This article considers contributions that the medical humanities have made to biomedical ethics. Philosophy has contributed methods of ethical justification to case analysis and has given birth to the New Professionalism movement. Taking biography as its paradigmatic resource, this movement has refocused medical education on the formation of physicians who not only have certain responsibilities to their patients, but also a regard for the role of the medical profession in working toward social justice. However, reliance on biography is now giving way to a renewed emphasis on autobiography, as educators seek to support medical students through the personal and spiritual journey that confrontation with death, disease, and injustice naturally entails. Reflective methods and techniques characterize this emerging era as educators and students try to preserve and reinvigorate the soul of medicine.  相似文献

The second part of this study of the six axes of head motion caused by translational seat vibration is concerned with the effect of fore-and-aft (x-axis) and lateral (y-axis) seat vibration. Seat-to-head transmissibilities have been determined at frequencies up to 16 Hz for each of the three translational and three rotational axes of the head during exposure to random vibration of the seat. Repeatability measures within a single subject and studies of the variability across a group of twelve subjects have been conducted with two seating conditions: a rigid seat with a backrest, and the same seat with no backrest. Fore-and-aft seat motion mainly resulted in head motion within the mid-sagittal plane (x-z plane). Without the backrest, transmissibilities for the fore-and-aft, vertical and pitch axes of the head were greatest at about 2 Hz. The backrest greatly increased head vibration at frequencies above 4 Hz and caused a second peak in the transmissibility curves at about 6 to 8 Hz. Lateral seat motion mainly caused lateral head motion with a maximum transmissibility at about 2 Hz. The backrest had little effect on the transmission of lateral vibration to the head. For both axes of excitation inter-subject variability was much greater than intra-subject variability.  相似文献

Vibration in the three translational (fore-and-aft, lateral and vertical) and the three rotational (roll, pitch and yaw) axes of the head has been measured during exposure to whole-body random vibration. Using an instrumented bar gripped between the teeth, the influence of variations in bite grip and bite-bar mass on movements of the head were found to be small up to a mass of 375 g. The repeatability of measures of seat-to-head transmissibility within a single subject and the variability in transmissibility across a group of twelve subjects have been determined with two seating conditions: a rigid seat with a backrest and the same seat with no backrest. Seat-to-head transmissibilities associated with vertical seat vibration are presented at frequencies up to 25 Hz for all six axes of head vibration both with and without a backrest. Head motion occurred principally in the fore-and-aft, vertical and pitch axes of the head. The backrest increased the magnitude of head vibration in most cases. Intra-subject variability was generally small compared to inter-subject variability.  相似文献

This paper examines the critical attitude of behavioral professionals toward spiritual phenomena, and the current growing openness toward a scientific study of spirituality and its effects on health. Health care professionals work amidst sickness and suffering, and become immersed in the struggles of suffering persons for meaning and spiritual direction. Biofeedback and neurofeedback training can facilitate relaxation, mental stillness, and the emergence of spiritual experiences. A growing body of empirical studies documents largely positive effects of religious involvement on health. The effects of religion and spirituality on health are diverse, ranging from such tangible and easily understood phenomena as a reduction of health-risk behaviors in church-goers, to more elusive phenomena such as the distant effects of prayer on health and physiology. Psychophysiological methods may prove useful in identifying specific physiological mechanisms mediating such effects. Spirituality is also a dimension in much of complementary and alternative medicine (CAM), and the CAM arena may offer a window of opportunity for biofeedback practice.  相似文献

The avian pineal gland, like that of mammals, displays a striking circadian rhythm in the synthesis and release of the hormone melatonin. However, the pineal gland plays a more prominent role in avian circadian organization and differs from that in mammals in several ways. One important difference is that the pineal gland in birds is relatively autonomous. In addition to making melatonin, the avian pineal contains photoreceptors and a circadian clock (thus, an entire circadian system) within itself. Furthermore, avian pineals retain their circadian properties in organ or dispersed cell culture, making biochemical components of regulatory pathways accessible. Avian pinealocytes are directly photosensitive, and novel candidates for the unidentified photopigments involved in the regulation of clock function and melatonin production, including melanopsin, pinopsin, iodopsin, and the cryptochromes, are being evaluated. Transduction pathways and second messengers that may be involved in acute and entraining effects, including cyclic nucleotides, calcium fluxes, and protein kinases, have been, and continue to be, examined. Moreover, several clock genes similar to those found in Drosophila and mouse are expressed, and their dynamics and interactions are being studied. Finally, the bases for acute and clock regulation of the key enzyme in melatonin synthesis, arylalkylamine N-acetyltransferase (AA-NAT), are described. The ability to study entrainment, the oscillator itself, and a physiological output in the same tissue at the same time makes the avian pineal gland an excellent model to study the bases and regulation of circadian rhythms.  相似文献
