermore, we observed altered small-world properties in patients, along with smaller degree of connectivity, increased n-to-1 connectivity, smaller absolute clustering coefficients and shorter absolute path length.

Conclusions/Significance

We suggest that the mTLE alterations observed in functional connectivity and topological properties may be used to define tentative disease markers.  相似文献

Characteristics of spatial-temporal relations of the frontal zones potentials with those of other brain centres were studied in five- to six-year old children during naming of visually presented objects. EEG cross correlation analysis has shown that during visual perception of familiar objects the correlations of frontal areas with the interior pariental area of the left hemisphere are enhanced; the rhythms of the inferior parietal and occipital zones in the left hemisphere precede the potentials in the frontal area, while in the right hemispere synphasic relations are set up between the frontal and occipital zones. Naming the objects by the appropriate word as compared with rest and showing of the object, leads to enhancement of both intra- and interhemispheric correlations between potentials in the frontal zone and the inferior parietal and temporal zones. The frontal lobe rhythms begin to precede the inferior parietal and occipital potentials and form synphasic relations with the temporal and motor zone potentials in the left hemisphere.  相似文献

Auditory verbal hallucinations (AVH) are not only among the most common but also one of the most distressing symptoms of schizophrenia. Despite elaborate research, the underlying brain mechanisms are as yet elusive. Functional MRI studies have associated the experience of AVH with activation of bilateral language-related areas, in particular the right inferior frontal gyrus (rIFG) and the left superior temporal gyrus (lSTG). While these findings helped to understand the neural underpinnings of hearing voices, they provide little information about possible brain mechanisms that predispose a person to experience AVH, i.e. the traits to hallucinate. In this study, we compared resting state connectivity between 49 psychotic patients with chronic AVH and 49 matched controls using the rIFG and the lSTG as seed regions, to identify functional brain systems underlying the predisposition to hallucinate. The right parahippocampal gyrus showed increased connectivity with the rIFG in patients as compared to controls. Reduced connectivity with the rIFG in patients was found for the right dorsolateral prefrontal cortex. Reduced connectivity with the lSTG in patients was identified in the left frontal operculum as well as the parietal opercular area. Connectivity between the lSTG and the left hippocampus was also reduced in patients and showed a negative correlation with the severity of hallucinations. Concluding, we found aberrant connectivity between the seed regions and medial temporal lobe structures which have a prominent role in memory retrieval. Moreover, we found decreased connectivity between language-related areas, indicating aberrant integration in this system potentially including corollary discharge mechanisms.  相似文献

目的:应用CT灌注成像技术观察帕金森病合并抑郁患者局灶脑血流灌注的特点,进一步探讨抑郁症发生与脑血流的关系.方法:将41例帕金森患者根据是否合并抑郁症分为帕金森病组22例、帕金森病合并抑郁症者为抑郁组19例、其中抑郁组分为经颅磁刺激(rTMS)治疗前组、治疗后组,3组均进行CT局部脑血流灌注显像,半定量分析各脑区血流灌注情况.结果:帕金森合并抑郁症组患者双侧额叶、颞叶和基底节的脑血流量测定(CBF)较帕金森病组显著下降(P<0.05);抑郁组左、右侧脑血流低灌注存在不对称性,左侧额叶、顶叶的CBF较右侧显著下降(P<0.01);rTMS治疗后脑血流灌注较治疗前改善,HAMD评分改善(P<0.05).结论:帕金森患者存在局灶性脑血流灌注降低,合并抑郁症患者额、顶叶下降更明显,经颅磁刺激治疗后脑血流低灌注改善.  相似文献

