Patterns of marriage and parenthood were studied in the families of 250 patients with severe hemophilia. The incidence of marriage among adult hemophiliacs was 82.5 percent that of age-matched males of the general United States population, as of 1968. Married hemophiliacs 30 years old or older had an average of 2.1 children each, as of 1971. In the periods 1950-1954 and 1960-1964, the numbers of children sired by hemophiliacs were 25 and 38 percent, respectively, of the numbers expected of age-matched men of the general United States population. This indicates a lesser ability or willingness to have children. The number of additional births to women who were already mothers of hemophiliacs was compared with that expected for women of the general population of the United States, matched for age and parity. In the years 1940 through 1954, the birthrates for the two groups were similar. However, in the years 1955-1968, the birthrate among carriers declined more rapidly than that in the general population. The improved medical treatment of hemophilia has apparently not resulted in a greater willingness of affected persons to transmit the defective gene.  相似文献

The range of values for the 24-hour thyroidal accumulation of radioactive iodine in euthyroid persons varies with geographic location. In the San Bernardino Valley region of Southern California the “normal range” is 6 percent to 33 percent in euthyroid subjects. This is lower than in studies from other areas of the United States. The urinary iodide excretion and the absolute iodine uptake of the thyroid are higher than in studies from many other areas of the United States, pointing to iodine abundance as the reason for this difference. The geographic variation and the possibility of changing dietary iodine intake of normal persons point to the necessity of current and local determinations of the “normal range” of the thyroidal uptake of radioiodine if the results of this thyroid function test are to be properly interpreted.  相似文献

Retroviral infections transmitted by blood transfusion   总被引:2,自引:0,他引:2  
Modifications in donor screening and the introduction of laboratory testing of donated blood for anti-HIV-1 and anti-HTLV-I have resulted in a significant reduction in the risks of retroviral infections from blood transfusion. Presently, the American Red Cross detects an average of eight carriers of human immunodeficiency virus, type 1 (HIV-1) per 100,000 otherwise acceptable blood donors (0.008 percent), compared with an average of 35 per 100,000 (0.035 percent) when testing for HIV-1 antibodies began in 1985. Surveillance studies in the United States indicate a small likelihood that HIV-2 carriers will pass current screening procedures and be accepted as blood donors. Even if an HIV-2-infected person were to be accepted as a blood donor, there is a 42-92 percent likelihood that this person's blood would be detected as infective for HIV-2 and excluded because of serological cross-reactions that occur in the EIA for HIV-1 antibodies. During 1989, which was the first year that donated blood was routinely tested for antibodies to human T-lymphotropic virus, type I (HTLV-I) in the United States, approximately nine in 100,000 donors (0.009 percent) were confirmed positive for antibodies to HTLV-I, and their donated blood was excluded. Subsequent testing has revealed that a significant number of these persons whose sera was reactive by the HTLV-I EIA were, in fact, infected by HTLV-II. Epidemiological studies of human retroviral infections (HIV-1, HIV-2, HTLV-I, and HTLV-II) continue to provide important data and direction for improving criteria for qualifying blood donors.  相似文献

It has been suggested that geographic variation in the early neonatal (0-6 days) and postperinatal (7-364 days) mortality rates for the United States derives from positive and negative associations, respectively, with the probability of an average couple sharing one or more HLA antigens. If this theory is correct, then the ratio of 0-6-day deaths to 7-364-day deaths among the offspring of migrant parents should be similar to that seen in the area from which the mother and the father originally migrated. Information on deaths among the offspring of migrant couples by the mother's region of birth was obtained from computerized vital statistics for Washington state for the period 1968-1977. Information on infant deaths occurring in the mother's region of birth was obtained from published vital statistics for the United States for 1980-1984. Ratios of 0-6-day deaths to 7-364-day deaths in 9 geographic regions for the period 1980-1984 were placed in 4 intervals. These intervals became the standard against which the 0-6/7-364-day ratios in the offspring of migrant parents were compared by means of a test for trend in proportions. As predicted, a significant positive trend was observed when the mother and the father were born in the same region but not when they were born in different regions.  相似文献

