Background

The causation of asthma is poorly understood. Risk factors for atopic and non-atopic asthma may be different. This study aimed to analyze the associations between markers of poverty, dirt and infections and wheezing in atopic and non-atopic children.

Methods

1445 children were recruited from a population-based cohort in Salvador, Brazil. Wheezing was assessed using the ISAAC questionnaire and atopy defined as allergen-specific IgE ≥0.70 kU/L. Relevant social factors, environmental exposures and serological markers for childhood infections were investigated as risk factors using multivariate multinomial logistic regression.

Results

Common risk factors for wheezing in atopic and non-atopic children, respectively, were parental asthma and respiratory infection in early childhood. No other factor was associated with wheezing in atopic children. Factors associated with wheezing in non-atopics were low maternal educational level (OR 1.49, 95% CI 0.98-2.38), low frequency of room cleaning (OR 2.49, 95% CI 1.27-4.90), presence of rodents in the house (OR 1.48, 95% CI 1.06-2.09), and day care attendance (OR 1.52, 95% CI 1.01-2.29).

Conclusions

Non-atopic wheezing was associated with risk factors indicative of poverty, dirt and infections. Further research is required to more precisely define the mediating exposures and the mechanisms by which they may cause non-atopic wheeze.  相似文献

House-dust mite allergy is a fairly common problem in West Bengal among individuals sensitive to dust inhalation. House-dust mites belonging to the genusDermatophagoides are abundant in the homes of asthmatic patients residing in urban as well as rural areas of West Bengal. The frequency of positive skin reaction to different dust-related allergens tested was higher (χ2=5.4777, df = 1;P < 0.05) among patients of urban areas compared with that among the patients of rural areas. Urban patients showed more frequent skin reaction towards cockroach allergen, while rural patients are more sensitive to hay-dust allergen and these are very much related to their local environmental conditions. Analysis of radioallergosorbent test (RAST) results against house dust (HD) and mites reveal that 73 and 90% patients of both urban and rural areas responded positively towardsDermatophagoides pteronyssinus (DP) andDermatophagoides farinae (DF) antigens, respectively. The present study indicates no significant difference in house-dust mite sensitivity and mite levels in homes among the rural and urban asthmatics of West Bengal, India as evidenced from the results of analysis of dust samples, allergy skin test and detection of mite-specific IgE antibodies by RAST.  相似文献

Weight, stature, arm circumference, and the triceps skinfold were measured in 1,410 school children, 6 through 14 years of age, from two urban colonias in the city of Oaxaca de Juarez (n = 479), and from two rural Ladino (n = 467) and two rural Zapotec (n = 464) communities in the Valley of Oaxaca in southern Mexico. Children from rural Ladino communities and urban colonias are significantly taller, heavier, and more muscular than children in rural Zapotec communities. The differences between rural Ladino and urban colonia children favor the former, particularly for weight and stature. These observations thus suggest (1) that children in the rural, indigenous communities in the Valley of Oaxaca are relatively undernourished compared to children in Ladinoized and urban communities, and (2) that rural-to-urban migration does not necessarily result in improved growth status.  相似文献

OBJECTIVE--To compare proportions of low birthweight babies and mean heights of schoolchildren between rural and urban areas at different levels of social deprivation. DESIGN--Cross sectional population based study classifying cases by Townsend material deprivation index of enumeration district of residence and by rural areas, small towns, and large towns. SETTING--Northumberland Health District. SUBJECTS--18,930 singleton infants delivered alive during January 1985 to September 1990 and resident in Northumberland in October 1990; 9055 children aged 5 to 8 1/2 years attending Northumberland schools in the winter of 1989-90. MAIN OUTCOME MEASURES--Odds ratios for birth weight less than 2800 g; difference in mean height measured by standard deviation (SD) score. RESULTS--Between the most deprived and most affluent 20% of enumeration districts the odds ratio for low birth weight adjusted for rural or urban setting was 1.71 (95% confidence interval 1.51 to 1.93) and the difference in mean height -0.232 SD score (-0.290 to -0.174). Between large towns and rural areas the odds ratio for low birth weight adjusted for deprivation was 1.37 (1.23 to 1.53) and the difference in mean height -0.162 SD score (-0.214 to -0.110). Results for small towns were intermediate between large towns and rural areas. CONCLUSIONS--Inequalities in birth weight and height exist in all rural and urban settings between deprived and affluent areas. In addition, there is substantial disadvantage to living in urban areas compared with rural areas which results from social or environmental factors unrelated to current levels of deprivation.  相似文献

BACKGROUND:

Sickle cell disease (SCD) is a prevalent genetic disorder in India and the rural and urban areas experience distinctly different healthcare facilities. In view of this, a comparative study of SCD-SS pattern children of age 8–15 years from rural and urban areas of Wardha district of Central India was carried out using anthropometric and hematological parameters.