带状疱疹后遗神经痛(postherpetic neuralgia,PHN)是临床上一种慢性顽固性神经病理性疼痛,然而,对于其潜在的中枢机制还知之甚少.为了进一步探讨带状疱疹后遗神经痛患者的相关脑区活动,利用功能核磁共振成像低频振幅振荡(ALFF)技术观察带状疱疹后遗神经痛患者的基础脑区活动.8名带状疱疹后遗神经痛患者与8名性别、年龄相匹配的健康者行静息态功能磁共振(f MRI)成像扫描,用SPM8中的多重回归分析,在控制被试年龄、性别、教育年限的影响下,将每个体素的ALFF值同每个被试的病程、视觉模拟评分(visual analog scale,VAS)进行相关分析.与健康志愿者相比,PHN组与VAS评分相关的ALFF值增高的脑区有:右侧小脑后叶、前额叶背外侧区域(BA11/46/47)、右侧顶叶(BA40)、右侧舌回(BA17/18/19);与VAS评分相关的ALFF值降低的脑区有:右侧颞中回(BA21)、左侧舌回(BA17/18)、右侧小脑前叶、左侧后扣带回(BA30/19)和右侧中央前回(BA3/4/6);PHN组与病程相关的ALFF值增高的脑区有:右侧小脑后叶、前额叶背外侧区域(BA9/10/11/47)、左侧颞上回(BA38)、右侧顶叶和右侧舌回(BA17/18/19);与病程相关ALFF值降低的脑区有:左侧海马旁回(BA28)、右侧小脑前叶、左侧扣带回(BA24)、右侧颞上回(BA13)、左侧中央前回和右侧顶下小叶(BA39/40).研究结果提示,涉及疼痛的情绪、警觉行为、注意的脑区在带状疱疹后遗痛慢性疼痛的产生和维持中发挥重要作用.  相似文献

It is a commonly held view that numbers are represented in an abstract way in both parietal lobes. This view is based on failures to find differences between various notational representations. Here we show that by using relatively smaller voxels together with an adaptation paradigm and analyzing subjects on an individual basis it is possible to detect specialized numerical representations. The current results reveal a left/right asymmetry in parietal lobe function. In contrast to an abstract representation in the left parietal lobe, the numerical representation in the right parietal lobe is notation dependent and thus includes nonabstract representations. Our results challenge the commonly held belief that numbers are represented solely in an abstract way in the human brain.  相似文献

The study of multiple indices of diffusion, including axial (DA), radial (DR) and mean diffusion (MD), as well as fractional anisotropy (FA), enables WM damage in Alzheimer's disease (AD) to be assessed in detail. Here, tract-based spatial statistics (TBSS) were performed on scans of 40 healthy elders, 19 non-amnestic MCI (MCIna) subjects, 14 amnestic MCI (MCIa) subjects and 9 AD patients. Significantly higher DA was found in MCIna subjects compared to healthy elders in the right posterior cingulum/precuneus. Significantly higher DA was also found in MCIa subjects compared to healthy elders in the left prefrontal cortex, particularly in the forceps minor and uncinate fasciculus. In the MCIa versus MCIna comparison, significantly higher DA was found in large areas of the left prefrontal cortex. For AD patients, the overlap of FA and DR changes and the overlap of FA and MD changes were seen in temporal, parietal and frontal lobes, as well as the corpus callosum and fornix. Analysis of differences between the AD versus MCIna, and AD versus MCIa contrasts, highlighted regions that are increasingly compromised in more severe disease stages. Microstructural damage independent of gross tissue loss was widespread in later disease stages. Our findings suggest a scheme where WM damage begins in the core memory network of the temporal lobe, cingulum and prefrontal regions, and spreads beyond these regions in later stages. DA and MD indices were most sensitive at detecting early changes in MCIa.  相似文献

Recent neuroimaging studies have shown that people with subcortical vascular cognitive impairment (sVCI) have structural and functional abnormalities in the frontal lobe and subcortical brain sites. In this study, we used seed-based resting-state functional connectivity (rsFC) analysis and voxel-mirrored homotopic connectivity (VMHC) techniques to investigate the alteration of rsFC in patients with sVCI. rsFC and structural magnetic resonance images were acquired for 51 patients with subcortical cerebrovascular disease. All patients were subdivided based on cognitive status into 29 with sVCI and 22 controls; patient characteristics were matched. rsFC of the posterior cingulate cortex (PCC) and VMHC were calculated separately, and rsFC of the PCC and VMHC between the two groups were compared. The regions showing abnormal rsFC of the PCC or VMHC in sVCI patients were adopted as regions of interest for correlation analyses. Our results are as follows: The patients with sVCI exhibited increases in rsFC in the left middle temporal lobe, right inferior temporal lobe and left superior frontal gyrus, and significant decreases in rsFC of the left thalamus with the PCC. sVCI patients showed a significant deficit in VMHC between the bilateral lingual gyrus, putamen, and precentral gyrus. Additionally, the z-memory score was significantly positively associated with connectivity between the left thalamus and the PCC (r = 0.41, p = 0.03, uncorrected) in the sVCI group. Our findings suggest that the frontal lobe and subcortical brain sites play an important role in the pathogenesis of sVCI. Furthermore, rsFC between the left thalamus and the PCC might indicate the severity of sVCI.  相似文献