Data from the World Health Organization, U.S. Vital Statistics, the Historical Abstracts of the United States and the census of the United States were used to compare current world and states data on cancer death rate and age with historical data that shows the changing percentage of elderly. Current annual death rates from malignant neoplastic disease among the 73 countries for which data exist are highly correlated with the percent of the population ≥ 60 yrs of age and a virtually identical relation exists presently among the 50 states of the U.S. The current world and states data provide a remarkably accurate snapshot of U.S. cancer death rates over the past century during which time the U.S. elderly population increased substantially. Data on U.S. lung cancer death rates since 1940 were similarly examined and these historical numbers also show good agreement with current values among countries that today span the same proportion of elderly that the U.S. had over this period. The agreement of current and historical CA death rate and age contrasts sharply with the picture that emerges when cardiovascular death rates and death rate from all causes are examined. The geographic and time invariance seen in the cancer data seems incosistent with identical exposure to carcinogens and suggests that biological aging is an important intrinsic factor in cancer.  相似文献

Vibrio vulnificus is a marine bacterium that causes human wound infections and septicemia with a high mortality rate. V. vulnificus strains from different clinical and environmental sources or geographic regions have been successfully characterized by ribotyping and several other methods. Pulsed-field gel electrophoresis (PFGE) is a highly discriminative method, but previous studies suggested that it was not suitable for examining the correlation of V. vulnificus strains from different origins. We employed PFGE to determine its efficacy for characterizing V. vulnificus strains from different geographic regions, characterizing a total of 153 strains from clinical and environmental origins from the United States and Taiwan after SfiI or NotI digestion. V. vulnificus strains showed a high intraspecific diversity by PFGE after SfiI or NotI digestion, and about 12% of the strains could not be typed by the use of either of these enzymes. For PFGE with SfiI digestion, most of the clinical and environmental strains from the United States were grouped into cluster A, while the strains from Taiwan were grouped into other clusters. Clinical strains from the United States showed a higher level of genetic homogeneity than clinical strains from Taiwan, and environmental strains from both regions showed a similarly high level of heterogeneity. PFGE with NotI digestion was useful for studying the correlation of clinical strains from the United States and Taiwan, but it was not suitable for analyzing environmental strains. The results showed that PFGE with SfiI digestion may be used to characterize V. vulnificus strains from distant geographic regions, with NotI being a recommended alternative enzyme.  相似文献

Lyme disease, caused by the bacterium Borrelia burgdorferi sensu stricto, is an emerging zoonotic disease in Canada and is vectored by the blacklegged tick, Ixodes scapularis. Here we used Bayesian analyses of sequence types (STs), determined by multilocus sequence typing (MLST), to investigate the phylogeography of B. burgdorferi populations in southern Canada and the United States by analyzing MLST data from 564 B. burgdorferi-positive samples collected during surveillance. A total of 107 Canadian samples from field sites were characterized as part of this study, and these data were combined with existing MLST data for samples from the United States and Canada. Only 17% of STs were common between both countries, while 49% occurred only in the United States, and 34% occurred only in Canada. However, STs in southeastern Ontario and southwestern Quebec were typically identical to those in the northeastern United States, suggesting a recent introduction into this region from the United States. In contrast, STs in other locations in Canada (the Maritimes; Long Point, Ontario; and southeastern Manitoba) were frequently unique to those locations but were putative descendants of STs previously found in the United States. The picture in Canada is consistent with relatively recent introductions from multiple refugial populations in the United States. These data thus point to a geographic pattern of populations of B. burgdorferi in North America that may be more complex than simply comprising northeastern, midwestern, and Californian groups. We speculate that this reflects the complex ecology and spatial distribution of key reservoir hosts.  相似文献

