During 1989 to 1990, human sera were collected by age groups in Okinawa (the northern, central and southern areas), Miyako and Ishigaki islands and examined for the neutralization (N) antibodies to two strains, Nakayama (vaccine strain) and C307 (Okinawan strain), of Japanese encephalitis (JE) virus. In Okinawa island, the N antibody positive rate to C307 was higher than that to Nakayama, while in Miyako and Ishigaki islands, the positive rate to Nakayama was higher than that to C307, suggesting that JE virus transmission rate was higher in Okinawa than in Miyako and Ishigaki islands. In Okinawa Prefecture, JE vaccine had not been administered to most of residents over 31 years of age at the time of serum collection. In residents over 31 years old, the positive rate to C307 was highest in the north of Okinawa (83.3%) and was lowest in Miyako (26.3%), with the second lowest in Ishigaki (33.3%). The distribution of N antibody titers to C307 gave hyperbolic patterns in the 0–5 age groups in Miyako and Ishigaki, and also in the 31–40, 41–50 age groups in Miyako and the 41–50 age group in Ishigaki, suggesting low rates of natural infection in these 4–5 decades in both islands. In residents of ages subjected to JE vaccine, a characteristic pattern was obtained, in which the curves to Nakayama shifted to higher titers than those to C307, suggesting that the first antigenic stimulation was caused by vaccine, not by natural infection of JE virus.  相似文献

为了解儿童中流行性腮腺炎(腮腺炎)的免疫情况,观察腮腺炎减毒活疫苗的安全性和免疫原性。选取353名1~15岁儿童进行血清流行病学调查;对157名城、乡3~5岁儿童进行疫苗效果观察,并将其随机分成实验组和对照组;采用酶联免疫吸附试验(ELISA)检测血清IgG。接种疫苗24h、48h、72h、96h后观察人体不良副反应;并分别在接种前及接种后1个月,分别采集免疫对象静脉血测定抗体。353名1~15岁儿童中腮腺炎抗体阳性179人,总阳性率为50.71%;0~1岁、2~6岁、7~12岁和13~15岁4组的抗体阳性率分别为21.84%、36.17%、69.41%和77.01%。实验组44名抗体阴性儿童接种了腮腺炎减毒活疫苗,1个月后抗体阳转率为93.18%,IgG呈4倍增长者23人,4倍增长率为52.27%。无一例出现严重的不良副反应。  相似文献

摘要 目的:探究HIV抗体不确定及后续随访阳转样本的WB条带和流行病学特征。方法:对太原市范围内2017年至2022年监测到的790份HIV抗体不确定及71例后续随访阳性人员的WB条带和流行病学信息进行分析。结果:(1)HIV抗体不确定样本主要来自医院、血液中心和自愿咨询与检测(VCT)机构,占比84.47%;阳转样本主要来自医院和VCT,占88.73%;不确定人群中的性病门诊就诊者和VCT人群阳转率最高,分别为50%、25.49%。(2)不确定和阳转样本以20-40周岁人员为主,占53.8%;不确定人群中的20-30岁阳转率最高,占15.57%。(3)不确定样本在学历、性别方面无明显差异,但阳转样本男性占比远高于女性、高学历人群相对较高。(4)不确定样本以已婚、无高危行为、就业人群为主,阳转样本以具有未婚、离异、丧偶、阳性配偶、男男同性恋(MSM)、学生、无业或待业等特点人群为主。(5)不确定样本条带以P24和P17条带和带型为主,阳转样本条带以P24和P17为主,以含有P24和gP160条带的带型为主。结论:不确定样本中具有性病门诊就诊者、VCT、MSM、20-30岁、未婚、无业等流行病学因素和具有P24或gP160条带等特点的样本后续阳转可能性较大,应以上述人群为防控干预重点,重点加强具有上述特点不确定样本的个案跟踪和随访检测。  相似文献