MATERIALS AND METHODS:

The data were collected using standard methods and the results showed a significant (P < 0.05) difference in the mean values for body weight, body mass index (BMI), hemoglobin, hematocrit, and white blood corpuscles (WBC). Statistical analysis of the data was done using SPSS 18.0 software. Individuals were screened by solubility test method. Sickle cell patterns (AS and SS) were determined by using electrophoresis technique.

RESULT:

The SCD-SS children from rural were significantly underweight than those from the urban area of Wardha district. BMI is a good indicator of nutritional status and BMI values of SCD children have less than desired.

CONCLUSION:

The study highlights an urgent need to conduct integrated investigations for SCD population of rural areas covering clinical, nutritional, and social aspects.  相似文献

451 rural children (group I) and 2000 urban children (group II) aged 10-16 years from Toruń province were inquired by a questionnaire to their parents or guardians. 9.09% of children in the country and 13.45% of those living in the city of Toruń suffered from hypersensitivity disorders; bronchial asthma was reported in 2.22% and 3.05% of cases, allergic rhinitis--in 3.77% and 7.15%, allergic conjunctivitis--in 1.33% and 2.75%, allergic edema--in 0.44% and 0.60%, urticaria--in 1.55% and 3.50%, and infantile eczema--in 0.44% and 2.10% of cases, respectively. The pollinosis prevalence rate was 2.00% in group I and 1.85% in group II. At least 2 various forms of hypersensitivity coexisted in 30.48% of allergic urban children (64.86% of patients with pollinosis among them); infantile eczema preceded allergic rhinitis and bronchial asthma symptoms in 6.29% and 11.47% of cases, respectively, while allergic rhinitis occurred before the onset of bronchial asthma in 24.59% of asthmatic children. 37.10% of individuals with positive family history of allergic conditions also fell ill with some diseases of this nature, while in those with negative family history allergy occurred only in 10.22% of cases. From environmental factors mother's diseases during pregnancy, bottle feeding and a regular diet during the first year of life, frequent respiratory infections in the early childhood and poor living conditions increased the risk of allergic diseases or aggravated their course in the population examined.  相似文献

The purposes of the current study were: (1) to describe growth and physical development and establish norms for schoolchildren from Rostov region in Russia; (2) to compare major characteristics of development between urban and rural children by sex and age.Nearly 200,000 children (198,712) aged between 7 and 17 years from 232 urban and rural schools of Rostov region (Southern Federal District of Russia) participated in the study. School age is a period of intensive growth and physiological and psychological development. Irregularities of personal development are caused by a multitude of factors, such as sex differences, heredity, socio-economic status of a family, standard of living, particular environmental conditions, and lifestyle.It has been established that children from the Southern Federal District of Russia had body mass index values higher than age-appropriate norms for all Russians (Total Russian, Rudnev et al., 2014) and World Health Organization charts. Children from urban settings were taller and heavier than children from rural settings.Sex is one of the most influential factors which play key role in determining specific characteristics of growth and personal development. According to our results, boys and girls both had similar age-related changes in weight and height, but their respective dynamics differed. Girls’ height and weight values accelerated at the age 10 to 12 years and plateaued after the age fourteen, whereas in boys height and weight steadily increased with age, showing slight acceleration at the age 12 to 13 years, and reached a plateau by the age of seventeen.  相似文献