BACKGROUND: An increase in the rate of gastroschisis has been documented by birth defects surveillance systems in the United States and in other countries. This study sought to evaluate historical trends in the rate of gastroschisis in Atlanta, Georgia, and to describe the epidemiology of gastroschisis over 33 years. METHODS: Gastroschisis cases were identified through the Metropolitan Atlanta Congenital Defects Program (MACDP) from 1968 through 2000. Poisson regression techniques were used to evaluate trends over time. Data on covariates were compared for three maternal age groups (< or =19, 20-24, and > or =25 years). RESULTS: From 1968 through 1975, the rate of gastroschisis was stable at 0.8 per 10,000 births. After 1975, the rate of gastroschisis was 2.3 per 10,000 births with no significant increase observed from 1976 through 2000. The rate of gastroschisis was six times higher among teenage mothers compared with mothers > or =25 years of age. Affected infants born to teenage mothers were less likely to be born to Black mothers compared to White mothers (rate ratio [RR], 0.4; 95% confidence interval [CI], 0.2-0.6). This was also true for mothers 20-24 years of age (RR, 0.5; 95% CI, 0.3-0.8) but not for mothers 25 years of age or older (RR, 1.6; 95% CI, 0.9-2.7). CONCLUSIONS: An increase in the rate of gastroschisis was observed in the mid-1970s, but no temporal trend has been observed since that time. In light of recent reports of an increasing prevalence of gastroschisis in the United States, continued monitoring of this birth defect is warranted.  相似文献

为探究中国和美国大豆疫霉的遗传关系, 采用简单序列重复区间扩增多态性(ISSR)技术, 对来自中国黑龙江省、福建省和美国的3个大豆疫霉地理群体的遗传多样性进行了分析。通过13个ISSR引物对供试的111株大豆疫霉菌株进行扩增, 共得到102个ISSR条带, 其中多态性条带为88个, 占86%。遗传变异分析表明, 美国群体具有更高的遗传变异度; Nei’s 遗传相似性和主成分分析均显示, 中国福建群体与美国群体间的遗传相似性最高, 而福建群体与黑龙江群体间遗传相似性最低; 聚类分析显示, 供试菌株在88%的相似性水平上可区分为7个聚类组, 且美国群体分布于更多的聚类组中; Shannon-Wiener多样性指数也表明美国群体的遗传多样性最为丰富。综合分析表明, 本研究的结果不支持关于美国的大豆疫霉可能来源于中国的推测。  相似文献

Results of rubella antibody tests performed by the California State Viral and Rickettsial Disease Laboratory on blood specimens collected in 1968 and 1969 from school children and women of childbearing age showed a slightly lower prevalence of rubella antibody in California than reported from most other areas of the United States. Among women of childbearing age, rubella hemagglutination-inhibition (hi) antibody was found in 72 percent of those tested in California compared with 80 percent to 90 percent in other areas of the country.Rubella antibody testing services offered by the State Virus Laboratory included situations in which a pregnant woman was exposed to a suspected case of rubella. It was shown that very few of these exposures constitute a significant risk to the fetus as most of the women already possessed antibody to rubella from past infection and in many instances the exposures were not to actual cases of rubella.The results of this study emphasized the urgency of obtaining blood specimens from pregnant women as soon as possible after exposure to rubella or development of symptoms of rubella. The urgency and anxiety attending these situations can largely be obviated if routine rubella antibody testing of women is carried out prior to pregnancy or at the first prenatal visit.  相似文献

Limited by the availability of data, conservation planners must use surrogates for biodiversity when selecting conservation areas. Although several methods have been proposed for selecting surrogates, no clear set of species attributes have been described that allow for the efficient a priori selection of surrogate groups. We used a database of 1449 species in two regions of the United States to (1) examine the consistency in the performance of simple taxonomic-based surrogates of biodiversity and (2) test five hypotheses proposed to explain surrogate performance. First, we compared the ability of sites selected to protect members of seven surrogate groups to protect non-surrogate species in the north-western United States and in the Middle-Atlantic region of the eastern United States. Then, in a separate analysis, we tested whether surrogate performance could be explained by (1) taxonomic diversity; (2) nested species distributions; (3) hotspots of biodiversity; (4) species range sizes; (5) environmental diversity. Our first analysis revealed little consistency in the performance of surrogates in the two different study regions. For example, butterflies provided protection for 76% of all other species in the north-western United States but only 56% of all other species in the eastern United States. Our second analysis revealed only weak associations between species characteristics and surrogate performance. Furthermore, these associations proved inadequate for selecting successful surrogates across study regions. Overall, our results suggest that in lieu of searching for optimal surrogate groups, research efforts will be better spent by developing alternative methods for assessing conservation value in areas where data on species distributions are limited.  相似文献