为了观察口服轮状病毒活疫苗在儿童服用后的安全性和免疫原性效果。于惠州市选择63名6月龄~3岁的儿童为观察对象。所用疫苗由兰州生物制品研究所生产,服苗前和服苗后4~5周采末梢血,以中和试验检测抗轮状病毒(G1、G2、G3、G4)型抗体及疫苗株(LLR型)的抗体水平。63名儿童服苗前、后采集的血清样本双份配对均有效者为37人。37人服苗后各型抗体阳转率为47.06%~72.72%;≥4倍增长率为43.40%~66.04%;各型中和抗体免疫前后平均增长2.67~3.01倍。63名服苗儿童中的不良反应为低热3例(24.67%)、中热1例(1.59%)、无高热反应。观察结果表明,轮状病毒具有良好的免疫原性和安全性。  相似文献

A seroepidemiological survey of varicella was made in the Nagoya area by the neutralization (NT) test. Of 1,473 recorded cases of varicella, 81.4% were under 6 years old and 9.6% were under one year old; of the 168 recorded cases under one year old, about 30% were under 5 months old. Examination of 11 pairs of mother and cord sera and 13 pairs of mother and infant sera showed that transfer of NT antibody was in general good, even in babies that were small for their age or smnall at birth after 28 weeks gestation. The transferred maternal antibody decreased rapidly, becoming undetectable in babies of 4 months old. Then with increase in age the percentgage of seropositive children gradually increased, being 53.3% at 4 to 5 years old, and 100% in those of over 9 years old, with a temporary decrease in young adults in their twenties.  相似文献

目的 调查泉州地区手足口病(HFMD)流行的特征,为该地区HFMD的防治提供参考。方法 分析2014年9月至2015年9月泉州地区HFMD的发病季节、地区、性别、年龄及不同类型病毒感染的情况。结果 泉州地区一年四季均有新发HFMD病例,发病高峰在4~7月,次高峰为9~10月。发病人群以散居儿童多见;其中男性患者占64.9%,女性患者占35.1%,男女发病比例为1.9∶1,总体发病率为0.2%。患者中<5岁的儿童占94.9%,其中1~2岁占54.3%。脑炎病例多数患者年龄<2岁,尤其以1~2岁为多发。咽拭子标本检测发现,66.5%为EV71感染,6.8%为CoxA16感染,26.7%为其他病毒感染。多数病人有多处皮疹。结论 泉州地区HFMD病例在夏秋季明显增多,人口密集的地区发病率较高,以EV71感染最为多见。5岁以下儿童,特别是1~2岁儿童为HFMD易感人群。  相似文献

Background

High rates of typhoid fever in children in urban settings in Asia have led to focus on childhood immunization in Asian cities, but not in Africa, where data, mostly from rural areas, have shown low disease incidence. We set out to compare incidence of typhoid fever in a densely populated urban slum and a rural community in Kenya, hypothesizing higher rates in the urban area, given crowding and suboptimal access to safe water, sanitation and hygiene.

Methods

During 2007-9, we conducted population-based surveillance in Kibera, an urban informal settlement in Nairobi, and in Lwak, a rural area in western Kenya. Participants had free access to study clinics; field workers visited their homes biweekly to collect information about acute illnesses. In clinic, blood cultures were processed from patients with fever or pneumonia. Crude and adjusted incidence rates were calculated.

Results

In the urban site, the overall crude incidence of Salmonella enterica serovar Typhi (S. Typhi) bacteremia was 247 cases per 100,000 person-years of observation (pyo) with highest rates in children 5–9 years old (596 per 100,000 pyo) and 2–4 years old (521 per 100,000 pyo). Crude overall incidence in Lwak was 29 cases per 100,000 pyo with low rates in children 2–4 and 5–9 years old (28 and 18 cases per 100,000 pyo, respectively). Adjusted incidence rates were highest in 2–4 year old urban children (2,243 per 100,000 pyo) which were >15-fold higher than rates in the rural site for the same age group. Nearly 75% of S. Typhi isolates were multi-drug resistant.