Abstract. Mosquitoes were collected on human bait over a 16-month period (September 1988 to December 1989) in an urban and a rural area of Kinshasa, Zaïre. P.falciparum malaria sporozoite rates were determined by ELISA. In the urban area Culex quinquefasciatus accounts for 96% of the 121 bites/ person/night (b/p/n). The only anopheline is Anopheles gambiae, sensu stricto, with an average of 5.1 b/p/n and a sporozoite rate of 1.86%. The entomological inoculation rate (EIR) averages 0.08 infective b/p/n. Malaria transmission is almost interrupted at the end of the dry season. In the rural area mosquito nuisance is small (20b/p/n), almost entirely due to six species of Anopheles including four vectors of malaria: An.gambiae (13.3 b/p/n), An.funestus (2.4b/p/n), An.nili (0.4b/p/n) and An.brunnipes (0.7b/p/n) with mean sporozoite rates of 7.85%, 6.60%, 6.63% and 0.53% respectively. An.paludis (0.4b/p/n) and An.hancocki (0.2b/p/n) were not found infective. Malaria transmission is intense and perennial: the overall EIR varies monthly between 0.60 and 3.29 infective b/p/n. The specific contributions of An.gambiae, An.funestus and An.nili average 1.07, 0.14 and 0.03 infective b/p/n respectively. Malaria transmission peaks during the rainy season in both study areas. The daily mean survival rates for An.gambiae were 0.91 and 0.78 in the rural and urban area, respectively. All An.gambiae examined belonged to the forest cytotype (Coluzzi et al., 1979). Through its effect on the sporozoite rate, the higher vector survival rate in the rural environment appears to be the major determinant of the greater malaria transmission rate in the rural area as compared to urban Kinshasa.  相似文献

The prevalence of head louse (Pediculus hamanus var. capitis) infestation in preschool and school children in 9 urban and 8 rural areas of Korea, was investigated. Of 11,865 children who were examined for head lice and nits, 2,900(24.4%) were found infested. The over-all infestation rate of rural children, 58.9%, was higher than that of urban children, 14.4%. There also existed significant differences in the infestation rate among various regions of urban as well as rural areas. Females were more infested than males. But there was no differences in infestation rate in either sex of preschool age. The infestation rate increased gradually from the age of 6, reached a plateau between 9-12 years of age and thereafter slowly decreased. The infestation rate of an orphanage children was higher than that of general childhood population. Health education and mass-delousing attempt are urgently required to lower such a high prevalence of head louse infestation in Korea.  相似文献

An epidemiological study and mass treatments of Enterobius vermicularis infection among children near Wonju area of Kangwon province were carried out. The children were divided into 4 groups according to their residing localities; children in the mountainous area, rural area, urban area and in orphanage. They were examined by adhesive cellotape anal swab technique, and egg positive rates were obtained. The rates of egg reduction and re-infection rates after repeated mass treatments were also observed. The results obtained were as follows: 1. The overall egg positive rate of E. vermicularis in the first screening was 19.9% (251 out of 1,262 examinees; 19.7% in males and 20.1% in females). The positive rates were 13.0% in the mountainous area, 11.9% in the rural area, 15.1% in the urban (medium-sized) area and 61.9% in orphanages. 2. The highest positive rates were observed in the kindergarten children, and 1st and 2nd grade children of primary schools (26.2-32.2%), and the lowest rate (13.6%) in 6-year grade children of primary schools. 3. Cumulative detection rates from 3 repeated anal swabs at 4-5 days interval were higher (70.8%) than those from single anal swabs (50.0-59.2%). 4. Out of the examinees who showed the highest cumulative positive rate (70.8%), about 39.2% were consecutively positive in 3 anal swabs. Among different groups of children, the higher the total egg detection rates (87.5%), the higher the consecutive positive rates (71.9%). 5. A total of 2,609 (male: female = 1:12.4) worms were collected from 17 egg-positive cases treated with anthelmintics. The harmful outcomes of nicotine self administration have been the focus of sustained global health education campaigns that have targeted tobacco smoking and to a lesser extent, smokeless tobacco use. 'Smokeless tobacco' infers that the nicotine is not burnt, and administration can be through a range of methods including chewing. The chewing of wild tobacco plants (Nicotiana spp.) is practiced across a broad inland area of Central Australia by traditional Aboriginal groups. Collectively these plants are known by a variety of names - one common name being 'pituri'. This is the first paper to examine the historical literature and consider the linkage between pituri use and health outcomes. Using a narrative approach, this paper reviews the literature generated since 1770 surrounding the term pituri and the behaviours associated with its use. The review examines the scientific literature, as well as the diaries and journals of nineteenth century explorers, expedition notes, and early Australian novels to expound the scientific evidence and broaden the sense of understanding related to pituri, particularly the behavioural elements. The evaluation considers the complexities of ethnobotany pertaining to language and distance and the ethnopharmacology of indigenous plant usage. The review compares the use of burnt and smokeless tobacco to pituri and establishes the foundation for research into the clinical significance and health outcomes of pituri use. Additionally, this review provides contemporary information for clinicians providing care for patients who chew pituri.  相似文献