Normative data on the in vivo size of the human brain and its major anatomically defined subdivisions are not readily available. In this study, high-resolution magnetic resonance imaging was used to measure regional brain volumes in 46 normal, right-handed adults (23 men, 23 women) between the ages of 22-49 years. Parcellation of the brain was based on neuroanatomical landmarks. The following brain regions were measured: the cerebral hemispheres, frontal lobe, temporal lobe, parietal lobe, occipital lobe, cingulate gyrus, insula, cerebellum, corpus callosum, and lateral ventricles. Males tend to be significantly larger than females, for the whole brain and for nearly all of its major subdivisions, including the corpus callosum. However, the proportional sizes of regions relative to total volume of the hemisphere are remarkably similar in males and females. Variation in size of region is always greater than variation in proportional representation. Asymmetries in brain regions are not profound, with the exception of the cingulate gyrus, which is larger in the left hemisphere. Brain regions are highly correlated in size, with the exception of the lateral ventricles. After controlling for hemisphere size, the volumes of the frontal and parietal lobes are significantly negatively correlated. The occipital lobe tends to be less sexually dimorphic than other major lobes, and less correlated with other brain regions for volume. These results have implications for understanding whether or not certain sectors of the brain have shown relative expansion over the course of hominid and hominoid evolution.  相似文献

Background

To identify changes in brain activation patterns in bipolar disorder (BD) and unipolar depression (UD) patients.

Methodology/Principal Findings

Resting-state fMRI scans of 16 healthy controls, 17 BD and 16 UD patients were obtained. T-test of normalized regional homogeneity (ReHo) was performed in a voxel-by-voxel manner. A combined threshold of á = 0.05, minimum cluster volume of V = 10503 mm3 (389 voxels) were used to determine ReHo differences between groups. In UD group, fMRI revealed ReHo increases in the left middle occipital lobe, right inferior parietal lobule, right precuneus and left convolution; and ReHo decreases in the left parahippocampalgyrus, right precentralgyrus, left postcentralgyrus, left precentralgyrus and left cingulated. In BD group, ReHo increases in the right insular cortex, left middle frontal gyrus, left precuneus, left occipital lobe, left parietal, left superior frontal gyrus and left thalamus; and ReHo decreases in the right anterior lobe of cerebellum, pons, right precentralgyrus, left postcentralgyrus, left inferior frontal gyrus, and right cingulate. There were some overlaps in ReHo profiles between UD and BD groups, but a marked difference was seen in the thalamus of BD.

Conclusions/Significance

The resting-state fMRI and ReHo mapping are a promising tool to assist the detection of functional deficits and distinguish clinical and pathophysiological signs of BD and UD.  相似文献

In recent years, amnestic mild cognitive impairment (aMCI) has attracted significant attention as an indicator of high risk for Alzheimer''s disease. An understanding of the pathology of aMCI may benefit the development of effective clinical treatments for dementia. In this work, we measured the cortical thickness of 109 aMCI subjects and 99 normal controls (NC) twice over two years. The longitudinal changes and the cross-sectional differences between the two types of participants were explored using the vertex thickness values. The thickness of the cortex in aMCI was found significantly reduced in both longitudinal and between-group comparisons, mainly in the temporal lobe, superolateral parietal lobe and some regions of the frontal cortices. Compared to NC, the aMCI showed a significantly high atrophy rate in the left lateral temporal lobe and left parahippocampal gyrus over two years. Additionally, a significant positive correlation between brain atrophy and the decline of Mini-Mental State Examination (MMSE) scores was also found in the left superior and left middle temporal gyrus in aMCI. These findings demonstrated specific longitudinal spatial patterns of cortical atrophy in aMCI and NC. The higher atrophy rate in aMCI might be responsible for the accelerated functional decline in the aMCI progression process.  相似文献