Conclusions

This systematic urban slum and rural comparison showed dramatically higher typhoid incidence among urban children <10 years old with rates similar to those from Asian urban slums. The findings have potential policy implications for use of typhoid vaccines in increasingly urban Africa.  相似文献

This study aimed to determine the role of influenza-like illness (ILI) surveillance conducted on Leyte Island, the Philippines, including involvement of other respiratory viruses, from 2010 to 2013. ILI surveillance was conducted from January 2010 to March 2013 with 3 sentinel sites located in Tacloban city, Palo and Tanauan of Leyte Island. ILI was defined as fever ≥38°C or feverish feeling and either cough or running nose in a patient of any age. Influenza virus and other 5 respiratory viruses were searched. A total of 5,550 ILI cases visited the 3 sites and specimens were collected from 2,031 (36.6%) cases. Among the cases sampled, 1,637 (75.6%) were children aged <5 years. 874 (43.0%) cases were positive for at least one of the respiratory viruses tested. Influenza virus and respiratory syncytial virus (RSV) were predominantly detected (both were 25.7%) followed by human rhinovirus (HRV) (17.5%). The age distributions were significantly different between those who were positive for influenza, HRV, and RSV. ILI cases were reported throughout the year and influenza virus was co-detected with those viruses on approximately half of the weeks of study period (RSV in 60.5% and HRV 47.4%). In terms of clinical manifestations, only the rates of headache and sore throat were significantly higher in influenza positive cases than cases positive to other viruses. In conclusion, syndromic ILI surveillance in this area is difficult to detect the start of influenza epidemic without laboratory confirmation which requires huge resources. Age was an important factor that affected positive rates of influenza and other respiratory viruses. Involvement of older age children may be useful to detect influenza more effectively.  相似文献

Toxoplasma antibodies prevalence was studied in the north of Tunisia where a mild climate prevails. Two groups of individuals were investigated: 857 living in rural area and 564 living in urb town. Sera were analysed by ELISA and indirect immunofluorescence. The overall prevalence was 58.4%. It roses from 24.5% at ten years to 52.1% at 20 years of age. A maximum level, around 70%, was reached by about 30 years. The risk of acute infection after this age seemed low as judging by the proportion of high antibodies titers observed in this group (14.2% before 30 years vs 3.7% after). A significantly higher prevalence was detected in urban residents (67% vs 52.8%). In this group, the rate of seroconversion seems the highest between ten and 20 years of age and the majority of women are infected before reaching childbearing age. In the rural area, the seropositivity is lower between ten-20 years and many women at childbearing age still susceptible to toxoplasmosis. The risk of acute infection seems higher in the youngest ones as showed by the proportion of high antibodies titers observed in the 18-30 age group (9.2%) compared to the one observed after 30 years (1.9%).  相似文献

目的 了解河南农村秋冬季节5岁以下儿童急性胃肠炎流行现状和疾病负担。方法 采用单纯随机整群抽样方法,抽取7个行政村的所有5岁以下儿童共513名。结果 5岁以下儿童秋冬季急性胃肠炎2周发病率为9.4%,疾病负担平均为2.7151 DALYs/千人,年龄越小疾病负担越高;0岁组最高,为6.5839 DALYs/千人。患儿例均经济负担为284.84元,其中直接经济负担为211.83元,间接经济负担为72.98元。结论 5岁以下儿童秋冬季急性胃肠炎发病率随年龄增长呈下降趋势,疾病负担主要集中于3岁以下儿童(尤其0~1岁婴幼儿);秋冬季应积极预防病毒性急性胃肠炎,防控重点应在2岁以下儿童;早就诊早治疗,以减少住院病例,降低疾病负担。  相似文献