The mortality rate due to heroin overdosage in San Francisco has increased dramatically since 1968 and now stands as one of the highest in the United States. While the numbers of heroin fatalities in many eastern United States cities have declined substantially in the past few years, the figures for San Francisco and the other West Coast areas continue to increase. The group of heroin overdose victims from the 1970 through 1973 period is more predominantly Caucasian and younger than from the 1963 through 1965 period. In nearly all of the victims, the presence of morphine (a heroin metabolite) was noted in bile or urine, and in about half the results of blood alcohol tests were positive. Measurement of blood morphine concentrations in the victims showed no significant difference from the concentrations noted in a control group of heroin addicts dying from causes other than overdosage.  相似文献

The ectomycorrhizal fungus Pisolithus tinctorius has been introduced to many areas around the world as a source of inoculum for pine plantations. However, little is known about the genetic structure of fungal populations in their introduced habitats. To study the genetics of exotic P. tinctorius populations, we developed and employed seven new microsatellite markers and compared samples from Puerto Rico (exotic range) and the eastern United States (native range). Bayesian cluster analysis, neighbor joining analysis and FST values all strongly separated Puerto Rican populations from North American populations. Consistent with a founder effect, populations from Puerto Rico had reduced allelic richness when compared to samples from the United States. The genetic structure of P. tinctorius populations in Puerto Rico is weak to modest and is not correlated with geographic distance reflecting anthropogenic movement of inoculum with forestry practices.  相似文献

Previous national and global conservation assessments have relied on habitat conversion data to quantify conservation risk. However, in addition to habitat conversion to crop production or urban uses, ecosystem alteration (e.g., from logging, conversion to plantations, biological invasion, or fire suppression) is a large source of conservation risk. We add data quantifying ecosystem alteration on unconverted lands to arrive at a more accurate depiction of conservation risk for the conterminous United States. We quantify ecosystem alteration using a recent national assessment based on remote sensing of current vegetation compared with modeled reference natural vegetation conditions. Highly altered (but not converted) ecosystems comprise 23% of the conterminous United States, such that the number of critically endangered ecoregions in the United States is 156% higher than when calculated using habitat conversion data alone. Increased attention to natural resource management will be essential to address widespread ecosystem alteration and reduce conservation risk.  相似文献

Little is known about how mutualistic interactions affect the distribution of species richness on broad geographic scales. Because mutualism positively affects the fitness of all species involved in the interaction, one hypothesis is that the richness of species involved should be positively correlated across their range, especially for obligate relationships. Alternatively, if mutualisms involve multiple mutualistic partners, the distribution of mutualists should not necessarily be related, and patterns in species distributions might be more strongly correlated with environmental factors. In this study, we compared the distributions of plants and vertebrate animals involved in seed‐dispersal mutualisms across the United States and Canada. We compiled geographic distributions of plants dispersed by frugivores and scatter‐hoarding animals, and compared their distribution of richness to the distribution in disperser richness. We found that the distribution of animal dispersers shows a negative relationship to the distribution of the plants that they disperse, and this is true whether the plants dispersed by frugivores or scatter‐hoarders are considered separately or combined. In fact, the mismatch in species richness between plants and the animals that disperse their seeds is dramatic, with plants species richness greatest in the in the eastern United States and the animal species richness greatest in the southwest United States. Environmental factors were corelated with the difference in the distribution of plants and their animal mutualists and likely are more important in the distribution of both plants and animals. This study is the first to describe the broad‐scale distribution of seed‐dispersing vertebrates and compare the distributions to the plants they disperse. With these data, we can now identify locations that warrant further study to understand the factors that influence the distribution of the plants and animals involved in these mutualisms.  相似文献

