Seven years after the publication of the first diagnostic reference levels (DRL) order, the analysis of the data allows the French Institute for radiation protection and nuclear safety (IRSN) to assess and evaluate the increase of the nuclear medicine departments involvement and of the level administered activities to the patients during the most common examinations performed in France. IRSN analyses show a good agreement between the distribution of transmitted examinations and the frequency of examinations performed in France, taking into account 95% of the number of examinations and 95% of the dose delivered to patients by nuclear medicine. These analyses highlight the necessary consistency between DRL regulation, the French society of nuclear medicine (SFMN) recommendations and national practice. The IRSN recommendations established from data analyses have leaded the authorities to publish a new DRL order in January 2012. This first update of the regulation takes into account actual clinical practice in nuclear medicine and introduces fundamental points as pediatric DRL. In the future, periodical updates will be implemented in order to take into account procedures and devices evolutions.  相似文献

The formation of new vessels, a process referred to as neoangiogenesis, is one of the key pathophysiological mechanisms in the development and progression of cancer. It contributes to tumour growth and dissemination of neoplastic cells and can determine response or resistance to anticancer therapies. It involves different signaling pathways including the vascular endothelial growth factor (VEGF) pathway and integrins, which are also preferred targets for the development of antiangiogenic therapies. Changes in the microvasculature induced by antiangiogenic treatments occur before morphological changes can be detected with conventional imaging approaches. The development of molecular tools enabling an assessment of these targets before initiating therapy, or early detection of response or recurrence during or following treatment is essential for the close monitoring of antiangiogenic treatments. These outstanding needs call for the development of specific probes enabling the characterization of the molecules and pathways involved. This review summarizes the major signaling pathway involved in promoting tumor neoangiogenes is, the different radiotracers recently developed in preclinical and clinical settings, as well as their potential use in humans in order to improve the management of patients treated with antiangiogenic treatments.  相似文献

Since previous ACOMEN's conference in 2005 on radiopharmaceuticals, many improvements have been encountered: active research has allowed the development of numerous new tracers of interest, with a large part dedicated for PET; clinical applications of radiopharmaceuticals have resulted in patients care improvement, both for management and survival; therapeutic applications are now fully recognized, as internal targeted radiotherapy could be considered as efficient in several cancer diseases; and regulation, despite remaining difficulties, will certainly become more favourable for radiopharmaceuticals. Thus we could make sure that radiopharmaceuticals use will be even more established in the next years.  相似文献

Gallium-68 is a radionuclide, which has, because of its favorable physical characteristics similar to those of fluorine-18, gained a great interest for labeling PET tracers. Furthermore, it is available onsite at the radiopharmacy, by elution of a 68Ge/68Ga generator that has a shelf life of around one year, which approximates the versatility of 99mTc in the labeling of tracers for scintigraphy. After a brief historical overview, this article lists the necessary equipment and components, and the choices to be made (currently and in the near future) to implement, in a radiopharmacy, 68Ga-labelling of PET tracers for a nuclear medicine department.  相似文献

Technetium99m (99mTc) shortage crisis regularly affect nuclear medicine activity and oblige the community to find solutions in order to perform most of the prescribed exams and avoid systematic substitutions by other non-nuclear medicine techniques. Firstly, some practical solutions can be set up in radiopharmacy departments such as using more than two generators together, realizing fractionated elutions, preparing radiopharmaceuticals with elutions providing from different generators… Then, it could be interesting to have a reflexion in nuclear medicine departments to convene patients the days when 99mTc supply is sufficient, to pool some exams or to make substitutions with more available isotopes.  相似文献

Nuclear imaging techniques have been occupying an increasingly large place in the study of potentially cancerous morphological abnormalities. A National French survey was conducted among members of the French Society of Nuclear Medicine (Société française de médecine nucléaire) to study the usual practices of nuclear medicine physicians regarding information and communication during the “pre-announcement” stage of a cancer diagnosis. Sixty-six doctors (with an average of 44.7 years of age and 16.3 years of experience) working in mainland France responded to the online survey between November 2013 and February 2014. Most physicians said that they were uncomfortable in this kind of situation. Half of the doctors interviewed faced that situation at least 5 times a month. Communication habits were varied: 20% of them adopted a systematic approach (chose ‘always’ or ‘never’ to communicate information regardless of the patients’ demands) and 80% adapted their approach to the situations and to the patients (communication ‘on case by case basis’ and ‘upon request’). The survey also showed that professionals have contrasting views on the relevance, interest and risks of informing patients about the results of the tests in this kind of situation. Our study shows that it is necessary to discuss the position of nuclear medicine physicians in the pre-announcement procedure set out in the French Cancer Plan so as to reduce the challenges facing them while at the same time organizing and defining their roles.  相似文献

The diagnosis of chronic orthopedic infections relies on the combination of clinical, laboratory and imaging findings. Although there is no perfect imaging examination, the various nuclear medicine techniques are a useful help for clinicians. Performance and image quality have improved dramatically since the development of hybrid cameras. The choice of the most appropriate examination for underlying infections is sometimes difficult as well as its interpretation. It is often essential to know the patient's history (surgical history, biology, etc.). We have tried to illustrate the sometimes-complex management of chronic infections of hip prostheses through these clinical cases from multidisciplinary staff committee of osteoarticular infections.  相似文献

IntroductionHyperparathyroidism due to a parathyroid ectopia is a rare disease; it is the most common cause of persistent or recurrent hyperparathyroidism. Its treatment is exclusively surgical and the results of the surgery benefit from the contribution of preoperative imaging techniques. The aim of this work is to illustrate the interest of the single photon emission computed tomography/computed tomography (SPECT/CT) in the detection and treatment of ectopic parathyroid orientation.Case reportClinical case 1: a 41-year-old patient was followed for persistent primary hyperparathyroidism revealed by brown tumors at the level of the lower extremity of the right femur and tibia. The rate of parathormone was 1987.4 pg/mL. The cervical echography and the scanning were not decisive. The SPECT/CT, performed during a 99mTc-MIBI scintigraphy and the same for the 2nd clinical case, has showed an area of high uptake projecting behind the left sternoclavicular joint. The diagnosis of ectopic parathyroid localization was retained. Resection and pathologic examination of the surgical specimen revealed parathyroid adenoma tissue. The evolution was marked by a normalization of the rate of parathormone. Clinical case 2: a 71-year-old patient who presents a clinico-biological board of primitive persistent hyperparathyroidism. A first scintigraphy (99mTc-MIBI) realized in 2010 was negative. The rate of parathormone was 969 ng/L, the rate of calcium was 2.90 mmol/L. Two cervical ultrasound were normal. The SPECT/CT has objectified an area of high uptake in projection at the upper floor of the middle mediastinum. The diagnosis of ectopic parathyroid localization was retained. Surgical resection and histological study revealed a parathyroid adenoma tissue. The evolution was marked by a normalization of the rate of parathormone.DiscussionThe SPECT/CT has an important role in the management of patients with persistent or recurrent hyperparathyroidism. It can usually visualize ectopic parathyroid injury, specify its size and anatomic relationships to optimize surgical management.  相似文献