目的 了解6a来广州地区头癣类型及病原菌分布特点.方法 采用病例回顾性分析,对2007年7月~2013年9月来广州市中山大学附属第三医院皮肤科门诊就诊的头癣患儿(平均为6.0±3.6岁)真菌直接镜检、培养及分离鉴定的结果进行分析.结果 70例头癣患儿真菌镜检阳性为69例,阳性率为99%,真菌培养阳性为47例,阳性率为81%.白癣为52例,占74%.病原菌包括犬小孢子菌19例(40%)、须癣毛癣菌13例(28%)、紫色毛癣菌5例(11%),石膏样小孢子菌3例(6%),其他病原菌为7例(15%).其中,4~7岁的头癣患儿为35例,占50%.结论 儿童头癣的主要临床类型为白癣.致病菌以亲动物性犬小孢子菌为主.患儿的主要年龄段为4~7岁.  相似文献

The seroprevalence of hepatitis E virus (HEV) among 997 healthy individuals aged 6 to 84 years, collected between July 1993 and June 1994 at Kaohsiung-Pingtung area in Southern Taiwan was studied. Of the study populations of vegetable farmers, elementary school children, volunteer blood donors and college students, the prevalence of IgG anti-HEV ranged from 6.4% to 8.8%. In suburban elemantary school children of Mang-Chou Village at Pingtung-Hsien, the seroprevalence rate (9.6%) was significantly higher than the positive rate (1.5%) found in rural aboriginal elementary school of San-Min Village at Kaohsiung-Hsien. IgG anti-HEV antibodies were widely distributed among all age groups, with a significantly higher percentage (13.1%) in the age group of 46-55 years old.  相似文献

为了解甘肃省现阶段乙型肝炎病毒感染的现状,分析乙肝病毒感染血清学指标的变化,采用多阶段整群抽样的方法,抽取5个县区1~59岁人群共2200人进行调查。以ELISA方法对血清标本统一检测乙肝病毒表面抗原(HBsAg)、乙肝病毒表面抗体(抗-HBs)、乙肝病毒核心抗体(抗-HBc)。结果显示,甘肃省1~59岁调查人群HBsAg阳性率为3.59%,抗-HBs和抗-HBc阳性率分别为49.45%、16.33%;HBV总感染率为27.50%。比1992年HBV总感染率下降了36.48%,HBsAg阳性率下降了45.94%;1~4岁儿童HBsAg阳性率仅为1.13%,比1992年(5.34%)下降78.84%。甘肃省1~59岁人群乙肝病毒感染率下降,抗-HBs阳性率升高,尤其在1~4岁儿童变化更为明显;甘肃省乙肝感染逐步呈现由中高流行转向低流行区的趋势;乙肝疫苗免疫取得显著效果。  相似文献

目的:了解本地区腹泻婴幼儿的A群轮状病毒感染情况及其流行特点。方法:采用胶体金法对我院2009年10月-2010年9月2104例有腹泻和肠炎特征的婴幼儿粪便进行A群轮状病毒抗原检测。结果:在2104例受检者中,A群轮状病毒感染的总阳性率是24.71%,其中男性感染率24.17%,女性为25.40%。不同年龄组间以1—2岁婴幼儿感染率最高,为32.13%,0-1岁为20.72%,2-5岁为12.03%。感染的季节特征是秋末冬初季(10—12月)阳性率最高,为42.82%,春末夏初季(4.6月)最低,为8.81%。结论:由A群轮状病毒感染引发的急性腹泻主要发生在1-2岁的婴幼儿,各个季节均有发生,以秋末冬初季为高发。  相似文献

Background

Infantile hepatitis B after neonatal immunoprophylaxis is a rare yet distinct disease. This study aimed to analyze the long-term outcomes and risk factors in immunized infants with hepatitis B.

Methods

The clinical parameters and outcomes of 41 infants born after universal immunization, and admitted for HBV-positive hepatitis were studied. All patients were followed for at least 6 months (median  = 4.4 years, range 0.6–18.1 years). Patient survival, changes of HBsAg and HBeAg status, and complications were analyzed.