Some demographic and epidemiological patterns of the rural population of Monte Negro, locality situated in the State of Rond?nia (Brazil), Western Amazonia, are described based on a sample of 924 randomly selected individuals, approximately 10% of the whole population. The main features of this sample are (1) the illiteracy rates in the parental generation were 23% for fathers and 20% for mothers. Among children, this figure dropped to 6%; (2) housing in Monte Negro is characterized by being constructed with wood (92%), and also a floor (75%). Nevertheless, only 32% of these houses had electric energy; (3) the mean ages for the parental generation were 41.9 for males and 36.3 for females. These values for the offspring generation were 12.2 and 10.5, respectively; (4) the sex-ratio of the offspring generation was 1.32;(5) the bioassay of kinship was estimated as.033 for this long range migrant population; (6) the prevalence of some macrophage dependent infectious disease was conspicuously high; (7) the reported number of malarial episodes among males and females was statistically different, suggesting that malaria may be, in part, a "professional" disease; (8) the prevalence of serum-positive reactions against B-hepatitis is distressing. It has a strong age dependence and reaches 74% among adult males. Conversely, signs of active infection (AgHbs) rises to 16% among children.  相似文献

Summary A histochemical and biochemical study of the activity of arylsulphatases A and B was carried out on the oviduct of female rabbits during the first days after mating. The histochemical results demonstrated that the ampullary and the isthmic epithelial cells have a positive reaction to the sulphatases during the whole of the postovulatory period tested. The enzymatic activity is mainly localized in the basal cellular cytoplasm. The biochemical results confirmed that both arylsulphatase A and B are active. Arylsulphatase A activity is more intense in the ampulla than in the isthmus and it increases during the whole of the postovulatory period; in the isthmus the activity increases up to 72 h, thereafter decreasing again. The arylsulphatase B activity is always lower than arylsulphatase A activity; maximum activity is reached between 66 to 72 h after mating. The arylsulphatase B is relatively higher in the ampulla than in the isthmus. The biological role of these enzymes is discussed in relation to the regulation of the sulphated glycoconjugates.  相似文献

The tree changes of 1.02 ha of montane forest at the Santa Lúcia Biological Station, southeastern Brazil, were analyzed using two surveys separated by an interval of 11 years with the aim of confirming the patterns of stability of structure and diversity over time. In the original survey all trees with diameter at breast height ≥6.4 cm were sampled. In second survey (this study), dead trees, survivors and recruits in the same forest were reported. The data suggest a dynamic balance of the forest structure because mortality (−1.06% year−1 for number of trees and −0.85% year−1 for basal area) was very close to recruitment (0.89% year−1) and ingrowth (1.05% year−1). The high diversity of the original survey (H′ > 5.2) was maintained by the turnover species. The main tree populations also showed stability of number of trees and basal area. This pattern was shared by most of the 28 local endemic species, ensuring the maintenance of their populations in the plot.  相似文献

Capture-recapture data were obtained for two populations of Great Tits Parus major in southeast Estonia, one rural, the other urban. These data were used to estimate and compare survival probabilities between sexes and populations using capture-recapture statistical models. As is generally assumed, urban birds survived better than rural ones. Females survived better than males, and the survival probabilities varied over time. The three sources of variation considered (sex, habitat and time) had additive effects on survival probability on a logit scale. The estimated annual survival probabilities varied from 0.19 to 0.70. The averages over time ranged from 0.26 to 0.47 and were generally lower than published estimates from other populations. The proportion of yearling breeders was higher among females than among males, contrary to what could have been deduced from survival patterns, and is possibly a consequence of complex recruitment mechanisms.  相似文献