Remington (1968) argued that 13 suture zones exist in North America. Remington defined a suture zone as, "a band of geographic overlap between major biotic assemblages, including some pairs of species or semispecies which hybridize in the zone" (p. 322). Although initially controversial, the idea that suture zones exist has picked up momentum over the past decade, due largely to the phylogeographic work of Hewitt, Avise, and their colleagues. Nevertheless, the reality of suture zones has not yet been subjected to rigorous analysis using statistical and geographic information system (GIS) approaches. To test for the existence of Remington's suture zones, we first identified 117 terrestrial hybrid zones in Canada and the United States through a literature search for the key words "cline," "contact zone," "hybrid zone," and "hybridization" in articles published between 1970 and 2002. The 117 hybrid zones were mapped using a GIS approach and compared with a digitized version of Remington's original suture zone map. Overall, there does appear to be an association between hybrid zones and suture zones, but this association is largely attributable to clustering of hybrid zones in only two of the 13 suture zones recognized by Remington. The results suggest that evolutionary biologists should retain some skepticism toward Remington's suture zones.  相似文献

Resightings of uniquely marked birds from 2001 to 2008 were used to determine winter distributions of 4 breeding populations of a species at risk, the piping plover (Charadrius melodus). Although considerable overlap exists, a distinct pattern in winter distributions was evident. Birds originating from eastern Canada wintered farthest north compared to other populations. Most individuals from the United States Great Lakes were found in South Carolina and Georgia in winter, whereas birds from eastern Canada were found primarily in North Carolina. Although the great majority of birds marked in Prairie Canada were observed wintering in Texas, particularly southern Texas, breeding plovers from the United States Great Plains were more widely distributed on the gulf coast from Florida to Texas. Very few large-scale movements of individuals in winter were reported either within or between years. This study highlights the significance of geographic regions for eastern Canada, the United States Great Lakes, the United States Great Plains, and Prairie Canada populations, and demonstrates relatively high winter site fidelity. This information will help focus conservation efforts for specific breeding populations during the winter. © 2011 The Wildlife Society.  相似文献

OBJECTIVE: To determine whether location of postgraduate medical training and other factors are associated with the emigration of physicians from Canada to the United States. DESIGN: Case-control study, physicians were surveyed with the use of a questionnaire mailed in May 1994 (with a reminder sent in September 1994), responses to which were accepted until Dec. 31, 1994. PARTICIPANTS: Physicians randomly selected from the CMA database, 4000 with addresses in Canada and 4000 with current addresses in the United States and previous addresses in Canada. OUTCOME MEASURES: Sex, age, location of undergraduate and postgraduate medical training, qualifications, practice location, opinions concerning residence decisions, current satisfaction and plans. RESULTS: The overall response rate was 49.6% (50.0% among physicians in the United States and 49.2% among those in Canada). Age and sex distributions were similar among the 8000 questionnaire recipients and the nearly 4000 respondents. Physicians living in the United States were more likely to be older (mean 53.2 v. 49.6 years of age), male (87% v. 75%) and specialists (79% v. 52%) than those practising in Canada. Postgraduate training in the United States was associated with subsequent emigration (odds ratio 9.2, 95% confidence interval 7.8 to 10.7). However, in rating the importance of nine factors in the decision to emigrate or remain in Canada, there was no significant difference between the two groups in the rating assigned to location of postgraduate training. Professional factors rated most important by most physicians in both groups were professional/clinical autonomy, availability of medical facilities and job availability. Remuneration was considered an equally important factor by those in Canada and in the United States. Six of seven personal/family factors were rated as more important to their choice of practice location by respondents in Canada than by those in the United States. Current satisfaction was significantly higher among respondents in the United States. Most physicians in each group planned to continue practising at their current location. Of Canadian respondents, 22% indicated that they were more likely to move to the United States than they were a year beforehand, whereas 4% of US respondents indicated that they were more likely to return to Canada. CONCLUSIONS: Factors affecting the decision to move to the United States or remain in Canada can be categorized as "push" factors (e.g., government involvement) and "pull" factors (e.g., better geographic climate in the US). Factors can also be categorized by whether they are amenable to change (e.g., availability of medical facilities) or cannot be managed (e.g., proximity of relatives). An understanding of the reasons why physicians immigrate to the United States or remain in Canada is essential to planning physician resources nationally.  相似文献