Results

Among the 41 cases (32 males, 9 females), 21 presented with fulminant hepatitis (FH), and 20 with non-fulminant hepatitis (NFH). Ninety-five percent (36/38) of the mothers were positive for hepatitis B surface antigen (HBsAg). Multivariate analyses revealed younger age of onset (age <7 months) and negative maternal hepatitis B e antigen (HBeAg) were associated with FH (p = 0.03 and p = 0.01, respectively). An infantile fulminant hepatitis B risk score using maternal/infant HBeAg positivity and onset age was proposed. Among the FH cases, the rate of mortality, HBsAg clearance, and chronic HBV infection were 47.6%, 38.1%, and 14.3%, respectively. Among the NFH cases, 35% developed chronic infection. Of the 9 chronically infected children received long-term follow-up, 8 had HBeAg seroconversion before 4 years of age. One case of FH developed hepatocellular carcinoma 14 years later.

Conclusions

Maternal HBsAg + /HBeAg- and early onset age were risk factors for FH in immunized infants. A significant portion of patients with FH or NFH evolve to chronic HBV infection, with HBeAg seroconversion in young childhood. Close surveillance for hepatocellular carcinoma is warranted in patients surviving infantile hepatitis B.  相似文献

摘要 目的:总结西安市2010-2015年 0-5岁儿童乙肝病毒感染的发病趋势和流行病学特征,寻找高危人群。通过随访研究获取HBV感染儿童疾病转归及乙肝监测系统存在的问题,为乙肝监测系统完善及制定防治策略提供科学依据。方法:采用描述性流行病学方法对西安市2010-2015年的0-5岁儿童乙肝患者进行三间分布描述分析;采用前瞻性队列研究方法,检测母亲及儿童的外周血HBV病毒学情况,获得母亲感染状况和患儿转归。结果:6年间,西安市0-5岁儿童共上报乙肝病例175例,年均HBV感染率为6.05/10万,2013年最高,为9.73/10万,以散发为主;0-1岁为高发年龄段,男童发病多于女童;未央区、雁塔区为高发地区。截止2016年8月,随访HBV感染学龄前儿童139例,仅17例完成流行病学调查和体检检测,失访率高达87.7%,17例HBV感染儿童中HBsAg慢性化高达88.2%,其中14例(82.3%)母亲为HBsAg阳性者。结论:西安市0-5岁儿童HBV感染的高危人群为HBsAg阳性母亲的儿童,与宫内感染/母婴传播有关。0-5岁HBV感染儿童转归结局不良,建议加强HBV宫内阻断,并对高危新生儿进行乙肝抗体监测。  相似文献

目的分析儿童肺炎支原体(MP)咽拭子快速液体培养的结果及与患儿年龄以及季节的关系,为临床诊断、治疗肺炎支原体感染提供依据。方法选取深圳市宝安区人民医院1217例急性呼吸道感染的患儿,采用咽拭子快速液体培养基进行筛查,观察其阳性率。结果在1217例患儿中,共检出阳性281例,阳性率为23.09%。〈1岁、1~3岁、3~6岁和6~14岁各年龄组的阳性率分别为15.13%、26.52%、28.31%和18.07%;不同季节MP感染率分别为春23.75%,夏20.11%,秋18.61%,冬29.72%。1~3岁,3—6岁小儿感染率明显高于6—14和〈1岁儿童(P〈0.01);冬春季阳性率较夏秋季高。结论MP快速液体培养鉴定对MP诊断具有重要的临床诊断价值。肺炎支原体感染全年均可发病,好发于冬季,以3—6岁小儿多见。  相似文献