本文研究了在保留最大化内在功能连接条件下抑郁症患者脑网络效率的改变,并探索了改变的拓扑效率和抑郁症病理学之间的关系.为此,我们收集了20例抑郁症患者和20例在年龄、性别和教育水平相匹配的健康被试的静息态功能磁共振图像数据.图论分析显示,与健康对照组比较,抑郁症患者的节点效率减少在左海马旁回、右杏仁核,左颞横回和左颞极(颞中回)减少.减少的节点效率表明,在抑郁症患者脑网络中这些区域传送信息到其他区域的能力减弱.此外,发现局部效率降低在左内侧额上回、左眶部额上回、右回直肌、左杏仁核、右顶上回、左丘脑和左颞极(颞中回).并且发现左内侧额上回、左杏仁核、左丘脑与PHQ-9得分呈负相关.降低的局部效率表明抑郁症患者脑网络中这些区域的局部网络信息传送能力受到抑制.这些结果进一步确认在抑郁症患者中涉及情感信息处理的前额-丘脑-边缘区域被破坏.我们的发现为抑郁症病人的辅助诊断提供了新的潜在生物学标记物.  相似文献

Significant differences in the area of cell nuclei of the left and right lobes of the interrenal glad have been revealed within the spawning period in two flounder species—the the barfin plaice Liopsetta pinnifasciata and the Korean flounder Glyptocephalus stelleri, which testifies to the different functional activities of these lobes and, accordingly, about the functional asymmetry of the interrenal glad. Individuals with a functionally more active right lobe of the gland prevail for the barfin plaice, whereas with the Korean flounder the number of individuals with a more active left lobe is equal to the number of individuals with a more active right lobe.  相似文献

An EEG cross-correlation analysis has shown that in children aged four to five years higher sensory analysis of verbal commands and their meaning was reflected in the nature of synchronous interactions between oscillatory processes and their spatial-temporal patterns. At the moment of perception of the command "listen" highly synchronous synphasic relations were recorded between biopotentials in the associative infero-parietal cortex and projection temporal centres of the left hemisphere. Oscillations of the parietal areas preceded the rhythms of the occipital, motor and frontal lobes in the left hemisphere; slow oscillations with a 3 osc/sec frequency predominated, and the intensity of the periodic processes increased. The command "look" evoked a high degree of synchronous synphasic relations of biopotentials in the parietal-occipital cortical parts of both hemispheres; oscillations with 6 osc/sec frequency predominated; their intensity rose; synphasic relations of oscillations in parietal and motor and temporal centres grew more manifest, while the rhythmic activity in the parietal zones preceded the potentials in the frontal lobes of both hemispheres.  相似文献

The contribution of each cortical zone to the organization of spatial synchronization of brain bioelectric potential s (SSBP) was estimated using cross-correlation matrices of 20-channell EEG during reading in boys and girls aged 12–13 years. Differences in the topical distribution of the SSBP levels in young boys and girls were revealed both in the resting state and during reading. In boys, the SSBP level in the left temporal lobe was lower than that in girls, and in the right parietal lobe it was higher than that in girls in all the states studied. A decrease of SSBP in central cortex zones was revealed, indicating the activation of motor cortical areas in all subjects during reading. Enhancement of SSBP in the parietal regions of the left hemisphere and the occipital zones of both hemispheres is observed in boys during reading, while in girls SSBP is increased bilaterally in the frontotemporal areas with the involvement of the left frontal lobe. Moreover, during reading intrahemispheric correlations between EEGs of the central and temporoparietal regions with emphasis on the right hemisphere were more pronounced in boys, while both intra- and interhemispheric correlations of the left temporal regions were pronounced in girls.  相似文献