An epidemiological study and mass treatments of Enterobius vermicularis infection among children near Wonju area of Kangwon province were carried out. The children were divided into 4 groups according to their residing localities; children in the mountainous area, rural area, urban area and in orphanage. They were examined by adhesive cellotape anal swab technique, and egg positive rates were obtained. The rates of egg reduction and re-infection rates after repeated mass treatments were also observed. The results obtained were as follows: 1. The overall egg positive rate of E. vermicularis in the first screening was 19.9% (251 out of 1,262 examinees; 19.7% in males and 20.1% in females). The positive rates were 13.0% in the mountainous area, 11.9% in the rural area, 15.1% in the urban (medium-sized) area and 61.9% in orphanages. 2. The highest positive rates were observed in the kindergarten children, and 1st and 2nd grade children of primary schools (26.2-32.2%), and the lowest rate (13.6%) in 6-year grade children of primary schools. 3. Cumulative detection rates from 3 repeated anal swabs at 4-5 days interval were higher (70.8%) than those from single anal swabs (50.0-59.2%). 4. Out of the examinees who showed the highest cumulative positive rate (70.8%), about 39.2% were consecutively positive in 3 anal swabs. Among different groups of children, the higher the total egg detection rates (87.5%), the higher the consecutive positive rates (71.9%). 5. A total of 2,609 (male: female = 1:12.4) worms were collected from 17 egg-positive cases treated with anthelmintics. Serological surveys of rubella antibody were carried out using the hemagglutination-inhibition test, with a view to studying the distribution of seroimmune individuals according to age and intermingling with other populations. Specimens were collected from different age groups including infants, children and adults, among the inhabitants of Montreal from 1963 to 1968. From the results obtained it was possible to establish the pattern of rubella antibody development in this urban community. Surveys were also conducted among the inhabitants of Les Iles de la Madeleine, a Canadian island in the Gulf of St. Lawrence, and among the population of Easter Island, an isolated island in the South Pacific remote from any large land mass.It was seen that, among the inhabitants of Montreal, presumably maternally acquired rubella antibody was present in 95% or more of the infants, the same percentage of seroimmune individuals as was found among the adult women 25 to 30 years of age. Passively acquired rubella antibodies decreased rapidly, attaining their lowest levels among children 1 to 2 years old. Rubella infection occurred in young children and its incidence rose steeply from school age to adolescence, leaving 7 to 9% of the adults without antibody. The highest geometric mean antibody titres were found among children 4 to 10 years of age.The same pattern of rubella antibody development was found among the population of Les Iles de la Madeleine, except that in adults the percentages of seropositives reached practically 100%. Antibody titres decreased with advancing age and became lower than those found among children.Detection of rubella antibody in serum samples derived from the inhabitants of Easter Island indicated that this population had experienced rubella infection not long before the Canadian Medical Expedition of 1964-1965. This status is determined from the high proportion of seroimmune individuals in each age group and the uniformly high antibody titre.Island populations appear to represent the ideal subjects for estimating the duration of the immunity conferred by any attenuated rubella vaccine that will eventually be licensed.  相似文献

In 1982 a two dose regimen was introduced in Sweden for the combined vaccination against measles, mumps, and rubella of children aged 18 months and 12 years. Since 1977 about half of the preschool children were vaccinated against measles annually, and since 1974 about 80% of 12 year old girls were vaccinated against rubella. During the period 1982 to 1985 90-93% of the eligible age cohorts of 18 month old children and 88-91% of the 12 year old children were immunised with the new combined vaccine. A study in 1982 of about 140 18 month old children who were nearly all seronegative before vaccination showed that 96%, 92%, and 99% seroconverted against measles, mumps, and rubella, respectively. A second study was carried out in 1983 of 247 12 year old children, of whom 11% lacked antibodies to measles, 27% to mumps, and 45% to rubella. This showed seroconversion in 82% and 80% against measles and mumps, respectively, and all children seroconverted against rubella. In the latest study in 1985 of 496 12 year olds 9% and 13% were seronegative against measles and mumps before vaccination, and 41% against rubella. Of these, 88% seroconverted to measles and 80% to mumps, and all converted to rubella when sera were tested by the haemolysis in gel method. After a neutralisation test against measles as well all children showed immunity to the disease. A low incidence of measles and declining figures for mumps and rubella were reported in 1984 to 1986. An outbreak of rubella during 1985 affected mainly boys in age cohorts in which only the girls had been vaccinated during the 1970s.  相似文献
